Purpose : The purpose of this study was to identify the effect of body weight support treadmill training (BWSTT) and parallel bar gait training(P-bar) on gait and balance ability of subacute stroke patients. The subjects were consisted of 27 patients with subacute stroke, and they were randomly devided into two groups which were BWSTT group and P-bar group. Method : The timed up and go(TUG), 10m gait speed were used to measure gait speed, Bergs balance scale(BBS) was used to measure dynamic balance ability, and balance performance monitor(BPM) was used to measure sway area, sway path, max velocity. Result : 1. The TUG and 10m gait speed of BWSTT group and P-bar group were significantly decreased (p<.05). The TUG and 10m gait speed were different significantly between BWSTT group and P-bar group(p<.05). 2. The BBS and sway area of BWSTT group and P-bar group were significantly decreased (p<.05). The BBS and sway area were not different significantly between BWSTT group and P-bar group(p>.05). 3. The sway path and max velocity of BWSTT group and P-bar group were significantly decreased (p<.05). The sway path and max velocity were not different significantly between BWSTI group and P-bar group(p>.05). Conclusion : The outcomes suggest that patient with subacute stroke can improve their gait and balance through body weight support treadmill training.
The purpose of this study was to investigate whether or not the physical therapist's explanation of the treatment prior to electrical stimulation affects the subjects level of anxiety. Eighty healthy subjects participated in this study and were assigned either to the experimental group receiving information on the treatment ($n_1$=40) or to the control group not receiving information on the treatment ($n_2$=40). Transcutaneus Electrical Nerve Stimulation (TENS), a sphygomamometer and the Spielberger State Anxiety Scale Questionnaire were used to investigate the threshold of anxiety. Analysis of data revealed significant differences in pulse and diastolic blood pressure compared to the control group when the subjects were stimulated electrically after receiving information on the treatment. We found significant differences between the two groups. The results of the study indicate that the patient's anxiety can be reduced by the physical therapist's explanation on the treatment.
Objectives: The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on anterior talofibular ligament injury induced by acute and chronic ankle sprain. Methods: From April 19, 2010 to May 30, 2011 the 79 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medical hospital, Dae-Jeon university with ankle sprain were performed heating-conduction acupuncture therapy on anterior taIofibular ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with pressure algometer and visual analogue scale(VAS) were applied before treatment and after 1st, 2nd treatment. Results: 1. The pain threshold and VAS score showed significant improvement on acute subacute, chronic anterior talofibular ligament injury group. 2. The difference in pain threshold and VAS score between acute, subacute, chronic phase group was not significant. Conclusions: Heating-conduction acupuncture therapy has clinical effects of pan reduction on patient with anterior talofibular ligament injury.
Purpose: The purpose of this study was to investigate the effects of a rehabilitation nursing program(RNP) for persons with disabilities. Method: a quasi-experimental pretest and post test study was used to examine the changes of patient's muscle strength, flexibility, self efficacy and health related quality of life at the completion of an 8-week education and physical exercise with a Thera-Band and Exercise Ball course. The subjects consisted of 40 adults who were disabled. Twenty-two experimental and 18 control subjects completed pre and post-test measures. Outcome variables were flexibility, hand grip, elbow flexor and extensor, knee flexor and extensor, self efficacy and quality of life. Independent t-test and ANCOVA were used to examine group differences by using SPSS 12.0 Result: Mean comparisons of the change scores revealed that the experimental group increased significantly in knee extremity flexor(p=0.035), extensor(p=0.039), flexibility(p=0.008), self efficacy(p=0.000), and quality of life(p=0.000). No significant group differences were found in each patient's hand grip, elbow flexor and extensor. Conclusion: RNP can improve lower extremity flexibility, muscle strength, self efficacy and health related quality of life in people with disabilities. The intervention to tailor exercise and education to the unique needs of the disability was suggested for further study.
Objective: This study was aimed at investigating the effects of lumbar stabilization exercise according to correct verbal instructions in pain and muscle strengthening of the low back pain patients. Design: A randomized controlled trial. Methods: Twenty subjects with low back pain were selected. They were randomly assigned to one of two groups (10 in each group): namely the lumbar stabilization exercise and lumbar stabilization exercise according to the correct verbal instructions group. The lumbar stabilization exercise group performed lumbar stabilization exercises for 6 weeks (5 times a week). The lumbar stabilization exercise according to correct verbal instructions group performed lumbar stabilization exercise according to correct verbal instructions for 6 weeks (5 times a week). We measured pain, muscle power, proprioception, and body balance before and after exercise by using visual analog scale (VAS), digital handheld dynanometer, Joint repositioning error, time up and go test respectively. Results: We found statistically significant differences in pain, muscle power, proprioception, and body balance in lumbar stabilization exercise and lumbar stabilization exercise according to correct verbal instructions group, before and after (p<0.05). Conclusions: We confirmed the effect of lumbar stabilization exercise according to correct verbal instructions. Thus we thought these results could be used as basic data and reference for low back pain. But we need more study effect of correct verbal instructions on other exercises.
Objectives This study was designed to compare the effects Gwibi-tang herbal-medication with Danggwisusan herbal-medication for patients with acute cervical and lumbar sprains due to a traffic accident. Methods This study was carried out on 119 acute cervical & lumbar sprain patients by traffic accident. We divided these patients into 2 groups. Group A was treated with Gwibi-tang and Group B was treated with Danggwisusan three times a day for three days. Each patient was treated with acupuncture, physical therapy and two types of herbal-medication. We measured the numerical rating scale (NRS) at two time points : pre-treatment (Tx). and post-Tx. Results 1. The NRS scores significantly decreased after treatment in all patients. 2. The changed NRS scores of nuchal pain showed a borderline significant difference between Group A and Group B (p=0.067). 3. In sub-group analysis, Gwibi-tang showed a greater effect on NRS scores of lumbago pain in female patients (p=0.068) and patients under 30s (p=0.054) compared to Danggwisusan. Conclusions In this study, both types of herbal-medication; Gwibi-tang and Danggwisusan were highly effective on acute cervical and lumbar sprain patients. Especially Gwibi-tang (Group A) was more effective than Danggwisusan (Group B) in reducing the initial nuchal pain. Also Gwibi-tang was more effective for lumbago pain in female and patients under 30s.
Objectives Upper lumbar disc herniation (LDH) (L1/2, L2/3) has specific anatomical characteristics and different outcome after conventional treatment compared to lower LDH (L3/4, L4/5, L5/S1). The purpose of this study was to compare the clinical features and effects of korean medical treatment of upper LDH between lower LDH. Methods We retrospectively reviewed the clinical data collected from 121 patients who was had admitted at the Haeundae Jaseng Hospital of Korean Medicine from June 1st, 2014 to August 31th, 2018. The patients who had treated at L1/2, L2/3 level LDH were grouped and compared with those treated at the L3/4, L4/5, L5/S1 level. We reviewed the patient characteristics such as age, the positive rate of Straight Leg Raise Test (SLR test), the presence or absence of previous lumbar surgery. Numeric Rating Scale (NRS) and Oswestry Disability Index (ODI) were evaluated at adimission and discharge. Results Mean age was significantly higher at upper LDH group and positive rate of SLR test was higher at lower LDH group. There was no significant difference of gender and previous lumbar surgery between two groups. Each group had a significant improvement of NRS, ODI scores. But between two groups, there was no significant difference of NRS, ODI scores. Conclusions Upper LDH group was older than lower LDH group. SLR test was useful tool to exclude upper LDH. Korean medical treatment was significantly effective to both upper and lower LDH groups. Further well designed prospective comparative studies are needed.
Purpose: Nurses play an important role in ensuring patient rehabilitation and are involved in all aspects of multidimensional rehabilitation. Therefore, strengthening rehabilitation nursing education is vital to ascertain high-quality rehabilitation and optimum outcomes. This study examined the effectiveness of a new teaching reform-a modified Six-Sigma-based training program-against a conventional educational program on rehabilitation specialist nurses' core competencies, post-training performance, and satisfaction. Methods: A quasi-randomized controlled trial was conducted to assess the effectiveness of the modified training program. We recruited 56 learners from the 2020 training course at the Hunan Rehabilitation Specialist Nurse Training Base as the control group. Sixty learners from the base's 2021 training course were recruited as the intervention group. Data were collected in a consistent manner from both groups after the training program was implemented. Results: Those who underwent the modified training program showed better improvement in all core competencies than those who underwent the conventional training program (p < .05); the scores for theoretical knowledge, clinical nursing lectures, reviews, and nursing case management improved significantly following the teaching reform (p < 0.05). Further, overall satisfaction as well as base management and theoretical teaching satisfaction improved significantly (p < .05). Conclusion: The modified training program strengthens rehabilitation nurses' base management abilities; enhances their core competencies; expands their interest in and breadth, depth, and practicability of theoretical courses; and updates the teaching methods.
