2021년 자살 관련 지표들과 추이 (Suicide Related Indicators and Trends in Korea in 2021)

  • Background: This study aimed to update suicide-related indicators, including the number of suicidal deaths, suicide rate, and the prevalence of suicidal ideation and suicide attempts. We observed trends in suicide-related indicators based on up-to-date information. Methods: This study used five data sources to evaluate the trends of suicide-related indicators: Statistics Korea (1983-2021), Korean Wealth Panel Study (KOWEPS, 2012-2021), and Korea Health Panel Survey (KHP, 2010-2013, 2016-2019), Korean National Health and Nutrition Examination (KNHANES, 2007-2013, 2015-2021), Korean Community Health Survey (KCHS, 2008-2009, 2013, 2017, 2021). Results: The suicide rate per 100,000 population increased from 25.7 in 2020 to 26.0 in 2021. The rates of suicidal ideation from recently available data were 4.28% (KNHANES, 2021), 6.52% (KCHS, 2021), 1.61% (KOWEPS) and 7.10% (KHP, 2019). The suicidal attempts rates were 0.46% (KNHANES, 2021) and 0.34% (KCHS, 2021). The annual percentage change (APC) of suicide rate showed that suicide rates increased in the younger population (APC=9.02% in <19 years, APC=5.13 in 20-39 years) although the rates decreased in the older population (APC=-3.37 in 60-79 years, APC=-2.25 in >80 years). Conclusion: The suicide rate and related indicators increased in 2021 compared to 2020. Thus, continuous observation and appropriate suicide prevention policies as well as studies about the factors that affected the increase in 2021 are needed.

  • This study investigated the factors associated with HINT-8 (Korean health-related quality of life instrument with eight items), including health habits, hand grip strength, chronic disease, chewing function, mental health, and nutrient intake. In this study, the health-related quality of life (HRQoL; HINT-8) was assessed using the data from the 2019 Korea National Health and Nutrition Examination Survey (KNHANES VIII) among Koreans aged over 65, with HINT-8 being evaluated based on the general characteristics. Of the participants from KNHANES VIII, 1,455 subjects were analyzed using SPSS statistics complex samples (Windows ver. 27.0). The HINT-8 score was significantly lower in females and in those aged 75 years or older, lowest educational level, lowest income level, and single households. Logistic regression analysis, adjusted for the general characteristics, revealed a significant association between a lower HINT-8 group and various factors: low aerobic physical activity (OR=1.71, 95% CI=1.25~2.35), low hand grip strength (OR=1.34, 95% CI=1.02~1.78), chewing difficulties (OR=2.17, 95% CI=1.62~2.92), poor self-rated health status (OR=6.11, 95% CI=4.42~8.45), stressful mental status (OR=3.57, 95% CI=2.27~5.63) and poor calcium status (OR=1.45, 95% CI=1.11~1.88). On the other hand, this study could not reveal a significant association between a lower HINT-8 group and factors such as smoking, drinking, chronic disease, and nutrient intake status, except for calcium intake. This study suggests that HINT-8 is associated with aerobic physical activity, hand grip strength, chewing function, mental health, and calcium intake, but not with smoking, drinking, chronic disease, and nutrient intake.

자가 응답식 자료에 근거한 유병률 및 건강기대수명 연구의 신뢰도 분석: 건강보험 표본코호트 DB와의 비교 (Reliability of self-reported data for prevalence and health life expectancy studies: comparison with sample cohort DB of National Health Insurance Services)

  • 한국 의료패널 데이터와 국민건강 영양조사는 추출된 표본의 자가 진단에 따른 건강상태(self-assessed health)와 그들의 의료기관 이용에 대한 자가응답식 자료(self-reported data)이다. 이러한 자료에 근거한 유병률 연구 및 그에 따른 건강기대수명 연구에 관하여 유병률의 신뢰도에 대한 검증이 선행되어야 한다는 주장은 이미 여러 연구에서 제기되었다. 반면 최근 공개된 건강보험공단의 표본코호트 DB는 전 국민을 대상으로 의료기관 이용에 관련된 모든 자료가 저장된 자료인 국민건강정보 DB로부터 추출된 객관적인 자료이다. 또한 추정된 질병별 유병률에 대하여 그 대표성 및 신뢰도가 확보되어 있음이 검증된 자료이다. 이에 본 논문에서는 우리나라 국민의 유병률에 대한 대표성 및 신뢰도가 확보되어 있는 표본코호트 DB와의 비교를 통하여 이들 응답식 데이터에서 도출된 유병률의 신뢰도에 대하여 논의하였다. 자가응답식 자료를 통한 유병률은 표본코호트 DB를 통한 유병률에 비교하여 보았을 때 과소추정되어 있고 이러한 과소추정은 건강기대수명의 과대추정 문제로 이어지고 있음을 확인할 수 있었다. 또한 표본코호트 DB를 제외한 우리나라 건강자료의 안정적이지 못한 표본의 문제는 추정된 건강기대수명의 트렌드를 왜곡하는 문제가 추가적으로 발생할 수 있음을 확인하였다.

  • Objectives: The purpose of the study is to investigate the health and oral health factors related to hypertension in Korean elderly. Methods: The study subjects were 1,527 elderly people${\geq}65$ years old who underwent physical examination and completed the health survey questionnaire of KNHANES 2014. Results: The risk of hypertension was higher in nonsmoking female elderly having poor subjective health status and low body mass index (BMI). The risk of hypertension was also higher in the elderly having poor subjective oral health status and no oral examination in the previous year. Conclusions: Health risk factors for hypertension and oral health factors may be useful measures to manage hypertension and enhance quality of life in the elderly.

2010년 국민건강영양조사 자료를 이용한 우리나라 성인 상용 음식의 1인 1회 섭취분량(Portion Size) 설정과 2005년 국민건강영양조사를 기반으로 한 설정량과의 비교 - 밥류, 면·만두류, 국·탕류 및 찌개류를 중심으로 - (Establishment of One Portion Size of Dishes Frequently Consumed by Korean Adults using 2010 KNHANES and Its Comparison with the One Portion Size using 2005 KNHANES - Focusing on Rice, Noodles, Soups, and Stews -)

  • The purpose of this study was to define a portion size for dishes frequently consumed by Korean adults especially focusing on main dishes like rice, noodles, soups, and stews. From the original 8,019 people who took part in the Korea National Health and Nutrition Examination Survey (KNHANES) 2010, we analyzed the data for 4,556 persons aged 19 to 64 years and selected dish items consumed based on an intake frequency of 30 or more by all participants. The portion size of each dish item was set on the basis of the median amount ($50^{th}$ percentile) in a single intake by a single person. In the rice category, 15 items were selected, of which the most frequently consumed item was steamed rice with a portion size of 250ml. Among noodles and dumplings, the most frequently consumed dish was ramyeon with a portion size of 500ml. As for soups, the portion size was $250m{\ell}$ for kongnamul guk and $300m{\ell}$ for miyeoguk. In stews, the most frequently consumed items were kimchi jjigae followed by doenjang jjigae with a portion size of $200m{\ell}$ and $125m{\ell}$, respectively. In a comparison of portion sizes between the KNHANES 2010 and KNHANES 2005, the portion size of some noodles, soups, and stews tended to decrease from 2005 to 2010. Portion sizes established in this study for dish items consumed regularly by adults may be conveniently and effectively used in dietary planning, in nutritional education programs, and in assessing the dietary intake status of Korean adults.

한국 소아.청소년의 고열량 저영양 식품 섭취와 비만 (The Relationship between High Energy/Low Nutrient Food Consumption and Obesity among Korean Children and Adolescents)

  • Consumption of foods with high energy and low nutrient values has been linked to various health issues including obesity and chronic diseases. This study investigated the high-energy/low-nutrient food consumption status and its relationship with obesity status and intake of energy and nutrients, using the Korea National Health and Nutrition Examination Survey (KNHANES) and the Korea Youth Risk Behavior Web-based Survey (KYRBWS). The prevalence of overweight and obesity among 2-18 year-olds was 8.4% and 10.8% in 2008 and 8.5% and 9.0% in 2009, respectively, in KNHANES. The prevalence of obesity among 12-18 year-olds was 13.9% in 2008 and 11.4% in 2009 in KNHANES, while it was 8.1% in 2008 and 8.2% in 2009 in KYRBWS. Consumption patterns of high-energy/low-nutrient foods were diverse depending on the particular food type. High-energy/low-nutrient foods such as cookies were most often consumed more frequently (2~3 times per week), but fast food type foods were generally consumed less frequently (once per week or less). No significant relationships between highenergy/ low-nutrient food consumption and level of energy and nutrient intakes were found in either datasets. Adolescents who were not obese more frequently consumed ramen (p < 0.001), cookies (p < 0.001) and fast food (p < 0.001) than those who were obese in KYRBWS, however, such relationships were not found in KNHANES. This direction of the relationship could be probably attributed to the cross-sectional nature of the datasets. This study was limited by the cross-sectional nature of the data, therefore, further longitudinal research using various study techniques would be necessary to understand the effects of high-energy/low-nutrient foods on child obesity.

외식관련 식생활 위험군 분석 및 외식 행동 전략 도출 - 제3기 국민건강영양조사 20세 이상 성인 자료 분석 - (An Analysis of Groups with Diet Problems Associated with Dining Out)

  • The principal objectives of this study were to identify diet problems associated with dining out, and to suggest dining out strategies for groups with diet problems. The data collected from adults(all over 20 years old) from the third Korea National Health and Nutrition Examination Survey III(KNHANES III), conducted in 2005, were used in this study. A total of 6,497 data were included in our statistical analyses, using SPSS 14.0. The results of this study demonstrated that there were significant differences in dining out frequency between different genders(p<0.001), ages(p<0.001), and economic status (p<0.001). With the crosstabulation analysis using the Chi-square statistics, the middle class males aged $20{\sim}29$(p<0.05), and lower class females aged 65 and over evidenced different degrees of compliance with the following guidelines. 'Eat a variety of foods' varied by frequency of dining out. In addition, the degree of compliance with the guidelines 'Increase activity and eat an adequate amount of foods for weight control', 'Enjoy the Korean traditional diet', and 'Control consumption of alcohol' also differed by dining out frequency in certain populations. With these results, dining out strategies were suggested for the groups with diet problems associated with dining out.

The association of dietary quality and food group intake patterns with bone health status among Korean postmenopausal women: a study using the 2010 Korean National Health and Nutrition Examination Survey Data

  • BACKGROUND/OBJECTIVES: Osteoporosis and osteoporosis-related disease are drawing a lot of attention in Korea as one of the serious health problems. Bone health status may be influenced by the general dietary quality and dietary pattern. SUBJECTS/METHODS: To determine the relationship between dietary quality and intake patterns and bone health status, the %RNI, NAR, DDS, and food group intake patterns were assessed according to their bone health status for 847 postmenopausal women using the 2010 KNHANES data after eliminating those of likely changing their diet under the advice of doctors or those taking estrogen. RESULTS: Bone health became worse as dietary quality deteriorated. All NAR and %RNI values were highly associated with bone health levels and the consumption frequency of Ca sources, DDS and the food group intake patterns also confirmed the findings. CONCLUSIONS: This study confirmed that dietary quality and dietary patterns were important for bone health. Nutritional education on eating foods from the five basic food groups has to be emphasized to prevent osteoporosis among older women.

폐경여성의 골밀도와 잔존치아의 관련성 (Association between bone mineral density and remaining teeth in postmenopausal women)

  • Objectives : The purpose of the study is to investigate the factors affecting the bone density and the relationship between bone density and remaining teeth by using data from 2007 to 2009 of the 4th Korean National Health and Nutrition Examination Survey(KNHANES). Methods : The subjects were 1,829 postmenopausal women over 50 years old from the data of the 4th KNHANES. Results : The bone density had the significant difference in 'age' and 'level of education' and remaining teeth had the significant difference in age, monthly income and level of education. The bone mineral density and remaining teeth had the significantly different from subjective general health, smoking, drinking, physical activity. The oral health behaviors and remaining teeth had the significantly associated in subjective oral health status, frequency of tooth brushing, use of auxiliary oral hygiene product, and recent oral examination. Conclusions : The number of remaining teeth is below 9.27 compared with the normal group.

거주지역별 노인의 자살생각과 관련요인 - 도시와 농촌의 비교 - (Suicidal Ideation and Associated Factors of the Elderly According to Residence Area - Focusing on the Comparisons between Urban and Rural Areas -)

  • Purpose: This study was conducted in order to investigate the incidence and associated factors of suicidal ideation among the elderly according to residence area. Methods: This cross-sectional study used secondary data from the 2011 Korean National Health and Nutrition Examination Survey (KNHANES V-2). A representative sample of 1,464 men and women aged 65 and over was selected. Rao-scott $X^2$-test, multiple logistic regression models based on sampling scheme of the data were used. Results: Of the 1,464 respondents, 22.4% had experienced suicidal ideation during the past year. Higher incidence of suicidal ideation was observed in rural seniors (23.7%) than in urban seniors (21.1%). The incidence of suicidal ideation showed correlation with age, education level, depression, stress, lying in a sick bed, and current smoking status among elderly living in urban areas. However, among elderly living in rural areas, the incidence of suicidal ideation showed correlation with household income, depression, stress, and daily activity. Conclusion: Based on the results, nurses should manage effective and individualized nursing interventions for elders in planning suicide prevention programs with consideration for residence areas, because there were differences in the factors affecting suicidal ideation among elders according to residence area.
