회상법과 빈도법을 이용하여 식이섭취조사를 할 경우 영양소에 따라 다소 차이는 있으나 일반적으로 모든 영양소에서 빈도법이 회상법 보다 개인의 섭취량이 과다하게 평가되는 경향을 보이고 있으며 영양소별 측정 오차가 식이조사 방법에 따라 가장 큰 차이를 보였던 영양소는 carotene이었고 이 영양소는 두 방법에 의해 추정된 값 사이에 통계적으로 유의있는 상관을 보이지 않고 있다. 그러므로 본 연구 결과는 연구 목적이 되는 영양소의 종류에 따라 식이조사 방법으로 히상법과 빈도법을 교차 선택할 수 있는영양소와 그렇지 않은 영양소가 있음을 시사하고 있다. 그러나, 본 연구는 각 방법이 지니는 상대적 타당성(validity)를 측정하는데 필요한 표준가치(reference value)가 없으므로 타당성을 측정할 수 없었고 단지 두 방법간의 차이만을 비교하는데 그치는 제한점이 있었다. 그러므로 사용된 방법의 타당성 측정을 위해서는 표준값을 제공할 수 있는 객관적인 방법인 실측법(precise weights method) 등이 비록 적은 표본 규모라도 진행될 필요성이 있다. 한편, 더 나아가 조사방법의 신뢰도 측정을 위해서는 정확성(precision)을 보기 위해 한 방법을 여러번 반복 조사하여야 방법의 정확성 등이 검토될 수 있겠다. 이러한 기초연구를 위한 많은 투자와 노력이 있어야만 역학조사에서 이용된 식이조사 방법의 신뢰성 확보가 가능함을 본 연구결과는 시사하고 있다.
The discovery of the relationship between nutrients and deficiency diseases during the 100 years from the mid-1800s to the mid-1900s was a breakthrough that led to advances in the study of nutrition. The Recommended Dietary Allowances (RDA) were created as a quantitative standard for avoiding diseases caused by nutrient deficiency. In addition, a reductionism paradigm has become generally accepted among nutrition scholars in health and disease, which focused on the properties of individual nutrients, content in foods, cellular levels, and mechanisms of action. The reductionist paradigm worked very well for the prevention and treatment of malnutrition diseases. However, as the incidence of nutrient deficiencies decreased and that of chronic diseases increased, the nutrition goals have been changed to secure safe and adequate nutrient intake and to reduce chronic disease risks. Accordingly, Dietary Reference Intakes (DRIs), a set of nutrient-based reference values, were designed to replace the RDA. The revised Korean DRIs were published for 40 nutrients in 2020. However, there is still room for improvement in the reference intake levels targeted at reducing the risk of chronic disease. The reductionist approach can no longer be practical because chronic diseases are related to the interactions between multi-components in the foods and multi-targets in the body. Therefore, a second innovative leap is needed following the nutrition development breakthrough made over 100 years ago. To this end, the nutrition paradigm must evolve from reductionism to a holism approach. Cutting-edge scientific technologies, such as metabolomics, transcriptomics, microbiomics, and bioinformatics, should also be acceptable in nutrition science based on the knowledge gained from basic nutrition studies.
Objectives: Recommended Menu (RM) prepared according to the Target Pattern expected to meet the dietary reference intake (DRI) of nutrients. Nutritional adequacy of RM in 'DRI for Koreans 2015' were analyzed to verify whether such expectation was fulfilled. Methods: Dishes in RM are categorized by 5 food groups, and number and types of dishes for main meal and between-meals were analyzed. The energy and 12 nutrients (protein, dietary fiber, Ca, P, Fe, Na, K, vitamin A, thiamin, riboflavin, niacin and vitamin C) contents in 10 RMs were calculated using the food composition table (CD) in 'DRI for Koreans'. Energy, energy contribution ratio, and nutrient contents in 10 RMs for 18 age groups were evaluated based on the 'DRI for Koreans 2015'. Results: Number of dishes per meal were 4.83, and representative table setting consisted of cooked rice + soup (or stew) +3 side dishes. Energy contents of RM were 75~109% of estimated energy requirement (EER). None of the RM met the DRI of all 12 nutrients examined. Calcium was the most insufficient nutrient. Only 1-2 years' RM met the DRI, all the other RMs did not meet the calcium DRI. Dietary fiber and potassium contents were also insufficient in most RM. In adult male's RM, only 1 nutrient, i.e. calcium did not meet the DRI, but in female adult's RM, 5~6 nutrients did not meet the DRI. Energy contribution ratio of carbohydrate, protein, and fat in RM were 59.0~70.4%, 15.7~17.5%, 12.1~23.5%, respectively. And 4 RMs out of 10 exceeded the upper limit of acceptable macro-nutrient distribution range (AMDR) of carbohydrate and 3 RMs out of 10 RM were below the lower limit of fat AMDR. Contribution ratio of nutrients were ${\geq}40%$ by food groups were as follows: grain group in energy and carbohydrate; meat fish egg legume group in fat, protein, and niacin; vegetables group in dietary fiber, vitamin A, and vitamin C; milk dairy products group in calcium. Conclusions: RM prepared according to the Target Pattern did not meet nutritional adequacy as expected. Especially calcium, potassium, and dietary fiber contents are needed to be increased in many RMs. Further, energy content in RM needs to be adjusted for fat sugar food group assigned in Target Pattern and condiment.
The purpose of this study was to examine household food insecurity and the associations of food insecurity with socioeconomic characteristics, food behaviors, health status and nutrient intake among elderly people in a rural area. This study included 191 elderly people (mean age = $72.0{\pm}7.4$) in Jeollabuk-do, Jangsu-gun. House food insecurity was measured using an adapted version of the USDA short form household food insecurity scale, and nutrient intake measured by a 24-hour dietary recall. Only 15.2% of the subjects were in food-insecure households, 84.8% of the elderly households were food-secure. The food-insecure households were significantly lower in the number of family, frequency of shopping, and perceived health status than the food-secure households. In addition, they had serious tooth problems and NSI (Nutrition Screening Index) scores. Food-insecure households were significantly lower in most areas of nutrient intake (energy, protein, fat, Ca, P, Fe, K, Na, Zn, Vit. $B_2$, niacin, Vit. E), and also lower in nutrient intake compared to dietary reference intake (energy, protein, P, Fe, K, Na, Vit. $B_1$, Vit. $B_2$, niacin, Vit. E). These results suggest that the household food insecurity measures used in this study were an important indicator of nutritional well-being among elderly persons in the rural area. Food insecurity should be considered an important issue to public health and food service programs should be expanded for food-insecure households in rural areas.

BACKGROUND/OBJECTIVES: There has been an increased interest in determining calcium magnesium, sodium, and potassium's distinct effects on hypertension over the past decade, yet they simultaneously regulate blood pressure. We aimed at examining the association of dietary calcium, magnesium, sodium, and potassium independently and jointly with hypertension using National Health and Nutrition Examination Survey data from 2007 to 2014. MATERIALS/METHODS: The associations were examined on a large cross-sectional study involving 16684 US adults aged>20 years, using multivariate analyses with logistical models. RESULTS: Sodium and calcium quartiles assessed alone were not associated with hypertension. Potassium was negatively associated with hypertension in the highest quartile, 0.64 (95% confidence interval [CI], 0.48-0.87). When jointly assessed using the high and low cut-off points, low sodium and corresponding high calcium, magnesium, and potassium intake somewhat reduced the odds of hypertension 0.39 (95% CI, 0.20-0.76). The sodium-to-potassium ratio was positively associated with hypertension in the highest quartile1.50 (95% CI, 1.11-2.02). When potassium was adjusted for sodium intake and sodium-to-potassium ratio assessed among women, increased odds of hypertension were reported in the highest quartile as 2.02 (95% CI, 1.18-3.34) and 1.69 (95% CI, 1.12-2.57), respectively. The association of combined minerals on hypertension using dietary goals established that men meeting the reference intakes for calcium and exceeding for magnesium had reduced odds of hypertension 0.51 (95% CI, 0.30-0.89). Women exceeding the recommendations for both calcium and magnesium had the lower reduced odds of 0.30 (95% CI, 0.10-0.69). CONCLUSIONS: Our results suggest that the studied minerals' association on hypertension is stronger when jointly assessed, mostly after gender stratification. As compared to men, women increased their risk of hypertension even with a low sodium intake. Women would also reasonably reduce their risk of developing hypertension by increasing calcium and magnesium intake. In comparison, men would somewhat be protected from developing hypertension with calcium intake meeting the dietary goals and magnesium exceeding the nutritional goals.
A untrition survey of early childhood was undertaken among 102 rural young children, aged 3 to 6, attending three day care centers of Hongcheon-gun, Kangwon province, in July of 1987, to investigate dietary and nutritional status. The results were obtained as follows: Mean values of height and arm circumference ranged from 96 to 97% of the KSRI's standards. However, mean value of weight met 91% of that standard. Mean value of hematocrit was 37.3±3.5%. According to the criterion established by the WHO, 3% of the subjects were proven to be anemic. Mean value of urinary urea nitrogen/creatinine ratio was 13.8±7.6; the higher the age lower the ratio was shown. Intake of energy and nutrients ranged from 63 to 88% of the RDAs. Carbohydrate provided 68% of total energy intake; protein accounted for 14%; fat provided 18%. Energy intake was divided among breakfast, lunch supper and snacks in a percentage ratio of 21:35:26:18. The survey clarified that the day care feeding largely supplemented the inadequate dietary intakes of these young children at home. Family factors, anthropometric measurements, biochemical results were positively correlated with nutrient intakes. Authors with this study can be contributed, as a reference, to develop the community nutrition programs as well as improving the quality of day care feeding.
Objectives: This study evaluated dietary behavior and nutritional status according to the metabolic syndrome status in Korean menopausal women. Methods: The subjects were 1,392 menopausal women aged 50 to 64 who took part in the Korea National Health and Nutrition Examination Survey of 2016 and 2017. Subjects were classified into normal (NOR) group, pre-metabolic syndrome (Pre-MetS) group, and metabolic syndrome (MetS) groups according to the number of metabolic syndrome risk factors present. Results: The overall prevalence of metabolic syndrome was 33.7%. Using the NOR group as a reference, the odds of belonging to the MetS group in Model 1 adjusted for age were higher at 53% (OR = 1.53, 95% CI:1.011-2.307) for 'not used' subjects compared to 'used' subjects of the nutrition labeling system. Using the NOR group as a reference, every 1g increase in the intake of monounsaturated fatty acids (MUFA) and polyunsaturated fatty acids (PUFA) decreased the odds of belonging to the MetS group in Model 1 adjusted for age by 3% (MUFA, OR = 0.97, 95% CI:0.946-0.991; PUFA, OR = 0.97, 95% CI:0.942-0.993). Conclusions: These results suggest that to reduce the number of risk factors of metabolic syndrome in menopausal women, nutritional education should emphasize the adequate intake of riboflavin, unsaturated fatty acids, protein, and calcium, and also encourage the recognition and use of nutritional labeling. Results of this study are expected to be utilized as basic data for the health management of menopausal women.
Objectives : The purpose of this systematic review was to investigate the association between dietary intake of citrus fruits and prostate cancer risk. Methods : Authors searched electronic databases and the reference lists of publications of diet and prostate cancer studies until August 2007. All of the epidemiological studies that obtained individual data on dietary intake of citrus fruits and presented risk estimates of the association between intake of citrus fruits and risk of prostate cancer were identified and included. Using general variance-based methods, study-specific odds ratios (OR)/ relative risk (RR) and associated confidence interval (CI)/ standard error (SE) for highest versus lowest intake of citrus fruits level were extracted from each paper. Results : Eleven articles including six case-control studies, one nested case-control study and four cohort studies, proved eligible. Overall summary OR using random effect model did not show an association in risk of prostate caner with intake of citrus fruits (summary OR=1.03, 95% CI=0.89-1.19) with large heterogeneity across studies that we were unable to explain ($I^2$=67.88%). The summary ORs in case-control studies and cohort studies were 1.10 (95% CI=0.97-1.22) and 1.05 (95% CI=0.96-1.14), respectively. Conclusions : Pooled results from observational studies did not show an association between intake of citrus fruits and the risk of prostate cancer, although results vary substantially across studies.
The purpose of this study was to develop children-friendly education materials based on food replicas for the dietary education of lower grade elementary students. Through a literature review on current dietary problems and dietary education for children, five education themes were chosen: balanced diet, seasonal food, traditional food, noodle culture, and making instant noodles. A total of 304 key food items for food replicas were selected based on the themes: six basic food groups (113 items), balanced meals (37 items), dietary habits (74 items), spring foods (13 items), summer foods (17 items), autumn foods (16 items), winter foods (11 items), Korean traditional foods (52 items), Korean native noodles (18 items), noodles from foreign countries (19 items), and making instant noodles (46 items). Next, a food replica database was developed consisting of name of the food, picture, size, ingredient, recipe, reference, and attached code. Powerpoint slides and teaching plans for all education themes were developed utilizing the food replicas in order to raise interest and understanding of serving size and intake amount. This research provides a basis for the development of food replicas as effective dietary education materials for elementary students and their use in extra-curricular activities, after school programs, community health centers, or food companies.

BACKGROUND/OBJECTIVES: The doubly labeled water (DLW) method is considered the gold standard for the measurement of total energy expenditure (TEE), which serves to estimate energy requirements. This study evaluated the accuracy of predictive dietary reference intake (DRI) equations for determining the estimated energy requirements (EER) of Korean adults by using the DLW as a reference method. SUBJECTS/METHODS: Seventy-one participants (35 men and 36 women) aged between 20 and 49 years were included in the study. The subjects' EER, calculated by using the DRI equation ($EER_{DRI}$), was compared with their TEE measured by the DLW method ($TEE_{DLW}$). RESULTS: The DRI equations for EER underestimated TEE by -36.3 kcal/day (-1.3%) in men and -104.5 kcal/day (-4.9%) in women. The percentages of accurate predictions among subjects were 77.1% in men and 62.9% in women. There was a strong linear correlation between $EER_{DRI}$ and $TEE_{DLW}$ (r = 0.783, P < 0.001 in men and r = 0.810, P < 0.001 in women). CONCLUSIONS: The present study supports the use of DRI prediction equations to determine EER in Korean adults. More studies are needed to confirm our results and to assess the validity of these equations in other population groups, including children, adolescents, and older adults.
