Changes in Adolescent Health Behavior and the Exacerbation of Economic Hardship During the COVID-19 Pandemic: A Cross-sectional Study From the Korea Youth Risk Behavior Survey

  • Objectives: This study investigated the association between exacerbated economic hardship during the coronavirus disease 2019 (COVID-19) pandemic and changes in the health behaviors of Korean adolescents. Methods: We analyzed data from the 2021 Korea Youth Risk Behavior Survey and included 44 908 students (22 823 boys and 22 085 girls) as study subjects. The dependent variables included changes in health behaviors (breakfast habits, physical activity, and alcohol use) that occurred during the COVID-19 pandemic. The aggravation of economic hardship by COVID-19 and the subjective economic status of the family were used as exposure variables. Multiple logistic regression analysis was utilized to calculate the prevalence odds ratios (PORs). Results: Severe exacerbation of a family's economic hardship due to COVID-19 was negatively associated with the health behaviors of adolescents, including increased breakfast skipping (POR, 1.85; 95% confidence interval [CI], 1.55 to 2.21 for boys and POR, 1.56; 95% CI, 1.27 to 1.92 for girls) and decreased physical activity (POR, 1.37; 95% CI, 1.19 to 1.57 for boys and POR, 1.38; 95% CI, 1.19 to 1.60 for girls). These negative changes in health behaviors were further amplified when combined with a low subjective family economic status. Conclusions: The experience of worsening household hardship can lead to negative changes in health behavior among adolescents. It is crucial to implement measures that address the economic challenges that arise from stressful events such as COVID-19 and to strive to improve the lifestyles of adolescents under such circumstances.

The Relationship Between Bullying and Risk of Suicide Among Adolescents During the COVID-19 Pandemic in Indonesia

  • Objectives: Although adolescents appear less vulnerable to coronavirus disease (COVID-19), the side effects of this pandemic can still be devastating. Bullying and suicidality are significant global issues with detrimental effects on young people, particularly during school closure. This study aimed to identify the relationship between bullying and suicide risk among adolescents in Indonesia during the COVID-19 pandemic. Methods: A cross-sectional study was conducted on adolescents aged 14-18 years in May 2020 in Bandung, Indonesia, using a web-based closed survey. The Adolescent Peer Relations Instrument and the Suicide Behavior Questionnaire-Revised were used to measure bullying and risk of suicide. Multinomial logistic regression analysis was performed. Results: This study included 268 participants in 2020 and 175 participants in 2019. In 2020, the prevalence of perpetrators and victims of bullying combined was 74.6%. Meanwhile, in 2019, the prevalence of perpetrators and victims of bullying combined was 82.9%. Risk of suicide increased from 26.1% in 2019 (before the COVID-19 pandemic) to 36.5% in 2020 (during the first wave of the COVID-19 pandemic). The risk of perpetrators and suicide victims was higher than that of perpetrators and victims alone (odds ratio [OR]=4.0, 95% confidence interval [CI]=1.5-6.6 vs. OR=1.3, 95% CI=1.0-2.9 and OR=1.6, 95% CI=1.1-2.8, respectively). Conclusion: Bullying can enhance the likelihood of suicide among adolescents in Indonesia, and the risk was highest for the combination of victims and perpetrators. It is very important to provide early risk prediction for youths with bullying behavior and improve the knowledge and understanding of families and schools regarding the negative effects of bullying behavior.

  • Purpose: The objective of this study was to develop a predictive model for the sexual experiences of adolescents using the random forest method and to identify the "variable importance." Methods: The study utilized data from the 2019 to 2021 Korea Youth Risk Behavior Web-based Survey, which included 86,595 man and 80,504 woman participants. The number of independent variables stood at 44. SPSS was used to conduct Rao-Scott χ2 tests and complex sample t-tests. Modeling was performed using the random forest algorithm in Python. Performance evaluation of each model included assessments of precision, recall, F1-score, receiver operating characteristics curve, and area under the curve calculations derived from the confusion matrix. Results: The prevalence of sexual experiences initially decreased during the COVID-19 pandemic, but later increased. "Variable importance" for predicting sexual experiences, ranked in the top six, included week and weekday sedentary time and internet usage time, followed by ease of cigarette purchase, age at first alcohol consumption, smoking initiation, breakfast consumption, and difficulty purchasing alcohol. Conclusion: Education and support programs for promoting adolescent sexual health, based on the top-ranking important variables, should be integrated with health behavior intervention programs addressing internet usage, smoking, and alcohol consumption. We recommend active utilization of the random forest analysis method to develop high-performance predictive models for effective disease prevention, treatment, and nursing care.

Impact of the COVID-19 Pandemic on Tooth Brushing and Handwashing of Adolescents: A Secondary Analayis of the Korea Youth Risk Behavior Web-Based Survey (2019~2020)

  • Background: In this study, we investigated the changes and factors that affect daily health behaviors due to COVID-19 among adolescents. We intend to provide basic data so that a systematic and comprehensive health education program that includes physical and oral health can be made. Methods: This study was a secondary data analysis of the from the 2019 and 2020 Korea Youth Risk Behavior Web-based Survey. A final sample of 112,251 participants was anlayzed using chi-square test, and ordinal logistic regression. Results: In the case of tooth brushing, the frequency of 'never' increased in most general characteristics compared to before COVID-19. In the case of handwashing, it was found that handwashing behavior increased (increased) after COVID-19 compared to before COVID-19. As a result of confirming the factors affecting tooth brushing behavior before and after COVID-19, it was found that girls brushed teeth 2.2 times more regularly after lunch than boys. As a result of adjusting all other factors, regular tooth brushing behavior after COVID-19 was reduced by 0.79 times compared to before COVID-19. Conclusion: It is judged that it is necessary to operate a comprehensive health management program in the school so that the most basic tooth brushing for oral health and handwashing for health can become a habit. In addition, by confirming the impact of the COVID-19 pandemic on handwashing and tooth brushing behavior through this study, it is expected to be used in the development of policy data such as raising awareness of brushing and improving behaviors applicable in the current and upcoming new pandemic situations.

일반계고와 특성화계고 남학생의 현재 흡연 영향요인: 제13차 청소년건강행태온라인조사(2017)를 활용하여 (Factors associated with Current Smoking among Male High School Students according to School Type: Using Data from the 13th(2017) Korea Youth Risk Behavior Web-based Survey)

  • Purpose: This study was conducted to examine the prevalence of current smoking and the factors that influence current smoking among male high school students in Korea according to school type. Methods: Data were extracted from the Korea Youth Risk Behavior Web-Based Survey (2017) and multiple logistic regression was used based on complex sample analysis. The study sample comprised of 15,777 male high school students: 12,583 from general high schools (GHS) and 3,194 from specialized vocational high schools (SVHS). Results: The prevalence of current smoking was significantly higher in SVHS students (19.6%) compared to GHS students (12.6%). Low academic achievement, depressive symptoms, current drinking, and high household economic status were commonly associated with current smoking in male students from both school types. However, exposure to secondhand smoking at home, non-exposure to anti-smoking advertisement, exposure to tobacco advertisement, participation in smoking prevention or cessation education were related to current smoking in GHS students, while living in rural area was associated with current smoking in SVHS students. Conclusion: To reduce current smoking among male high school students, it is necessary to develop smoking prevention policies reflecting the factors associated with current smoking that are different by school type.

  • Objectives: The purpose of this study was to investigate the relationship between oral health factors and suicidal ideation in adolescents in order to provide basic data for adolescent suicide prevention and oral health promotion. Methods: This study used data from the Korea Youth Risk Behavior Web-Based Survey held in 2017, in which a total of 62,276 youth participated. Data were analyzed by SPSS Ver. 22.0. Complex logistic regression analysis was used to analyze oral health factors related to suicidal ideation. Results: The effects of oral health on male students' suicidal ideation were as follows: brushing teeth 2-3 times a day was associated with decreased suicidal ideation (p<0.01), while suicidal ideation was increased in individuals who had experienced scaling within the past year (p<0.001), or, among experiences of oral disease within the last year, had reported prickling and throbbing pain (p<0.001), pain on the tongue and inside the cheeks (p<0.001), or bad breath (p<0.001). The effects of oral health on female students' suicidal ideation were as follows: brushing teeth 2-3 times a day was associated with decreased suicidal ideation (p<0.001), while suicidal ideation was increased in individuals who, among experiences of oral disease within the last year, had reported pain when chewing (p<0.001), pain on the tongue and inside the cheeks (p<0.01), or bad breath (p<0.001). Conclusions: The above results demonstrate that adolescent suicidal ideation and oral health factors are correlated, and it will be necessary to incorporate oral health interventions when planning or developing projects and programs for suicide prevention in adolescents.

청소년의 건강행태 및 스마트폰 사용 특성과 비만과의 관련성 (Relationship among the Korean Adolescents Obesity, Health Behavior and Smart phone Usage)

  • Purpose: Regarding the rapid growth of the obesity in Korea's youths, environmental factors as well as biological tendencies have been major focus of years of study. Methods: This study, as a secondary analysis research, bases its source upon the raw data from the 13th on-line survey of the Korean adolescents' health behaviors. In particular, it is a descriptive research for clarifying the relevance of the Korean youths' smart phone use with their health behaviors and obesity. Results: In logistic analysis, variables showing a significant difference are as follows: soft drinks frequency(p<.001), sweet drinks intake frequency(p<.001), fast food intake frequency(p<.001) and smoking status (p=.005). Based on the amount of time the teenagers using smart phones, there have been different results in the risk of obesity. Four to six hours a week of smart phone use (p=.002) has a 1.132 times higher risk, six to eight hours(p<.001) are 1.212 times higher, over eight hours(p=.020) are 1.132 times higher, than less than two hours'. Conclusion: This study has significance in that it has emphasized the new lifestyle, teenagers' smart phone use, as an additional important factor for the increased risk of obesity.

성관계와 관련된 성적지향이 고등학생의 흡연행위에 미치는 영향 (Smoking Prevalence according to Sexual Orientation among Korean High School Students)

  • Objectives: This study was to investigate the effects of sexual orientation related to sexual intercourse on smoking behavior. Methods: The study includes 33,309 Korean high school students. Of these, 31,213 had no experience of sexual intercourse, 1,739 had sexual intercourse with the heterosexual, 191 had sex with the homosexual, and 166 had sexual intercourse with the bisex. The study used 12th(2016) Korea Youth Risk Behavior Web-based Survey with complex sample logistic regression. Results: Adjusted confounding variables were analyzed, and it showed that smoking behavior was higher in the group had experienced sexual intercourse. The group had homosexual intercourse (OR=1.84, 95%CI=1.31-2.57) or bisexual intercourse (OR=2.78, 95%CI=2.06-3.76) showed higher risk of smoking behavior than the group had heterosexual intercourse (OR=1.67, 95%CI=1.48-1.90 Conclusions: The study results showed sexual orientation related to sexual intercourse was associated with smoking behavior. Further follow up studies and evaluations are needed.

한국 청소년의 건강관련행태와 구강질환 증상경험의 관련성 (Relationship between health behaviors and oral symptoms in Korea adolescents)

  • Objectives: The purpose of this study is to provide basic data on the developement of oral health programs to maintain correct oral health in adolescence by identifying the relevance of health-related behavior and symptom experience to adolescents. Methods: Using the 14th(2018) online survey of youth health behavior, this study was conducted on students in grades1 middle school to grade5 high school in Korea and 30,463 boys, 29,577 girls were selected as the final analysis targets. Results: During the oral disease symptom experience, there was a statistically significant difference in smoking experience with teeth pain controlled general characteristics during eating. Conclusions: It is expected that it will be used as basic dat for the development of oral health programs to maintain the correct oral health status of adolescents by identifying the effects of health-related behaviors and related variables on oral diseases in Korea.

  • Purpose: This study was conducted to compare and analyze the smoking-related social-ecological factors affecting attempts to quit smoking by adolescents using tobacco cigarettes only and also those adolescents using tobacco cigarettes along with electronic cigarettes. Methods: This study, as secondary analysis research, used the raw data from the 14th Korea youth risk behavior survey 2018. The data was analyzed by frequency analysis, the Rao-scott χ2-test and logistic regression analysis when considering the complex sample's analysis. Results: On logistic regression analysis, during their first smoking period, intense physical activities and having friends who smoked were associated with significantly more attempts to quit smoking by cigarettes smokers, and their first smoking period and experiences of undergoing smoking cessation education were associated with significantly more attempts to quit smoking for dual smokers of both tobacco cigarettes and electronic ones. Conclusion: As a result of this study, we identified the need to differentiate different types of smoking cessation counseling and education according to the types of smoking in adolescents.
