Relationship Between Serum Concentrations of Organochlorine Pesticides and Metabolic Syndrome Among Non-Diabetic Adults

  • Objectives: This study was performed to investigate if organochlorine pesticides (OCPs) were associated with metabolic syndrome and insulin resistance among non-diabetes. Methods: Among subjects who participated in a community-based health survey, 50 non-diabetic subjects with metabolic syndrome and 50 normal controls were selected. Insulin resistance was measured by the homeostasis model assessment (HOMA-IR). Eight OCPs were selected. Results: After adjusting for confounders except for body mass index (BMI), beta-hexachlorocyclohexane ($\beta$-HCH) and heptachlor epoxide were positively associated with metabolic syndrome. Odds ratios across tertiles of $\beta$-HCH and heptachlor epoxide were 1.0, 3.2 and 4.4, and 1.0, 4.0 and 6.0, respectively (p for trend = 0.01 and <0.01). After additional adjustment for body mass index (BMI), heptachlor epoxide still showed an increasing trend with adjusted odds ratios of 1.0, 4.1, and 4.6 (p for trend = 0.10). When the five components of metabolic syndrome (with the definition of high fasting glucose ($\geq$ 100 mg/dL) were separately analyzed, all components were positively, but not significantly, associated with heptachlor epoxide. As the serum concentration of heptachlor epoxide increased, HOMA-IR increased significantly in subjects with metabolic syndrome even after adjusting for BMI (p value <0.05 and <0.01). Conclusions: Despite the small sample size, this study suggests that the background exposure to some OCPs may be associated with metabolic syndrome.

A Case Report of Giant Cell Tumor of the Occipital Bone (후두골에 발생한 거대세포종 (giant cell tumor)의 수술적 치험례)

  • Purpose: Giant cell tumors of the bone are rare, locally aggressive lesions that primarily affect the epiphysis of the long bones in young adults. These tumors occur very rarely on the skull, principally in the sphenoid and temporal bones. The occipital bone is an unusual site. We report a rare case of a giant cell tumor of the occipital bone with a review of the relevant literature. Methods: A 7-year-old boy presented with a mass of the right occipital area, which was accompanied by localized tenderness and mild swelling. The mass was first recognized approximately 1 year earlier and grew slowly. There was no significant history of trauma. The physical examination revealed a nonmobile and non-tender bony swelling on the occipital region. The neurological evaluation was normal. The serial skull radiography and CT scan showed focal osteolytic bone destruction with a bulged soft tissue mass in the right occipital bone. The patient underwent a suboccipital craniectomy and a complete resection of the epidural mass. The lesion was firm and cystic. The mass adhered firmly to the dura mater. Results: The postoperative clinical course was uneventful, and the patient was discharged 5 days later. The histopathology report revealed scattered multinucleated giant cells and mononuclear stromal cells at the tumor section, and the giant cells were distributed evenly in the specimen, indicating a giant cell tumor. Conclusion: Giant cell tumors are generally benign, locally aggressive lesions. In our case, the lesion was resected completely but a persistent long term follow up will be needed because of the high recurrence rate and the possible transformation to a malignancy.

Association of Metabolic Syndrome with Exercise Capacity and Heart Rate Recovery after Treadmill Exercise Test

  • Heart rate recovery (HRR) immediately after a treadmill exercise test is a function of vagal reactivation. A delayed heart rate recovery is associated with an increased risk for overall cardiovascular mortality. The purpose of this study is to find out if metabolic syndrome is associated with autonomic nerve function and exercise capacity in healthy adults. We measured the treadmill exercise capacity (METs) and heart rate recovery in 119 subjects through a medical checkup at $J$ General Hospital. The metabolic syndrome was defined according to the National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATP III) criteria. The value for the HRR was defined as the difference between the heart rate obtained during the peak exercise and the heart rate obtained at first minute during the recovery period. The subjects with the metabolic syndrome had significantly lower exercise duration ($9.6{\pm}1.5$ vs $8.7{\pm}1.4$), METs ($11.6{\pm}1.7$ vs $10.4{\pm}2.5$), and HRR ($37.5{\pm}14.3$ vs $27.1{\pm}8.9$). The waist circumference in subjects with the metabolic syndrome was more strongly correlated with HRR ($r$=-.517, $P$ <.001) than in normal subjects. Furthermore, delayed HRR was associated with high resting heart rate and increased waist circumference ($P$=.032, $P$ <.001, respectively). In conclusion, delayed HRR during the first minute after a treadmill exercise test was associated with the metabolic syndrome risk factors. Delayed HRR was also associated with high resting heart rate and increased waist circumference.

  • Objectives: The current study was performed to assess the distribution of intraocular pressure (IOP) and its association with metabolic syndrome (MS) in a community. Methods: We measured IOP and MS components from 446 adults, age 20 or more years old, who reside in a community in Kyunggi Province, South Korea. We compared the level of IOP according to the number of metabolic abnormalities and between normal and abnormal metabolic components. Linear regression analyses were used to determine the relationship between IOP and metabolic components. Results: No significant difference in IOP (mean${\pm}$SE) was found between men ($12.24{\pm}2.42$) and women ($12.55{\pm}2.41$ mmHg, p > 0.1), while IOP of men tended to decrease as age increased (p for trend < 0.01). After adjusting for age, IOP of subjects with abdominal obesity in men and high blood pressure in women were significantly higher than those without abdominal obesity or high blood pressure (p < 0.05). Female subjects with MS showed significantly higher IOP than those without MS. Participants with more metabolic disturbances tended to have a greater IOP elevation with a linear trend after adjusting for age and sex. In the univariate regression analysis, age and waist circumference were significantly associated with IOP in men, but systolic and diastolic blood pressure were associated with IOP in women. In final multiple regression model, age, systolic blood pressure, and triglyceride were associated with IOP in women, and age in men. Conclusions: These findings suggest that MS and its components may be important determinants of elevated IOP.

In Vitro Cultivation of Cymatocarpus solearis(Brachycoeliidae) Metacercariae to Obtain the Adult Stage without the Marine Turtle Definitive Host

  • In vitro cultivation of trematodes would assist studies on the basic biology of the parasites and their hosts. This is the first study to use the yolk of unfertilized chicken eggs as a simple and successful method of ovocultivation and the first time to obtain the adult-stage of the trematode Cymatocarpus solearis Braun, 1899 (Digenea: Brachycoeliidae). Chicken eggs were inoculated with metacercariae from the muscle of the spiny lobster, Panulirus argus (Latreille, 1804). The metacercariae were excysted and incubated for 576 hr (24 days) at $38^{\circ}C$ to obtain the adult stage. Eggs in utero were normal in shape and light brown color. The metacercariae developed into mature parasites that have been identified as the adultstage found in marine turtles. The adult lobsters collected in Quintana Roo State, Mexico, showed the prevalence of 49.4% and the mean intensity of 26.0 per host (n=87). A statistical study was performed to determine that no parasitic preference was detected for male versus female parasitized lobsters. Morphometric measurements of the adult-stage of C. solearis obtained in our study have been deposited in the National Helminths Collection of the Institute of Biology of the National Autonomous University of Mexico. This study is significant because it is the first time that a digenean of the family Brachycoeliidae has been demonstrated to develop in vitro from metacercariae into adults capable of producing eggs using the yolk of unfertilized chicken eggs. Secondly, this technique allows to obtain the adult stage of C. solearis without the presence of its marine turtle host, allows us to describe the mature parasites, and thus contribute to our understanding of the biology of C. solearis.

HPV 16 E6/E7 Transgenic Mice have Hyperkeratosis and Elevated ROS Related Enzyme Activities

  • Human papillomavirus type 16(HPV16) has been known to the major factor for the development of uterine cervical carcinomas. We have extended these studies to investigate the in vivo activities of HPV-16 E6/E7 when expressed in squamous epithelia of transgenic mice. Grossly, hK14HPV16E6/E7 transgenic mice had multiple phenotypes, including wrinkled skin that was apparent prior to the appearance of hair on neonates, thickened ears, and loss of hair in adults. In the transgenic mice, the wrinkled skin phenotype on the body and legs died at the age of 3∼4 weeks. Histological analysis of demonstrated that E6/E7 causes epidermal hyperplasia in multiple transgenic lineages with high penetrance. This epithelial hyperplasia was characterized by an expansion of the proliferating compartment and an expansion of the keratinocyte and was associated with hyperkeratosis. These transgenic mice expressed E6/E7 transgene mainly in skin, heart, pancreas and kidney. Hyperplasia was found at the skin. The enzyme activities of GR, GPx and CuZnSOD were measured from the transgene cause keratinocyte at the skin. The specific enzyme activities were significantly higher in transgenic mice skin compared to the normal mice skin. Thus these transgenic mice may be useful for the develpment of antioxidant enzymes or other therapies for HPV-associated hyperkeratosis.

Morphological Specificity in Cultured Starry Flounder Platichthys stellatus Reared in Artificial Facility

  • The starry flounder Platichthys stellatus, like all flatfish, exhibits conspicuous lateral asymmetry in numerous traits, most obvious of which is the migration of one eye to the other side of the head during metamorphosis. Additional changes related to eye migration include asymmetrical pigmentation, and a behavioral shift from larvae that exhibit upright, open-water swimming to juveniles and adults that lie on the ocean floor, eye side up. However, the morphology of these juveniles has been quite plastic in recent years, a phenomenon which is thought to be related to a diverse suite of semi-intensive and intensive larviculture methods. The cause of morphological abnormalities in the farmed flatfish is poorly understood. In the present study, we observe the features of morphological specificity and abnormality of immature fish (mean total length 23 cm) and survey the occurrence frequency of the specificity and abnormality of juvenile (mean total length 6.70 cm) in artificial culture facility. We find 2 types of abnormality (e.g., albino in ocular side and hypermelanosis in blind side) and 1 type of specificity (e.g., lateral polymorphism). These considerably differ from normal individuals (has sinistral eye and pigmented on only one side) by several characteristics (dextral eye, ocular side albinism, blind side hypermelanosis). The incidences of albinism, hypermelanosis, and body reversal are $10.1{\pm}2.56%$, $91.7{\pm}1.7%$, and $13.1{\pm}1.1%$, respectively. These suggest that these morphometric and morphological differences occur more in artificial environment during and just after metamorphosis.

A Study on Adult Learners' Perception on the Development of Life Competencies in Busan and Ulsan (부산·울산지역 성인학습자들의 생애능력개발에 대한 인식 분석)

  • The purpose of this study was to search and analyze the realities and needs of life long education, the necessity and perception of life competencies and the difference of competencies on experiences of education to support life long education system for adult learners in Busan and Ulsan. In this study, we conducted a questionnaire survey of 234 adult learners living in Busan and Ulsan. The results of this study were summarized as follows. First, adult learners perceived that life long education was very important to adults and appreciated an educational institution being managed by government. Second, adult learners thought that basic literacy competency and basic work competency were important and late fifties perceived that professional job is in important. Third, adult learners perceived that self directed learning and capacity of management are poor competencies. And they perceived that the government support adult learners who has economical difficulty. Forth, adult learners perceived that it was normal that 'professional job' and 'job transition abilities', 'women', 'the middle aged and old','the unemployed' and' low incomers' perceived that they have poor level. Fifth, according to the results of ${\chi}^2$ test for determining the statistical significance of differences in experience in lifelong learning among adult learner groups divided by their individual background, significant differences were observed in a few competencies(p<.05, p<.01).

Rectus Femoris Action Potentials under 4 Positions during Straight Leg Raising (하지거상운동시 자세변화에 따른 대퇴직근의 활동전위)

  • The purpose of this study was to investigate which of 4 positions produced the highest action potential in the rectus femoris muscle of normal adult subjects. Testing was performed in supine with the right leg performing a simple straight leg raise with the knee fully extended. The left leg, however, was placed in 4 different positions: 1. Full support with $0^{\circ}$ flexion. 2. Flexed on the plinth with $60^{\circ}$ knee flexion and foot flat. 3. Same as N0.2 but with $90^{\circ}$ knee flexion. 4. Left leg hanging over the end of the plinth with $90^{\circ}$ knee flexion, $0^{\circ}$ hip flexion and no foot support. This study was designed to compare the level of electromyographic activity of the rectus femoris under 4 positions. Fourty-three healthy young adults performed three trials of each exercise condition in random order in the supine position. Electromyographic activity was recorded from surface electrodes. Rectus femoris action potentials in all 4 positions were significantly different. The highest action potential at the end of movement of the right leg occurred with the left leg hanging over the end of the plinth with $90^{\circ}$ knee flexion. It is therefore recommended the straight leg raising be performed with the contralateral leg flexed at $90^{\circ}$ over the end of the supporting surface to obtain a maximum rectus femoris isometric contraction.

Evaluation of proximal contact strength by postural changes

  • STATEMENT OF PROBLEM. Proper proximal contact is important for maintaining and stabilizing the dental arch. However, the proximal contact strength (PCS) is not a constant value and can be affected by a variety of factors. PURPOSE. This study examined the influences of postural changes on the posterior PCS. MATERIAL AND METHODS. Twelve adults with a normal occlusion and had not undergone prosthetic treatment or proximal restoration were participated in this study. A metal strip was inserted into the proximal surface and removed at a constant velocity. The contact strength was measured in every contact point between canine to second molar in both arches. The PCSs were obtained initially in the upright position, secondly in the supine position and finally in the upright position again. All measurements were repeated after a 2 hour period. Statistical analysis was carried out using the Friedman test (P < .05). RESULTS. Generally, a decrease in PCS occurred when the posture was changed from the initial upright to supine position, while it increased when the posture was changed from the supine to upright position. A significant change was observed in all areas except for between the canine-first premolar in the maxilla and between the first molar-second molar in the mandible areas. CONCLUSION. The posterior PCS, which dentists generally believe to be a static feature of occlusion, is affected significantly by posture.
