일 지역사회거주 치매의심 노인에서 Apolipoprotein E ${\varepsilon}4$와 인지기능 간의 연관성 (An Association Between Apolipoprotein E ${\varepsilon}4$ and Cognitive Function in Community-Dwelling Elders with Questionable Dementia)

  • Objectives : It was the aim of the present paper to examine the impact of the apolipoprotein E(APOE) ${\varepsilon}4$ on cognitive performance in community-dwelling elderly samples with 'questionable dementia'. Methods : Total 295 samples who were diagnosed with 'questionable dementia' in the recent year and completed the Korean version of the Consortium Establish a Registry for Alzheimer's Disease(CERAD-K) neuropsychological assessment protocol, were recruited. The CDR test established score of 0.5. Genomic DNA was extracted from the venous blood and APOE genotyping was done in this group. Their cognitive performance was compared by the occurrence of the APOE ${\varepsilon}4$ allele. Results : The impact of ${\varepsilon}4$ allele was significant in the Word List Recall Test(WLRT, F=4.511, df=1, p=0.035). The 'young-old' group aged 75 years and under had a significantly lower performance on the Word List Recall Test(WLRT, F=5.090, df=1, p=0.015), but the 'old-old' group over 75 years of age had not significantly different performance on the all the item of tests in ${\varepsilon}4$+ allele group. Conclusion : The conclusion to be drawn here is that community-dwelling elderly samples with ${\varepsilon}4$ allele in 'questionable dementia' had a significantly lower performance on the Word List Recall Test in the CERAD-K neuropsychological test batteries and the effect was prominent in the 'young-old' age group.

대학생의 수면과 감각처리 특성의 상관관계 (Correlation of Sleep and Sensory Processing Patterns With University Students)

  • 목적 : 본 연구는 비숙면군 대학생의 감각처리특성과 수면의 질의 상관관계를 알아보고자 한다. 연구방법 : 부산광역시, 경상남도, 대구광역시, 그리고 경상북도에 소재한 6개 대학의 만 18세~25세 대학생 191명을 대상으로 하였다. 연구도구는 한글판 청소년/성인 감각 프로파일(Adolescent/Adults Sensory Profile; AASP)과 피츠버그 수면의 질 지수(Pittsburgh Sleep Quality Index; PSQI)를 사용하였다. 분석방법은 감각처리특성과 수면의 질의 관계를 알아보기 위해 Pearson 상관분석을 사용하였다. 결과 : 본 연구의 참가자들 중 비숙면군에서 감각처리특성과 수면의 질의 상관관계검증 결과, 등록 저하, 감각 민감성, 감각 회피, 감각 추구의 모든 감각특성이 수면의 질 항목 중 수면 방해와 정적 상관관계를 나타냈다. 수면의 질 항목 중 주간 기능장애는 등록 저하와 감각 민감성과 정적 상관관계를 나타냈다. 결론 : 대학생의 수면장애와 감각처리특성을 이해하는데 도움을 제공하며, 관련 연구의 기초 자료로 사용할 수 있다. 앞으로 대학생만이 아니라 아동, 성인, 노인을 대상으로 수면과 감각처리특성에 관한 연구가 필요하리라 생각된다.

죽음준비교육이 노인의 죽음에 대한 태도와 우울에 미치는 효과 (Effects of Death Education on Attitude toward Death and Depression in Older Adults)

  • 본 연구는 '노인을 위한 죽음준비교육'의 효과를 검정하기 위한 연구이다. 노인은 인간발달단계로 볼 때 죽음을 가장 가까이 접하고 있는데, 죽음에 대한 태도는 긍정적 혹은 부정적으로 복합적일 수 있다. 노인이 죽음에 대해 긍정적인 태도를 가지게 되면 그들의 삶의 질이 좋아지고 편안한 죽음을 맞이할 수 있다고 보는 견해가 지배적이다. 이에 본 연구는 10주간의 죽음준비교육 프로그램을 주 1회 100분씩 60세 이상의 노인 38명에게 실시하여 노인의 죽음에 대한 태도와 우울에 대해 긍정적인 변화를 검정하고자 하였다. 교육은 2기에 걸쳐 실시하였다. 1기 22명의 노인에게는 2008년 1월 16일 부터 3월 26일까지, 2기 16명의 노인에게는 2008년 4월 16일부터 6월 18일까지 교육을 실시했다. 10주간 이루어진 강의주제는 "죽음준비교육의 필요성, 존엄한 죽음을 위한 3가지 대안, 죽음 끝이 아니다(I, II), 호스피스(I, II), 죽음의 9가지 유형(I, II, III), 죽음을 알면 자살하지 않는다."이다. 죽음준비교육을 실시한 다음 죽음에 대한 태도와 우울 점수를 교육 실시 전 점수와 t-test로 검정한 결과, 교육실시 이후에는 실시 이전보다 노인의 죽음에 대한 태도가 유의미하게 긍정적으로 변화하였다. 우울 정도의 점수는 감소하였으나 그 감소는 통계적으로 유의하지 않았다. 또한 노인대상자의 죽음준비교육 실시 이전과 실시 이후에 죽음태도와 우울간의 상관관계는 Pearson correlation coefficients로 분석한 결과, 교육 실시 이전과 실시 이후 모두 강한 역 상관관계를 보여 죽음태도가 긍정적일수록 우울정도가 낮았다. 그러나 교육 전·후 우울 정도와 상관관계의 변화는 없었다. 그러므로 본 연구는 노인계층 대상으로 일정기간 동안 규칙적으로 죽음준비교육프로그램을 실시할 수 있다면, 죽음에 대한 노인들의 태도를 긍정적으로 변화시킬 수 있음을 입증하였다. 아울러 지금까지 국내에서 체계적으로 이루어지지 못하고 있는 노인을 위한 죽음준비교육 프로그램 개발과 활용 기반을 확장하는데 기여할 수 있을 것이다.

노인요양시설물리치료사들의 노인복지정책과 노인질환에 대한 이해도 및 직무 환경에 대한 조사 (Survey for the Understanding of Elderly Disease and Work Environment for Physical Therapists in Elderly Care Facilities)

  • The results of researching on duties of physical therapist, understanding of elderly diseases and policies related with elderly welfare came out from the survey before beginning of education to physical therapists who attended National Elderly Welfare Physical Therapist Workshop during Sep. 11th to 14th, 2007. They have already understood about long-term elderly care insurance from the policy, and responded that those policies will bring more advantage to elders. On contrary, negative response had reason of low service quality. About elderly care system, there were positive answer of appropriate alternatives to support them, but also other critics had opinion of inevitable choice for living conservation. About understanding of elderly diseases, there were more comprehension of diseases as age gets higher, but most did not understand dementia and paralysis. Past experience and process of medical treatment for their disease were such arthritis, paralysis, broken bone, and dislocation of joint. Prognosis of technical training was mostly answered positively, but linking with other related disease programs were not welcomed. Most respondents thought that professional training were needed. The results of understanding diseases during senescent had high percentage of understanding as age and education was high, and it had eye upon statistics of the survey (p < 0.05) Most of those questioned responded that it is appropriate to have six working days per week and eight hours of daily work, however, present wage is not reasonable. Most of those questioned had negative response of work independence, and had an-swered that there should be certificates needed in related fields. For difficulties during work period, they had hardship instituting direction of treatment by lack of program, level of wage, and physical difficulties. Physical therapists averagely wished to have fewer than thirty patients per therapist. After the research, physical therapists who work in those facilities of elderly care systems agreed on policies, though it should not lower service quality. In addition, they were pros to have elderly care system, yet they are still inevitable option to maintain living. For work environment, the period of work was appropriate, but had different opinions for work independence and regular number of patients. Therefore, there should be more organized treatment program that also should be linked to other practical programs at this beginning point of the policy enforcement and the time that many therapists hope to work in elderly care system.

노인이 경험한 대체요법의 양상에 관한 연구 1 (A Study on the Patterns of Alternative Therapy Experienced by the Aged)

  • This study looks at the various alternative therapy methods used in day to day life by elderly, over 60 years of age. The elderly have come to know and practice these methods for the following reasons it is good for the health ; it is the method used in the of fen days when there wesn't modern medicine ; it has been passed down from generations ; it can be done at home without having the need to go to the hospital ; acupuncture or poulticing can be used ; it can be done at home, which was an important factor in rural areas where hospitals are few and far between ; and 〔herbal〕 medicine could be prepared at home at no cost ; it derives from experience ; it is impossible to ignore tradition passed down through the generations. Diet control and plants (herbs) are methods most often used. as they are easy to find and can be readily used in critical situations. Other methods include oriental medicine practices of moxibustion with moxa cone, negative therapy, hand and finger acupunture, finger press method. ordinary acupunture, manual healing methods of massage. diaphoretic therapy and meditation to reach a state of calm, and qigong dirigation. The reasons for its use are as follows ; it has been used before ; it is effective ; there is some improvement after the treatment ; it is not harmful to the body ; medicine cannot be obtained and it is the only thing available ; it is not good for an old person to go to the hospital everyday. the symptoms are not serious enough to go to a hospital : and acupuncture is for these things. The means that the elderly have come to practice these methods are : it has been used since the past ; it has been told by the elders ; they have been told by friends ; it was part of their knowledge ; and they have come to know by watching their mother. Further, to regain vitality lost through old age, the elderly have relied on hot soup. a hearty meal. brewed honey water, pumpkin, or ginseng. Humans, by instinct. would rub or massage the areas that caused pain. These actions, combined with a breathing technique have been recognized in Tong-Eui-Bo-Gam(the essential of eastern medicine), the complete work of early modern medicine, are a useful means to revive chi(기). This knowledge is thought to have greatly affected our heathy lifestyle. Furthermore, though the demand for medical services would increase with age, the elderly have not always been able to tend to their needs at the hospital for reasons economic or other. Hence, these alternative therapy methods seem to have been practiced as a temporary means of relief. The excellence of our traditional therapeutic custom has not received full recognition due to the argument relating to its scientific merits. As a result, it has become vital to prove their effectiveness through scientific and other experimental means. The potency of moxibustion with moxa cone and hand and finger acupunture have been proven scientifically. but diet and herbal methods appear to be practiced as a result of customs passed down from generations. In addition, it is submitted that the effectiveness of the traditional methods of disease control and our heathy lifestyle that are easily found in the nursing field must be verified.

농촌노인의 건강문제와 지각된 건강상태에 관한 연구 (A study Health problem and Perceived health status of the rural elderly)

  • Purpose: The purpose of this study is to provide a basic data that may help develop health promotion programs by identifying health problems and perceived health status of the rural elderly. Method: The subjects of the study were 366 elders recruited from 24 villages located in Mari Myun, Geochang Gun, Korea. The sample was selected using a quota sampling method. The instruments used in the study included 117 items for health problems and 4 items for perceived health status. The data were analyzed with descriptive statistics, Pearson correlation coefficient, t-test, ANOVA and scheffe test using SPSS program. Results: 1) The most prevalent health problem was ailments in 'musculoskeletal system', followed by 'fatigue', 'eyes and ears', 'genitourinary system', 'mood & temper patterns', 'cardiovascular system', 'digestive system', 'nervous system', 'respiratory system' and 'skin'. 2) Womens health problems were more prevalent than men's health problems. 3) The mean score of perceived health status was 7.68. 4) Health problems of the rural elderly were significantly correlated with demographic variables such as education (F=9.532, p=0.000), gender (t=-4.246, p=0.000), marital status (t=-3.531, p=0.000), family type (F=5.742, p=0.00l), and occupation (t=3.356, p=0.001). 5) Perceived health status of the rural elderly was significantly correlated with demographic variables such as education (F=6.408, p=0.002), gender (t=2.949, p=0.003), marital status (t=0.802, p=0.034), family type (F=4.844, p=0.003), and occupation (t=-2.485, p=0.011). 6) Health problems of the rural elderly were significantly correlated with life style pattern variables such as drinking (F=5.223, p=0.006), smoking (F=4.087, p=0.007), salty food intake (F=3.424, p=0.034), greenish yellow vegetables intake (F=6.343, p=0.002) and fat food intake (F=5.327, p=0.005). 7) Perceived health status of the rural elderly was significantly correlated with life style pattern variables such as sleeping hours (F=3.966, p=0.020) and drinking (F=7.231. p=0.001). Conclusion: The findings of this study indicate that nurses need to understand health problems and perceived health status of the rural elderly and to develop health promotion programs for them in the future in the consideration of regional and environmental elements.

일부 지역사회 저소득층 주민을 대상으로 한 가정간호사업의 효과에 관한 연구 (A Study on Effectiveness of the Home Care Services for the Low Income Inhabitants)

  • This study was intended to assess the need of home nursing care and analyze the effect of home nursing care, and find out the problems during the performance of home nursing care for the chronic patients among the low-income people in urban area. Data collection by interview was carried out from Nov. 1991 to Jul. 1992. The main results were as follows; 1) Total subjects for the need assessment of home nursing care were 123 households wi th 488 persons in a urban poor area. Over half of households $(57.7\%)$ was teenage family. The overall living conditions were poor and the average monthly income was 580 thousands won. $74.8\%$ of subjects was covered by medical care insurance and only $4.7\%$ was covered by public assistance. The morbidity rate was $8.2\%$ among 488 subjects and $27.5\%$ of them was not treated at all, $30\%$ was treated in utilizing pharmacies or local clinics. 2) The subjects of home nursing care were 46 with Hypertension or DM who agreeded the participation of study among registered patients at a public health center in Incheon. Home visiting was performed at intervals per one month for one year. Most of them were the elder(mean age=61 years) and long term patients(7.8 years continued). Home nursing care was effective. That is, blood pressure(including systolic and diastolic pressure) was significantly reduced (t(n=22)=2.31, P=.031, t(n=24)=4.16, P=.000 respectively) and knowledge of disease(t(N=46)=-7.63, P=.000), attitude of disease (t(N =46) = -4.92, P=.000), and self-care(t(N =46) = -4.89, P= .000) were significantly improved through home nursing care. But there was no difference in blood sugar for diabetics between the beginning and the end of visits. At the beginning of visit for home nursing care, sex$(\beta=-0414,\;t=-3.012)$ and nursing need({3=.310, t=2.164) were influencing self-care, and duration of disease$(\beta=.297,\;t=2.106)$ and nursing need $(\beta=.385, t=20417)$ were influencing blood pressure, blood sugar level. Namely, the subjects who were male and had higher nursing need showed better self-care and the longer duration of disease and the higher nursing need were relationship with the better blood pressure and blood sugar level. At the end of visit for home nursing care after one year, the blood pressure and blood sugar level was influenced by age $(\beta.320,\;t= 2.242)$, duration of disease ($(\beta.352,\;t= 2.395)$ and nursing need $(\beta=.350,\;t=2.623)$ and self-care had no influencing factor. The higher age and the longer duration of disease and the higher nursing need were relationship with the better blood pressure and blood sugar level. 3) The problems that were found out during the performance of home nursing care were the absent of useful protocols for services and the clear evaluation base, and the difficulty of teaching elders who were the major part of our subjects.

노년 남성의 의복착용 실태조사 -체형 변화와 기성복 만족도를 중심으로- (Clothing Condition of Elderly Men Focusing on Somatotype Variation and Satisfaction with Ready-to-Wear)

  • The purpose of this study was to understand clothing condition of elderly men focusing on somatotype variation and satisfaction with ready-to-wear and to provide basic data in designing ready-to-wrar for the elderly men. The questionnaire method was used and one-to-one interviews were held for the elderly residents in Seoul, Deajon, Jonju, and Yeosu areas. The total of 275 questionnaires (from 148 elderly men in the 60's, 99 in the 70's and 28 in the 80's) were collected and used for statistics. SAS package was used for the statistics and the statistics were based on frequency, X2-test, ANOVA, Duncan's multifle range test. The results of the study were as follows; 1) Height (p< .05), weight (p< .001) and Rohrer index (p< .05) decreased significantly with ageing 2) The somatotypes of 75.6% were changed and the 70's 8t the 80's showed significantly (p< .05) higher changing rate compared to the 60's and more changes took place significantly (p< .001) with ageing. 3) The most preferred out wear was suit & tie and next was jumper or shirt. 4) For the mode of getting suit, the highest number 55.3%) purchased ready-to-wear, next (32.0%) was tailored and the least (12.7%) neither purchased nor tailored. The percentage of buying ready-to-wear significantly (p< .01) decreased with ageing. The main reason for getting tailor-made was not well fitting of ready-to-wear (20.7% out of 32.0%). And for the mode of getting suit, there was significant differance with income (p< .001) and pocket money (p< .01). 5) The most important factors in suit and shirt werefirst, size (fitness) and second, comfort in movement. 6) For the wearing feeling of suit, 55.3% showed discomfort. There was significant difference according to the age (p< .05) and pocket money (p< .05). The Discomfort increased with ageing and less pocket money. 7) 62.4% answered it uneasy to Pick out well fitting If comfortable suit. This showed their dissatisfaction with ready-to-wear. 8) As for the items of not well fitting, 64.4% answered in jacket, 60.8% in trousers, 46.0% in shirt. The above result shows that elderly men's somatotype changes as they age, suit is found uncomfortable and it's not easy for elders to pick out comfortable ready-to-wear. So, they seem to have a certain dissatisfaction in ready to wear. Therefore, there is a need for reorganization of size and developing pattern design according to measurement of elderly men.

전국보건소 치과위생사의 노인의치보철사업 현황 및 전망 (The Present State and Prospect of Geriatric Denture Prosthetic Dentistry Affairs among Community Dental Hygienists)

  • 본 연구는 국민기초생활수급대상 65세 이상 노인들의 삶의질 향상과 구강건강을 유지, 증진 시키는데 필수적인 노인무료의치보철사업의 개선발전을 위하여 전국 각 구의 보건소 치과 위생사를 대상으로 노인무료의치보철사업의 현황을 파악하였으며 결과를 요약하면 아래와 같다. 현재 노인무료의치 보철사업의 홍보는 비교적 잘되고 있으며 대상자는 주로 동사무소에서 1년에 20~30명 정도를 선정하여 시행하고 있다. 대상치과는 47% 정도 치과의사협회와 연계하며, 주로 총의치를 많이 제작하고 있다. 제작 후 치과에 의존하고 있는 실정이라서 사후관리의 문제점이 많이 나타나고 있다. 또한 음식물섭취의 이로움과 무료시술이라는 점에서 64%의 높은 만족도를 얻고 있지만 의치장착 후 동통, 이물감 등으로 인해 불편함을 49%나 호소하는 것으로 나타났다. 이러한 결과를 토대로 노인의치보철사업의 문제점 및 만족도를 조사하여 향후 보다나은 노인 의료복지사업 중 하나인 노인무료의치보철사업의 앞으로 나아갈 방향에 대한 제언은 다음과 같다. 첫째, 의치 제작 후 지속적인 사후 관리가 이루어지기 위해서는 기초생활수급대상 노인들을 담당하는 방문 간호팀이나 사회복지사와 연계하여 의치 사용유무 및 관리상황 등을 점검하고, 의치관리법에 대한 교육이 시행되어야한다. 둘째, 의치가 가장 필요한 대상자에게 혜택이 주어질 수 있도록 대상자 선정 시 연령 기준이 하향 조정되어야하며, 보다 장기적인 대책으로 노인의치에 대한 보험급여화가 제기되어야한다. 셋째, 보다 양질의 무료의치보철사업을 수행하기 위해서는 환자의 구강 상태에 따른 정확한 수가 적용이 요구되며, 무엇보다도 무료의치보철사업을 시행하는 보건소 담당자와 치과의사의 봉사정신이 바탕이 되어야한다.

독거노인과 가족동거 노인의 시간관리능력과 일상생활활동의 비교 (Comparison of Time-Management Ability and ADL between Elderly People Living Alone and Living with Family)

  • 목적 : 본 연구는 대전광역시에 거주하는 독거노인과 가족동거 노인들을 대상으로 시간관리능력과 일상생활 활동을 비교하고, 집단 간의 관계를 파악함으로써 노인의 건강관리 방안을 찾는 기초자료를 제공하고자한다. 연구방법 : 연구대상은 65세 이상의 독거노인과 가족동거 노인을 MMSE-K로 선별한 후 각 40명씩 총 80명을 선정하였다. 시간관리능력은 설문지를 사용하였고, 일상생활활동은 FIM을 사용해 평가하였다. 연구기간은 2013년 5월부터 6월까지 진행되었다. 본 연구의 자료분석은 SPSS 20.0 프로그램을 사용하였다. 결과 : 독거노인과 가족동거 노인의 시간관리능력 및 FIM에 대한 총점의 점수비교는 두 집단 간에 통계적으로 유의한 차이를 보이지 않았다. 집단 간 세부항목 점수의 비교에서는 FIM의 세부항목 중 사회적 인지항목에서 사회적 상호작용, 문제해결능력, 기억력 모두 두 집단 간에 통계학적으로 유의한 차이를 나타냈다. 결론 : 본 연구를 통해 가족동거 노인에 비해 상호작용의 기회가 부족한 독거노인이 사회적 인지기능에 저하가 있음을 알 수 있었다. 이를 바탕으로 향후 지역사회 작업치료 안에서 노인의 인구 사회학적인 특성을 고려한 다양한 프로그램 개발과 연구가 이루어져야 할 것이다.
