Comparative Observational Study on the Effects of Intra-articular Hominis Placenta Pharmacopuncture and Acupoint Hominis Placenta Pharmacopuncture for Knee Osteoarthritis Patients

  • The aim of this case report was to observe the effects of intra-articular hominis placenta pharmacopuncture (HPP). Based on the medical records patients who received intra-articular treatment or received acupoint pharmacopuncture treatment, a comparison was made. There were 35 patients who were hospitalized for degenerative osteoarthritis of the knee joint from the 1st October 2019 to 26th September 2020. There were 14 patients who were treated with HPP in the intra-articular joint space (Group A), and 14 patients who were treated with HPP at specific acupoints (Group B). The outcome effects were measured using the Korean Western Ontario and Mc (KWOMAC) the visual analog scale (VAS) before the first treatment, and after the fifth treatment. The KWOMAC (p < 0.001) and the VAS scores (p < 0.001) in Groups A and B significantly improved after treatment compared with before treatment. When comparing Group A improvement with Group B improvement using the KWOMAC there was no statistically significant difference however, when using the VAS scores, Group A treatment was statistically more effective compared with Group B (p = 0.002). This study indicated that HPP may be an effective treatment for knee osteoarthritis. Moreover, intra-articular HPP may be more effective than acupoint HPP for knee osteoarthritis.

Effects of Korean Traditional Medicine Treatment on Spontaneous Osteonecrosis of the Knee: A Case Report

  • Spontaneous osteonecrosis of the knee (SONK) is a common form of osteonecrosis of the knee and mainly affects the medial condyle due to localized vascular insufficiency. We report a case of SONK in a 65-year-old woman who had severe knee pain in her left knee whichimpeded her capacityto walk beyond 10 minutes. Bilateral knee X-rays revealed degenerative osteoarthritis of both knees and magnetic resonance imaging revealed R/O SONK in the lateral aspect of the medial femoral condyle, as well as a medial meniscal posterior horn root tear, and a Grade 1 medial collateral ligament injury. She was hospitalized at Jaseng Hospital of Korean Medicine for 21 days and receivedcombination therapy includingacupuncture, pharmacopuncture, and herbal medicine. Patient-reported scales indicated that her pain and physical functional limitations were alleviated. Combination therapy consisting of Korean traditional medicine may bean alternative nonoperative treatment approach for patients with SONK.

퇴행성 무릎관절염 변증도구의 신뢰도 평가 및 무릎 ROM, VAS, WOMAC과의 상관관계에 대한 탐색적 연구 (A Pilot Study to Evaluate the Reliability of Pattern Identification Tool for Knee Osteoarthritis and to Analyze Correlation between Pattern Identification Tool and Knee Range of Motion, Visual Analog Scale and Western Ontario & Mcmaster Universities Osteoarthritis Index)

  • Objectives To evaluate the pattern identification tool for knee osteoarthritis and to investigate the relationship between pattern identification tool and knee ROM (range of motion), VAS (visual analog scale) and WOMAC (Western Ontario & McMaster Universities osteoarthritis index). Methods We studied 50 patients who are diagnosed as degenerative osteoarthritis. With one patient dropping out, total of 49 patient went through pattern identification tool. Re-test of the pattern identification tool was implemented after about a week. The reliability was analyzed by calculating ICC (intraclass correlation coefficient). Also, reliability of each pattern identification score was calculated along with influence factor and correlation between knee ROM, VAS and WOMAC. Results Reliability of the pattern identification tool was evaluated as 'poor agreement beyond chance' with ICC value of 0.396. Reliablility of each pattern identification score was calculated and four out of five were measured 'good' with one 'moderate' reliability. Two of inter-item consistency were 'good', two were 'acceptable' and one was 'questionable'. Correlation between each pattern identification and knee ROM showed 'clear negative linear relationship' with two patterns. VAS showed 'clear positivie linear relationship' with all five patterns. WOMAC showed 'positivie linear relationship' with all five patterns. Conclusions According to the results, pattern identification tool for knee osteoarthritis seems to have meaningful relationship with other agnedas. Also, further research is needed to develop the tool.

뜸요법과 세라밴드운동이 퇴행성 골관절염 노인의 신체적, 심리적 변수에 미치는 효과 (Effects of Moxibustion and Theraband Exercise on Physical and Psychological Variables of the Aged with Degenerative Osteoarthritis)

  • Purpose: To examine the effects of a nursing intervention including moxibustion and theraband exercise on pain, fatigue, flexibility, and depression in aged patients with degenerative osteoarthritis. Methods: This study had a one group pretestposttest design. Thirty people over 60 yr-of-age with degenerative osteoarthritis were recruited from a community health center from November 1, 2005 to June 14, 2006. The 6 week treatment intervention included twice weekly 60 min sessions of of theraband exercise (30 min) and moxibution (30 min). Data was analyzed by SPSS Ver. 12.0. Results: Intervention was associated with significant decreases in pain (t=8.28, p<.001) and fatigue (t=5.19, p<.001), and significant increases in knee flexibility (t=-3.54, p=.001), dorsal flexibility (t=-9.05, p<.001), and plantar flexibility (t=-4.09, p<.001). Depression was significantly decreased after intervention (t=-4.09, p<.001). Conclusions: Moxibustion and theraband exercise should be utilized as an effective nursing intervention for the aged with degenerative oateoarthritis.

천수근 약침을 이용한 퇴행성 슬관절질환 치험 1례 (The Case Report of Degenerative Diseases of Knee Joint with Harpagophytum Radix Pharmacopuncture)

  • Objectives : The purpose of this clinical case is to evaluate the efficacy of Harpagophytum radix. Pharmacopuncture on Degenerative diseases of knee joint. Methods : We used Korean Medicine treatment and Harpagophytum radix. pharmacopuncture, for this patients. This case is measured and assessed by Neumerical Rating Scale(NRS), Korea Western Ontario MacMaster Universities Osteoarthritis Index(KWOMAC) and Physical examination of knee joint. Results : The NRS decreased from 8 to 5(Left knee joint) & 5 to 4(Right knee joint) and KWOMAC total points decreased from 77 to 41. Also Physical examination of knee joint improved. Conclusions : Harpagophytum radix. pharmacopuncture with Korean Medicine treatment is proved to be helpful to the pain relief and function recovery of knee joint.

봉독약침이 Monosodium Iodoacetate로 유발된 흰쥐의 골관절염에 미치는 영향 (Effects of Bee-venom Treatment on the Monosodium iodoacetate-induced Osteoarthritis in Rats)

  • This study was to investigate the effects of Bee-venom Treatment on the monosodium iodoacetate(MIA)- induced osteoarthritis in rats. Arthritis was induced by injection of MIA(0.5 mg) into knee joints of rats. Arthritic rats were divided into control(n=8) and treated(n=8) group. Control group was injected with normal saline once a day for 20 days, while treated group was injected with Bee-venom extract once a day for same duration. Body weights were measured at 0, 5, 10, 15, 20 days after injection. At the end of experiment, gross and histopathological examination on the articular cartilages of the knee joints were performed. Proteoglycan contents of articular cartilages were analysed by safranine O staining method. The contents of $TNF-{\alpha}$, $IL-1{\beta}$ and IL-6 in synovial fluids were analysed by ELISA method. And also, COX-2 and iNOS immunohistochemical examination on the knee joints were performed. Body weights of the treated group were increased compared with control group at 20 days after injection. Grossly, the severity of osteoarthritis in the treated group were alleviated compared with control group. PG contents in articular cartilages of the treated group were significantly increased compared with control group. Histopathologically, degenerative and necrotic lesion of articular cartilages in the treated group were alleviated compared with those of the control group. $TNF-{\alpha}$ contents in synovial fluids of the treated group were decreased compared with control group. Positive reactions of COX-2 in chondrocytes and synovial membranes of the treated group were decreased compared with the control group. On the basis of these results, we concluded that Bee-venom treatment has anti-arthritic effects on the monosodium iodoacetate-induced osteoarthritis in rats. And it's effects were related with reduced secretion of $TNF-{\alpha}$ and COX-2 from osteoarthritic chondrocytes and synovial membranes.

Monosodium Iodoacetate 유발 골관절염 모델: 장기적인 약물효능 평가에 적합한가? (Osteoarthritis Model Induced by Monosodium Iodoacetate: Good Model for Long-term Drug Efficacy Test)

  • Objectives This study was aimed to offer basic data in long term drug efficacy test using monosodium iodoacetate-induced osteoarthrits model. Methods Sixty male rats were divided into normal and osteoarthritic group. Rats of normal group were injected with 0.1 ml physiological saline, and rats of osteoarthritic group were inected with 0.1 ml monosodium iodoacetate (3 mg/ml) into each left and right knee joint cavities. Gross examination, proteoglycan contents and histopathological examination on the knee joint were performed at 10, 20, 40, and 60 days after injection. Results Grossly, degenerative changes at 10 days, desquamation at 40 days, and ulceration of articular cartilages at 60 days were observed. Proteoglycan contents in articular cartilages were decreased rapidly to 40 days, after than decreased gradually. Osteoarthritic scores were increased rapidly to 20 days, after than increased gradually to 60 days. Conclusions From above results, osteoarthritis model induced by a single intra-articular injection of monosodium iodoacetete is useful model for long term drug efficacy test.

  • Arthritis may be broadly classified as degenerative - related to defects in cartilage and other joint constituents, often age-associated - or inflammatory disease. Inflammatory arthritis called as rheumatoid arthritis (RA) is a chronic inflammatory arthropathy and characterized by a destructive arthritis. RA encompasses infectious arthritis, arthritis caused by intra-articular deposits of crystalline material (gout), syndromes associated with genetic defects (familial Mediterranean fever), and the immune-mediated inflammatory arthropathy. Degenerative arthritis called as osteoarthritis (OA), which is most frequently occurring, causes degenerative figures of knee, waist and knuckle, and accompanies severe pain around the cartilage. Also, it may cause morning stiffness, gelling effect, tenderness, bone swelling, crepitus, and motion disorders.

퇴행성 슬관절염에 대한 근위취혈과 원위취혈 침 치료의 효과 비교에 대한 임상연구 (A Randomized Clinical Trial of Local Acupoints Compared with Distal Acupoints in Degenerative Osteoarthritis on Knee)

  • Objectives : To compare the efficacy of local acupoint with distal acupuncture at relieving pain and improving function in knee osteoarthritis. Designs : A randomized, single-blinded, crossover clinical trial. Settings : One outpatient clinic(department of acupuncture & moxibustion) located in academic teaching hospital, South Korea. Patients : 17 patients with osteoarthritis of the knee(mean age 62.76[$SD{\pm}4.37$] years). Interventions : The trial had 4 stages : baseline(2weeks), phase I and II(each 2weeks), washout period(2weeks). Patients were randomly assigned to either group A or group B. Group A received acupuncture at local acupoints during phase I, then acupuncture at distal acupoints in phase II. Group B received the treatments in reverse order. In each phase, the patients were treated with acupuncture for 6 times. Measurements : The primary outcome was subjective pain as measured by a 100mm visual analogue scale(VAS) ranging from 0(no pain) to 10(worst pain ever). Secondary outcomes were changes in the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC) total and pain scores. Measurements were obtained at baseline, 1st day of phase I and II, and 2 days after last treatment of phase I and II. Results : The 17 participants in 2 groups were well matched for age, sex, target knees, baseline VAS score, WOMAC pain score and WOMAC score. Participants in local acupoint group experienced greater improvement than distal acupoint group at 2 days after last treatment in WOMAC total score(mean difference, -10.65[95% CI, -20.56 to -0.74] ; P=0.036) but not in VAS(mean difference, -12.41[95% CI, -29.56 to 4.73] P=0.15) and WOMAC pain score(mean difference, -1.82[95% CI, -3.98 to 0.33] ; P=0.094). Conclusions : Local acupoints are more effective than distal acupoints at relieving pain and improving function in knee osteoarthritis.

퇴행성 골관절염에 대한 HPL-04의 효과 (Effects of HPL-04 on Degenerative Osteoarthritis)

  • 본 연구에서는 HPL-04가 골관절염의 예방 및 치료 약물로서의 가능성을 탐색하여 다음과 같은 결론을 얻었다. HPL-04는 연골세포 생존율과 연골형성과 관련된 collagen type II, SOX 9 그리고 aggrecan의 유전자 발현을 유의성 있게 증가시킬 뿐만 아니라 염증성 인자와 관련 있는 MMP-2, 9도 유의성 있게 감소시켰다. HPL-04는 MIA에 의해 유도된 퇴행성 골관절염에서 관절연골의 파괴와 골 침식 등 연골의 변성을 억제했으며 proteoglycan의 소실을 유의성 있게 감소시켰다. 이에 본 연구는 HPL-04가 부작용이 적고 약리효과가 뛰어나 골관절염 예방 및 치료제 개발에 활용될 수 있을 것으로 본다.
