A Case Report of Erythemato-telangiectatic Rosacea Patient Treated with Yangdokbaekho-tang and Hwangryunhaedok-Tang Wet dressing (양독백호탕(陽毒白虎湯) 처방과 황련해독탕(黃連解毒湯) 습포(濕布)를 병행한 주사의 한방 치험 1례)

  • Objectives : The aim of this study is to report the effects of Yangdokbaekho-tang and Hwangryunhaedok-Tang wet dressing on the treatment of erythrotelangiectatic rosacea patient. Methods : A patient with Erythrotelangiectatic Rosacea treated with Yangdokbaekho-tang, Hwangryunhaedok-Tang wet dressing and acupuncture. We evaluated the score(frequency ${\times}$ intensity) of the hot flush, the main symptom of rosacea and NRS(numeral rating scale) for the subjective symptom. Results : After treatment, the score of the hot flush decrease from 20 to 6. the NRS changed and NRS score improved from 7 to 1. There are no adverse effects and no relapse of erythrotelangiectatic rosacea after 18 days of discharge. Conclusions : According to the result, Korean Medicine such as Yangdokbaekho-tang and Hwangryunhaedok-tang wet dressing can be effective for the treatment of erythrotelangiectatic rosacea.

A Statistical Analysis of Outpatients in Ophthalmology and Otolaryngology of Korean Medicine at Kyung Hee University Korean Medicine Hospital in 2012-2016 (2012-2016년 경희대학교 한방병원 한방안이비인후과 외래환자에 대한 통계적 분석)

  • Objectives : The purpose of this study is to research on the characteristics of outpatients and distributions of diseases in ophthalmology, otolaryngology of Korean medicine in 2012-2016 and compare the result with previous studies. Methods : This study is based on the analysis of medical records of total 2,319 outpatients in ophthalmology, otolaryngology of Korean medicine at Kyung Hee Korean medicine hospital from January 1, 2012 to December 31, 2016. We searched on the demographic characteristics of outpatients and the distributions of diseases in order of frequency by age and sex. Then, we compared these results to previous results of papers at same hospital in 2000 and 2007. Results : The common diseases in 2012-2016 were dry eye syndrome in ophthalmology(32.14%), tinnitus and hearing loss in otology(72.69%), rhinitis in rhinology(82.20%) and laryngopharyngitis in laryngology(67.77%). The most increasing diseases in the distribution between 2004-2006 and 2012-2016 were retinopathy in ophthalmology(+366.77%), etc including the eustachian tube disorder in otology(+151.55%), olfactory disorder in rhinology(+167.50%) and laryngopharyngitis in laryngology(+126.20%). Conclusions : If we accumulate these kinds of epidemiologic studies on ophthalmology, otolaryngology of Korean medicine, it will be great help to make Korean medical preparations for the increasing diseases on these fields.

Case Study of Tinea Corporis Treatment Diagnosed by KOH Test (한의과에서 KOH 검사로 진단한 체부백선 치료 증례)

  • Objectives : The purpose of this study is to report the treatment of the Tinea corporis treated by Korean medicine. Methods : The skin lesion with keratin were scraped and placed on a slide glass, and the KOH solution was dropped and observed under a microscope. After the hyphae was confirmed, a ChoPho-bang was prescribed and applied twice a day. Results : In the KOH test performed again 3 weeks after treatment, no hyphae was found and the skin lesion was completely disappeared. Conclusions : For Tinea Corporis, the KOH test is recommended to distinguish it from other skin diseases, and application of ChoPho-bang is effective for treatment.

The Effect of Needle-Embedding Therapy on Peripheral Facial Paralysis (말초성 안면신경마비에 대한 매선요법 복합치료 효과)

  • Objective : This study was performed to investigate the effect of Needle-Embedding Therapy on peripheral facial paralysis. Method : We investigated 60 cases of patients with peripheral facial paralysis, and devided patients into two groups : We treated one group by complex korean medical treatment with Needle-Embedding therapy, and did the other group by complex korean medical treatment without Needle-Embedding therapy. Yanagihara grading system at baseline and final were used for evaluating the effect of the treatment. Results : 1. In Needle-Embedding therapy group and non Needle-Embedding therapy group, compared with baseline, at final, Y score was significantly increased.2. At final, there was significant difference in improvement between Needle-Embedding therapy group and non Needle-Embedding therapy group. Conclusions : Needle-Embedding therapy seem to be effective to improve symptoms of peripheral facial paralysis. Further studies will be needed to identify the beneficial of Needle-Embedding therapy on peripheral facial paralysis.

  • Objectives : The purpose of this study is to analyze outpatients who visited for treatment for sudden sensorineural hearing loss in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean medical hospital from January 1st, 2013 to December 31th, 2021. Methods : We retrospectively analyzed outpatients who visited for treatment for sudden sensorineural hearing loss in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean medical hospital from January 1st, 2013 to December 31th, 2021. according to year, season, gender, age, clinical symptoms, number of visits and initial degree of hearing loss. The statistical analysis performed using IBM SPSS 25.0 for Windows. Results : The results of analyzing the medical records of 194 patients are as follows. 1. The number of patients showed a gradual increase from 2014 to 2021 and spring was highest in season. Acc ording to gender, women visited more than men and the age group that visited the most was middle-aged, including 40s and 50s. 2. Patients experienced 13 symptoms and the average number was 4.06. In the analysis of the major symptoms according to gender, hyperacusis and headache were found to be more common in female. 3. The average of initial hearing loss was 52.54dB and was higher in male. The loss was relatively higher in 60s or older than in the younger age group. Conclusions : If treatment and management methods that reflect these symptoms are developed, it is thought that it will greatly helpful to improve the quality of life of patients with sudden hearing loss.

Comparative Analysis of Outpatients Visiting Korean Medicine Hospital and Department of Ophthalmology, Otolaryngology & Dermatology of Korean Medicine Pre- versus Post- COVID-19 (COVID-19 전후 단일 한방병원 및 한방 안이비인후피부과 외래 환자들에 대한 비교 분석)

  • Objectives : The objective of this study is to predict how future infectious diseases will affect healthcare utilization patterns within korean medicine hospitals and, specifically, departments of ophthalmology, otolaryngology and dermatology of Korean medicine. Methods : We retrospectively analyzed medical records of patients who visited Kyung Hee University Korean Medicine Hospital during Pre COVID-19(from March 1st, 2018 to Feb 29th, 2019) and Post COVID-19(from March 1st, 2019 to Feb 28th, 2022). The data was analyzed by visit type and diagnosed disease code. Results : 1. After the COVID-19 break-out, the number of first visit patients increased but those of revisit patients, total patients, and revisit/first visit ratio decreased in all departments. 2. In Kyung Hee University Korean Medicine Hospital, the decrease rate of the number of total patients diagnosed with diseases of respiratory system(J00-J99) was the highest, while that for diseases of circulatory system(I00-I99) was the lowest. 3. In the department of Ophthalmology and Otolaryngology of Korean Medicine, the decrease rate of the number of total patients with nose disease was the highest, followed by eye disease and ear disease. 4. In the department of Dermatology of Korean Medicine, the number of total patients with dermatitis remained consistent, while the number of total patients with skin appendage disease showed relatively high decrease rate. Conclusions : Severe or chronic diseases showed relatively low decrease rate of the number of total patients. While mild diseases or diseases with high probability of exposure to COVID-19 infection during treatment showed high decrease rate of the number of total patients.

  • Objectives : The prevalence of Allergic Rhinitis (AR) has increased during the last decade. However, a lot of decisions on the assessment and treatment of AR are made depending on the personal experiences and choices of physicians. To suggest preferable approaches and to minimize the variations according to physicians, guidelines for assessing anterior nasal cavity using nasal endoscopy on AR is needed. Methods : The Korean Oriental Medical Ophthalmology, Otolaryngology and Dermatology Society and the Korean Oriental Preventive Medical Society has set out to develop the guidelines for the assessing anterior nasal cavity using nasal endoscopy on AR. Results & Conclusions : This is the first Korean guideline for the assessment of AR and should be revised and supplemented in the future as new evidence becomes available.

Methods : It was applied to clinical practice and considered the adequacy of the clinical practice program using reflective journal for students who complete the clinical practice. Result : Students are given high marks for self-directed learning and Korean medicine ophththalomology & otolaryngology & dermatology professional learning.

  • Objectives : This study retrospectively analyzed the medical records of patients admitted to Korean medicine ophthalmology, otolaryngology and dermatology department of Daejeon Korean medicine hospital from March 2018 to February 2023 to analyze the characteristics of patients receiving inpatient treatment. Methods : We retrospectively analyzed inpatients who admitted to ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean medicine hospital from March 1st, 2018 to February 28th, 2023, according to gender, age, year, season, detailed subdivision. The statistical analysis performed using IBM SPSS 29.0 for Windows. Results : 1. Examining the gender distribution of the patient group, there were 367 female patients, accounting for 71.7% of the total patients, and 145 male patients, accounting for 28.3% of the total patients. 2. When analyzing inpatients by subdivision, otology accounted for more than half of the total number of inpatients, and the combined number of otology and dermatology accounted for more than 80% of the total. 3. As a result of analyzing inpatients by frequent disease, Sudden hearing loss was a significantly higher number of patients, accounting for 22.7% of all inpatients. Conclusions : It was found that the proportion of patients with otologic diseases was very prominent. It is thought that further research is needed to see if the trend of increasing demand for otologic diseases continues.

Inter- and Intra-rater Reliability of Pattern Identification Using Nasal Endoscopy for Allergic Rhinitis (비내시경을 활용한 알레르기 비염에 대한 한의학적 변증 지표의 관찰자간, 관찰자내 신뢰도 연구)

  • Objectives : We performed a pilot study to investigate inter- and intra-rater reliability of pattern identification using nasal endoscopy for allergic rhinitis(AR). Methods : Eight experts of ophthalmology, otolaryngology and dermatology of Korean medicine evaluated 20 nasal endoscopy photograph cases of AR patients with pattern identification index using nasal endoscopy for AR including the nasal membrane color(pale / hyperemia), nasal membrane humidity(dryness / dampness), rhinorrhea(watery / yellow), and membrane edema (atrophic / edematous) on nasal endoscopy. Results : Intra-rater agreement(%) and Kappa coefficient was generally from 'moderate' to 'good'(% agreement: 73.13-90% / Kappa coefficient: 0.547-0.748). Inter-rater agreement(%) and Kappa coefficient was also from 'moderate' to 'good' (% agreement: 65-85% / Kappa: 0.475-0.778) except 'humidity(dryness / dampness)' item (% agreement: 55.98% / Kappa: 0.340). In findings of subgroup analysis according to affiliation of raters, Inter-rater agreement(%) and Kappa coefficient of raters in same affiliation was higher than inter-rater agreement(%) and Kappa coefficient of raters in different affiliation except 'dryness / dampness' item. Conclusions : It is necessary to improve objectivity and reproducibility of pattern identification using nasal endoscopy for allergic rhinitis(AR) through the development of detail-oriented criteria and enhanced training of clinicians with development of standard operating procedures(SOPs).
