Background: The utility of computed tomography (CT) in the differential diagnosis of patients with chronic obstructive pulmonary disease (COPD) exacerbation remains uncertain. However, due to the low cost associated with CT scan along with the impact of Koreas' health insurance system, there has been a rise in the number of CT scans in the patients with initial diagnosis of COPD exacerbations. Therefore, the utility of CT in the differential diagnosis was investigated to determine whether performing CT scans affect the clinical outcomes of the patients with an initial diagnosis of COPD exacerbation. Methods: This study involved 202 COPD patients hospitalized with an initial diagnosis of COPD exacerbation. We evaluated the change in diagnosis or treatment after performing a CT scan, and compared the clinical outcomes of patient groups with vs. without performing CT (non-CT group vs. CT group). Results: After performing CT, the diagnosis was changed for two (3.0%) while additional diagnoses were made for 27 of the 64 patients (42.1%). However, the treatment changed for only one (1.5%), and six patients (9.3%) received supplementary medication. There were no difference in the median length of hospital stay (8 [6-13] days vs. 8 [6-12] days, p=0.786) and intensive care unit care (14 [10.1%] vs. 11 [16.7%], p=0.236) between the CT and non-CT groups, respectively. These findings remained consistent even after the propensity score matching. Conclusion: Utility of CT in patients with acute COPD exacerbation might not be helpful; therefore, we do not recommend chest CT scan as a routine initial diagnostic tool.
This paper describes the implementation of a computerized radial pulse diagnosis by Elds of a clinical expert. On this base, we composed of the radial pulse diagnosis system in korean traditional medicine. The system composed of a radial pulse wave detection system and a radial pulse diag nosis system. With a detection system, we detected Inyoung and Cheongu radial pulse wave and processed It. Then, we have got the characteristic parameters of radial pulse wave and also quantified thats according to the method of Inyoung-Cheongu Comparison Radial Pulse Diagnosis. We defined the jugement standard of radial Pulse diagnosis system and then we confirmed the PossibiliLy for realization of automatic radial Pulse diagnosis in korean traditional medicine.

Purpose: Duchenne muscular dystrophy (DMD) is the most common lethal muscular dystrophy and is caused by the genetic variants of DMD gene. Because DMD is X-linked recessive and shows familial aggregates, prenatal diagnosis is an important role in the management of DMD family. We present our experience of prenatal molecular diagnosis and carrier detection based on multiplex polymerase chain reaction (PCR), multiplex ligation-dependent probe amplification (MLPA), and linkage analysis. Materials and Methods: During study period, 34 cases of prenatal diagnosis and 21 cases of carrier detection were performed at the Seoul National University Hospital. Multiplex PCR and MLPA was used to detect the exon deletions or duplications. When the DMD pathogenic variant in the affected males is unknown and no DMD pathogenic variant is detected in atrisk females, linkage analysis was used. Results: The prenatal molecular diagnosis was offered to 34 fetuses. Twenty-five fetuses were male and 6 fetuses (24.0%) were affected. Remaining cases had no pathogenic mutation. We had 24 (80.0%) cases of known proband results; exon deletion mutation in 19 (79.2%) cases and duplication in 5 (20.8%) cases. Linkage analysis was performed in 4 cases in which 2 cases (50.0%) were found to be affected. In the carrier testing, among 21 cases including 15 cases of mother and 6 cases of female relative, 9 (42.9%) cases showed positive results and 12 (57.1%) cases showed negative results. Conclusion: Prenatal molecular diagnosis and carrier detection of DMD are effective and feasible. They are useful in genetic counseling for DMD families.

Objectives: This study was designed to analyze the effectiveness of the diagnosis and treatments of Moya moya disease in oriental medicine. Results: This study showed that clinical symptoms of two Moya moya patients were improved by the diagnosis and treatments of oriental medicine. Conclusions: Diagnosis and treatment of two patients with Moya moya disease are accurate and effective. However, more cases are required to study in oriental medicine in order to prove the availability and to be applied universally.
Objectives This study is aimed to develop a standard instrument of diagnosis and assessment for the original symptom of Taeeumin. Methods 1. Based on the clinical practice guidelines, the works of Dong-mu, and the expert clinical judgment of the research team, the items to be used in the instrument of diagnosis and assessment for the original symptom of Taeeumin were extracted, and translation was proceeded in plain Korean for easy application and feasible use in daily clinical practice. As a result, candidate items for the diagnosis and assessment tool were derived. 2. An online questionnaire survey was conducted on the candidate items derived through the above process to the expert advisory group for (1) inclusion/exclusion as diagnosis and assessment index (2) evaluation of importance and (3) the validity of translation. Results & Conclusions Through survey of expert advisory group, the number of candidate items were reduced and the preliminary weights were assigned. And based on this, was developed. There were a total of 9 items for Exterior Cold[Wiwanhan] original symptom diagnosis and assessment tool, 21 items for Interior Heat[Ganyeol] original symptom, 15 items for Exterior Disease Unfavorable Patterns[Wiwanhan-paejo] original symptom, and 17 items for Interior Disease Unfavorable Patterns[Ganyeol-paejo] original symptom, and the weights were reflected in each response score so that the final total score could be calculated.

Sasang Constitutional Medicine (SCM) suggests the concept of health condition which is owned of different properties by each Sasang Constitution. But SCM theory has concentrate to explaining different symptoms that distinguish health condition, not much concerned with practical methods like face features, pulse diagnosis, skin surface properties and voice features. Physical properties of practical diagnosis methods from literature finding have been researched. As a result, complexion and face moisture were described to be physical properties related to health State. In scope of pulse diagnosis, depth, rate, contour, width and pressure are related. Skin surfaces' elasticity, moisture, fraction properties were also suggested as factors of health State of Sasang Constitution. We assume that this study would contribute further studies for objectifying and quantifying diagnosis methods to evaluate health state grades of Sasang Constitution.
The tongue diagnosis is a diagnostic method in the oriental medicine that uses shape, substance, coating, and movement of the tongue to determine the condition of health and disease characteristics in human. Since this information, however, could be affected by subjective sense and visual information, it is difficult to obtain the objective and reproducible results. This research aims at building a reproducible tongue diagnosis system using color chart that is attached close to the face contact region. The picture of color chart is taken simultaneously with a tongue and applied to color revision. The system, in addition, is focused on providing a clear tongue image through securing a sufficient photographing distance with a surface coating mirror. The lightning part which can suppress the reflection by sputum in maximum is implemented for the objectification and quantification of the tongue diagnosis system. The face contact region is designed for consideration of a testee's convenience. To evaluate the reproducibility of the system, the CVs (coefficient of variance, %) of $L{\ast}$, $a{\ast}$ and $b{\ast}$ of red, green and blue regions in color chart are calculated, respectively. The results of all CVs shows that the tongue diagnosis system is re liable and those consequences contribute to the objectification and quantification of the tongue diagnosis system.

This study is aim to evaluate pulse diagnosis as indicators for classification of the pattern identifications in stroke patients. To get the clinical information, we recruited the onset 1 month stroke patients through the multicenter network which consists of 13 oriental hospitals. The clinical informations about three pairs of pulse wave form and levels of their significancy based on the case report form (CRF) were collected and their distribution in each pattern identification were analyzed. The results are as follows Fire-Heat group shows high portions of floating pulse, rapid pulse and solid pulse. Qi Defficiency group has a greater portion of deep pulse, slow pulse, deficient pulse. The well-defined character of Phlegm-Retained Fluid, Yin Defficiency, Static Blood groups cannot be explained by pulse wave form. These results show a rough relationship between the pulse diagnosis and pattern identifications of stroke therefore, further studies are required to determine the pulse diagnosis as significant indicators of stroke pattern identification.
Objective : The chonguinyoung pulse diagnosis method is convenient than twelve meridian pulse diagnosis method The $\ll$Maek-bup(脉法)$\gg$ are founded at Mawangtweo(馬王堆) of the han dynasty tomb in 1973, but it could diagnosis the exsitance of illness of twelve meridian also .
