일 지역사회 간호사들의 금연지도활동에 대한 조사연구 (Smoking Cessation Counselling Activity among Nurses in a Community)

  • Purposes: The purposes of this descriptive correlational study were to assess knowledge and attitude toward smoking and smoking cessation counseling activity among nurses in a community, and to identify predictors of their smoking cessation counseling activity. Method: Nurses employed by institutions such as university hospital, hospital, clinic, and health department in a community were surveyed using a structured questionnaire. Of the nurses invited to participate in this study, 760 (88.5%) responded with completed questionnaires. Results: Nurses had relatively positive attitude toward their roles and responsibilities about smoking cessation counseling activity. However, smoking cessation counseling activity was not a routine part of their nursing practice. Moreover, the level of engagement with smoking cessation counseling activity was significantly different by working place. Nurses who were working at the university hospital, hospital, and health department were more actively engaged with smoking cessation counseling activity than nurses who were working at the clinic. Smoking cessation counseling activity was significantly correlated with knowledge of smoking. attitudes toward smoking-related issues, and self-efficacy for smoking cessation counseling activity. In the final stepwise multiple regression, smoking cessation activity was predicted by the nurses' working place, attitudes toward smoking-related issues, and self-efficacy for counseling knowledge and skills. Conclusion: In conclusion, nurses need to participate routinely and actively in smoking cessation counseling activity. To help nurses counsel and intervene patients regarding smoking cessation more effectively, it is essential to integrate educational information on smoking cessation intervention into curriculums of nursing schools as well as to offer smoking cessation intervention as a continuing education program available for currently practicing nurses.

영양상담을 통한 식사행동의 변화가 운동선수의 운동능력 향상에 미치는 효과 (A Behavior-Modification Approaches to Improved Exercise Performance for Athletes Through the Multipe Nutritional Counseling)

  • Management of nutrient intakes through behavior-mordification can be important for improving exercise performance in athletes. The purpose of this study was to determine the effectiveness of nutritional counseling for improving exercise performance capability in athletes. The subjects were seven golfers and fourteen ju-do athletes from Hoseo university, and they have nor received professional hel[ from dieticians. Prior to nutritional counseling, their dietary intakes, lifestyles and dietary habits were evaluated. Nutritional counseling sessions were conducted by a trained dietician every 2-3 weeks for 3-4 month. After 5 counseling sessions, nutrient intakes, lifestyle and dietary habits were re-evaluated. Exercise performance capability was measured by maximal exercise stress test using treadmills. Individual data were assessed as quality index represents the better nutritional status. The common dietary and lifestyle problems were overeating binging, overeating, snacking and drinking alcohol prior to the nutritional counseling. After counseling sessions, the quality index of dietary habits seemed to decrease, especially in ju-do athletes, representing nutritional problems were resolved. Body fat and body mass index decreased in ju-do athletes, but not in golfers. Both athletes have shown to increase oxygen uptake at anaerobic threshold and maximal oxygen uptake status, which represents that the athletic performance capacity was improved after counseling sessions. Their respiratory quotients were decreased. In conclusion, nutritional counseling sessions over five times are an effective and efficient approach to change dietary habits to improve exercise performance capacity. Furthermore, clients can have good dietary habits and learn how to manago muscle strength by behavior modification through multiple nutritional counseling sessions. 복약지도 만족도가 복약순응도에 미치는 영향 : 외래환자를 대상으로 (Effect of Patient's Satisfaction with Pharmacist's Medication Counseling on Medication Compliance Among Outpatients)

  • Medication compliance is essential to improve person's health status through pharmacotherapy. Since separation of dispensing and prescription has been implemented, the importance of pharmacist's role on medication counseling have been emphasized, especially among outpatients. The objective of this study is to investigate the effect of patient's satisfaction with pharmacist's medication counseling on medication compliance among outpatients. We used the 2005 Korea National Health and Nutrition Survey data to examine medication compliance of persons who visited pharmacy. Ordered logistic regression analysis was conducted to assess whether patient's satisfaction with pharmacist's medication counseling was associated with medication compliance. Overall, 86.17% of study subjects(5,494) reported to take the prescribed medicines correctly and 80.54% of respondents were satisfied with pharmacist's medication counseling. Logistic regression analysis presents patient's satisfaction with pharmacist's counseling is significantly associated with medication compliance after controlling patient's characteristics(proxy-measured by age, family income, educational attainments, comorbid conditions). Interestingly, even patients who are dissatisfied with pharmacist's counseling have slightly higher odds of better compliance to medication therapy than those without pharmacist's counseling. In addition, higher educational attainments and older age groups are associated with better medication compliance. In conclusion, pharmacist's medication counseling appears to be effective in improving medication compliance. Further research that assess medication counseling in detail needs to develop strategies to improve medication adherence.

Psychological effects and risk perception after genetic counseling

  • Purpose: Demand for genetic counseling on cancer predisposition syndrome is increasing. We evaluated the psychological effect on counselees after genetic counseling at a clinic in a single center. Materials and Methods: We surveyed a total of 72 enrolled participants who visited a genetic counseling clinic at the Samsung Medical Center (SMC). The initial survey was conducted before the first genetic counseling session, and the second survey was conducted after the second genetic counseling session. A total of 43 participants completed both the initial and second surveys. Results: The initial survey of 72 participants indicated higher feelings of guilt in the group with religion, higher depression and anxiety in the group with a diagnosis of self, and higher anxiety in the group on self-referral to the genetic counseling clinic. In the completed survey of 43 participants, overall decreased depression was observed after the second genetic counseling session (P=0.013). Risk perception and anxiety decreased in the group diagnosed with benign variant/variant of uncertain significance (BV/VUS, 25/3) and increased in the group diagnosed with pathogenic variant (PV, 15). Risk perception and anxiety differed between the BV/VUS and PV groups (P<0.001 and P=0.03, respectively). Conclusion: The genetic counseling clinic at the SMC was effective in ameliorating the depression score. Assessment of survey results revealed different depression scores, feelings of guilt and anxiety, and different effects of the genetic counseling clinic, depending on the subgroups. Understanding the needs and psychological characteristics of different groups is necessary for improving genetic counseling services.

부부상담 프로그램 개발 및 효과성 연구 : 부부갈등해결을 위한 통합적 접근 적용 (Study on the Development and Effectiveness of Marital Counseling Program : Applied Integrated Approach for Marital Conflict Solution)

  • The purpose of this study was to develop Marital Counseling Program for marital conflict solution and to verify the effects of the developed program both quantitatively and qualitatively. This marital counseling program consisted of conjoint marital counseling(5 session) and group marital counseling(6 sessions). Each 13 couples of the experiment group and the control group were analysed. The effectiveness of this program was evaluated by pretest, post test, and follow-up after 3 months. It was analyzed by Mann-Whitney U Test and Wilcoxon Matched Pairs Signed-Rank Test. Also, as for the experiment group, the level of objective change perception, qualitative evaluation, and program evaluation were investigated. The major results were as follows: This marital counseling program for conflict solution was effective to decrease the level of marital conflict and explosive conflict coping and to increase the level of emotional interaction, communication skill, marital satisfaction, and self-esteem among experiment group. These positive effects maintained at follow-up. Additionally, participants(experiment group) reported positive changes of psycho-emotional state, spouse interaction behavior, marital communication and marital relationships. However, in the control group, there were no change according to the time. Future research and practical implications were added.

실직 가족의 부인을 대상으로 한 집단상담 프로그램 효과 연구 - Satir 성장 모델을 중심으로 - (A Study of the Effects of Group Counseling Program for the Wives of Jobless Families - Focusing on Satir′s Growth Model -)

  • This study is designed to conducted group counseling programs applied to the wives of jobless families by Satir's growth model and to help them to recognize the value of themselves and families in their jobless situation and to possess desire and hope for the life of their families in the future. The researcher provided 7 wives of jobless families with group counseling at the family education room of a university from April to June, 2003. The group counseling program consisted of a total of 8 sessions, and each session lasted for three hours to four hours. The researcher came to have a in-depth understanding of the experiences of 5 subjects, who took part in more than 7 sessions in a total of 8 counseling sessions, in the group counseling to which Satir's growth model was applied and analyzed the effects of counseling changing the subjects. The researcher analyzed the following: viewing the subjects themselves and their spouses in a new way, rendezvous with the true self and its acceptance, objective insight into their families, learning how to communicate, and expecting hope through changes. All in all, the wives of jobless families were able to newly recognize the value of the existence of themselves and families and find desire and hope for the life of their families in the future through the group counseling program to which Satir's growth model was applied.

사상의학진료에서 상담기법의 활용 (Application of Counseling in Sasang Constitutional Medicine)

  • Objectives The purpose of this study was to investigate the relationship between counseling skills and Sasang constitutional medicine(SCM). Methods Reviewing the texts relevant to SCM, comparison between counseling skills and the excerpt from the relating books was conducted in terms of the attitude of the counselor, the observation method, the evaluation of the counselee, the relationship between the counselor and counselee, and the counseling method. Results As the attitude of the counselor, many kinds of attitudes and skills including respecting the counselee and reciprocal belief were needed. The excerpt from "Gyeokchigo" meant that the counselor or a doctor should have the virtue of Truth. To evaluate the patient's problems in SCM and the counseling, diet, digestion, and sleep could be asked, but questions about the emotions such as Joy, Anger, Sorrow and Pleasure should be more carefully modified in a modern way, when applying clinical practices. Empathy as the main principle of the counseling could be attributed to the main idea of Dongmu Lee Jema, in which all the people could be in the same status in a natural way reflecting that they like the virtue and dislike the evil. Recommendations of regimen and directives could be followed according to SCM Conclusions Several items of counseling skills and SCM coincide but some of the modern counseling skills are still needed to apply SCM to make the patients with psychiatric problems treated efficiently.

The Effects of Counseling about Death and Dying on Perceptions, Preparedness, and Anxiety Regarding Death among Family Caregivers Caring for Hospice Patients: A Pilot Study

  • Purpose: This pilot study aimed to examine the influence of death counseling on perceptions, preparedness, and anxiety regarding death and dying among family caregivers of hospice patients. Methods: Death counseling developed based on the SPIKES model was provided to 37 family caregivers in a hospice and palliative care unit. Perceptions, preparedness, and anxiety regarding death were assessed with a self-administered structured questionnaire, and participants' scores before and after counseling were compared using the paired t-test. Results: Significant changes were found in perceptions, preparedness, and anxiety regarding death after counseling. Compared to before counseling, the scores for perceptions of death (t=-4.90, P<0.001) and preparedness for death and dying (t=-16.23, P<0.001) improved, while anxiety (t=3.72, P=0.001) decreased after counseling. Some changes were also found in the types of support that family caregivers needed to prepare for the death of their family members in the hospice care unit. Conclusion: The findings demonstrate that death counseling could help family caregivers prepare for the death of their loved ones. Hospice and palliative care providers should play a key role in supporting family caregivers of hospice patients by developing strategies for counseling.

상담실무자가 지각한 건강가정지원센터의 상담업무와 근무환경 (Counseling Services and Work Conditions Perceived by Counselors in Healthy Family Support Centers)

  • The present study investigated counseling services and work conditions perceived by counselors in Healthy Family Support Centers. Questionnaire survey by mail was used to collect data from 32 counselors in Healthy Family Support Centers. Major findings of this study were as follows. First, many counselors in Healthy Family Support Centers perceived work load of counseling services heavily though they thought counseling work attractive and they would keep on working. Second, many counselors had a positive view of work conditions as a workplace but not a few of them complained about physical work conditions including poor facilities like lack of counseling room space in Centers. In addition to this, many suggestions were proposed to improve the counseling services and work conditions perceived by counselors in Healthy Family Support Centers.

사이버 상담 활성화를 위한 탐색 연구 (A Preliminary Study on the Effectiveness of Cyber Counseling)

  • The main purpose of this research was to explore the factors that encourage people to use the cyber counseling in resolving difficulties in their lives. Another purpose of this research was to investigate whether cyber counseling really helps people feel better. For this study, a structured survey questionnaire was distributed both on- and off-line, and 429 respondents returned completed survey forms. The major statistical methods used for data analysis were frequency, factor analysis, 1-test and ANOVA. The results of this study revealed that variables such as gender, age, occupation, internet skills, time of internet use, personality, and the awareness of cyber counseling made a difference in people's use of cyber counseling whenever they felt they needed it. However, cyber counseling seemed to fail to increase feelings of well being in people's lives. The implications of these findings are discussed in this article.
