A Case Report of Korean Medicine Treatment for a Patient with Acute Cholangitis Accompanied by Common Bile Duct Stone (총담관결석을 동반한 급성 담관염 환자의 한방치험 1례)

  • Objective: This case report outlines an investigation into the efficacy of Korean medicine treatment in a patient who presented with acute cholangitis accompanying common bile duct stones. The patient underwent percutaneous transhepatic gallbladder drainage and subsequently experienced resolution of the common bile duct stones. Methods: The patient was treated with Shihosogan-tang, general acupuncture, ear acupuncture, moxibustion, and cupping therapy. Changes in symptoms were evaluated using a self-reported numerical rating scale (NRS) score and a visual analog scale (VAS) score each morning. Laboratory tests were conducted to examine serum amylase, serum lipase, and liver function. Results: After 29 days, the NRS scores for indigestion and fatigue and the VAS score for abdominal pain all decreased. Additionally, the laboratory test results showed improvement. Discussion: The results suggest that Korean medicine could be effective in treating symptoms of acute cholangitis. However, further research is necessary.

  • 韓醫學的現代化, 並不意味著在自然科學方法論的框框限制下實現韓醫學的現代化, 而是使傳統韓醫學的理論和醫療技術與現代社會及文化得以圓滿溝通, 開發新理論和新醫療技術, 從而在現有的保健醫療體系內更有效的發揮韓醫學的醫療技術. 韓醫學理論的現代化當保持傳統韓醫學理論的長處, 並著眼現代之角度彌補不足之處, 從而進一步完善理論之體系. 同時, 還應提示在現代醫療體制下能夠創造出新的醫療技術的理論, 並與包括西醫學在內的所有學科和政治經濟社會文化等領域做到圓滿溝通. 韓醫學現代化應當按照以下順序來進行, 首先, 以經釋經或應用醫史學的硏究方法整備傳統韓醫學理論, 其次, 當對韓醫學理論的概念和關系, 采用現代科學的語言重新進行描繪和解釋, 然後與現代的其他學問和理論進行學科間的合作研究. 再其次, 在臨床方面, 當以現代科學的測定儀器代替人的感覺器官, 或者根據傳統韓醫學的診斷方法, 開發新的診斷儀器以實現更爲客觀的診斷, 尤其需要結合和利用電腦來進行辯證分析, 至少應以電腦爲輔助, 從而得以確立韓醫學的新的醫療形式. 最後, 創出有關韓醫學及醫學, 醫療技術等各各方面適用可能的理論.

Usage of Korean Herbal Medicine for Climacteric Symptoms - Analysis of Korean Clinical Studies (갱년기 증상에 대한 한약 사용 - 국내 한의 임상연구 분석)

  • Objective : To investigate the usage of Korean herbal medicine (KHM) for climacteric symptoms, this study reviewed clinical studies conducted in Korea. Method : Literature search was performed on three Korean database; OASIS, RISS and NDSL. Studies published before July 2023 were categorized and analyzed according to the study type and herbal prescriptions. Results : Fifty-eight studies including 7 controlled trial, 27 case series and 24 single case reports were included in our review and the total number of case patients was 420. 52 prescriptions based on syndrome identifications and 11 prescriptions of Sasang constitutions were used and the most frequently used prescriptions were Soyosan(逍遙散) and Guibitang(歸脾湯). Thirty-two prescriptions were also mentioned in previous delphi research or clinical practice guideline, and the rest were not. Thirty-five studies used acupuncture combined with KHM and 10 studies used pharmacopuncture and 5 studies used herbal hipbath. Conclusion : To increase the therapeutic effect of KHM and establish KHM as an alternative to hormone treatment, various prescriptions should be used according to syndrome identifications in the clinical field. High-quality RCTs on frequently used prescriptions and the development of various type of KHM are also necessary.

A review on the treatment of Herbal Medicine for asymptomatically infected COVID-19 patients (코로나-19 무증상 감염 환자의 한약 치료 방안에 대한 고찰)

  • Objectives: Objective: The purpose of this study is to investigate the treatment of herbal medicine for patients with COVID-19 asymptomatic infections. Method: In English, search engines such as 'PubMed', 'Science Direct', and 'Cumulative Index to Nursing and Allied Health Literature (CINAHL)' were used. In Chinese, search engines such as 'China National Knowledge Infrastructure (CNKI)' and Wanfang were used. The keywords for the search engines were 'COVID-19', 'asymptomatic infection', 'Korean Medicine', 'traditional Chinese medicine', 'herbal medicine', and etc. Only clinical studies using herbal medicine for patients without fever or respiratory symptoms were selected. We excluded the cases that do not fit the research topic. Results: A total of 5 studies were finally selected. Among them, four of them used integrated herbal medicine and Western medicine, and one of the studies treated exclusively for herbal medicine. There were a total of seven prescriptions for herbal medicine used in the study. Outcome variables were used as following: lab test, nucleic acid conversion time, hospitalization period, chest CT, and etc. In the RCT study, herbal medicine and Western medicine decreased nucleic acid conversion time, average hospitalization time compared to the control group, but it was not statistically significant. No other adverse reactions were reported in all studies. Conclusion: According to the results, integrated herbal medicine and Western medicine might be an effective treatment for patients with COVID-19 asymptomatic infection reducing hospitalization period, time of nucleic acid turning negative. No severe adverse effects were reported. However, it is thought that better-designed research will be needed in the future.

  • Objectives : The aim of this study was to introduce the development process of Korean medicine health aging program for the older that is suitable for applied at public health center. Methods : For the development of the draft of health promotion program, we conducted a literature review of previous studies through searching international and domestic databases. The opinions of several experts were also colleted. Results : The details of 8-week of Korean medicine healthy aging program are as follows: 8-week of taking herbal medicines (Yukmijihwang-tang or Palmijihwang-tang, or Jaeumganghwa-tang) based on counseling with Korean medicine doctor, qigong, acupuncture on head, abdominal hotpack, taking nuts, and health education Conclusions : This healthy aging program can help to develop the healthy lifestyle habits and increase the general health status of community seniors.

Statistical Analysis of Outpatients Trends at Korean Medicine Hospitals of Daejeon University by Region before and after COVID-19 (COVID-19 전후에 따른 지역별 대전대학교 한방병원의 외래 환자에 대한 통계적 분석)

  • Objectives: This study is designed to statistically analyze data of outpatients visiting for recent 3 years. The purpose is to identify tendencies of patients who visit the hospitals before and after COVID-19. Methods: This study retrospectively analyzed the medical records of 452,487 patients who visited to the Daejeon·Cheonan·Cheongju Korean Medicine Hospital of Daejeon University from January 1, 2018 to August 31, 2020. The data is classified according to year, month, gender, age, and visit type. The statistical analysis was performed using IBM SPSS 25.0. Results: The total number of patients decreased in 2020 compared to before 2020, and the number of patients in each hospital also decreased from the previous year. According to the year of each hospital, the difference by year was not statistically significant at Daejeon Hospital, but Cheonan and Cheongju Hospital showed statistical difference. The change in monthly according to the year by hospital has continued to decrease from January to March in 2020, unlike the previous year, which recovered from a decline in March. In the analysis of patients by age, there was no statistically significant difference in the number of patients in the above 60s according to the year, while those under 60s have a tendency to significantly decrease. Conclusion: We expect that the results of this study will be used as reference materials in analyzing effects of COVID-19 at health care utilization.

Statistical Analysis on Hospitalized Patients with Thoracolumbar Compression Fracture at Single Korean Medicine Hospital: Retrospective Review (일개 한방병원에서의 흉요추 압박골절 입원 환자에 대한 통계적 분석: 후향적 차트리뷰)

  • Objectives: This study was designed to statistically analyze patients hospitalized for thoracolumbar compression fractures at Daejeon University Daejeon Korean Medicine Hospital from January 1, 2017 to December 31, 2021. Methods: A total of 62 patients were analyzed through electronic medical records in this study. The extracted data was analyzed using IBM SPSS ver.27.0. Results: 1. Traffic accident patients were more likely to reduce pain by more than half compared to falls and other patients. Patients without spinal disease were more likely to reduce pain by more than half compared to those without. The shorter the absolute bed rest and the longer the hospital stay, the higher the probability of pain reduction by more than half. 2. The duration of pain half-reduction was longer in patients with diabetes than in patients without diabetes. 3. The longer the hospitalization period and the shorter the absolute bed rest period, the less pain was reported upon discharge. Males complained of less pain at discharge than females, and patients without spinal disease complained less than those without. Patients who did not receive absolute bed rest complained of less pain upon discharge than those who did not. Conclusions: This study included patients hospitalized for thoracolumbar compression fractures and showed that etiology, absolute bed rest period, hospitalization period, gender, spinal disease, diabetes statistically affected the degree of pain reduction.

A Study on the Interrelation Establishment of Herbal Formulas that Clear Heat (청열방제간(淸熱方劑間)의 연관성(聯關性) 확립(確立)에 대(對)한 문헌적(文獻的) 연구(硏究))

  • Objectives : In traditional herbal medicine, as clinical experience increased, the number of herbal formulas also increased. For example, there are 61,739 herbal formulas in Pujifang, and 96,592 in Dictionary of Traditional Chinese Medicine Prescriptions. In chapter of Clear Heat (CH; 淸熱劑) of textbook, the 38 formulas were presented. For enhancing understanding of herbal formula, a large majority of books explained each individual formula with description or figure about interactions between constituent herbs. But there is no study on the interrelationship of each CH herbal formulas. This study investigated the interrelationship of CH herbal formulas. Methods : The CH herbal formula were analyzed with the ingredients, actions and clinical applications by literature study. Results : To improve cognition the interrelationship of CH herbal formulas, we analyzed the formulas and established the 16 interrelations of herbal formulas. Conclusions : Finally, this study established the interrelation of herbal formula as one figure. This result could contribute to improve cognition on the interrelationship of CH herbal formulas.

  • Objectives : The process of disease treatment in traditional medicine is defined as differentiating patterns and determining treatment. As an important part of this process, the herbal formulas are specific combinations or configurations of medicinal herbs that serve as tools for treatment. As clinical experience increased, the number of herbal formulas also increased. For example, there are 61,739 herbal formulas in Pujifang, and 96,592 in Dictionary of traditional chinese medicine prescriptions. In chapter of Release the exterior syndrome (RES; 解表劑) of textbook, the 42 formulas were presented. For the understanding on the formula, most of books explained with description or figure about the formulas. But there is no study on the interrelationship of each herbal formulas. This study investigated the interrelationship of RES herbal formulas. Methods : The RES herbal formulas were analyzed with the ingredients, actions and clinical applications by literature study. Results : To improve cognition the interrelationship of RES herbal formulas, we analyzed the formulas and established the 19 interrelations of herbal formulas. Conclusions : Finally, this study established the interrelation of herbal formula as one figure. This result could contribute to improve cognition the interrelationship of RES.

A Case Report of an Ataxia Patient with Sensory Loss Diagnosed with Thalamic Infarction (감각장애를 동반한 Thalamic Ataxia 환자 치험 1례)

  • This report is about a case of an ataxia patient with sensory loss diagnosed with thalamic Infarction. The patient had symtoms of gait disturbances and dysmetria. We used Korean medicine treatment modalities including acupunture, electroacupuncture, moxibustion and herb medicines. The improvement of ataxia was evaluated by International Cooperative Ataxia Rating Scale and Balance master system. After treatment, decreased scores of International Cooperative Ataxia Rating Scale and change of the trace of Balance master system showed that symtoms of ataxia were improved. Sensory deficits and other conditions were also getting better. This report suggests that Korean medicine could have a therapeutic effect for Thalamic ataxia.
