흉막액 Cholesterol 농도에 대한 진단적 의의 (A Diagnostic Significance of Pleural Fluid Cholesterol Level)

  • The purpose of this study is to investigate the utility of the pleural fluid cholesterol level in separating the exudates from the transudates, and in differentiating tuberculous exudates from non-tuberculous exudates, 52 patients with pleural effusion were involved in this prospective study. By predefined criteria, 40 of these effusions were classified as exudates (Group I) and 12 as transudates (Group II). Group I was subdivided into tuberculous exudates (Group A) and non-tuberculous exudates (Group B). The followings are parameters used in separating the exudates from the transudates; pleural protein (P-PROT) 3.0 g/dl, pleural protein/serum protein ratio (P/S PORT) 0.5, pleural LDH (P-LDH) 200 IU, pleural LDH/serum LDH ratio (P/S LDH) 0.6, pleural cholesterol (P-CHOL) 50 mg/dl and pleural cholesterol/serum cholesterol ratio (P/S CHOL) 0.4. Mean values of the parameters in each group were compared, and then misclassified rate and the dignostic efficiency for each parameter were calculated. The results were as follows; 1) Mean P-CHOL ($94.98{\pm}73.86\;mg/dl$) in Group I was higher than that ($36.5{\pm}26.5\;mg/dl$) in Group II (p<0.05), but there was no significant difference in mean P-CHOL between Group A and Group B. 2) Mean P/S CHOL ($0.64{\pm}0.39$) in Group I was also higher than that ($0.27{\pm}0.15$) in Group II (p<0.01), but no difference was observed in mean P/S CHOL between Group A and Group B. 3) Misclassified rates for each parameter in separating the exudates from the transudates were as follows; P-PROT 1.9%. P/S PROT 3.8%. P-CHOL 9.6%, P/S CHOL 11.5%, P/S LDH 11.5%, and P-LDH 17.3%. 4) Diagnostic efficiencies for each parameter in separating the exudates from the transudates were as follows; P-PROT 98.1%, P/S PROT 96.2%. P-CHOL 90.4%. P/S CHOL 88.5%, P/S LDH 88.5%, and P-LDH 82.7%. In conclusion, we think that the pleural fluid chloesterol level could be used as a supportive parameter in separating the exudates from the transudates, but could not be used as a parameter in differentiating tuberculous exudates from non-tuberculous exudates.

가미제습순기탕(加味除濕順氣湯)이 고지혈증(高脂血症)을 유발시킨 고혈압(高血壓) 백서(白鼠)에서의 혈액학적(血液學的) 변화(變化)에 미치는 영향(影響) (A study on the Effect of Kamijesubsungitang on Hyperlipidemia)

  • KJST is the oriental herbal medicine of hyperlipemia which consisted of a herb of 17. We induced hyperlipemia to Spontaneous Hypertensive Rat(SHR) and studied the treatment effect by KJST. Hyperlipidemia was induced by hyper-lipidemic diet fed for 4weeks. Total cholesterol, LDL-cholesterol, HDL-cholesterol, Triglyceride, Total bilirubin, Lactic dehydrogenase(LDH), Glucose and Total protein were measured on the serum after an oral administration of KJST. Total cholesterol, LDL-cholesterol, Glucose and LDH was significantly lower in the KJST treated animals, and HDL was significantly raised in the KJST treated animals. These results suggest that KJST is effective for the treatment of hyperlipidemia.

늑막염의 임상적 고찰 (Clinical Observation of Pleural Effusion)

  • 연구배경 : 늑막염 환자는 흔히 보는 호흡기질환으로 결핵성에 의한 늑막염이 제일 많은 것으로 보고되고 있으나 근래에 와서 진단방법의 발전, 결핵이환율의 감소, 인간수명의 연장등으로 다른 원인 질환이 증가하고 있으며 특히 폐암 및 악성종양이 증가하여 이에 따른 암성과 다른 늑막염과의 빠르고 정확한 구별을 하여 신속한 치료를 요하기 위해 늑막염 환자의 제반검사와 임상양상에 대하여 알아보았다. 방법 : 1989년 1월부터 1992년 12월까지 4년간 315명을 대상으로 임상증세와 나이, 성별을 관찰하였고 흉부 X-선 촬영 및 늑막천자, Cope침을 사용한 늑막침생검, 임파선 조직검사, 기관지경검사 그리고 동반된 폐질환 등을 이용하여 원인규명 및 진단을 내렸고 늑막천자후 단백질양, LDH 양, Cholesterol 농도와 이들의 혈청에 대한 늑막액의 비를 측정하여 여출액과 삼출액을 비교하고 그 오류빈도를 비교하였다. 결과 : 315예중 삼출액이 288예(91.4%), 여출액이 27예(8.6%)였으며 삼출액중에서 결핵성이 82.3%, 암성이 12.2%, 감염성이 5.5%였다. 주소는 호흡곤란(58.7%), 흉통(54.9%), 기침 (50.2%) 순이었고 늑막액의 위치는 우측(51.4%), 좌측(41.3%), 양측(7.3%) 순이며 늑막액의 양은 소량(42.9%), 중등량(39.4%), 다량(17.7%) 순이었다. 비중은 1.030~1.039 범위가 51.4%, 1.020~1.029 범위가 23.5%순이었고 백혈구 수는 삼출액에서 $1000{\sim}5000/mm^3$ 범위가 51.4%, $250{\sim}1000/mm3^$ 범위가 23.3%순이며 백혈구 구분은 70% 이상이 결핵은 89.5%, 암성은 82.9% 였다. 삼출액과 여출액의 구별은 단백질양은 3.0g%, LDH 양은 200단위, Cholesterol 농도는 60mg/dl과 이들의 혈청에 대한 늑막액의 비를 0.5, 0.6 그리고 0.3을 기준으로 하였을때 삼출액의 오류빈도를 비교하면 5.2%, 13.9%, 8.0%와 7.6%, 9.7%, 5.6%를 나타내었다. 늑막침생검상 결핵성이 60.8%, 암성이 13.6%, 감염성이 2.3% 그리고 비특이성 염증이 23.3% 였다. 결론 : 이상의 결과에서 우리나라에서는 결핵성 늑막염이 제일 많으며 여출액 및 삼출액의 감별에 단백질양과 LDH양 및 이들의 혈청에 대한 늑막액의 비와 더불어 Cholesterol 농도와 혈청 Cholesterol에 대한 늑막액의 Cholesterol의 비도 유용한 지표가 될 수 있으며, 늑막침생검을 다른 검사에 우선하여 시행하여야 하며 비특이성 염증시 반복생검은 진단율을 높일 수 있어 정확한 진단 및 신속한 치료에 도움이 될 것으로 생각된다.

사육방법 및 크기에 따른 조피볼락의 혈청 화학성분 변화 (Changes of Serum Chemistry Values on Size and Cultured Method in Korean Rockfish, Sebastes schlegeli)

  • 조피볼락 친어 3개 그룹(서해자연산, 서해가두리양식산, 동해자연산)과 치어 5개 그룹(서해자연산, 서해육산양식산, 서해육상 가두리양식산, 서해 축제식 가두리양식산, 서해축제식양식산)에 대하여 사육방법과 크기의 차이에 따라서 9개의 혈청 화학성분이 어떠한 변화를 나타내는지 알아보고자 하였다. 그 결과, 7개 항목인 총단백, 알부민, 총콜레스테롤, AST, ALT, LDH, ALP의 함량에서 친어 및 치어그룹 간에 유의적인 차이가 관찰되었다. 즉, 총단백, 알부민, 총콜레스테롤, ALP는 친어그룹이 치어그룹에 비해 높은 농도를 보였으며, 반대로 AST, ALT, LDH는 치어그룹이 친어그룹에 비하여 높은 농도를 나타내었다. 이러한 결과는 사육방법과 크기(친어, 치어)가 서로 다른 조피볼락의 생리적인 변화와 관련이 있는 것으로 판단된다.

한국산 치자(梔子) 엑스 및 Geniposide의 약물성(藥物性) 간장해(肝障害)에 대한 보호효과(保護效果) (Protective Effects of Geniposide and Extract of Korean Gardeniae Fructus -On Hepatic Injury Induced by Toxic Drugs in Rats-)

  • This study was attempted to investigate the effect of Gardeniae Fructus on GOT, GPT, Al.p, LDH activities and level of total cholesterol in serum of $CCl_{4}$ and $_{D}-galactosamine$ intoxicated rats, and bile excretion. The geniposide and extract caused a remarkabel decrease of GPT activities, level of total cholesterol in serum of $CCl_{4}$ intoxicated rats at EtOH Ex. 300, 500 mg/kg p.o., MeOH Ex. and geniposide 100 mg/kg p.o., and GOT, Al.p, LDH activities were significantly decreased compared with control group. It caused a remarkable decrese of GPT, Al.p, LDH activities in serum of $_{D}-galactosamine$ intoxicated rats, and GOT activities was significantly decreased compared with control group. The geniposide and extract caused a remarkable increase of bile excretion, when administration of EtOH extract 500 mg/kg p.o., MeOH extract 100 mg/kg i.d., MeOH extract 50 mg and geniposide 50 mg/kg i. v. compared with normal-control group.

영지(靈芝)엑기스가 백서(白鼠)의 실험적(實驗的) 간장중독(肝臟中毒) 및 고지혈증(高脂血症)에 미치는 영향(影響) (Effect of Ganoderma lucidum Extract on Experimentally Induced Hepatic Damage and Hyperlipemic Rats.)

  • This study was attempted to investigate the effect of Ganoderma lucidum (Young-Jii) extract on the activities of GPT GOT Al. P LDH and the level of total bilirubin and total cholesterol in serum of $CCl_4$-intoxicated rats, and on the level of total lipids triglyceride phospholipids and total cholesterol in the serum of experimentally induced hyperlipemic rats, and on the effect of body and liver weight in rats. The results were shown as follows; In $CCl_4$-intoxicated rats, the extract showed a significant decrease in the activities of GPT and Al.P, a slight decrease in the activity of GOT and LDH; The level of total bilirubin was slightly affected, but significantly decreased at a dose as high as 500 mg/kg; the level of total cholesterol was increased dose dependently. In hyperlipemic rats, the extract caused a significant decrease in the level of total lipids and triglyceride and the rate of decrease was more pronounced with repeated treatments for 10 days; the level of phospholipids and total cholesterol were slightly decreased with repeated treatment of the extract at a dose of 300 mg/kg for 10 days; A significant body weight gain was shown with the treatment of the extract.

태충(太衝) 및 양지(陽池)의 전침(電針)이 실험적(實驗的)으로 유발(誘發)된 백서(白鼠)의 간보호효과(肝保護效果)에 미치는 영향(影響) (Hepatoprotective effects of Electro-Acupuncture at Taechung (LR3) and Yangji (TE4) on experimental liver injury in rats)

  • 목 적 : 본 연구는 태충(太衝) 및 양지혈(陽池穴) 에 대한 전침(電鍼)이 galactosamine 을 이용한 백서의 간독성(肝毒性)을 실험적으로 유발시킨 모델에서 예방효과(豫防效果)를 알아보고자 혈청중의 $\gamma\;-GTP$, GOT, GPT, LDH, total bilirubin, total cholesterol, triglyceride 의 변화를 관찰하였다. 방 법 : 간독성은 각 군들은 간독성을 유발하지 않고 무처치한 정상군, 간독성을 유발하고 난 후 무처치한 대조군, 각각 10 Hz, 50 Hz, 100 Hz 전침을 20 일간 10 회 시행한 후 간독성을 유발한 Pre 10, Pre 50, Pre 100군 등으로 분류하였다. 결 과 : 태충(太衝) 양지혈(陽池穴)에 대한 전침(電鍼) 치료(治療)의 예방효과(豫防效果)에서는 Pre 10 군에서는 ${\gamma}\;-GTP$, GOT, GPT, total cholesterol, triglyceride이 Pre 50군에서는 ${\gamma}\;-GTP$GOT, GPT, LDH, total bilirubin, total cholesterol이, Pre 100 군에서는 ${\gamma}\;-GTP$, GOT, LDH, total bilirubin, total cholesterol이 대조군에 비해 감소하였다. 결 론 : 위의 결과를 종합해보면 간독성에 대하여 태충(太衝) 양지혈(陽池穴)의 전침(電鍼)을 시행한 모든 군에서 간능과 지절대사에서 간손상에 대한 유의한 예방효과를 나타내었다.

당뇨한약복합처방(Herbal Remedy for Diabetes Mellitus-01, HRDM-01)이 당뇨병성 흰쥐의 간 및 혈중 지질에 미치는 영향 (Effects of Herbal Remedy for Diabetes Mellitus-01 (HRDM-01) on Liver and Serum Lipid Level in Diabetic Rats)

  • Objective:The Herbal Remedy for Diabetes Mellitus-01 (HRDM-01) is composed of 11 species of medicinal plants. HRDM-01 is used as anti-hyperglycaemic agent. Anti-hyperglycaemic agents in western medicine often have hepatotoxicities. Therefore, we investigated safety of HRDM-01, especially in liver functions. Methods:We investigated the effects of HRDM-01 on liver function measuring serum AST, ALT and LDH levels and histopathological changes of liver tissue using photomicroscope. In addition, we also investigated the effects on serum lipid levels such as total cholesterol, HDL-cholesterol and triglyceride. Results:In our experiment, single injection of streptozotocin elevated levels of AST and ALT in serum. But LDH level was not affected. In addition, our animal model showed elevated levels of total cholesterol and triglyceride in serum. 30 days treatment with HRDM-01 lowered serum AST level. Serum levels of ALT and LDH did not affected. In addition, HRDM-01 lowered serum triglyceride level significantly. In histopathological observation, we did not find any abnormal changes in all experimental groups. Conclusions:Briefly, HRDM-01 did not elevate serum levels of ALT, LDH, total cholesterol and did not affect histopathological changes in liver tissues. Moreover, HRDM-01 lowered serum AST, triglyceride, which are elevated by induction of diabetes mellitus. These results suggest the safety of HRDM-01 in diabetic treatment.

  • Objective : This study was undertaken to determine if Plantaginis Semen Aquacupuncture(PSA) has a protective effect against $CCl_4$ induced Hepatoxicity in Rats. Methods : The experimental group were divided into Normal group(untreated group), Control group, PSA group. Rats were administered orally $CCl_4$(0.1 ml/kg) for 4 days. In experiments for PSA effect, rats received 0.1 ml of PSA extraction in both sides of corresponding $G{\bar{a}}nsh{\bar{u}}$(BL18) of human body for 3 days after treated $CCl_4$. Variation of weight and biochemical assays(GOT, GPT, LDH, ALP, total cholesterol, triglyceride, albumin) were performed. Results : In Control group, $CCl_4$ increased serum GOT, GPT, LDH, ALP and albumin and decreased weight, total cholesterol and triglyceride. PSA significantly decreased serum GOT, GPT, LDH and increased total cholesterol as compared with Control group. Conclusion : These results indicate that PSA could be used in prevention and treatment of hepatoxicity. However, precise mechanisms of PSA protection remain to be determined.

제조 엑기스의 마우스 급성독성(急性毒性) 및 흰쥐의 사염화탄소(四鹽化炭素) 간장장애(肝臟障碍)에 미치는 영향(影響) (The Acute Toxicity of Liocolae vermiculus Extract in Mice and its Effect on Hepatic Damages induced by $CCl_4$ in Rats)

  • This study was attempted to investigate the acute toxicity of Liocolae vermiculus(Liocola brevitarsis) extract in mice, the effect on GOT, GPT Al.p, LDH activities and level of total cholesterol in serum of $CCl_4-intoxicated$ rats. In acute toxicity test, Liocolae vermiculus extract showed 10% mortality at 2,000 mg/kg, p.o. and at 1,000 mg/kg, i.p.. The Liocolae verculus extract caused a remarkable decrease in serum transaminase as well as Al.p activities in $CCl_4-intoxicated$ rats at $300{\sim}1,000{\;}mg/kg$ dosage ranges. The activities of -LDH and the level of total cholesterol were significantly decreased in all sample-treated group, when compared with the control group. The body weight decreased, and the liver and spleen weight increased in $CCl_4-intoxicated$ rats were significantly recovered by the administration of the extracts.
