Herbal Medicine for The Treatment of Pityriasis rosea: A Systematic Review and Meta-analysis (장미색 비강진의 한방 탕약 치료에 대한 연구 : 체계적 문헌 고찰)

  • Objectives : Pityriasis rosea is an acute and subacute papular scaly disease, and is a disease that mainly resolves spontaneously. However, since the outbreak of COVID-19, the possibility that the disease may occur as a sequelae of the disease and side effects of vaccination has been announced. Methods : We searched articles related to oriental treatment of pityriasis rosea at organized in domestic and foreign DBs. Articles were searched for RCT. The results were summarized in tables and graphs, and meta-analysis was performed. Also, we assessed the risk or bias. Results : As a result of setting the herbal medicine alone treatment as an intervention for pityriasis rosea and Western medicine alone as the control group, total 9 articles were searched. We analyzed them and got the results. The efficient rate of the treatment group was statistically significantly higher than that of the control group. In addition, adverse events were significantly less in the treatment group than in the control group. Conclusions : Herbal medicine treatment can be more effective than Western therapy(mainly anti-histamines and glycyrrhizin injection) for pityriasis rosea. In addition, herbal medicine treatment can be superior to Western treatment for safety. However, the limitations of this study include the small number of articles, no other evaluation indicators other than the efficient rate, and insufficient data such as pathological examination.

The Effects of Injincheonggan-tang on Two Patients with HBeAg-Negative Chronic Hepatitis B : Case Report

  • Objectives: This case report was executed to confirm the efficacy and safety of Injincheonggan-tang on two patients with HBeAg-negative chronic hepatitis B. Methods: We reviewed the changes of clinical progress and laboratory records of patients with chronic hepatitis B who did not want to take any antiviral or interferon therapy. One patient has visited the department of internal medicine I of Kyung Hee University Korean Medicine Hospital, from October 2010 and the other, from October 2006. Both of them were prescribed with Injincheonggan-tang and laboratory tests were conducted as well. Results and Conclusions: AST, ALT and HBV DNA which are significant indicators of liver function were controlled in the normal range during the herbal treatment period and subjective clinical symptoms were also improved after taking Injincheonggan-tang.

Distinctive clinical features of HPeV-3 infection in 2 neonates with a sepsis-like illness

  • We report a human parechovirus-3 (HPeV-3) infection in 2 neonates who had prolonged fever (>5 days) with palmar-plantar erythema. This distinctive rash was observed 4-5 days after fever onset, just before defervescence. Elevated aspartate aminotransferase, lactate dehydrogenase, and ferritin levels were characteristic laboratory findings in the 2 cases, suggesting tissue damage caused by hypercytokinemia. Case 1 was treated with intravenous immunoglobulin, considering the possibility of severe systemic inflammatory responses. The initial ferritin level was 385 ng/mL (range, 0-400 ng/mL); however, the level increased to 2,581 ng/dL on day 5 after fever onset. Case 2 presented with milder clinical symptoms, and the patient recovered spontaneously. HPeV-3 was detected in cerebrospinal fluid and/or blood samples, but no other causative agents were detected. The findings from our cases, in accordance with recent studies, suggest that clinical features such as palmar-plantar erythema and/or hyperferritinemia might be indicators of HPeV-3 infection in neonates with sepsis-like illness. In clinical practice, where virology testing is not easily accessible, clinical features such as palmar-plantar erythema and/or hyperferritinemia might be helpful to diagnose HPeV-3 infection.

A Study on the Clinical Utilization of Personal Health Records of Stool and Urine in Korean Medicine (대소변 개인건강기록의 임상연계 활용 연구)

  • Objectives : In this study, we analyze the medical significance of feces symptoms so that the daily records of the feces of individuals can be not only used as a measure of individual health monitoring in daily life, but also more actively connected to the medical treatment of the Korean Medicine (KM). Methods : Categories and clinically significant attributes for symptoms of Urination and defecation in the KM ontology DB are determined, and connected to KM related dialectical indicators by experts' common criteria including Viscera and Bowels [臟腑], eight principles [八綱], Qi Blood fluid and humor phlegm-retained fluid static blood [氣血津液痰飮瘀血], six excesses [六淫]. Results : The analysis of the symptoms of feces in the Korea Medicine ontology shows that the symptoms of stool in categories of 'stool stiffness', 'blood swelling', 'discomfort' are highly ranked among the overall clinical symptom categories. In the case of urine symptoms, symptoms corresponding to 'urine color,' 'urine discomfort,' and 'urine volume' are the top rankers among other total clinical symptoms. In the case of stool, the relationship between the symptom of stool and the categories of spleen, stomach, and colon is increased as the weighted symptom is considered. The relationship between the symptom of urine and the categories of the small intestine and the bladder is increased in the same way. Conclusions : This study could help better utilize the personal generated health records of feces in clinical practice of Korean Medicine.

Acupuncture Treatment for Shoulder Impingement Syndrome: A Review of Randomized Controlled Trials

  • To evaluate the effectiveness of acupuncture treatment for shoulder impingement syndrome (SIS) a literature review was conducted of randomized controlled trials (RCTs) where acupuncture was used as an intervention for patients diagnosed with SIS. Relevant clinical studies (N = 181) were retrieved from several databases based on the inclusion/exclusion criteria, and the interventions and results were analyzed. Six RCTs were selected to review based on the inclusion and exclusion criteria. In all 6 studies, the acupuncture treatment group showed significant positive changes in indicators evaluating pain, disability, and quality of life. A significant decrease in the evaluation indicators (Shoulder Pain and Disability Index, numeric rating scale, Visual Analogue Scale, Constant Murley Shoulder Assessment Score, patient's global assessment, and doctor's global assessment) and a significant increase in the questionnaire scores (UCLA, AL-score, EuroQol 5 Dimension Self-Report Questionnaire, and disabilities of the arm, shoulder, and hand) were observed. In addition, 1 study showed similar improvements in pain and quality of life measures in the acupuncture group and corticosteroid injection group. No major side effects were reported. Acupuncture may be an effective and safe treatment for SIS however, further RCTs are required.

  • Objectives: This study analyzed the prescription antibiotics characteristics of Acute respiratory infection outpatients. It provides a basis for establishing the correct evaluation project on appropriate prescribing indicators. Methods: The research data were collected from the National Health Insurance Corporation's 2014 sample cohort for Internal Medicine, Pediatrics, Otorhinolaryngology, Family Medicine and General practitioner clinics classification of diseases codes J00-J06, J20-J22, J40 outpatients. Results: The antibiotic prescription rate on the evaluation project for appropriate prescribing indicators of Health Insurance Review & Assessment Service was 43.54%, whereas in this study it was about 10% higher because the analysis targeted the entire acute respiratory infection diagnosis. Conclusions: There is a need to identify the correct antibiotic prescription by expanding the current assessment standard. Such standard must include acute lower respiratory infections and minor diagnosis because current evaluation projects on appropriate prescribing indicators targets only the major diagnosis of acute upper respiratory infection.

Effects of herbal mixture on blood glucose and lipid metabolism in type 2 diabetic mellitus mouse (천연물 복합제가 제 2형 당뇨 마우스에서 혈당 및 지질대사에 미치는 영향)

  • Objectives: This study aims to investigate the effects of diabetes mellitus care mixture (DCM) on blood glucose and lipid metabolism in type 2 diabetic mellitus mice. DCM consisted of lagerstroemia speciose, allium hookeri, momordica charantia, amaranthus tricolor, and boesenbergia rotunda, which have been proven to have antidiabetic properties. Methods: In this study, we researched the effects of DCM in type 2 diabetic mellitus mice. C57BLKS/J mouse groups had no treatment, db/db mouse randomly assigned to 2 groups, and treated with distilled water and DCM (200 mg/kg/day). Blood glucose levels and body weight were checked every week. After 4 weeks of treatment, liver function indicators (AST, ALT, and LDH) and lipid metabolites (triglyceride, total cholesterol, LDL-cholesterol, HDL-cholesterol) were measured with a biochemistry analyzer. Diabetic factors (insulin, resistin, and leptin) were measured with ELISA. Results: DCM was decreased blood glucose, diabetic factors, liver function indicators, triglyceride, total cholesterol, and LDL-cholesterol significantly. Also, HDL-cholesterol was significantly increased in DCM group. The bodyweight of DCM group decreased but, no significant difference with the control group. DCM may have the potential to improved diabetes mellitus by regulating blood glucose levels and diabetic factors. Also protecting from diabetic complications by adjusting liver function indicators and lipid metabolites. Conclusions: These results suggest that DCM to be used as an oriental medicine for diabetes, the results of clinical trials are needed.

Systematic Review and Meta-Analysis on Herbal Medicine for Generalized Anxiety Disorder: Focusing on Clinical Studies over the Past 5 Years (범불안장애의 한약 치료에 관한 체계적 문헌고찰 및 메타분석: 최근 5년 임상연구를 중심으로)

  • Objectives: This study investigated the effectiveness of herbal medicine for generalized anxiety disorder (GAD) based on recent clinical studies. Methods: Studies were searched through four databases. Clinical research studies on herbal medicine treatment for GAD patients were included. The studies were analyzed according to study design, diagnostic criteria, population, and intervention. A risk of bias assessment was performed to assess the quality of the included randomized controlled trials (RCT). If the intervention applied to the treatment and control groups was the same and two or more studies were reporting the same items as outcome indicators, a meta-analysis was performed. Results: A total of 19 studies, including 12 RCTs were selected. The most common pattern identification was 'Phlegm fire disturbing upward' (痰熱上擾), and the most used herb for therapeutic purposes was 'Rhizome of Poria cocos' (茯苓). Meta-analysis results of three studies showed that there were no significant differences in effectiveness between the herbal medicine intervention and the Western medicine intervention. Meta-analysis results of five studies showed that the Hamilton Anxiety Rating Scale was significantly reduced in the case of herbal medicine intervention compared to Western medicine intervention. Conclusions: The results of our study demonstrated that herbal medicine treatment for GAD is effective in alleviating anxiety symptoms and chief symptoms of GAD. However, this study has several limitations; there was a lack of placebo-controlled RCT and an absence of objective diagnostic criteria in case reports. Therefore, further well-designed clinical studies, conducted based on the results of this study, are recommended.

Development of Cerebral Amyloid Positivity Predicting Models Using Clinical Indicators (임상적 지표를 이용한 대뇌 아밀로이드 단백 축적 여부 예측모델 개발)

  • Objectives Amyloid β positron emission tomography (Aβ PET) is widely used as a diagnostic tool in patients who have symptoms of cognitive impairment, however, this diagnostic examination is too expensive. Thus, predicting the positivity of Aβ PET before patients undergo the examination is essential. We aimed to analyze clinical predictors of patients who underwent Aβ PET retrospectively, and to develop a predicting model of Aβ PET positivity. Methods 468 patients who underwent Aβ PET with cognitive impairment were recruited and their clinical indicators were analyzed retrospectively. We specified the primary outcome as Aβ PET positivity, and included variables such as age, sex, body mass index, diastolic blood pressure, systolic blood pressure, education, dementia family history, Mini Mental Status Examination (MMSE), Clinical Dementia Rating (CDR), Clinical Dementia Rating-Sum of Box (CDR-SB), hypertension (HTN), diabetes mellitus (DM) and presence of apolipoprotein E (ApoE) E4 as potential predictors. We developed three final models of amyloid positivity prediction for total subjects, mild cognitive impairment (MCI) and Alzheimer's disease (AD) dementia using a multivariate stepwise logistic regression analysis. Receiver operating characteristic (ROC) curve analysis was performed and the area under curve (AUC) value was calculated for the ROC curve. Results Aβ PET negative patients were 49.6% (n = 232), and Aβ PET positive patients were 50.4% (n = 236). In the final model of all subjects, older age, female sex, presence of ApoE E4 and lower MMSE are associated with Aβ PET positivity. The AUC value was 0.296. In the final model of MCI subjects (n = 244), older age and presence of ApoE E4 are associated with Aβ PET positivity. The AUC value was 0.725. In the final model of AD subjects (n = 173), lower MMSE scores, the presence of ApoE E4 and history of HTN are associated with Aβ PET positivity. The AUC value was 0.681. Conclusions The cerebral amyloid positivity model, which was based on commonly available clinical indicators, can be useful for prediction of amyloid PET positivity in MCI or AD patients.

Review on Korean Herbal Medicine Complex Prescriptions and Single Herbal Medicines for Osteoporosis - Focused on Domestic Research Published Since 2013 (골다공증의 한방 복합 처방 및 단일 본초에 대한 국내 연구 고찰 - 2013년 이후 발표된 국내 연구를 중심으로)

  • The purpose of this study was to investigate the effect of single herbal medicines and Korean herbal medicine complex prescriptions for osteoporosis in Korea. We targeted studies published in domestic journals from January 1, 2013 to April 1, 2021. For domestic databases, RISS, NDSL, OASIS, DBpia, KISS, KISTI, and Korean Traditional Knowledge Portal were used. Words such as "osteoporosis", "herbal medicine", "herbal combination prescription" and "herbal" were combined and searched. A total of 32 studies were selected. Among them, 8 studies were conducted exclusively for animal studies, 18 studies were conducted on cell experiments, and 6 studies were conducted on both animal and cell experiments. There were 18 studies using a single herb and 14 studies using a combination of Herbal medicine. As a result of the investigation of a single herb, it was found that 補陽藥 was most widely used, and it was found that it was mainly used in the herbal combination prescription, mainly for 補陽藥, 補氣藥, and 補陰藥. Studies in animal experiments revealed significant results mainly in bone mineral density, serological indicators, and morphological indicators of bone tissue. In the study of cell experiments, the effects were found in factors related to the differentiation of osteoclasts and inhibitors of inflammation. Through the results of this study, we are able to confirm the effective single herbal medicines and herbal medicine complex prescriptions for the treatment of osteoporosis, and it is expected that they will contribute to clinical treatment.
