국소한약외용제의 알레르기성 비염 치료 효과에 대한 문헌 고찰 (A Literature Study of Topical Application of Chinese Herbal Medicine for Allergic Rhinitis)

  • Objectives The purpose of this study is to analyze studies about the effects of topical application of Chinese herbal medicine for allergic rhinitis within randomized controlled trials (RCT). Methods The search database includes KJTK (Korean Traditional Knowledge Portal), OASIS (Oriental Medicine Advanced Searching Integrated system), CNKI (China National Knowledge Infrastructure), PubMed, and EMBASE. We used the following key search terms: "allergic rhinitis", "traditional Chinese medicine", "traditional Korean medicine", "Chinese herbal medicine", "randomized controlled trial", "clinical study", "nasal spray", and "nasal drop" Results Five studies were selected for analysis. Three studies used nasal spray of Chinese herbal medicine and two studies used nasal drops of Chinese herbal medicine for the intervention. Frequently used herbs were Magnoliae Flos (辛夷), Scutellariae Radix (黃芩), Coptidis Rhizoma (黃連), and Astragali Radix (黃芪). For the outcome measures, studies used symptom score, questionnaire of Quality of Life, nasal airway resistance, or laboratory studies. From all 5 studies, it has been shown that topical application of Chinese herbal medicine was effective for improving symptoms of allergic rhinitis. Two out of five studies showed statistical difference between study group and control group. Conclusions This study shows that topical application of Chinese herbal medicine can improve symptoms of allergic rhinitis. Well-designed RCT studies with low risk of bias and studies with outcome measures for assessing the immunomodulatory effects are necessary to confirm these findings.

야경증의 한약 치료에 대한 임상연구 동향 (Trend of Clinical Studies on Herbal Medicine Treatment for Night Terrors)

  • Objectives The purpose of this study is to review relevant clinical studies to investigate the effectiveness of herbal medicines for night terror in children. Methods Studies on herbal treatment for night terrors were searched and analyzed through electronic databases such as PubMed, China National Knowledge Infrastructure, Wanfang, CiNii, J-STAGE, Oriental Medicine Advanced Searching Integrated System and Science ON. Studies included Randomized controlled trials, case series, and case reports. Results A total of 25 studies were selected and analyzed. In most studies, improvement of night terrors was observed after treatment, such as high total effectiveness. In one study, the frequency of night terrors was decreased significantly in herbal medicine group. Conclusions Based on the results of the clinical studies, we identified that herbal medicine is an effective treatment for night terror. But it is considered necessary to prove the effect and safety through additional systematic clinical studies.

Four Case Reports on Treatment of elevated aminotransferase levels with Herbal medicine containing Artemisia capillaris as Principal Component

  • Background: Many herbal medications have been used to treat various liver diseases. But the concerns of herbal medicine induced liver injury also existed. In this respect, we would like to report several cases with imporved elevated liver aminotransferase after treating herbal medicine including Artemisia capillaris. Case Reports: We report four patients with elevated aminotransferase levels (ATLs), which indicate hepatocellular damages. After receiving herbal medicines therapy containing Artemisia capillaris as principal component (HMA), the patients' ATLs were improved. In the first case, the patient's ATLs decreased into normal range after administration of HMA, although they have not been improved with hepatotonics for a long period. In the second case, the patient's ATLs have been elevated after taking anticoagulants. The ATLs were improved with HMA without stopping anticoagulants. In the other two cases, the patients' ATLs were also improved after taking HMA. In addition, there were no changes of previous drugs for treating the corresponding underlying diseases and no adverse events during HMA applications. Result and Conclusion: The four patients received the herbal medicine containing Artemisia capillaris as principal component and showed improvement of ATLs. These cases suggest that HMA can be considered as alternative or complementary remedies to improve various liver diseases.

한약 정보 표준화를 위한 의약품 식별 표준 (IDMP) 분석 및 고찰 (Review of Identification of Medicinal Products (IDMP) Standards for Standardization of Herbal Medicine Information)

  • Objectives : The purpose of this study was to apply informations related to herbal medicines to IDMP (Identification of Medicinal Products), an ISO standards related to medicinal products substances, for systematic collection of data through the integration of informations on distribution, manufacturing, and management of herbal medicines. Methods : By analyzing ISO 11238 and ISO/TS 19844, elements that can be used in the information model of herbal medicine were derived from the identification of medicinal products information model on substances. The labeling specified in the safety and quality control regulations for herbal medicines was mapped to the IDMP information model, and ginseng was applied as an example. Results : Herbal medicine corresponded to substance in IDMP. Among the five types of substances specified by IDMP, herbal medicines were expressed as structurally diverse. Scientific name was used as an invariant property of herbal medicine, and the substance level included information about source material and modification, and specifically included information about the scientific name, medicinal part, fraction, and processing. In addition, the specified substance level had information on the constituents, characteristic attributes, manufacturing, and grade of the herbal medicine. Conclusions : It is necessary to establish a code system for identifying herbal medicines. In order to apply the IDMP standards, research on the development of standard terms is required to express the characteristics of herbal medicines. In addition, information for identification of herbal medicines is also required, and information from production to consumption should be systematically accumulated and managed for actual application.

2012년 첩약 건강보험 급여화 시범사업 정책 결정에 관한 연구 (A Study on the policy decision-making for the pilot project of herbal decoctions coverage in the National Health Insurance in 2012)

  • Backgrounds : To reduce the patients' economic burden of herbal decoctions use, in 2012, Korean government decided to implement the pilot project of herbal decoctions coverage in the National Health Insurance. Objectives : This study aimed to analyze the policy decision-making process for the pilot insurance project in 2012. Methods : Official documents, research papers, statistical reports, and news articles, etc. on the coverage of herbal decoctions were searched and collected. We used the Kingdon's Policy Stream Model to analyze how the policy of pilot project of herbal decoctions coverage was decided, and who were the main activists for the decision-making process. Results : Components to be included in the 'Problem stream' were the decline in the profits of Korean Medicine institutions, the contraction of the herbal decoctions use, and the fiscal surplus of National Health Insurance. In the 'Policy stream', there were several model studies for herbal decoctions coverage, and examples of herbal benefits in other social insurances. In the 'Political stream', there were the legislative initiatives by member of the National Assembly and the promotion of insurance coverage by the Association of Korean Medicine(AKOM), etc. Policy window for herbal decoctions coverage was opened by the combination of these three streams with the efforts of policy activists, such as the executives of AKOM, and policy researchers. Conclusions : The policy decision process for health insurance coverage of herbal decoctions was analyzed using Kingdon's model, and the analysis shows that the combination of political streams and entrepreneurs' competencies can be an important driving force in policy decision making.

Spargue-Dawley 랫드를 이용한 구미강활탕의 급성독성 연구 (Acute Toxicity Study on Gumiganghwal-tang(Jiuweiqianghuo-tang) in Sprague-Dawley Rats)

  • Objectives : This study was conducted to evaluate the acute toxicity and safety of Gumiganghwal-tang (Jiuweiqianghou-tang) in Sprague-Dawley rats though the current regulatory guideline. Methods : The preliminary study showed that the single oral administration of Gumiganghwal-tang(Jiuweiqianghou-tang) did not induce any toxic effect at a dose level of 2000 mg/kg. Based on the results, 2000 mg/kg was selected as the limited dose. In this study, 10 rats of each sex were randomly assigned to two groups of 5 rats each and were administrated singly by gavage at dose levels of 0 and 2000 mg/kg. Mortalities, clinical signs, and body weight changes were monitored for the 15-day period following administration. At the end of observation period, all animals were sacrificed and complete gross postmortem examinations were performed. Results : Throughout the study period, no treatment-related deaths were observed. There were no adverse effects on clinical signs, body weight, and gross findings at all treatment groups. Conclusions : These results showed that the single oral adminstration of Gumiganghwal-tang(Jiuweiqianghou-tang) did not cause any toxic effect at the dose levels of 2000 mg/kg in rats. In conclusion, the $LD_{50}$ of Gumiganghwal-tang (Jiuweiqianghou-tang) was considered to be over 2000 mg/kg body for both sexes.

한약으로 관해가 유도된 극심한 설사의 궤양성대장염 환자 증례보고 (Remission of ulcerative colitis with severe diarrhea by herbal medicine treatment: A case report)

  • Ulcerative colitis (UC) is one of inflammatory bowel diseases (IBD) with unknown etiopathogenesis. This case report details remission of UC induced by Korean herbal medicine treatment. A 31-year-old male patient diagnosed as UC after a series of examinations including endoscopy and fecal calprotectin (FC) test. He had severe bloody diarrhea over 10-20 times a day despite taking 5-aminosalicylic acid and steroid. He was given individualized Korean herbal medicine Dowha-tang according to pattern identification. After taking Dowha-tang for 7 months, clinical remission was achieved. The symptoms disappeared and FC level went down to normal level. No adverse events were reported. This case report shows that Korean herbal medicine treatment may have the potential for clinical remission of UC. Further investigation is warranted.

항정신병약물 복용 환자의 대사증후군에 대한 한약치료의 체계적 문헌고찰 (A Systematic Review of Traditional Herbal Medicine Treatments for Metabolic Syndrome in Patients Receiving Antipsychotic Drugs)

  • Objectives: This study aimed to systematically review the effectiveness and safety of traditional herbal medicine treatments for metabolic syndrome (MS) in patients receiving antipsychotic drugs (APs). Methods: We searched 11 domestic and foreign databases for randomized controlled trials (RCTs) using traditional herbal medicines for MS in patients receiving APs. Results: Twenty RCTs conducted in China were included in the analysis. The most common disease among the participants was schizophrenia. Most studies used the 2004 Chinese Diabetes Society (CDS) standards as the diagnostic criteria for MS. The most used prescription was Yukultang (Liuyu-tang). The most used herbal material was Pinelliae Rhizoma, followed by Poria, Citri Reticulatae Pericarpium, and Glycyrrhizae Radix et Rhizoma. Traditional herbal medicines were mostly reported to improve the symptoms of MS. Conclusions: Traditional herbal medicines may be effective in improving the symptoms of MS in patients receiving APs. However, the quality of the included studies was low, and the studies were heterogeneous. Methodologically rigorous clinical studies on traditional herbal medicine treatments for MS in patients receiving APs are needed.

일개 보건소에서 시행한 불면증에 대한 한약치료의 효과분석 (Clinical Effect of Herbal Medicine Treatment on Insomnia in a Public Health Center)

  • Objectives: We conducted a public health promotion program in Korean medicine for participants suffering from chronic insomnia in 2016~2017. A significant improvement effect was obtained through the 8-week herbal medicine treatment program. The object of this study is to report the results of this public health promotion program. Methods: Seventy patients diagnosed with chronic insomnia participated in the program, 49 of whom completed the program. Participants were treated with the herbal medicine for insomnia treatment for 8 weeks. A sleep diary was used to assess the total sleep time, sleep latency, and number of times the participants woke. The treatment satisfaction was then evaluated. Results: Through the 8-week herbal medicine treatment program, the total sleep time was significantly increased by $109.59{\pm}70.77$ minutes, the sleep latency was significantly decreased by $63.53{\pm}47.79$ minutes, and the number of times the participants woke was significantly decreased by $1.54{\pm}1.19$ times compared to the baseline (all, p=0.000). Among the 24 patients who were examined for the treatment satisfaction, the average score was 4.33 points of a total of 5 points. Six patients had adverse events. Except for one patient, the symptoms of other patients were mitigated by changing or discontinuing the herbal medicine. Conclusions: For patients with chronic insomnia, the 8-week herbal medicine treatment program was effective, with few side effects and high treatment satisfaction. This study has significance as this is the first report in Korea on the effect of a herbal medicine treatment program for chronic insomnia through Korean medicine public health promotion.

건선 환자에서 아시트레틴 치료에 추가한 한방 약욕 효과 : 체계적 문헌 고찰과 메타 분석 (Add-on Effect of Herbal Bath Combined with Acitretin Treatment in Patients with Psoriasis : Systemic Review and Meta-analysis)

  • Objectives : This study aims to evaluate the add-on effect of herbal bath combined with acitretin in patients with psoriasis. Methods : We searched randomized controlled trials(RCTs) reporting the effect of herbal bath and acitretin combined treatment for psoriasis in 10 electronic databases from their inception to January 2023. Study collection, data extraction, and risk of bias assessment were performed by two independent researchers. Data synthesis and risk of bias evaluation was conducted using Revman 5.4. Results : Ten RCTs with 1,008 subjects were included. Herbal bath and acitretin combined treatment group showed higher average total effective rate than acitretin alone group(RR 1.39; 95% CI: 1.15-1.68, P=0.0006, I2 =0%). Herbal bath and additional treatments such as phototherapy(NB-UVB) and oral herbal medicine, combined with acitretin group also showed higher average total effective rate than acitretin alone group(RR 1.81; 95% CI: 1.47-2.22, P<0.00001, I2 =43%). Herbal bath and phototherapy(NB-UVB) combined with acitretin group also showed higher average total effective rate than acitretin alone group(RR 1.51; 95% CI: 1.18-1.93, P=0.0009, I2 =0%). The overall risk of bias of the included studies was unclear or of high risks. Conclusions : This review showed that the add-on of herbal bath combined with acitretin treatment could be more effective than acitretin alone treatment in patients with psoriasis. However, because of few included studies, heterogeneity between studies, and insufficient quality of included studies, further well-designed RCTs are needed to confirm the add-on effect of herbal bath on psoriasis.
