CEA, AFP, CA125, CA153 and CA199 in Malignant Pleural Effusions Predict the Cause

  • Determination of the cause of malignant pleural effusions is important for treatment and management, especially in cases of unknown primaries. There are limited biomarkers available for prediction of the cause of malignant pleural effusion in clinical practice. Hence, we evaluated pleural levels of five tumor biomarkers (CEA, AFP, CA125, CA153 and CA199) in predicting the cause of malignant pleural effusion in a retrospective study. Kruskal-Wallis or Mann-Whitney U tests were carried out to compare levels of tumor markers in pleural effusion among different forms of neoplasia - lung squamous cell carcinoma, adenocarcinoma, or small cell carcinoma, mesothelioma, breast cancer, lymphoma/leukemia and miscellaneous. Receiver operator characteristic analysis was performed to evaluate sensitivity and specificity of biomarkers. The Kruskal-Wallis test showed significant differences in levels of pleural effusion CEA (P<0.01), AFP (P<0.01), CA153 (P<0.01) and CA199 (P<0.01), but not CA125 (P>0.05), among the seven groups. Receiver operator characteristic analysis showed that, compared with other four tumor markers, CA153 was the best biomarker in diagnosing malignant pleural effusions of lung adenocarcinoma (area under curve (AUC): 0.838 (95%confidence interval: 0.787, 0.888); cut-off value: 10.2U/ml; sensitivity: 73.2% (64.4-80.8)%, specificity: 85.2% (77.8-90.8)%), lung squamous cell carcinoma (AUC: 0.716 (0.652, 0.780); cut-off value: 14.2U/ml; sensitivity: 57.6% (50.7-64.3)%, specificity: 91.2% (76.3-98.0)%), and small-cell lung cancer (AUC: 0.812 (0.740, 0.884); cut-off value: 9.7U/ml; sensitivity: 61.5% (55.0-67.8)%, specificity: 94.1% (71.2-99.0)%); CEA was the best biomarker in diagnosing MPEs of mesothelioma (AUC: 0.726 (0.593, 0.858); cut-off value: 1.43ng/ml; sensitivity: 83.7% (78.3-88.2)%, specificity: 61.1% (35.8-82.6)%) and lymphoma/leukemia (AUC: 0.923 (0.872, 0.974); cut-off value: 1.71ng/ml; sensitivity: 82.8% (77.4-87.3)%, specificity: 92.3% (63.9-98.7)%). Thus CA153 and CEA appear to be good biomarkers in diagnosing different causes of malignant pleural effusion. Our findings implied that the two tumor markers may improve the diagnosis and treatment for effusions of unknown primaries.

Stimulation of bone formation in the expanding inter-premaxillary suture by vitamin E, in rat (백서 전상악골 봉합부 확장시 비타민 E에 의한 골형성 촉진)

  • Objective: The aim of this study was to evaluate the effects of vitamin E ($\alpha$-tocopherol) administration on bone formation in response to expansion of the inter-premaxillary suture, in rats, histomorphometrically. Methods: Thirty 50 - 60 day old Wistar rats were separated into five equal groups (one control and four experimental). All groups were subjected to inter-premaxilla expansion with 50-gram of force. Six control animals received saline solution (Group I) and three experimental groups were treated with a single dose of $\alpha$-tocopherol injected into the inter-premaxillary suture after one day after appliance placement (Group II: 2 mg/kg; Group III: 10 mg/kg; and Group IV: 50 mg/kg). A further group of six animals received three injections of 10 mg/kg $\alpha$-tocopherol, one each on days 3, 6, and 9 (Group V). Bone formation in the suture was evaluated by bone histomorphometry. Kruskal-Wallis rank and Mann-Whitney U tests were used for statistical evaluation at p < 0.05 level. Results: New bone area, bone perimeter, feret's diameter and newly formed bone measurements were significantly higher in the experimental groups than the control (p < 0.001). Bone architecture in $\alpha$-tocopherol administrated groups was improved, and bone formation during the expansion period was stimulated significantly, in a dose-dependent manner. Conclusions: The application of $\alpha$-tocopherol during the early stages to orthopedically expanded inter-premaxillary suture areas may stimulate bone formation and shorten the retention period, in rats.

Influence of Some Commercially Available Mouthwashes on Teeth (일부 시판 구강양치액이 치아에 미치는 영향)

  • The purpose of this study was to investigate the chemical properties of some commercially available mouthwashes and to ascertain whether the mouthwashes accelerated mineral loss in dental enamel. Five commercially available mouthwashes were selected from the three largest malls in Korea: Perio Total 7 Aqua Cool Mint Strong $Fresh^{TM}$ (PS; LG Household & Health Care Ltd.), Garglin $Original^{TM}$ (Dong-A Pharmaceutical Co., Ltd.), Garglin $Zero^{TM}$ (Dong-A Pharmaceutical Co., Ltd.), Listerine Naturals $Citrus^{TM}$ (LC; IDS Manufacturing Ltd.), and Listerine Cool $Mint^{TM}$ (LM; IDS Manufacturing Ltd.). The composition, pH, and titratable acidity of the mouthwashes were investigated. Six bovine teeth specimens were prepared for each mouthwash group. Each of the six specimens was individually immersed in 30 ml aliquots of mouthwash for 1 minute, 30 minutes, 90 minutes, and 120 minutes, and the samples were placed in a $36.5^{\circ}C$ stirred incubator. The degree of mineral loss (${\Delta}F$) of the tooth surface area exposed to mouthwash, compared with normal teeth, was analyzed by quantitative light-induced fluorescence-digital. The difference in ${\Delta}F$ among mouthwash groups was examined by the Kruskal-Wallis H test (${\alpha}=0.05$). The contents of mouthwashes differed between Listerine and other products, and the pH ranged from 4.09 to 6.75. The titratable acidity of PS was the lowest at 0.63 ml and highest at 9.25 ml for LM. Minor mineral loss was observed when dental specimens were immersed in the Listerine products (LC and LM) for more than 90 minutes, but the degree of mineral loss for Listerine products was not statistically significantly different from that for groups without mineral loss. In conclusion, all five commercially available mouthwashes showed no harmful effects on tooth enamel.

An Analytical Study on Differences between the Types of Disputes in Food Service Franchises and the Relevant Corporate Information (외식 프랜차이징 분쟁 유형과 기업정보 간의 차이분석 연구)

  • This study aimed to analyze the differences between the types of disputes in food service franchises and the relevant corporate information. For this aim, the types of disputes were analyzed on the basis of the cases reported by the Korea Fair Trade Commission, and whether there are any differences from the relevant corporate information was analyzed using SPSS WIN(V.17.0). According to analysis results, the most often seen types of disputes were failure to provide the information disclosure sheet (58.5%) and failure to deposit franchising fee (15.1%). In Kruskal-Wallis test between the type of disputes and related corporate information variables, significant difference was found under significance level p<.05 for sales volume, the establishment period and the number of franchisees, and under p<.1 for the number of full-time employees. In correlation coefficient, the establishment period, the number of full-time employees and the number of franchisees showed positive correlation(p<.05), and the number of full-time employees showed correlation in the number of franchisees and the number of brands under the significance level p<.05. As a solution for the dispute, there should be franchisors' responsibility to comply with the principle of good faith, the authorities' strengthening exemplary transaction criteria and legal systems, as well as experts' consulting for prospective franchisees prior to establishing business.

Comparison of apical transportation and change of working length in K3, NRT AND PROFILE rotary instruments using transparent resin block (Transparent resin block을 이용한 K3, NRT, PROFILE의 apical transportation 및 working length 변화양상의 비교)

  • Objectives: The purpose of this study is to compare the apical transportation and working length change in curved root canals created in resin blocks, using 3 geometrically different types of Ni-Ti files, K3, NRT, and Profile. Materials and Methods: The curvature of 30 resin blocks was measured by Schneider technique and each groups of Ni-Ti files were allocated with 10 resin blocks at random. The canals were shaped with Ni-Ti files by Crown-down technique. It was analyzed by Double radiograph superimposition method (Backman CA 1992), and for the accuracy and consistency, specially designed jig, digital X-ray, and CAD/CAM software for measurement of apical transportation were used. The amount of apical transportation was measured at 0, 1, 3, 5 mm from 'apical foramen - 0.5 mm' area, and the alteration of the working length before and after canal shaping was also measured. For statistics, Kruskal-Wallis One Way Analysis was used. Results: There was no significant difference between the groups in the amount of working length change and apical transportation at 0, 1, and 3 mm area (p = 0.027), however, the amount of apical transportation at 5 mm area showed significant difference between K3 and Profile system (p = 0.924). Conclusions: As a result of this study, the 3 geometrically different Ni-Ti files showed no significant difference in apical transportation and working length change and maintained the original root canal shape.

Effects of thickness and background on the masking ability of high-trasnlucent zirconias (고투명도 지르코니아의 두께 및 하부 배경에 따른 색조 차단 효과)

  • Purpose: The purpose of this study was to compare and evaluate the masking ability of three types of high translucent zirconia according to the various thicknesses and backgrounds. Materials and Methods: Using three types of high-translucency zirconia (Ceramill zolid fx white, Ceramill zolid ht+ white, Ceramill zolid ht+ preshade A2), 10 cylindrical specimens were fabricated in 10mm diameter and each with four thicknesses (0.6 mm, 1.0 mm, 1.5 mm, 2.0 mm), respectively by CAD/CAM method. The background was 10 mm in diameter and 10 mm in thickness. A1, A2, A3 flowable resin backgrounds, blue-colored core resin background, and Ni-Cr alloy background were prepared, and black, white backgrounds provided by the spectrophotometer manufacturer (x-rite, Koblach, Austria) were used. zirconia specimens and the background specimen were stacked to measure L, a*, b* with Spectrophotometer (Color i5, x-rite, Koblach, Austria) and the ΔE value with the other background is calculated. The Calculated mean ΔE values were compared based on perceptibility threshold 1.0 and acceptability threshold 3.7. Nonparametric tests such as Kruskal-Wallis test were performed to verify statistical significance (α = 0.05). Results: There was a significant difference in the mean ΔE value according to the zirconia type, background and thickness change (P = 0.000). Conclusion: According to the results of this study, the pre-colored high-translucent zirconia can obtain the desired zirconia shade when it is restored on teeth, composite resins, and abutments except for the blue resin core.

SURFACE ROUGHNESS AND MICROLEAKAGE OF CLASS V COMPOSITE RESTORATIONS : EFFECT OF SURFACE SEALING (치경부 5급 와동 수복물의 표면 거칠기와 미세누출에 관한 연구 : 표면 전색의 효과)

  • The purpose of this study was to compare the effect of surface sealing materials on microleakage and surface roughness in Class V composite restorations. Twenty five standardized Class V cavity preparations were made on the facial surface of human premolars and were randomly assigned to 5 groups. The teeth were restored with Z-250 after applying Single Bond. Following 7 days storage in distilled water at $37^{\circ}C$, the restorations were sealed as following systems : No sealing ; Single Bond Adhesive ; Biscover ; Fortify ; Optiguard. Then, toothbrush abrasion test was conducted using a wear testing machine. Surface roughness was measured by means of profilometer before and after toothbrushing and the results were statistically analysed by using a paired t-test and ANOVA. The bonded interfaces and the changes of surface roughness were examined by SEM. For microleakage test, specimens were stained in a $2\%$ methylene blue solution, then longitudinally sectioned and analyzed for leakage at occlusal and cervical interfaces using stereomicroscope. The results were statistically analysed by using a Kruskal-Wallis and Mann-Whitney U test. Surface roughness was increasing in all groups after toothbrushing, but no statistically significant differences. In SEM observation, surface sealant was partially retained and partially detached in bonded interfaces. Especially, microgap was identified in cervical margins. In microleakage test, there was better seal in the enamel region and a significant difference between groups at occlusal margin. Control group and Single Bond group had significantly better marginal seal at enamel margin than cervical margin.

A Derivation of Rainfall Intensity-Duration-Frequency Relationship for the Design of Urban Drainage System in Korea (우리나라 도시배수시스템 설계를 위한 확률강우강도식의 유도)

  • This study is to derive the rainfall intensity formula based on the representative probability distribution in Korea. The 11 probability distributions which has been widely used in hydrologic frequency analysis are applied to the annual maximum rainfall. The parameters of each probability distribution are estimated by method of moments, maximum likelihood method and method of probability weighted moments. Four tests such as $x^2$-test, Kolmogorv-Smirnov test, difference test and modified difference test are used to determine the goodness of fit of the distributions. The homogeneous tests (Mann-Whitney U test, Kruskal-Wallis one-way analysis of variance of nonparametric test) are applied to find the stations with rainfall homogeneity. The results of homogeneous tests show that there is no representative appropriate distribution for the whole duration in Korea. The whole region could be divided into five zones for 12-durations. The representative probability distribution of each divided zone for 12-durations was determined. The GEV distribution for I,II,V zones and the 3-parameter Weibull distribution for III,IV zones were determined as the representative probability distribution. The rainfall were obtained from representative probability distribution for the selected return periods. Rainfall intensity formula was determined by linearization technique for the rainfall.

Comparison of Desiccation Methods after Hand Washing for Removing Bacteria (손 씻기 후 손 건조방법에 따른 세균 제거 효과 비교)

  • Purpose: The purpose of this study was to analyze effects of hand drying methods after hand-washing. Hand drying methods consisted of spontaneous evaporation, paper towel, hand dryer with rubbing, and hand dryer without rubbing. Methods: The research design was a crossover design. The subjects were eighteen university students and randomly assigned in four group. Each group had the schedule of treatment. The schedule tested one drying method per a day and proceeded for 4 days. The data collection was performed from July 27 to July 30, 2010. Data were analyzed by Kruskal-Wallis, Mann-Whitney and Wilcoxon test. Results: There was significant difference between the after CFU between natural dry, paper towel, air dryer with hand rubbing, and air dryer without hand rubbing(p=.006). The after CFUs of paper towel, natural dry and air dryer without hand rubbing groups were statistically less than air dryer with hand rubbing group. Conclusion: It was shown that the paper towel, spontaneous evaporation and without rubbing hand dryer are effective hand drying interventions on reducing CFU on hand. However additional researches with large sample and strict methodology are needed.

Cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider, and the One G glide path instruments in double-curvature canals

  • Objectives: The aim of this study was to compare the cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider and One G glide path instruments in artificial doublecurvature canals. Materials and Methods: This study included 15 WaveOne Gold Glider (size 15/0.08), 15 ProGlider (size 16/0.08), and 15 One G (size 16/0.06) nickel titanium files. The files were used in accordance with the manufacturer's instructions until they were broken in artificial double-curvature canals made of stainless steel. The time to fracture was recorded via a digital stopwatch and the number of rotations until fracture was also calculated. The data were statistically analyzed via the Kruskal-Wallis test. Results: The highest average number of rotations until fracture of the files was found for the WaveOne Gold Glider, followed by ProGlider and One G in order. Statistically significant differences were present between all groups of files (p < 0.05). Conclusions: In our study, the resistance of the WaveOne Gold Glider nickel-titanium (Ni-Ti) file to cyclic fatigue in S-shaped curved canals was found to be higher than that of the ProGlider and One G Ni-Ti files.
