키네시오 테이핑이 고령자의 슬관절 등속성 근 기능 및 통증에 미치는 영향 (Effect of Kinesio Taping on Isokinetic Muscular Function and Pain of Knee Joint in Elderly Women)

  • 본 연구는 테이핑 요법이 슬관절의 등속성 기능 및 통증에 미치는 영향을 알아보기 위하여 슬관절 부위에 통증을 호소하는 $65{\sim}70$세 여성 노인 12명을 대상으로 슬건근, 대퇴사두근, 슬개골 부위에 폭 5cm의 테이프를 8주 동안 주당 3회씩 총 24회를 적용시킨 후 등속성 근 기능인 최대근력, 상대근력, 근 지구력(총 운동량), 근 지구력(운동피로도), 동측 근력 비율, 주관적 통증정도인 시각적 상사 척도와 구술적 평정 척도의 변화를 분석한 결과 다음과 같은 결론을 얻었다. 즉, 8주간의 테이핑 요법 후 최대근력, 상대근력, 근지구력(총 운동량), 동측근력비율은 각각 유의하게 증가하였고 (p<.05; p<.01), 근지구력(운동피로도) 및 시각적 상사 척도와 구술적 평정 척도 점수가 각각 유의하게 감소하였다(p<.05).

만성 뇌졸중 환자의 보행속도와 보행 비대칭에 영향을 미치는 무릎근력과 발목 관절가동범위 (Knee Strength and Ankle Range of Motion Influencing Gait Velocity and Gait Asymmetry in Patients With Chronic Stroke)

  • The common features of walking in patients with stroke include decreased gait velocity and increased asymmetrical gait pattern. The purpose of this study was to identify important factors related to impairments in gait velocity and asymmetry in chronic stroke patients. The subjects were 30 independently ambulating subjects with chronic stroke. The subjects' impairments were examined, including the isokinetic peak torque of knee extensors, knee flexors, ankle plantarflexors, and ankle dorsiflexors. Passive and active ranges of motion (ROM) of the ankle joint, ankle plantarflexor spasticity, joint position senses of the knee and ankle joint, and balance were examined together. In addition, gait velocity and temporal and spatial asymmetry were evaluated with subjects walking at their comfortable speed. Pearson correlations and multiple regressions were used to measure the relationships between impairments and gait speed and impairments and asymmetry. Regression analyses revealed that ankle passive ROM and peak torque of knee flexors were important factors for gait velocity ($R^2=.41$), while ankle passive ROM was the most important determinant for temporal asymmetry ($R^2=.35$). In addition, knee extensor peak torque was the most significant factor for gait spatial asymmetry ($R^2=.17$). Limitation in ankle passive ROM and weakness of the knee flexor were major contributors to slow gait velocity. Moreover, limited passive ROM in the ankle influenced the level of temporal gait asymmetry in chronic stroke patients. Our findings suggest that stroke rehabilitation programs aiming to improve gait velocity and temporal asymmetry should include stretching exercise for the ankle joint.

편마비 환자들의 환측 슬관절 중심으로 등속성 및 등장성 운동 비교연구 -Cybex II+ Isokinetic Dynamometer를 이용한 평가를 기준으로- (The Comparative Study with Isokinetic and Isotonic Exercise Knee Joint Involved Side in Hemiplegia Patients)

  • The purpose of this study was to find out the effect of muscular strength of quadriceps, hamstring of knee joint to isokintic, isotonic exercise treatment in stroke. Data was collected from 80 stroke from January 10. 1994 to april 10, 1994 in a Chungnam university hospital. The results were as follows; 1. Peak torque was higher isokinetic than isotonic excercise group both in quadriceps and hamstring of knee joint, 6 weeks, 8 weeks 10 weeks showed statistically significant in isokinetic exercise training group. 2. Total work was high isokinetic than isotonic exercise group both in quadricaps and hamstring of knee joint, 10 weeks 12 weeks showed statistically significant in isokinetic exercise training group. 3. Average peak torque and total work was higher isokinetic than isotonic excercise group both in quadriceps and hamstring of knee joint, showed statistically significant in isokinetic exercise training group (p<0.05).(p<0.01). Isokinetic and isotonic exercise treatment were needed for sutable walk training in stroke. But I think that better method was isokinetic exercise treatment in muscular strength and endurance.

전자-기계식 클러치를 이용한 장하지 보조기용 무릎관절 자동 제어 장치의 개발 (Development of the Automatic Knee Joint Control System for a Knee-Ankle-Foot Orthosis Using an Electromechanical Clutch)

  • A new knee-ankle-foot-orthosis(KAFO) which uses an automatically-controlled electromechanical wrap spring clutch for the knee joint was developed in the present study. It was found that the output voltage from the foot switches of the developed KAFO was proportionally increased with respect to the applied load. The output voltage from the infrared sensor also decreased as the knee flexion angle increased. The knee joint system for the new KAFO weighs only 780g lighter than any other commercially available developed system. In addition, the solenoid reduces the reaction time for the automatic control of the knee joint. The static torque of the clutch was measured for three persons, and it satisfied the normal knee extension moment during the pre-swing. Three-dimensional gait analyses for three different gait patterns (normal gait, locked-knee gait, controlled-knee gait) from five normal subjects were conducted. Controlled-knee gait showed the maximum knee flexion angle of 40.56$\pm9.55^{\circ}$ and the maximum knee flexion moment of 0.20$\pm$0.07Nm/kg at similar periods in the normal gait. Our KAFO system satisfies both stability during stance phase and free knee flexion during the swing phase at the proper period during the gait cycle. Therefore, our KAFO system would be very useful in various low extremity orthotic applications.

여자대학축구선수와 일반여자대학생의 슬관절 신전근 및 굴곡근의 등속성 운동 비교연구 (A Study on Comparison of Isokinetic Testing of the Knee Extensors and Flexors for the Woman College Soccer Players and General Waman College Students)

  • The subjects of this study were 15 woman soccer players and 15 untrained woman students at H-Cheju University. Each subject was measured on the muscle strength and peak torque of knee extensors and knee flexors with isokinetic load ($60^{\circ}$/sec, $180^{\circ}$/sec). The results were as follows: 1. There was no significant difference between right and left leg strength measured isokinetically in each group. 2. The peak torque of knee extensors and flexors was significantly higher in scoccer players than untrainded students (p<0.001). 3. For the angle of knee joint showing the highest peak torque, there was significant difference between soccer players and untrained students in knee extensors(p<0.05) and right flexors (p<0.01). 4. For knee H/Q ratio, there was significant difference between two groups(p<0.05). 5. For the total work and average power at $180^{\circ}$/sec, there was significant difference between two groups in knee extensors(p<0.001) and flexors(p<0.01). 6. In soccer players at $60^{\circ}$/sec, there were significant correlations between peak torque and height and between peak torque and weight.(p<0.001). 7. For the muscular contraction velocity at $60^{\circ}$/sec, there was significant difference between two groups in flexors (p<0.01). 8. For the muscular indurance at $180^{\circ}$/sec in extensors and flexors, there was no significant difference between two groups(p<0.05). In conclusion, there were significantly higher in peak torque, contraction velocity, knee joint's angle with the highest peak torque, H/Q ratio, total work and average power in woman scoccer player than general students. But there was no significant difference in muscular endurance. Soccer performance is based on the various components including muscular endurance that is one of the most important components. So it is necessary that the training method to improve the various components (especially including muscular endurance) should be done.

정적인 스트레칭과 Evjenth-Hamberg 스트레칭이 슬관절 등속성 근력에 미치는 효과 (The Effect of Static Stretching and Evjenth-Hamberg Stretching for Isokinetic Muscle Strength of Knee Joint)

  • Purpose: The purpose of this study was to examine the effects of static stretching and Evjenth-Hamberg stretching on isokinetic muscle strength of knee flexors and extensors. Methods: The subjects were composed of eighty healthy males without weight-training experience. ROM of knee joint measured active maximal extension and isokinetic Peak Torque measured $60^{\circ}/sec,\;120^{\circ}/sec$ using an the En-Knee. Three tests(Baseline, 4 weeks, 8 weeks, respectively) was operated to examine change of each variable. Data were analyzed with a $2{\times}3$ analysis of variance ($group{\times}test$) for repeated measures on last factor by SPSS package 10.0. The data analysis revealed muscle strength were dependent on stretching method. Results: The results were as follows. First, Evjenth-Hamberg stretching(E-HS) was more effective than static stretching(SS) on ROM. Second, Peak Torque of knee flexors and extensors was improved in both methods by each time. but E-HS was more improved than SS. Conclusion: In conclusion, This study indicates that E-HS is more efficient than SS on muscle strength improvement.

  • We proposed the above-knee prosthesis using rotary MR damper in which knee joint is semi-actively controlled by microprocessor. Dissipation torque in the knee joint can be controlled by the magnetic field which is induced by applying current to a solenoid, Tracking control of knee joint angle was tested by 3-DOF Leg simulator. The experimental results show that the proposed above-knee prosthesis system had good performance in swing phase tracking and repetitive controller in conjunction with a computed control law and PD control law, reduced RMS tracking error as the repetitions of tracking. Moreover, desired knee angle trajectory was generated based on the estimation of gait period with the gyro signal and the tracking control was performed.

무릎 골관절염 환자의 보행속도에 따른 하지 관절 강성 변화 (Changes of Lower Limb Joints Stiffness with Gait Speed in Knee Osteoarthritis)

  • Spring-like leg models have been employed to explain various dynamic characteristics in human walking. However, this leg stiffness model has limitations to represent complex motion of actual human gait, especially the behaviors of each lower limb joint. The purpose of this research was to determine changes of total leg stiffness and lower limb joint stiffness with gait speed in knee osteoarthritis. Joint stiffness defined as the ratio of the joint torque change to the angular displacement change. Eight subjects with knee osteoarthritis participated to this study. The subject walked on a 12 m long and 1 m wide walkway with three sets of four different randomly ordered gait speeds, ranging from their self-selected speed to maximum speed. Kinetic and kinematic data were measured using three force plates and an optical marker system, respectively. Joint torques of lower limb joints calculated by a multi-segment inverse dynamics model. Total leg and each lower limb joint had constant stiffness during single support phase. The leg and hip joint stiffness increased with gait speed. The correlation between knee joint angles and torques had significant changed by the degree of severity of knee osteoarthritis.

Effects of Lumbar Mobilization and Transcutaneous Electrical Nerve Stimulation on Proprioception and Muscular Strength in Volleyball Players with Chronic Knee Pain

  • Background: The spinal nerves, which start at the lumbar level, are connected to the nerve innervation in the knees. Currently, there is a lack of research on the treatment of knee pain through lumbar mobilization. Objectives: To investigate the effects of lumbar joint mobilization (LJM) and transcutaneous electronic nerve stimulation (TENS) on proprioception and muscular strength in volleyball players with chronic knee pain. Design: Two group pre-posttest. Methods: A total of 26 professional volleyball players with chronic knee pain were allocated to the LJM (n=13) and TENS (n=13) groups. In the LJM group, grade III - IV amplitude was applied 3 times for 1 minute (80 times per minute) at the affected lumbar (L2-3) facet joint in the prone position. In the TENS group, the TENS treatment device was used to directly apply or 15 minutes to the area of chronic knee pain (100 Hz, 150 ㎲). Proprioception was measured by knee flexion and extension angles, and muscle strength was evaluated using an isokinetic test. Measurements were taken before and after interventions. Results: In the eye opened conditiond, proprioception significantly increased during both knee extension and flexion after LJM, while only knee extension was significantly increased in the TENS group. There was also a significant difference in knee extension between the two groups. In the eye close conditiond, proprioception was significantly improved only during knee extension in the LJM group, and the difference in knee extension between the groups was also significant (P<.05). The maximum torque of the affected knee joint was significantly improved at 60°/sec in both groups (P<.05); however, there was no difference between the two groups. There was no significant difference in the maximum flexion torque within or between the groups. Conclusion: This study suggests that LJM improved proprioception and muscular strength in volleyball players with chronic knee pain.

국소적 부하와 전신적 부하가 슬관절 위치 감각에 미치는 영향 (The effects of knee joint position sense following local and general load protocols)

  • The purpose of this study was to compare the effects of knee joint position sense following local and general load protocols in 25 healthy male subjects. Proprioception of the knee joint was evaluated by measuring absolute angular errors at matching angles before, after and between 2 different types of load protocols. Proprioception tests(on the dominant knee) were performed in which proprioception of the passivepassive reproduced and active-active reproduced knee position was measured. Local load was provided with maximum isokinetic knee extension-flexion on the isokinetic dynamometer(Cybex), and general load was 10 minutes running on a treadmill. Peak torque(knee extension and flexion) and heart rate(beats per minute) was evaluated as an indicator of local and general fatigue during load protocols. The results were as follows: 1. For pasive-pasive reproduced knee position test, significant difference in absolute angular errors after general load protocol was detected compared with that before general load protocol(P<.05), significant difference in absolute angular errors after local load protocol was detected compared with that before local load protocol(P<.05). However, no significant difference in absolute angular errors of general load protocol was detected compared with that of local load protocol (P>.05), no significant difference in absolute angular errors of local load protocol was detected compared with that of general load protocol(P>.05). 2. For active-active reproduced knee position test, significant difference in absolute angular errors after general load protocol was detected compared with that before general load protocol(P<.05), significant difference in absolute angular errors after local load protocol was detected compared with that before local load protocol (P<.05). Also, significant difference in absolute angular errors of general load protocol was detected compared with that of local load protocol(P<.05), significant difference in absolute angular errors of local load protocol was detected compared with that of general load protocol(P<.05). 3. A significant decrease of peak torque of knee extensors and flexors was seen after local load, although heart rate was significantly increased(P<.05). No significant change of peak torque of knee extensors and flexors was seen after general load(P>.05), although heart rate was also significantly increased(P<.05). The previous study revealed that knee proprioception is significantly altered when the muscle mechanoreceptors are dysfunctional due to muscle fatigue, although the joint mechanoreceptors have no significantly effect on knee proprioception when the presence of knee muscle fatigue. However, the results of this study are different from those of the previous study in that muscle weakness of the knee could not be seen after general load. This study shows that general load may diminish motor control by the central nervous system. Proprioceptional decline without muscle weakness of knee after general load suggests a change in the proprioceptional pathway without influence from muscle mechanoreceptors.
