  • Objectives : The Phrase, Ki goes to Essence and Essence goes to Change in Eumyangeungsangdaeron in Suwen have contradiction in direction of Ki. Because this phrase linked to relation of Taste, Shape, Ki, Essence and Change, each others are supposed to be treated systematically. Methods : We will review on annotations of past doctors and new opinions of resent scholars. Results : Ki has two meanings in this phrase ; Ki of food and Ki of ability in human nature. Conclusions : From the Taste through Shape to Ki of food are the way of Eum aspect and from the Ki of ability through Essence to Change are the way of Yang aspect.

Recovery of High Purity KI Crystal from Aqueous Waste of Polarizing Film Manufacturing Process (LCD용(用) 편광(偏光)필름제조폐용액(製造廢溶液)으로부터 고순도(高純度) KI결정(結晶) 회수(回收)에 관한 연구(硏究))

  • A laboratory study was carried out to recover KI crystals with high purity from a waste solution generated from the production of polarizing film for LCD industry. The waste solution contains 1 to 4% KI, and other impurities such as B, PVA and etc. More than 95% purity of KI crystals were produced through refining process such as vacuum evaporation and fractional crystallization. Most of B compounds and impurities were removed by concentrating the waste solution until KI content reached about 50%. The KI crystals were washed with solvents to remove most of PVA which gave result in producing 99.5% purity of crystals. The overall recovery of KI was about 90% during the concentration process.

Ki-67 Can Predict the Response to the Gemcitabine, Oxaliplatin And L-asparaginase Regimen (GELOX) and Prognosis in Patients with Nasal Natural Killer/T-cell Lymphoma

  • GELOX (gemcitabine, oxaliplatin and L-asparaginase) regimen showed an impressive result in our previous study, but the effect of this new regimen is still dissatisfying for some patients, so it is necessary to identify which patients will benefit from this regimen. A total of fifty-one cases with nasal natural killer/T-cell lymphoma receiving initial GELOX chemotherapy were enrolled in this study. The ki-67 expression detected by immunohistochemistry (IHC) in the specimens ranged from 10% to 90%, with a median value of 70%, so cases higher than the median value (${\geq}70%$) were defined as high ki-67 expression, and the others were designated as low ki-67 expression. The response rate had no statistical difference between low ki-67 expression group and high ki-67 expression group (P=0.291) though the value in the former group was relatively high. After a median follow-up of 18.03 months, the 3-year progression-free survival (PFS) for patients with low ki-67 expression was significantly higher than those with high ki-67 expression (83.8% vs. 47.9%, P=0.038). In the stage I/II subgroup, 3-year PFS and overall survival (OS) were statistically higher in the patients with low ki-67 expression than those with high ki-67 expression. Multivariate analysis revealed high ki-67 expression was an independent prognostic factor for PFS. These results suggest that low ki-67 expression can predict a good response of GELOX in these patients, and the combination of ki-67 expression and early stage is helpful to identify an excellent prognosis subgroup from patients receiving GELOX in this disease.

Assessment of Ki-67 for Predicting Effective Prognosis in Breast Cancer Subtypes

  • Ki-67 has been widely performed and become an important biomarker in worldwide clinics, but the standard cut off value of Ki-67 index in breast cancer is still controversy. The objective study was to understand the Ki-67 in breast cancer subtypes and to investigate relative risk of breast cancer subtypes according to Ki-67 cut off value in Korean breast cancer. Immunohistochemical staining (IHC) for estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2), and Ki-67 index was examined from 123 breast cancer patients. Ki-67 index was significantly overexpressed in PR, ER, and HER2 hormone negative groups. Ki-67 index in Triple negative and HER2 subtypes was shown significantly higher than that in Luminal A and Luminal B subtype. Then, we compared the relative risk of each subtype according to 14% and 20% Ki-67 cut off value, which were applied in most clinics. Especially, 20% Ki-67 cut off value in HER2 and Triple negative subtypes was shown 8.41 fold and 2.83 fold higher relative risk than this in Luminal A subtype. Moreover, Ki-67 index in HER2 2+ or 3+ status showed significantly overexpressed than this in HER2 1+ status. At the 20% Ki-67 cut off value, HER2 1+ or 2+ status and 3+ status showed significant difference. Therefore, the 20% Ki-67 cut off value will be useful as a precise prognostic management and helpful for interpreting diverse outcomes of other subtypes in breast cancer patients.

  • Objectives: The aim of the present study was to widen a clinical use by investigating literatures about the acupoint of Pu-ryu(KI7) and Um-gok(KI10) concerning Kidney-Eum(vital essence of the kidney) and Kidney-Yang(vital function of the kidney). Methods: We investigated the first literature about $Pu-ryu(KI7)\;{\cdot}\;Um-gok(KI10)$ and a second name, a location and a characteristic of them. We made a comparative study about the chief virtue and combination of $Pu-ryu(KI7)\;{\cdot}\;Um-gok(KI10)$. Results: Pu-ryu(KI7) is the 7th acupoint of Kidney Meridian of Foot Soeum(少陰), which reinforces a meridian of belonging and has the efficacy of warming the Kidney Yang, clearing heat, excreting dump and regulating water passage. Um-gok(KI10) is the 10th acupoint of Kidney Meridian of Foot Soeum(少陰), which has the virtue of nurishing the Liver and Kidney Eum, promoting lower heater and marinating the free flow of Gi Conclusions: The chief virtue of Pu-ryu(KI7) is to remove edema due to disturbance in Gi activity by dificiency of Kidney-Yang because of warming Yang to induce diuresis. To Um-gok(KI10), it is to treat instability of Kidney-Gi by Eum dificiency of the Liver and Kidney because of nurishing the Liver and Kidney Eum.

The effect of acupuncture treated by tonification and sedation manipulation at LI4 and KI7 on lacking of sweating in cold environment(I) (합곡(合谷) 복류(復溜) 보사(補瀉) 시술(施術)이 소한(少汗)에 미치는 영향(影響)(I))

  • Objectives : The purpose of this study was to identify the effect of the tonification & sedation manipulation for LI4 and KI7 on lacking of sweating in cold environment. Methods: The participants were divided into 3 groups as intact group without acupuncture, acupuncture group inserting and twisting with LI4 tonificaton and KI7 sedation method(LI4-tonify KI7-purge) and acupuncture group inserting and twisting with LI4 sedation and KI7 tonificaton method(LI4-purge KI7-tonify). We obtained baseline data at cold condition with the temperature $14{\pm}1^{\circ}C$. After making the participant staying in cold condition for 10 minute, we measured sweating rate in skin surface, body temperature, oxygen saturation, pulse rate, systolic blood pressure, diastolic blood pressure, deoxy-Hb and oxy-Hb in subcutaneous. Results : After acupuncture in cold environment, the sweating rate in skin surface significantly decreased in KI7-purge LI4-tonify group comparing the intact groups. The body temperature significantly increased in LI4-tonify KI7-purge and LI4-purge KI7-tonify groups comparing the intact group. The oxygen saturation significantly increased in LI4-tonify KI7-purge group comparing the intact group. Conclusions : These findings indicated that LI4 tonificaton and KI7 sedation method(LI4-tonify KI7-purge) could be effective for production of sweating in cold condition.

Issues and Significance of the Li Theory of Nature in the 19th Century (19세기 성리학(性理學)의 쟁점과 그 의미)

  • Two schools represent the Li theory of nature in the 19th century. They are the Li's superintendence school of Lee Hangno, Ki Jungchin and Lee Jinsang and the Li and Ki's mutual superintendence school of Chun Woo. They share a theoretical concern about the blockage of Ki's free rein. However, their concepts of superintendence are very different. Thus, various disputes arise among them and consequently different solutions are proposed. The Li's superintendence school applies Li's power even in the actual world, which has completely dominated Ki by conferring dynamic power on Li. However, Chun Woo, a representative theorist of Li and Ki's mutual superintendence, accepts only Ki's dynamic power. By denying Li's dynamic power, he argues that there is no other thing than Ki which takes the lead in reality. His solution to block Ki's free rein is to make Ki(mind) a disciple of Li(standard or nature) and to make Ki follow Li's lead.

Anti-Inflammatory Effect of Polygoni Avicularis Herba Herbal-Acupuncture at KI10 on LPS-Induced Nephritis in Rats (음곡(KI10) 편축 약침의 LPS 유도 급성신장염 억제 효과)

  • Objectives : This study aimed to evaluate the effects of Polygoni avicularis Herba Herbal-acupuncture( PaH-HA) at KI10(Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods : Rats were allocated into normal, control, and 3 experimental groups. The rats in the control group were intra-peritoneally injected with LPS for nephritis induction. The rats in the groups of experiment 1, experiment 2, and experiment 3 were treated with single needle prick, saline injection, and PaH-HA, respectively at KI10 three times for a week and then injected with LPS. To evaluate the effects of PaH-HA at HI10, WBC count in blood, serum CINC-1, renal TNF-${\alpha}$ and renal MPO were measured. Results : Needle prick at KI10 suppressed the increase of WBC in blood and CINC-1 in serum of LPS-stimulated rats. Saline injection at KI10 suppressed the increase of WBC in blood. PaH-HA at KI10 suppressed the increase of WBC in blood, CINC-1 in serum, and MPO in kidney of LPS-stimulated rats. Conclusions : PaH-HA at KI10 has an anti-inflammatory effect on LPS-induced nephritis in rats and there may be a synergism between KI10(Umgok) stimulation and PaH-HA Solution.

Effects of the Herbal-Acupuncture with Acanthopanacis cortex extract at KI10 (Eumgok) on osteoporosis in ovariectomized DDY mice (음곡(陰谷)(KI10)에 시술한 오가피(五加皮) 약침(藥鍼)이 난소적출 생쥐의 골다공증(骨多孔症)에 미치는 영향)

  • Objective : The purpose of this study is to observe the effects of herbal-acupuncture with Acanthopanacis cortex extract (AC-HA) at KI10 (Eumgok) on osteoporosis in ovariectomized (OVX) ddy mice. Methods : We carried out several experimental items to analyze the changes in body weight, uterine weight, uterus index, tibial length, the ash bone weight, tibial BMD, serum ALP, serum creatinine, serum osteocalcin, serum Ca, and the levels of Ca, P, Ca/P ratio in tibia, and we performed histological and histomorphological analysis as well. Results : Tibial BMD was significantly decreased by OVX, while significantly increased by AC-HA at KI10. Serum creatinine level was significantly increased by OVX, while significantly decreased by AC-HA at KI10. Serum osteocalcin level was significantly increased by OVX, while significantly decreased by AC-HA at KI10. Tibial TBV(trabecular bone volume) was significantly decreased by OVX, while significantly increased by AC-HA at KI10. AC-HA at KI10 significantly suppressed the OVX-induced increase of GPL(growth plate length) of tibia in mice. Conclusion : These results suggest that AC-HA at KI10 has a therapeutic effect on osteoporosis in OVX mice.

A View on Hyung-Ki in Donguibogam (『동의보감(東醫寶鑑)』의 '형기론(形氣論)'에 대한 소고)

  • The narration of "Donguibogam" is focused on humans whilst medical books before it focused on diseases. This is shown from the first subtitle of External Body(身形門) chapter, [The Origin of Hyung-Ki(形氣之始)]. The contents of [The Origin of Hyung-Ki] explains that the human body is composed of 'Hyung(形)' and 'Ki(氣)' This perspective is shown throughout "Donguibogam". First, in the aspect of construction, the viewpoint is shown from the JipRye(集例). It divides the body into inside and outside, which is a result of Hyung-Ki perspective. This continues in the table of contents. Naegyeongpyeon(內景) and Oehyeongpyeon(外形) describes the inside and outside of the body, in other words 'Hyung-Ki'. Japbyeongpyeon(雜病) describes complex illnesses with mixed insides and outsides. Tangaekpyeon(湯液) and Chimgupyeon(鍼灸) can be understood as division of treatment methods into inside and outside. When we look at the contents, the human body is created on the basis of the essence and vital energy of the world. Cheon-Ki becomes the 'Ki' that forms the functions of the body, while Ji-Ki becomes the 'Hyung' that constructs the bodily structure. It is considered that 'Hyung' is composed of SaDae(the body essence, vital energy, mentality and blood), and 'Ki' is composed of OSang(the five Jang organs and six Bu organs). 'Hyung' and 'Ki' show various appearances according to physiology and pathology. 'Hyung' is especially shown by obesity or thinness, 'Ki' by color. The obese may have Ki deficiency, coldness, dampness or phlegm. The thin could have blood deficiency, heat, fire or dryness. The color could show the pathology of the five Jang organs by the five colors, but it can simply be divided into black and white, where black means Eum deficiency, and white means Yang deficiency. It is said that the distinctive feature of traditional Korean medicine is that it is a constitutional medicine. In this perspective, further study on 'Hyung-Ki' is of value. If "Donguibogam" was the foundation of the Sasang Constitutional Medicine(四象醫學), study on 'Hyung-Ki' means searching for the origin of the Sasang Constitutional Medicine. Also, the study on Sasang Constitutional Medicine and Hyungsang medicine(形象醫學) will show the modern image of "Donguibogam". Hence, interchange study between "Donguibogam", Sasang Constitutional Medicine and Hyungsang medicine through the term 'Hyung-Ki' could hopefully lead to academic progression.
