식이내 단백질 수준이 성장기 흰 쥐의 신장 기능에 미치는 영향 (The Effect of Level of Dietary Protein on Kidney Development and Function in Growing Rats)

  • 본 연구에서는 식이단백질 수준이 성장기 흰쥐의 신장 발달과 기능에 미치는 영향을 조사하기 위해 평균체중이 97.5$\pm$1.9g인 숫컷 흰쥐를 5%, 15% 그리고 50% casein 식이로 6주간 사육하였다. 체중증가량은 단백질 수준이 높을수록 높았으며 특히 단백질수준이 신장무게에 미친영향이 간, 비장, 흉선 등이 다른 장기에 비해 컸다. 신장내 총 DNA와 RNA 함량에서는 유의적인 차이가 없었다. 총 protein과 단위 g당 protein 함량은 식이내 단백질수준이 높을 수록 증가했다. 사구체여과율(GFR)은 식이단백질 수준이 높을수록 높았으나 신혈류(RBF)는 각 군 사이에 유의적인 차이를 보이지 않았다. 뇨질소배설량은 단백질 수준에 비례하여 유의적인 차이를 보였고 노칼슘배설량은 5%군이 낮은 경향을 보였다. 본 연구 결과로 성장기의 흰쥐에서 고단백식이는 신장의 무게증가를 촉진하며 사구체여과율을 증가시킴을 알 수 있다.

Can distinction between the renal cortex and outer medulla on ultrasonography predict estimated glomerular filtration rate in canine chronic kidney diseases?

  • Background: Quantitative evaluation of renal cortical echogenicity (RCE) has been tried and developed in human and veterinary medicine. Objectives: The objective of this study was to propose a method for evaluating RCE quantitatively and intuitively, and to determine associations between ultrasonographic renal structural distinction and estimated glomerular filtration rate (eGFR) in canine chronic kidney disease (CKD). Methods: Data were collected on 63 dogs, including 27 with normal kidney function and 36 CKD patients. Symmetric dimethylarginine and creatinine concentrations were measured for calculating eGFR. RCE was evaluated as 3 grades on ultrasonography images according to the distinction between the renal cortex and outer medulla. The RCE grade of each kidney was measured. Results: There was a significant difference in eGFR between the group normal and CKD (p < 0.001). As mean of RCE grades (the mean values of each right and left kidney's RCE grade) increases, the proportion of group CKD among the patients in each grade increases (p < 0.001). Also, severity of RCE (classified as "high" if any right or left kidney evaluated as RCE grade 3, "low" otherwise) and eGFR is good indicator for predicting group CKD (p < 0.001). Conclusions: The degree of distinction between the renal cortex and the outer medulla is closely related to renal function including eGFR and the RCE grade defined in this study can be used as a method of objectively evaluating RCE.

$^{99m}Tc$-DMSA 신장검사에서 획득 계수의 차이에 따른 기능 및 형태 평가 (The Quantitative Assessment of Renal Function and Size by Differences of Acquisition Counts in $^{99m}Tc$-DMSA Renal Scan)

  • 핵의학 검사에서 정적 영상을 획득하는 방법은 총 계수 설정법(preset count method)과 시간 설정법(preset time method)이 있다. $^{99m}Tc$-DMSA를 이용한 정적 신장 검사는 총 계수 설정법을 주로 사용하고 있지만 시간 설정법을 사용하는 곳도 있다. 총 계수 설정법을 사용하는 경우는 매번 동일한 계수를 획득하게 되지만 검사 시간은 차이가 있고, 시간 설정법의 경우는 검사 시간은 일정하지만 동일하지 못한 계수가 획득되게 된다. 따라서, 본 연구에서는 획득되는 계수에 따른 좌우 신장의 기능 및 형태 정보에 어떠한 차이가 있는 지 알아보고자 하였다. 2010년1월11일부터 3월31일까지 서울아산병원 핵의학과에서 $^{99m}Tc$-DMSA 신장 검사를 시행한 환자 중 한쪽 신장의 상대적 기능이 40~60%인 30명(남:여=11:19)의 환자를 대상으로 하였으며, 신장 내에 냉소나 열소가 존재하여 신장 피질의 구분이 명확하지 않은 경우는 제외하였다. 대상자의 나이는 $14.83{\pm}22.07$세로서 성인과 소아를 따로 구분하지 않았으며, 검사 시 이용한 장비는 PHILIPS사의 BrigthView 감마카메라를 사용하였다. 신장의 기능 및 형태 분석을 위해서는 동일 회사의 JET stream release 3.0 version을 이용하였고, 통계 분석은 SPSS 12.0을 사용하여 기술통계, 대응 표본 t 검정 (paired t-test)을 시행하였다. 영상은 다른 검사인자들을 모두 동일하게 설정하고 획득 계수를 10만 계수와 30만 계수, 그리고 획득 시간을 7분 (30만 계수 초과)으로 변경하여 연속적으로 획득하였고, 신장의 기능 및 형태 정보를 평가하기 위해서 좌우 신장의 상대적 기능(renal relative function)과 기하평균(geometric mean) 그리고 신장의 크기(Size)를 측정하였으며 그에 따른 각각의 차이를 비교 분석하였다. 좌우 신장의 상대적 기능(좌측 신장 기준)의 경우 10만 계수는 $50.52{\pm}3.61%$, 30만 계수는 $50.38{\pm}3.66%$, 7분 획득은 $50.31{\pm}3.64%$였으며 각 방법 간에 통계적으로 유의한 차이는 없었다. 기하 평균(좌측 신장 기준)의 경우 10만 계수는 $50.08{\pm}3.25%$, 30만 계수는 $49.89{\pm}3.40%$, 7분 획득은 $49.91{\pm}3.24%$였으며 이 역시 통계적으로 유의한 차이는 없었다. 신장의 크기 중 좌측의 경우 10만 계수는 $8.23{\pm}1.96$ cm, 30만 계수는 $8.35{\pm}1.97$ cm, 7분 획득은 $8.45{\pm}2.01$ cm이었고, 각 방법 간에는 통계적으로 유의한 차이를 나타내었다(p<0.05). 우측 신장의 경우 10만 계수는 $7.91{\pm}1.88$ cm, 30만 계수는 $8.12{\pm}1.90$ cm, 7분 획득은 $8.25{\pm}1.96$ cm이었으며, 각 방법 간에는 통계적으로 유의한 차이를 나타내었다(p<0.05). 본 연구 결과에 의하면 획득 계수에 따른 좌우 신장의 상대적 기능과 기하 평균은 통계적으로 유의한 차이가 없었지만, 신장의 형태는 획득 계수가 증가할수록 크기가 증가하는 경향을 보였으며 통계적으로도 유의한 차이를 나타내었다. 따라서, 정적 신장 검사의 신뢰성 있는 정량 평가를 위해서는 시간 설정법보다 총 계수 설정법을 사용하는 것이 바람직할 것이다. 특히, 추적검사의 경우는 시간 설정법을 사용하면 검사 시마다 획득되는 계수가 다르게 되어 신장의 형태 평가에 차이를 발생시킬 것이다.

경기 남부 일개 병원에 입원한 코로나 19 환자들의 신기능 현황 (Status of Kidney Function in Hospitalised COVID-19 Patients in the Southern Gyeonggi Province, South Korea)

  • 코로나바이러스 감염증 19는 중증급성호흡기증후군 코로나바이러스 2의 명칭에서 명명되었다. 이 연구는 코로나바이러스 감염증 19 환자의 신기능 상태를 조사하는 것을 목적으로 하였다. 본 연구는 경기도 남부 한 병원에서 코로나바이러스 감염증 19로 입원한 649명 환자를 대상으로 하였다. 이번 연구에서 검토한 코로나바이러스 감염증 19 환자의 혈액요소질소와 크레아티닌은 증가된 패턴을 보였다. 본 연구의 대상자 중 남성의 혈액요소질소는 정상 범위보다 높았다. 특히 60대, 80대 이상에서 높게 나타났고, 크레아티닌 수치도 비슷하게 나타났다. 이 연구에서 환자의 전해질 검사는 정상 범위에서 벗어나는 결과는 관찰되지 않았다. 이 연구의 대상에서 80세 이상의 남성은 60 mL/min/1.73 m2 이하의 만성신장질환 역학협력 사구체여과율을 나타냈다. 최근 연구에 따르면 COVID-19의 일부 심각한 사례는 코로나바이러스에 감염되기 전에 기저 신기능의 문제가 없었던 사람들에게도 콩팥 손상의 징후를 보이고 있다. 따라서 여러 지표 평가의 신기능 검사는 검사결과의 검토는 코로나19 환자의 비정상 신기능 발견에 도움이 될 수 있다.

개의 신장기능에 미치는 Guanabenz의 영향 (Effects of Guanabenz on Renal Function in Dog)

  • In this study attempts were made to observe the effects of guanabenz on renal function in dog, which manifests the antihypertensive action by inhibition of sympathetic tone through stimulating the presynaptic adrenoceptor (${\alpha}_2-adrenoceptor$). Guanabenz, when injected at a dose of $30.0{\mu}g/kg$, or infused at a dose of $3.0{\mu}g/kg/min$ intravenously, produced diuretic action with increased amounts of $Na^+\;and\;K^+$ in urine, and with decreased reabsorption rates of $Na^+\;and\;K^+$ in renal tubules. It was also observed that the rates of osmolar and free water clearances were increased, but the glomerular filtration rate and renal plasma flow were not changed. Guanabenz injected at a dose of $3.0{\mu}g/kg$ into a carotid artery or infused intravenously at a dose of $3.0{\mu}g/kg/min$ in a state of water diuresis elicited the diuretic action of the similar aspect as a case of guanabenz given intravenously. The diuretic action produced by guanabenz was completly blocked by pretreatment of i.v. prazosin, ${\alpha}_1-adrenoblocking$ agent, or of i.v. yohimbine, ${\alpha}_2-adrenergic$ blocking agent. Prazosin, when given into a renal artery, inhibited the diuretic action by i.v. guanabenz in only injected kidney, whereas in case of yohimbine the action was inhibited in both kidney. Guanabenz infused at a dose of $1.0{\mu}g/kg/min$ into a renal artery exhibited no significant changes of renal function in both kidney. In denervation experiments, guanabenz given intravenously produced typical diuretic action in innervated kidney, whereas in denervated kidney, it did not affect the action at initial period but exhibited the action with increase of only free water clearance at later period. These results suggest that guanabenz produced diuretic action in dog by inhibition of electrolyte reabsorption rates in renal tabules, mainly proximal tubule and of ADH release, which is mediated by stimulating of central sympathetic ${\alpha}_2-receptor$.

주산기 가사가 있는 신생아에서 theophylline의 신기능 개선 효과에 대한 체계적 문헌고찰 및 메타분석 (The Effect of Theophylline on Improvement of Renal Function in Asphyxiated Neonates: A Systematic Review and Meta-analysis)

  • The purpose of this systematic review and meta-analysis was to assess the preventive effect of theophylline on acute kidney injury and the ameliorative effect of theophylline on renal function in asphyxiated neonates. A literature search of the PubMed/Medline, Embase, and Cochrane Library databases for information published up to February 2019 was conducted. All studies that reported the incidence rate of acute kidney injury, serum creatinine level, and glomerular filtration rate after the randomized administration of theophylline or placebo were included. In total, eight studies involving 498 neonates were eligible. The incidence rate of acute kidney injury was significantly lower in the theophylline group than in the placebo group (risk ratio [RR]: 0.42, 95% confidence interval [CI]: 0.32-0.55, p < 0.001). The changes in serum creatinine level in the theophylline group were significantly higher than those in the placebo group from the first day of life to 3 and 5 days of age (weighted mean difference [WMD]: -0.51, 95% CI: -0.62 to -0.40, p < 0.001, and WMD: -0.26, 95% CI: -0.34 to -0.18, p < 0.001, respectively). The changes in glomerular filtration rate in the theophylline group were significantly higher than those in the placebo group from the first day of life to 3 days of age and the last day of follow-up (WMD: 12.30, 95% CI: 9.39-15.21, p < 0.001, and WMD: 9.35, 95% CI: 6.43-12.27, p < 0.001, respectively). These results suggested that theophylline has a beneficial effect on the prevention of acute kidney injury in neonates with perinatal asphyxia.

대학생 체질량지수와 혈액검사결과 상 비만 분류군간 특성 및 비만 연관 요인 분석에 대한 연구 (The Characteristics in Obesity Classification Group of College Student by Analyzing Their BMI and Blood Test and the Association between Factors Contributing to Obesity and Obesity according to BMI)

  • Objectives The purpose of this study was to investigate the characteristics in obesity classification group of college students having health checkup by analyzing their BMI and blood test and determine the association between factors contributing to obesity and obesity according to BMI. Methods A group of 2992 test subjects took their medical examination and their body composition, height, weight, blood pressure were measured and blood test was done. With these results we diagnosed obesity, and analysed relationship between obesity and cholesterol, triglyceride, LDL (low density lipoprotein)-cholesterol, HDL (high density lipoprotein)-cholesterol, liver function, renal function and blood pressure. Results 1. Overweight individuals showed high total cholesterol, LDL-cholesterol, triglyceride, GOT (glutamic oxalacetic transaminase), GPT (glutamic pyruvate transaminase), Urea-nitrogen, Creatinine and low HDL-cholesterol. 2. BMI showed a significant association with other factors. BMI has a negative correlation with sex and HDL-cholesterol. BMI had positive correlations with other factors. 3. There was no relation between BMI and sex. Those six factors, liver function, cholesterol, blood pressure, kidney function, neutral fat, and blood sugar were related to BMI. Conclusions There was a significant relation between college students' BMI and their liver function, cholesterol, blood pressure, kidney function, neutral fat, and blood sugar. The diseases related to liver function, cholesterol, blood pressure, kidney function, neutral fat, and blood sugar were clearly associated with obesity.

복류(復溜).음곡혈(陰谷穴)의 주치별(主治別) 차이(差異)에 대(對)한 문헌적(文獻的) 비교(比較) 고찰(考察) (A Philological Comparative Study on the Difference of the Chief Virtue of Puryu(KI7) and Um-gok(KI10))

  • Objectives: The aim of the present study was to widen a clinical use by investigating literatures about the acupoint of Pu-ryu(KI7) and Um-gok(KI10) concerning Kidney-Eum(vital essence of the kidney) and Kidney-Yang(vital function of the kidney). Methods: We investigated the first literature about $Pu-ryu(KI7)\;{\cdot}\;Um-gok(KI10)$ and a second name, a location and a characteristic of them. We made a comparative study about the chief virtue and combination of $Pu-ryu(KI7)\;{\cdot}\;Um-gok(KI10)$. Results: Pu-ryu(KI7) is the 7th acupoint of Kidney Meridian of Foot Soeum(少陰), which reinforces a meridian of belonging and has the efficacy of warming the Kidney Yang, clearing heat, excreting dump and regulating water passage. Um-gok(KI10) is the 10th acupoint of Kidney Meridian of Foot Soeum(少陰), which has the virtue of nurishing the Liver and Kidney Eum, promoting lower heater and marinating the free flow of Gi Conclusions: The chief virtue of Pu-ryu(KI7) is to remove edema due to disturbance in Gi activity by dificiency of Kidney-Yang because of warming Yang to induce diuresis. To Um-gok(KI10), it is to treat instability of Kidney-Gi by Eum dificiency of the Liver and Kidney because of nurishing the Liver and Kidney Eum.

Prevalence of autoantibodies that bind to kidney tissues in cats and association risk with antibodies to feline viral rhinotracheitis, calicivirus, and panleukopenia

  • Background: The feline viral rhinotracheitis, calicivirus, and panleukopenia (FVRCP) vaccine, prepared from viruses grown in the Crandell-Rees feline kidney cell line, can induce antibodies to cross-react with feline kidney tissues. Objectives: This study surveyed the prevalence of autoantibodies to feline kidney tissues and their association with the frequency of FVRCP vaccination. Methods: Serum samples and kidneys were collected from 156 live and 26 cadaveric cats. Antibodies that bind to kidney tissues and antibodies to the FVRCP antigen were determined by enzyme-linked immunosorbent assay (ELISA), and kidney-bound antibody patterns were investigated by examining immunofluorescence. Proteins recognized by antibodies were identified by Western blot analysis. Results: The prevalences of autoantibodies that bind to kidney tissues in cats were 41% and 13% by ELISA and immunofluorescence, respectively. Kidney-bound antibodies were observed at interstitial cells, apical border, and cytoplasm of proximal and distal tubules; the antibodies were bound to proteins with molecular weights of 40, 47, 38, and 20 kDa. There was no direct link between vaccination and anti-kidney antibodies, but positive antibodies to kidney tissues were significantly associated with the anti-FVRCP antibody. The odds ratio or association in finding the autoantibody in cats with the antibody to FVRCP was 2.8 times higher than that in cats without the antibody to FVRCP. Conclusions: These preliminary results demonstrate an association between anti-FVRCP and anti-cat kidney tissues. However, an increase in the risk of inducing kidney-bound antibodies by repeat vaccinations could not be shown directly. It will be interesting to expand the sample size and follow-up on whether these autoantibodies can lead to kidney function impairment.

The Potential of Renal Progenitor Cells in Kidney Diseases: Preclinical Findings

  • The kidney is a highly complex organ, and acute or chronic renal diseases can occur with various complications such as diabetes and hypertension. So far, no target specific treatment is available in acute or chronic renal failure, necessitating the development of alternative therapeutic strategy. Recent experimental findings suggest that the renal function and structure can be restored after being treated with various sources of stem/progenitor cells. In this review, we discuss up-to-date findings of the potential of renal progenitor/stem cells in alleviating renal injuries with a focus on preclinical studies. We also review cellular mechanisms underlying the therapeutic function of these cells.
