Acupotomy for Carpal Tunnel Syndrome: A Systematic Review and Meta-Analysis (수근관증후군에 대한 도침치료의 효과: 체계적 문헌 고찰과 메타분석)

  • Objectives The objective of this study was to assess the effectiveness of acupotomy for carpal tunnel syndrome. Methods Based on seven domestic and foreign databases. We analyzed the randomized controlled trials using acupotomy for carpal tunnel syndrome. The treatment group was treated with acupotomy and the control group was no restrictions on treatment methods. Results In this study, the effect of acupuncture treatment for carpal tunnel syndrome was investigated. Each study reported that acupotomy could be an effective treatment for carpal tunnel syndrome. However, as a result of meta-analysis of Levine carpal tunnel syndrome questionnaire scores, visual analog scale, and sensory nerve conduction velocity, the results were more clinically significant than those of the control group. Due to the small number of randomized controlled trial studies and the nature of acupotomy treatment, blinding of interventions was impossible, resulting in a high risk of bias. Conclusions In this regard, it is thought that well-planned randomized controlled studies on patients with carpal tunnel syndrome are needed in the future to secure the clinical evidence for acupotomy treatment.

  • Cervical spondylotic myelopathy (CSM) is a severe disease that usually needs surgical treatment. We have treated two CSM patients with Korean traditonal medicine, which includes acupuncture, pharmacopuncture, herbal medicine and manual treatment (Chuna). For objective evaluation, we used Nurick's classification, JOA (Japanses Orthopedic Association) score, grip and release test and hand grasping power measurement. Post-treatment analysis has shown that grip and release test score and grasping power of two patients were both remarkably improved, along with the improvements on both Nurick's classification and JOA score. This study showed that Korean traditional medicine could be effective conservative treatment for CSM.

Effect of East-West Integrative Rehabilitation on Activities of Daily Living and Cognitive Functional Recovery in Stroke Patients: A Retrospective Study (뇌졸중 환자의 일상생활 및 인지기능 회복에 대한 의·한의 협진 재활치료의 효과)

  • Objectives This study was conducted to verify the effectiveness of east-west integrative rehabilitation therapy on activity of daily living and cognitive functional recovery in stroke patients by comparing with integrative rehabilitation therapy group and conventional rehabilitation therapy group in a single institution. Methods The medical records of 106 stroke patients hospitalized in Department of Rehabilitation Medicine, Dongguk University Bundang Oriental Hospital from January 1, 2017 to February 28, 2019 were reviewed. After screening and dividing it into conventional rehabilitation (CR) group and integrative rehabilitation (IR) group, Korean version of Modified Barthel Index (K-MBI), functional independence measure (FIM), clinical dementia rating-sum of boxes (CDR-SB) were statistically analyzed. Results IR group showed significant improvement in K-MBI, FIM, and CDR-SB after treatment (p<0.001) and there was a statistically significant difference in K-MBI and CDR-SB score changes than CR group (p<0.05). And chronic patient of IR group showed significant improvement in K-MBI, FIM, and CDR-SB after treatment (p<0.01) and there was a statistically significant difference in CDR-SB score changes than CR group (p<0.05). In particular, the earlier the treatment initiation time, the more the improvement in function and when the treatment started within 2 years from the onset and patients took acupuncture and pulsed electromagnetic therapy, all scales significantly improved (p<0.001). Conclusions IR showed more improvement on activities of daily life and cognitive functional recovery than CR in this study.

Recent Domestic and International Trends on Non-Surgical Treatment of Lumbar Spinal Stenosis (요추 척추관 협착증의 비수술적 치료에 대한 최근 국내·외 연구 동향)

  • Objectives The present study examined the recent domestic and international trends of non-surgical treatment of lumbar spinal stenosis (LSS). Methods The studies on non-surgical treatment of LSS were investigated via searching Korean web databases and PubMed. As a result, 39 studies were analyzed according to the authors, the types of study, the relationship to surgical treatment and the method of treatment. Results The first authors' occupations were identified as 17 doctors, 12 Korean medicine doctors, six physiotherapists, three chiropractic doctors and one doctor of science. The most common type of study in Korea was the case report and most of the studies were retrospective. Overseas, there were many prospective or experimental studies including randomized controlled trials (RCT). In Korea, non-surgical treatment was performed independently of surgical treatment in most cases. Among non-surgical treatments, the most researched treatment was traditional Korean medicine (TKM), followed by injection therapy, exercise therapy, and physical therapy. Conclusions Through this review, we can determine the effectiveness of TKM and its research direction. TKM should be studied experimentally including RCT and it should be conducted not only on acupuncture, but also on other treatment methods such as acupotomy, chuna, and herbal medicine. It is also necessary to conduct studies on TKM before and after surgery to compare the effects of surgery and TKM together, as well the effects on non-surgical treatments.

  • The purpose of this study is to evaluate the effectiveness of traditional Korean medicine treatment of cervical vertigo. Three patients were diagnosed as cervical vertigo with correlating symptoms of imbalance and dizziness with neck pain. The diagnosis of cervical vertigo is also dependent on excluding other vestibular disorders on the basis of history, examination, and vestibular function tests. They were treated by acupuncture, transcutaneous electrical nerve stimulation therapy and low-intensity pulsed ultrasound at acupoints and sympathetic ganglion chain of their cervical and upper thoracic region. The evaluation of clinical outcome was done by numeric rating scale (NRS), dizziness handicap inventory (DHI) and neck disability index (NDI), EuroQol-five dimensions questionnaire (EQ-5D) index. After the treatment, the value of their NRS, NDI, DHI was significantly decreased and their EQ-5D index was significantly increased. The traditional Korean medicine treatment at cervical and upper thoracic region could be an effective way to treat cervical vertigo.

Retrospective Study on the Characteristics of Patients with Scoliosis at the Korean Medicine Hospital (한방병원에 내원한 척추측만증 환자의 특성에 대한 후향적 연구)

  • Objectives This study was conducted to characterize scoliosis patients visiting Korean medicine hospital and to analyze the demands and factors affecting discomfort. Methods This retrospective study analyzed 33 scoliosis patients who visited Korean medicine hospital from March, 2021 to October, 2021. The data analysis consisted of three factors: (1) demographic characteristics, (2) characteristics of demands on Korean medicine (reasons for choosing Korean medical treatment, preferred treatment methods, most uncomfortable part, treatment priorities) and (3) discomfort factors (treatment experiences, diagnosed age and Cobb's angle). Statistical analyses were performed and a p-value≤0.05 was considered to be statistically significant. Results 43.75% of the patients chose 'effectiveness' for the reason why they preferred Korean medicine treatment. 'Chuna treatment' was the most preferred treatment method. The patients chose 'lower back' for the most uncomfortable part and 'pain' for the highest priority of improvement. The Cobb's angle of included patients was 16.02±7.65° and it is not much differ to average of Cobb's angle in Korean. Discomfort was more severe in the patients with treatment-experienced than treatment-naive. The score of discomfort in appearance and psychological were higher in the patients diagnosed in childhood or adolescent period than who were diagnosed after adult. Classification based on Cobb's angle showed no statistical difference. Conclusions Not only Cobb's angle but also other clinical factors should be considered for effective treatment in scoliosis. Also, It is necessary to pay attention to adult scoliosis patients.

  • Objectives : This study was performed to report the present situation of the cupping treatment to make standardization of cupping treatment in Korea. Methods : We searched relevant case reports, surveys, and review articles using a databases of online bibliography. And we had research to oriental medical doctor with questionnaire about the cupping treatment. Results : 1. Cupping treatment is used for diagnoisis, protection and treatment for many kinds of diseases such as musculoskeletal diseases, internal diseases, sequela of cerebral attacks and so on in Korea. 2. Adequate cupping area is the area of lesion. 3. Cupping time and pressure are various. 4. Adequate amount of venesection is 10cc. 5. Adequate dry cupping term is 1 time/day and adequate wet cupping term is 1 time/2~3days. 6. Cognition of adverse reaction of cupping treatment is different among the doctors. 7. Method of disinfection of cup is different among the doctors. Conclusions : The result of this study will help to make the a guideline of cupping treatment. And we have to go ahead studying to make standardization of cupping treatment.

The Effect of Acupuncture combined with Conventional Rehabilitation Treatment for Cerebral palsy: A Systematic review of Randomized Controlled Trials(RCTs) (뇌성마비(Cerebral palsy) 소아의 재활치료와 재활과 침치료의 병행치료를 비교한 체계적 문헌 고찰)

  • Objectives This study aims to evaluate the effect of acupuncture combined with rehabilitation treatment for cerebral palsy in children. Methods 7 electro-database (PubMed, CENTRAL, CNKI, DBpia, KISS, OASIS, KJTK) were searched for using the keywords "cerebral palsy AND acupuncture" which study is designed in randomized controlled trials (RCTs). Results 10 out of 303 studies were included in analysis after screened. Acupuncture combined with rehabilitation treatment had a significant effect on motor function. Only 1 study reported minor adverse effects. Conclusions All of the studies have shown acupuncture combined with conventional rehabilitation treatment was effective in cerebral palsy. Additional well-designed studies are needed to justify the effectiveness of acupuncture in cerebral palsy.

The Clinical Effects of Heating-Conduction Acupuncture Therapy for Lumbago Induced by Iliolumbar Ligament Sprain (전열침이 장요인대 염좌로 인한 요통에 미치는 임상적 효과)

  • Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on lumbago induced by iliolumbar ligament sprain. Methods : From September 25 to December 30, 2009, the 8 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medical hospital, Dae-Jeon university, with acute lumbago were performed heating-conduction acupuncture therapy on iliolumbar ligament sprain. To evaluate the efficiency of heating-conduction acupuncture therapy, visual analogue scale(VAS) and short-form McGill pain questionnaire(SF-MPQ) were applied before treatment and after 1st, 2nd treatment. Results : 1.The VAS and SF-MPQ scores showed statistically significant improvement after 1st treatment. 2. The VAS and SF-MPQ scores after 2nd treatment showed better improvement than those of before and 1st treatment, but had no statistical significance compared with those of before and 1st treatment. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with acute lumbago induced by iliolumbar ligament sprain.

A Case Report of Nocturnal pain in Patients Diagnosed with HIVD of L-spine Treated by Samul-tang(Siwu-tang) Gagam and Chuna Manual Treatment (야간통을 주증으로 한 요추 추간판 탈출증 환자에 대한 사물탕가감방과 추나요법병행치료 증례보고)

  • Objectives : To report cases of HIVD of L-Spine successfully treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam. and Chuna manual treatment. Methods : Three patients diagnosed with HIVD of L-Spine were treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment. Numeric Rating Scale(NRS), Oswestry low back pain Disablility Index(ODI) scores and EQ-5D were collected before and after treatment for comparison. Results : There was a significant decrease in NRS, ODI scores.And, there was a significant increase EQ-5D. The mean NRS score decresed from 10 to 1.33, ODI from 66.66 to 8.88. Tne mean EQ-5D score incresed from 0.225 to 0.856. Conclusion : The result suggests Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment can be an effective treatment on patients with HIVD of L-Spine.
