Maternal and Child Factors Associated with Early Detection of Cerebral Palsy (뇌성마비아 조기발견과 관련된 모자인자)

  • To investigate the maternal and child factors associated with early detection of cerebral palsy, 74 mothers of cerebral palsy children who were born since January 1, 1980 and being treated at Taegu Rehabilitation Center for the Handicapped, Rehabilitiation Center of Taegu University, St. Paul Children's House and Pusan Welfare Association of Cerebral Palsy Children were interviewed from February to April 1987. There is no association between age of child when parents noticed the child's abnormality and educational level of father but it tend to be detected earlier when education level of mother is college or above compared with high school or under. There is a trend of earlier detection of child's abnormality although statistically not significant in case father is professional or managerial worker, monthly income of father is over \610,000, child is first-born, age of the parents is 34 years or under, child is a boy, and child has periodic well-baby check-up. The child's abnormality is detected earlier when mothers had 7 prenatal visits or more compared with those who had 6 visits or less (p<0.05). Parents noticed the child's abnormality first in 85.1% of the cases whereas doctors detected it first in 2.7% and this percentage was not different whether the child had periodic well-baby check-up or not. The first physician's diagnosis of the children was cerebral palsy in 36.5% and the rest was normal, need for observation, uncertain, etc. Parents took the child to doctor for diagnosis 2-3 months after they noticed the child's abnormality and after the child was diagnosed as cerebral palsy parents either took no therapeutic measure or brought the child to physiotherapy or acupuncture or gave herb medicine before they started specific rehabilitative therapy. For early detection of the cerebral palsy children, teaching of evaluation method for child development should be reinforced both in medical school and clinical training course and should train the specialist for diagnosis and treatment of crippling conditions. Also, public education is needed for the importance of early detection of crippling conditions and currently available methods for diagnosis and treatment.

Questionnaire-based analysis of growth-promoting attempts among children visiting a university growth clinic (대학병원 성장클리닉을 내원한 아동에서 설문 조사를 통한 키성장 관리 실태분석)

  • Purpose : Growth-promoting attempts are widespread in Korea, but little is known about their prevalence or associated factors. This study was designed to assess the prevalence of growth-promoting attempts among children visiting a university growth clinic. Methods : A questionnaire-based survey was carried out with 823 children (416 boys, 407 girls) who visited the growth clinic at Paik Hospital. Results : The mean age of the subjects was $10.4{\pm}2.6$ yr, and the height z-score was $-1.58{\pm}0.91$. Approximately 33.4% of the children had tried growth promotion. Among the height-gain methods, herbal medicine was the most frequently used (37.8%), followed by health-promoting supplements (37.1%), exercise or machine (3.0%), and growth hormone treatment (2.9 %). The mean age at which the parents began to worry about their children's height was 7.7 yr. The mean age at which they started height-gain methods was 8.9 yr for herbal medicine, 9.1 yr for health-promoting supplements, 9.4 yr for exercise or machine, and 9.9 yr for growth hormone treatment. Motivating factors included advice from relatives or friends (36.0%), advertisements in the Internet or newspaper (28.4%), advice from pharmacist (16.8%), and advice from their medical doctor (5.5%). The degree of satisfaction from the height-gain methods was 29.1% with growth hormone treatment, 6.6% with exercise or machine, 6.4% with herbal medicine, and 2.8% with growth-promoting supplements. Conclusion : Approximately one third of the children reported use of growth-promoting methods, but the satisfaction rate was not high. The benefits of growth-promoting methods should be carefully weighed against their costs and side effects.

  • In this county, the gap between the urban 'haves' and the rural 'have-nots' continues to be an increasing problem. WHO and UNICEF see primary health care(PHC) as the key to achieving an acceptable level of health throughout the world as a community development. PHC is essential health care made accessible to individuals and families in the community by means acceptable to them. It is the first level of contact of individual, the family, and community with the national health system. It includes at least education on health system. It includes at least education on health problems, promotion of food supply, MCH including family planning, immunization against infectious diseases, control of endemic diseases, treatment of common diseases and injuries, promotion of mental health, and provision of essential drugs. However, of the aboves, education concerning of mental health problems and the methods to identify, prevent, and control them is the principal step of establishment. In Korea, the category of PHC worker includes the physician as public doctor and nurse as primary health care practitioner and community health leader as village health worker. PHC workers of the aboves will thus function best if they are appropriately trained to respond to the health needs of the community. However in this country, since the national PHC service project launched in 1980, the government has not developed and performed appropriate and enough education and training activities. In light of above reasons, several categories of health education activities had been planned and performed being aimed at above specific target groups and the main focus was on the village health workers for about one year from July 1991 to July 1992 in Yeoju Kun of Kyonki Province. At the end of the period, evaluation of education input was carried out to measure the improvement of healthful life of people in terms of awareness, attitude, and practice. The totals of 80 village health workers, 13 public health practitioners and 9 public docters took in the course of health education for a few hours at every month and the evaluation works of educational effect were taken. The results the study were as follows. 1) Number of persons who realized the maxim "health care of the people is a duty of the government" increased after the education course, On the other hand, the rate of satisfaction on the effort of government for health promotion of the people decreased. 2) Public doctors and primary health care practitioners(nurses) liked and enjoyed the education schedule as a meeting of peer group. It provided chances of communication with staffs of Korea University Hospital. It was said that lectures covered great deal of knowledge and technic they urgently needed in the field. 3) After finishing the education course, more of village health workers(VHW) thought they adapted themselves to their roles and functions showing increased number of home visit and contact with primary health care practitioners by month. 4) In case of patient refer, VHW preferred primary health care practitioners to public doctors. 5) Capability of VHWs in most of their functions increased dramatically after when the education course finished except tuberculosis control.

Study on the Evaluation of Renal Function According to Set a Partial Region of Interest in 99mTc-DMSA scan of the Pediatric Patient with a Duplicated Ureter (중복요관을 가진 소아환자의 99mTc-DMSA 검사에서 부분적 관심영역 설정에 따른 신기능 평가에 관한 연구)

  • Purpose: A duplicated ureter is congenital renal malformations with ureter in two. Patients with duplicated ureter are in force to $^{99m}Tc-DMSA$ scan at surgery before and after. In existing examination, at produce result after $^{99m}Tc-DMSA$ scan, didn't compare to upper pole and lower pole with malformed kidney and compared to only relative uptake ratio. Therefore, this study will examine about utility of set a partial region of interest and to functional recovery of renal cell through change of upper pole uptake ratio of malformed kidney by setting each partial region of interest in upper pole and lower pole of malformed kidney in $^{99m}Tc-DMSA$ examination in surgery before and after. Materials and Methods: Pediatric patients with malformed kidney of incomplete duplicated ureter, 15 patients were enrolled in this study. Scanning were scan 3 to 4 hours after injection of $^{99m}Tc-DMSA$ 1.5 ~ 1.9 MBq/kg. Region of interest were each set in normal kidney, upper pole and lower pole with malformed kidney. Region of interest were set with same condition and method to images of surgery before and after that radio technologist 1 person, resident of nuclear medicine 1 person and doctor of urology together. Therefore, this study were compared to uptake ratio (A: B: C) that normal kidney (A), lower pole of malformed kidney (B) and upper pole of malformed kidney (C) about uptake ratio changes of malformed kidney in follow-up examination of surgery before and after. Results: When compared to 15 patients, uptake ratios were increased 7 persons and decreased 8 persons. Among increased 7 persons, it were periods of follow-up examination that 2 persons were 14 months, 4 persons were 12 months and 1 person was 8 months after surgery. Among decreased 8 persons, it were periods of follow-up examination that 4 persons were 12 months 3 persons were 6 months and 1 persons were 4 months after surgery. Conclusion: Existing study could not see the exact uptake ratio changes of malformed kidney because using only the overall Left-Right kidney uptake ratios. But a setting partial region of interest was able to see exactly what changes in the uptake of each upper pole and lower pole of malformed kidney. Because recovery of renal parenchymal cells is difficult in an evaluation of short period of time, follow-up examination should be made in long period of time. How to set up partial region of interest be thought that it would be useful.

The Effect of Breathing Meditation Qigong Therapy on the Recovery of Olfactory Disorders and Voice Handicap Index in Parkinson's Disease Patients (호흡명상기공테라피가 파킨슨병 환자의 후각 및 음성 기능장애 개선에 미치는 효과)

  • Objective : The purpose of this study is to determine whether An's breathing meditation qigong therapy (ABMQT) delivers bioenergy to the frontal lobe, prefrontal lobe, the olfactory tract in the mesolimbic pathway, olfactory bulb, CV22, CV21, olfactory area and vocal-related areas in Parkinson's disease (PD) patients to help improve olfactory disorders (anosmia) and vocal functions. Methods : The subjects of this study were 4 patients with idiopathic PD (3 males/1 female, 65.0±NA/68.7±10.2 years old). ABMQT was applied once a week, 120 minutes per session for 12 weeks in a non-invasive and noncontact manner, and the test before and after ABMQT application included olfactory impairment test the Korean version of Sniffin' stick test (KVSS), voice acoustic test, aerodynamic test, vocal handicap index (VHI-30), and auditory perception scale test tools. The results before and after the experiment were analyzed assuming a normal distribution, and a chi-square test was performed using a continuity correction, and the significance level was set to p<0.05. And the medical diagnosis and findings of the examiner (doctor in charge) before and after the experiment were described. Results : KVSS was significant as 0.2±0.5 and 9.0±0.0 before and after the experiment. There was no significant difference between the voice acoustic test FO and Jitter, the vocal aerodynamic test MPT, SP, AE, the vocal disorder index test, and the auditory perception test. However, the medical diagnosis findings of four study subjects showed that olfactory disorders, voice disorders, and laryngeal function were improved before and after the application of ABMQT. Conclusions : The breathing meditation qigong program showed significant effects on improving the olfactory disorders (anosmia) and speech function of each study subject. However, to produce meaningful results, it is thought that experiments involving a larger number of research participants are necessary, and additional blood and FMRI tests are conducted to verify metabolic activities and the olfactory neuron signal transmission system.

A Study on the Current Status of the Curriculum Operation of the Basic Medical Sciences in Nursing Education (간호학교육에서 기초의.과학 교과운영에 대한 연구)

  • The purpose of this study was to investigate the current status of curriculum operation of the basic medical sciences in nursing education at college of nursing, department of nursing and junior college of nursing, ultimately to provide the basic data to improve a curriculum of basic medical science in nursing education. 78 professors who were in charge of basic medical science at 22 colleges of nursing and department of nursing, and 20 junior colleges of nursing responded the questionnaire consisted of 22 question items about the status of objectives, lectures, laboratory practice and characteristics of professors, and mailed to the author. The findings of this study were as follows : 1. The subjects of basic medical science were identified as physiology, anatomy, biochemistry, pathology, microbiology, pharmacology in the most colleges of nursing and junior colleges of nursing. 2 colleges of nursing and department of nursing(9.1%) and 19 junior colleges of nursing(95%) did not open biochemistry, 1 college of nursing and department of nursing(5%) did not open pathology and pharmacology. 2 Junior colleges of nursing(10%) did not open pharmacology, 1 junior college of nursing(5%) did not open pathology, the other 1 junior college of nursing did not open microbiology. 2. Credits of the subjects were ranged from 1 to 4. Lecture hours of one semester of physiology at school of nursing and junior college of nursing was average 103.6 and average 102.67, that of anatomy was average 127.1 and average 98, that of microbiology was average 109.7 and average 86.33, that of biochemistry was average 105, that of pathology was average 91 and average 94, that of pharmacology was average 86 and average 85.75. 3. Most of schools used 1 textbook for lectures, 3 school of nursing and department of nursing recommended references without using textbook, while all 36 junior colleges of nursing used textbooks. 4. 5 among 10 schools of nursing and department of nursing had a laboratory practice in physiology, 4 among 7 schools in anatomy, 4 among 6 schools in biochemistry, 2 among 6 schools in pathology 5 among 6 schools in microbiology. Not all the schools had a laboratory practice in pharmacology. 4 among 9 junior colleges of nursing had a laboratory practice in physiology. 1 among 4 schools in anatomy, 2 among 7 schools in microbiology. Not all the junior colleges of nursing had a laboratory practice in pathology and pharmacology. 11 among 20 colleges of nursing and department of nursing, 4 among 7 junior schools of nursing used a textbook of laboratory practice. 5. All the subjects at school of nursing and department of nursing responded that content of lectures and laboratory practices of basic medical science should be different from that of medical education, 34 junior schools of nursing responded that content of lecture of basic medical science in nursing education should be different from that of medical education. 33 junior schools of nursing responded that content of practice of basic medical science in nursing education should be different from that of medical education. 6. The final degree of 25 professors who were in charge of basic medical science were doctors of. medicine, that of 5 professors were masters of medicine, that of 5 were doctor of pharmacology, that of 2 were a master of pharmacology, that of 1 was physical science. The final degree of 8 professors who were in charge of basic medical science were masters of medicine, 7 doctors of medicine, 4 masters of nursing science, 4 masters of pharmacology, 2 doctors of nursing, 2 doctors of physical science, 2 doctors of pharmacology and 1 master of public health. 9 full professors, 13 associate professors, 11 assist ant professors, 3 full time instructors, and 6 part time instructors were in charge of basic medical science at college of nursing and department of nursing, 20 part time instructors, 8 associate professors, 6 assistant professors, and 2 full professors were in charge of has basic medical science at junior college of nursing. Based on these results, curriculum of basic medical science in nursing education should be reviewed deeply based on nursing model.

  • 이 논문(論文)은 1973년(年) ICSH주최로 열린 panel에서 적혈구파괴(赤血球破壞) 장소(場所)를 결정(決定)하기 위한 생체(生體) 체표계측법(醫表計測法)의 표준화(標準化)에 관(關)한 토론(討論) 결과(結果)를 초록(抄錄)한 것이다. 체표계측(體表計測)은 체외(體外)에서 계측기(計測器)를 이용(利用)하여 각(各) 장기(臟器)에서의 방사표지물질(放射標識物質)의 분포(分布) 및 시간경과(時間經過)에 따른 변화(變化)를 측정(測定)하는 것으로서 $^{51}Cr$를 사용(使用)하여 적혈구수명(赤血球壽命)을 측정(測定)할 때 간(肝), 비(脾), 심장(心臟)의 방사능(放射能)을 계측(計測)한다. 이 방법(方法)은 각(各) 장기(臟器)에서의 적혈구파괴(赤血球破壞)의 정도(程度)를 예측할 수 있다. 특(特)히 용혈성(溶血性) 빈혈환자(貧血患者)에서 비장적출(脾臟摘出) 여부를 결정(決定)하는데 도움이 된다. 이 panel에서는 주(主)로 오차(誤差)의 원인(原因)이 되는 여러가지 요인(要因)에 대(對)하여 토론(討論)하였으며 일반적으로 다음과 같은 것에 의견(意見)의 일치(一致)를 보았다. 즉(卽) 비장(脾臟)의 위치(位置)는 $^{99m}Tc$로 비주사(脾走査)를 실시하여 결정(決定)하는것이 좋고, $^{51}Cr$은 체중(體重) 1kg당 $1.5{\mu}Ci$를 사용하여, 계측기(計測器)는 NaI crystal(직경이 5cm이상, 두께가 3.75cm이상)의 scintillation doctor를 사용하고, 계측(計測)은 $^{51}Cr$로 표지(標識)된 적혈구(赤血球) 주입후(注入後) 15분(分) 이후(以後)에 하고 다음날 계측(計測)한 후(後) 2주(週) 동안에 적어도 6번 계측(計測)한다. Nutrition Survey in a Korean Mountainous Farming Area (산간지(山間地) 농촌주민(農村住民)의 영양실태조사(營養實態調査))

  • The main character of the Korean diet has been found to be low in protein both quantity and quality and high in carbohydrate. The purpose of this survey was to study the amount of salt intake related to the dietary pattern in Korea. The nutrition survey was conducted in a mountainous farming area located in Auhchun-ri, Gaebuk-myon, Changsoo-gun, Chunbuk Province, February 14-19 in 1973 (7 days). The precise weighing method was used in evaluating the kinds of foods and nutrients intake for 24 households during a three day period. The physical examinations were performed by a doctor on 120 persons and a detailed biochemical test on both blood and urine was made on 42 persons over 40 years old. The results obtained are summarized as follows: (1) Average nutrients intake of an adult per day: calorie intake was 2,446 Cal and its components-protein(61.1g) was 10 percent, fat(12.9g) was 5 percent and carbohydrate(521g) contributed 85 percent of the total calories. Other nutrients-calcium (443mg), thiamine(1.09mg), riboflavin (0.90mg), niacin (14.4mg) and vitamin C (63.2mg) were lower than the recommended daily allowance but vitamin A(2,083 I.U.), iron(11mg) and phosphorous(998mg) were slightly higher than that. (2) To evaluate the nutritional deficiences, clinical examinations were conducted. Angular stomatitis was present in 16.7 percent of those examiners. No edema was found. The rate of osteoarthritis, hepatomegaly diseases appeared in 20 percent of the total subjects and the symptoms appeared highest among those Iron 50 to 59 years old. (3) The following chemical components of blood serum were analyzed and found to be within the normal range: glucose, blood urea nitrogen, uric acid, total protein, albumin, globulin, bilirubin, total cholesterol, inorganic phosphate, alkaline phosphatase, sodium, potassium, chloride, calcium and lecithine dehydrase. One case of each of the following were found: hyperglycemia, hypocholesterolemia, renal problem, hypoproteinaemia and diabetes mellitus, and two persons were classified as showing hypoglycemia and hyponaturemia. (4) The sodium content in urine was 199.6 mEq/L, potassium content was 24.6 mEq/L. The sugar, pH and specific gravity in the urine was shown to be normal.

  • The purpose of this study was to compare the different environment among the young children of large and small cities and rural areas in Korea, in order to know how these different environment have an effect on the children's intellectual and emotional development. For this subjects, 2,700 questionares with 51 items were distributed to the infants' mothers in each area to interview and answer. About 1,800 questionaires were gathered from 3 metropolitan, 6 cities and 6 rural areas in (each 2 farming, fishing and mining villages) The results of analyzing these questionaires were like following; 1. The average number of children of each family was 2.5 in large cities, 3.0 in small cities and 3.6 in the rural areas. 2. White about 75% of infants' parents of large cities graduated college education, only 6% of the rural parents did it. Most infants' parents of the rural areas have only graduated the elementary school. 3. About 90% of the rural, small and large cities family have had radios and T.V sets, and 90% of infants watched T.V program for 2 hours a day in average. 4. While about 50% of large cities' young children were not reared by their mother's milk but by milk and other foods, about 95% of rural infants by breasting mills. 5. Young children of large cities were wearing about 5 months earlier than those of the rural. 6. While 20% children of cities were taught in the kindergarten, most children of the rural areas could not be taught in the kindergarten. 7. About 45% young children of the rural areas and cities were understood and taught reading, writing letters and numbers by their parents, brothers and sisters before entering primary school. 8. While 50% young children of large cities have had pianos and were taught music in kindergarten or piano tutor's, most of the rural areas have not had pianos and could not be taught music. 9. Most children's favorite music songs were T.V signals or C.M songs in both the rural and cities. 10. While most children of cities have had lots of children's pictures or fairy tale books. most infants of the rural areas have had nothing or a few. 11. As lots of infants could not find their pleasure resorts of sport tools outside, they used to play in side streets or publicroads with their friends. 12. While most infant's parents in cities wanted to make their infants lawyer or medicine doctor, most parents in rural areas wanted to make their children teacher or technician. 13. About a half of Korean infants have had their own rooms or have lived in it together with their brother or sister. In conclusion, as children of large cities have had more various kinds of cultural circumstances than the rural areas in aspects of cultural institutions' tools and environment of their parents' education, books, toys, pleasure resorts and their own rooms, the intellectual development of the former could be considered to surpass those of the latter. In other words, the average IQ points of urban's young children are 10 point higher than those of the rural areas, which means the better circumstance would affect the infant's intellectual development. Therefore, the government must support to make good circumstances of the children in the rural areas.

An Analysis on the Contents Related to Health in the Three Major Monthly Women's Magazine (3개 주요 월간 여성잡지에 나타난 건강관련 기사 내용분석 (1997년 3월-1998년 2월 중심으로))

  • The purpose of this study was to analyze the contents related to health in the three major monthly women's magazine that was printed from March, 1997 to February, 1998. The unit of analysis was the section of health, living life, and reaing child. The total number of related story of health was 317. The analysis was done according to the target, health management, topics and the informer by using frequency and number. The findings are as follows : 1. The contents were categorized by target people. The number of items for women was 113(35.6%), child(17.4%), family(15.5), husband(4.7%) and others(26.8%). Others were the contents that are impossible to classify. 2. The number of items of contents by health management was the related to health maintenance and promotion 120(47.8%), health care when they were deviated from health 26(10.3%), and those of both attributes 105(41.8%). 3. The number of items of contents by topics was the related to the occurrence, prevention and treatment of various kinds of disease 41(12..9%), diet 37(11.7%), academic information 11.3%, women's disease 10.4%, the effect of foods 6.0%, child's growth & development and child care 5.75, various kinds of therapy for health care 5.7%, sexual life 5.4% and exercise 4.7%. 4. The number of items of contents by informer was medical doctor 215(49%), lay person's case report or report of a struggle against a disease 12.3%, relevant organization of a related story(eg. family therapy research institute, physical therapist's or teacher's) 11.2%, and the contents of existing books or lecture 8.9%. Among those contents, only the one case from nurse or Korean nurses association as informer was founded, which is related to "skinship child care." In conclusion, this finding showed that women's magazine took a role as a important resource providing informations of health to women. it provides a challenge to the health professionals to have concerns on women's needs, and the content, source, and accuracy of the health related information and take part in the process producing information through such as screening and examining so as to give accurate information to women. Then women's magazine can take a role as a major resource for maintaining and promoting women's health. Finally, nurses's, who are professional health care providers, important and active role as informers toward the lay persons, especially for the women who are non-professional care givers at the family unit should be stressed. More active and continual monitoring and analyzing the contents related health in the mass media including magazine and Internet network in detail, and participating in establishing the system of adequate and precise information for women and lay persons from the nursing profession are required absolutely.
