한의사들에게 적합한 고문헌 지식체계에 대한 고찰 -"동의보감(東醫寶鑑)"증보를 중심으로- (A Study of Classic Korean Medicine Knowledge System which is Suitable for Korean Medicine Doctors -The process of expanding DONGUIBOGAM-)

  • Objectives: The objectives of this study are to determine both what information Korean Medicine(KM) doctors want from medical classics and how classic KM knowledge should be structured. Methods: KM doctor survey was conducted in October 2012 via E-mail to determine what information Korean Medicine doctors want to collect from a larger volume of existing classics. Specialist survey was made in October 2012 through questionnaire and 8 workshops were held between April 2012 ~ February 2013 to discuss how to reorganize classic KM knowledge. Results: With two surveys and 8 workshops, we built a conceptualization of the classic KM knowledge system for KM practice. KM doctors wanted to engage with the design of KM knowledge system informed by authentic medical classics, and hoped that more information on common disorders would be added to the table of contents of classic KM books than now. A comprehensive knowledge system was also required to maximize information sharing. Conclusions: Future KM knowledge systems need to be more comprehensive and include more information about disorders with which patients commonly consult KM doctors.

  • Recently, there has been fierce argument between oriental and western doctors in the medical field. The use of medical devices has particularly come to the fore lately. Appropriate medical devices are required to diagnose and treat patients' conditions or illnesses accurately. At issue recently in medical device sector are diagnostic instruments using radiation, magnetic resonance imaging, ultrasound, IPL(Intense Pulse Light), and instruments used for tonometry. Relating to this issue, Association of Korean Oriental Medicine and The Association of Korean Medicine are sharply opposed. It is predicted that more accusations of this kind will be seen in the future. As oriental medicine contends, ultrasonic imaging itself seems to cause no harm to humans and its use may have an advantage for national health. The use of western diagnostic equipment can expand the diagnostic range of oriental doctors. However, unless new legislation is made, it is against the law for oriental doctors to use this equipment. Both law and medical science require grounds and predictability on the correctness of a decision and all of its consequences. Additionally, oriental medicine's use of ultrasounds and other medical devices should be established by standards and grounds which make same the diagnosis with repetition. Therefore, the scope of oriental medicine can be expanded following a revision of the Oriental Medicine Promotion Act and it is estimated that the state of national health will be greatly improved by the mutual respect of both sides of the health profession.

『관아외편(串雅外編)』의 외치료법(外治療法)에 관한 연구 - 내과질환을 중심으로 - (The Observation on the Five Vicera's A Study of External Treatment on the 『Chuan-Ya-Wai-Bian(串雅外編)』 -part of internal medicine-)

  • 1) Objective External treatments have various curative effects. So it had been used to cure various patients. But, it has a limited sphere of application in the present South Korea. And treatments of wander doctors had been disregarded by general doctors and the public. But Zhao-Xue-Min(趙學敏) had thought that there are useful treatments in it. So, the "Chuan-Ya(串雅)" had been written by Zhao-Xue-min in 1759. The book had included treatments of wander doctors. Especially, it had included various external treatments. Therefore we would like to bring out it's external treatments theory 2) Conclusions We have researched external treatments of "Chuan-Ya", and then we have arrived at following conclusions. "Chuan-Ya" has 48 sentences that have related to external treatments in internal medicine. And it had recorded fire conducting method(引火法) to explain process of external teratment principle.

월경통 환자에 대한 한방진단시스템의 진단일치도 연구 (A Study for Diagnostic Correspondent Rates between DSOM and Korean Medical Doctors' Diagnosis about Menstrual Pain)

  • Objectives Diagnosis System of Oriental Medicine (DSOM) was made as a computerized assistant program for oriental medicine doctors to be able to diagnose with statistical basis. Then DSOM uses questionnaires filled out by respondents with explanatory guide. But if the respondents misunderstand the meaning of the passages, the results were quite the opposite. Methods This study was designed to investigate the diagnostic correspondent rates between DSOM and TKM practitioners. First, let the respondents answer to DSOM. After that, three doctors diagnosed the respondents and marked 'p' when they diagnose that the respondent had the pathogenic factors, marked 'n' when they diagnose that the respondent had the pathogenic factors but not severs, and did not marked when they diagnose that the respondent didn't have the pathogenic factors. Finally, this study was investigated the correspondent rates of diagnosis between DSOM and doctors. Results In the pathogenic factor of three including insufficiency of Yin (陰虛), the correspondent rates were 90%. In the pathogenic factor of nine including deficiency of qi (氣虛), the correspondent rates were 80%. In the pathogenic factor of four including blood stasis (血瘀), the correspondent rates were 70%. In HH and HL, they showed the correspondent rates of 61.77%. The correspondent rate of heat (熱) was highest (96.88%). The correspondent rate of insufficiency of Yang (陽虛) was lowest (0%). In LH and LL, they showed the correspondent rates of 88.31%. The correspondent rate of blood stasis (血瘀) was lowest (71.76%). They all showed the correspondent rates of over 70%. Conclusions In DSOM and Doctors' diagnose, they showed the correspondent rates of 83.60%.

한방병원 근무 한방수련의의 기본심폐소생술에 대한 태도와 교육 효과 (Attitudes Toward and the Educative Effect of Basic CPR in Apprentice Doctors in an Oriental Hospital)

  • Objectives : To save the lives of patients with cardiac arrests, CPR must be performed rapidly and precisely. Regarding its critical necessity, there is a growing tendency to encourage whomever witnesses the cardiac arrest to perform basic CPR. However, the attitude toward basic CPR nor its current state of education in Korean Oriental Medical arena, which is one of the dual axes unique to Korean medical system, are not known. This is a kind of pilot study to research attitudes toward and the educative effect of basic CPR among oriental medical doctors in Korea. Methods : We carried out surveys and tests targeting nineteen apprentice doctors working in Dongguk University Ilsan Oriental Hospital, who were receiving training for 'AHA BLS course for healthcare providers', on the attitude toward and educative effect of basic CPR before and after the education. Results : The pre-educational survey showed that most of the participants felt the necessity to be trained in the BLS course, were not confident about performing CPR, and not well-acquainted with the whole process of performing CPR. After education, however, none of them responded with a lack of confidence to perform basic CPR, and the score of the confidence to perform basic CPR was significantly increased, too (from $2.05{\pm}0.71$ to $4.05{\pm}0.40$, n=19, p<0.001). Moreover, all of them answered they were satisfied with this BLS course, and it aroused their sense of responsibility as oriental medical doctors. All the participants passed the exam, and their average score for BLS scheme performance was $28.2{\pm}1.3$. Conclusions : It is necessary, in the future, to set up a management system for intensive, regular and continuous reeducation and expand the research on the perception and attitude targeting larger numbers and more multilevel groups of oriental medical doctors.

한방임상사용 의료기기의 실태 조사를 통한 개선방향 제시 (Actual Condition Survey and Improvement of Medical Devices in Korea Traditional Medicine)

  • We conduct a survey of actual condition of medical devices in korea traditional medicine on 157 oriental medicine doctors and 183 manufacturers. The 61% percent of doctors indicate the trouble in use of medical devices which is applied to korea traditional medicine. The manufacturers replied that devices have a problem with diagnosis(53%) and treatment(36%). To overcome these problems, it is necessary to develop devices based on the study of basic oriental medicine and the reliable clinical research. This study contributes the improvement of quality and the revision of rules on medical devices.

한국형 중풍변증진단 표준안의 필요성에 대한 설문조사 보고 (Report on the Survey for the Necessity of Korean Standard Differentiation of the Symptoms and signs for the Stroke)

  • This survey was accomplished in order to find out the necessity of creation of Korean standard differentiation of the symptoms and signs for the stroke(KSDS) in clinical practice. The survey questions were developed by the consensus from the professors who major in internal traditional Korean medicine(TKM). May 11th to July 31th 2005, the questionnaire was given to 800 TKM doctors in the whole country of Korea listed on the National Korean Medical Hospital Associations address book dy postal mail. Total of 358(44.7%) questionnaires were retrieved out of the 800 subjects. The 311 (86.9%) of 358 TKM doctors given an answer to the survey responded to need development of KSDS, and 176(56.6%) of 311 TKM doctors who answered to need development of KSDS responded to have to consider consensus of clinical experts on the stroke for creation of KSDS. These results support a role for new KSDS might be usefully applied in diagnosis and treatment of the stroke.

의사-환자의 치료적 관계에 대한 질적 사례연구 (A Qualitative Case Study of the Medical Doctor-Patient Therapeutic Relationship)

  • Objectives: The purpose of this study was to identify the doctor-patient relationship perceived by doctors in clinical settings and the effect of doctor-patient relationships on treatment schemes. A qualitative case study was conducted for this purpose. Methods: In-depth interviews were conducted with five oriental medicine doctors and doctors working in clinical settings using a semi-structured questionnaire. Transcription and coding were performed to analyze the data. By analyzing each case individually through within-case analysis, we attempted to find themes that emerged from the research subjects' experiences with establishing relationships with patients. Afterward, a cross-case analysis was conducted to identify the meaning of the experiences through commonalities and differences. Results: Within-case analysis confirmed the thoughts and emotions of the research participants in recognizing, defining, and participating in doctor-patient relationships while delivering treatments. Case-to-case analysis derived two themes, seven categories, and 20 meaningful units for doctor-patient relationships. Conclusions: The study found that a doctor-patient relationship regarding patient treatment could be established based on the doctor's 'professional qualifications' and 'human qualities'. In the future, it is necessary to present an educational model for relationship-based intervention techniques and personality maturity. Follow-up research should be conducted to enable the establishment of therapeutic relationships between doctors and patients.

한방 의료분쟁과 한의사의 적절한 진료 가이드라인에 대한 고찰 (Medical Dispute and the Proper Guideline for Medical Practice in Korean Medicine)

  • Medical disputes are increasing year by year in korean medical institutions as the frequency of patient's visiting korean medical institutions goes high and the reliability relations between korean medical doctors and patients are getting worse than the past. Frequently occurring accidents in korean medical acts fall into some categories such as medical treatment of acupuncture${\cdot}$moxibustion${\cdot}$cupping, korean herbal medication, korean medical physical therapy, wrong diagnosis. Frequently occurring accidents include these cases. Infection due to improper treatment process of acupunctrue or wet-cupping ; skin-burns caused by moxibustion, infrared therapy or haet therapy ;toxic hepatitis or kidney dysfunction induced by herbal medication ; progression of disease or missing of opportunity of timely transfer because of wrong diagnosis. It will prevent medical accidents to a great extent for korean medical doctors to have scientific diagnostic techniques and lab. test results at hand in korean medical institutions.

최근 3년간 보건지소 한방진료실의 현황에 대한 통계적 연구 (The Statistical Analyses of Oriental Medical Office of Sub-health Center During Recent 3 years)

  • Objective: We investigated the actual condition of oriental medical office in Sub-health Center. Method: We classified patients who visited an oriental medical office in a sub-health center of Jeollanam-do during recent 3 years, according to age, time(year), sex, disease, medical instruments, insurance, living condition... etc and studied statistical researches. Result: Almost all patients who visited an oriental medical office in a sub-health center were in 50, 60s. The number of patients were on the decrease. The number of female patients was 2.4 times more than the number of male patients. The most diseases of patients in sub-health center were diseases in connection with bones and sinews system. Generally, medical instruments depended on acupuncture. Conclusion; These results provide actual informations about condition of oriental medical office in Sub-health Center. Oriental medical doctors and offices of sub-health center are supposed to contribute to society, however essentially many problems exist. Public medical doctors of oriental medicine and local administration must renew their efforts for health of people in farming and fishing villages.
