Fear of Falling, ADL/IADL disability, and Depressive Symptoms in Korean Community-Dwelling Middle-aged and Older People (중노년기의 낙상두려움이 ADL/IADL장애와 우울증상에 미치는 영향)

  • This study examined the relationships among fear of falling, ADL/IADL disability, and depressive symptoms among Korean community-dwelling middleaged and older adults based on the disablement process model. The panel data from the Korean Longitudinal Study of Aging(KLoSa) were used to test whether fear of falling affects disablement process as a risk factor and an exacerbator. The results of path analysis showed that middle-aged and older adults who reported fear of falling were more likely to experience ADL/IADL disability and depressive symptoms, and fear of falling influenced depressive symptoms through increasing the risk of ADL/IADL disability. In addition, fear of falling exacerbate the risk of ADL/IADL disability among middle-aged and older adults who had chronic illnesses. The results indicated that fear of falling is not only risk factor but also excerbator in predicting ADL/IADL disability and depressive symptoms prospectively.

Comparison of Activities of Daily Living Differences with Dementia Stage (치매 단계별 일상생활수행능력의 차이 비교)

  • This study aims to obtain information about the ADL and I-ADL functions of people with dementia living in the community and nursing care facilities regarding their dementia stages for the purpose of supporting occupational therapists working in dementia care centers. The information was collected from 100 people with dementia living in the community and 100 people with dementia staying in care facilities from October 1 to November 20, 2015. 15 facilities participated in this study, including nursing homes, day care centers, health centers, geriatric hospitals and metropolitan dementia centers. All of the assessors were trained in the use of the instruments, ADCS-ADL, S-IADL, S-ADL and MMSE-K, by the researcher before conducting any evaluations or interviews. In order to compare the differences in the levels of ADL according to the dementia stage, an analysis was made via the post-hoc test after conducting a one-way ANOVA. An analysis of the residence type and dementia stage was made through the independent t-test. The comparison of the level of ADL between the people with dementia living in the community and those living in nursing homes showed differences in the moderate level of dementia for all items of the I-ADL and ADL (p<.01). The post-hoc test showed that there is a difference among the minimum, mild, and moderate levels. When the differences in ADL according to the residence type were examined, there were no significant differences in any of the stages (p>.05). Considering these results, it may be critical to identify the changes in the I-ADL rather than those in the ADL for the outcome index when conducting future intervention studies on dementia.

The Correlation between Problematic Behaviors and Activities of Daily Living of Elderly People with Dementia in Patients in a Geriatric Hospital

  • The purpose of this study is to determine the correlations between problematic behaviors and activities of daily living(ADL) targeting 106 demented elderly people hospitalized in a geriatric hospital. To examine the cognitive function of the subjects, the study used Korean Mini Mental State Examination(K-MMSE). For problematic behaviors and ADL, the study collected data based on Patient Assessment Forms in the geriatric hospital. Among problematic behaviors, apathy/indifference had the highest correlation with the items of ADL. Irritability/lability, agitation/aggression, depression/ dysphoria, night-time behavior and wandering also showed to be correlated to items of ADL(p<.05). This study demonstrated that cognitive function, problematic behaviors and ADL of the demented elderly hospitalized in the geriatric hospital are correlated to each other.

Potential Allelic Association of Microsatellite Markers on Chromosome 1 with Economic Traits in Korean Native Chicken (한국재래닭 1번 염색체내 초위성체 유전표지를 이용한 경제형질 연관 지역 탐색)

  • A total of 17 polymorphic microsatellite markers on chromosome 1 were used for allelic association tests with phenotypic traits in Korean native chicken. Chi-square tests were performed to compare the frequencies of individual alleles between the high and the low trait groups. The frequency of allele 123 of MCW0160 showed a significant difference between the high and the low groups in the trait of egg weight (EW). Three markers, namely ADL0234, UMA1.125 and ADL0101, were found to show significant differences in allelic distribution for the trait of the first lay day (FLD). UMA1.117, ADL0020, UMA 1.019, LMA1 and ADL0238 were found to show significant differences in allelic distribution for the trait of body weight (BW). ADL0101 and ADL0238 were found to show significant differences in allelic distribution for the trait of number of egg production(EP). In this study, we identified the QTL for economic traits at around 94 (MCW0160), 151 (ADL0234), 170 (UMA1.125), 225 (UMA1.117), 285 (ADL0020), 387 (UMA1.019), 418 (LMA1), 500 (ADL0101) and 520 (ADL0238) cM on chromosome 1 in Korean native chicken. The results provided a useful guideline for identification of positional candidate gene and marker-assisted selection for economic traits in Korean native chicken.

Effects of Dual Task Balance Training on Balance and Activities of Daily Living in Stroke Patient (이중과제 균형 훈련이 뇌졸중 환자의 균형과 일상생활동작에 미치는 효과)

  • Purpose : The purpose of study was to investigate effects of dual task balance training on balance and activities of daily living(ADL) in Stroke patient. Methods : The purpose of study was to investigate effects of dual task balance training on balance and activities of daily living(ADL) in Stroke patient. Results : The results of this study were summarized as follows: There were significant improvement in balance and ADL following the training in experimental group. There were significant improvement in a part of balance following the training in control group, whereas there were no significant improvement in ADL following the training in control group. There were significant difference following training in both groups in balance and ADL. The level of statistical significance was <05. Conclusion : Based on the results of this study, dual task balance training have an effects on balance and ADL in stroke patients.

The Health and Dietary management of Impaired elderly by ADL in Gyeonggi, Korea (ADL에 의한 도움필요 노인의 건강과 식생활관리 -경기지역 농촌노인을 중심으로-)

  • The purpose of this study is to provide information searching for health promotion, nutrition improvement, and health care of the impaired elderly by ADL(Activity of Daily Living) and IADL(Instrumental Activity of Daily Living). The subjects were divided into the Assistant Needed Group and No Assistant Needed Group for living in line with the responses of ADL(10 items like dressing, washing, move etc.) and IADL(10 items like housekeeping, using transportation, shopping, Phone call etc.). Survey was made for health behavior, health risk habit, dietary management status and diet intake by 24 hr-recall and questionnaire method. 242 subjects were collected in 12 cities or Gun districts in Gyeonggi Province, S. Korea. Survey was carried out by regional home extension workers using interview method. Statistical analyses were made using SAS (Version 8.1). Chi-Square Tests and General Linear Models. The subjects of impaired ADL elderly was 26.5% and it composed 30% of the total male and 22.2% of the total female. The demographic status of the impaired ADL elderly showed no difference from that of the normal elders, elementary school educated (73.4%), with spouse (43.8%) or with adult children(37.5%), using monthly living cost of 500-1,000 thousand won(35.9%). Mean age was 74.05 years compared to 72.25 years of normal elders. However, there was no significant difference from the normal and impaired ADL group, regular exercise(60.0%), with walking (90.0%), no-smoke(54.7%) and no-drink(48.4%). Kind of disease was not different from the one in impaired and normal group, with cardiovascular disease(32.3%), with diabetes mellitus(8.1%), joint lumbago neuralgia(32.3%) and osteoporosis(9.7%). Gastrointestinal complaints of the impaired ADL group were nausea(57.8%), chronic indigestion (23.4%), constipation (14,0%) and vomiting(3.7%). Sleeping time required for the impaired was longer than that for the normal group by 10hours(4.7%) or 8-10hours(20.3%), which consisted 1.7% and 16.6% respectively. Nutrient intake of the impaired ADL group was low compared to normal range elders: Energy(1260kca1), Protein(52.75g). There was gender difference in nutrient intake; the male impaired group showed no significant difference from the normal group but it was significantly lower in female impaired group. These results suggest that low quality of life and low economic status of the impaired ADL elderly require congregate meal in village hall to cover the lack of side dishe variety. And nutrition education program including community assistance would be required for the impaired ADL elderly together with the sufficient food and exercise practice. By operating nutrition education program, the impaired ADL elderly would maintain more enhanced quality of life and ameliorate the ADL capability.

Age-Related Physical Function(ADL, IADL) and its Related Factors of Elderly People in Korea (우리나라 고령자의 연령에 따른 신체적 기능(ADL, IADL)과 관련요인)

  • This study was performed to determine the levels of physical function (ADL, IADL) and to reveal its association with the related factors in the elderly people. The study subjects were 1,756 (male 872, female 884) people aged over 70 who received medical check-ups and long-term care services between 2009 and 2012 from the National Health Insurance Corporation. As a result, the distribution of impaired ADL and IADL increased significantly with age. Logistic regression showed that the risk ratio of impaired ADL was increased significantly in the following groups: female, urban, low weight, stroke history group, smoking, alcohol drinking, and not regular exercise group. The risk ratio of an impaired IADL were increased significantly in the group of females, low weight, smoking, alcohol drinking. On the other hand the risk ratio of an impaired ADL and IADL was similar in each age group. As above results, the levels of ADL and IADL in the study subjects are closely related to the socio-demographic characteristics and health related behaviors. In particular, they suggested that the levels of ADL and IADL were lower in the poor group of the health-related behaviors, such as smoking, alcohol drinking, and regular exercise.

Relationship of Activity of Daily Living and Self-Care Agency in Patients with Spinal Cord Injuries (척수손상환자의 일상생활동작과 자가간호역량에 관한 상관연구)

  • Purpose: This study examined the relationship between level of activity of daily living (ADL) and Self-Care Agency in patients who have had a spinal cord injury. Method: The participants in the study were 78 persons who had a spinal cord injury and were between 20 and 70 years of age. The data were collected from November 1, 2003 to January 30, 2004. The instruments used in this study were the ADL Check List and Self-Care Agency Scale. The data were analyzed using the SPSS program and included numbers, percentages, t-test and ANOVA, and Pearson correlation coefficients. Results: The total mean score for activity of daily living was $44.21{\pm}16.63$. The total mean score fur Self-Care Agency was $107.45{\pm}15.71$, There was a significant difference between scores for ADL and Self-Care Agency. Conclusion: In order to increase the level of ADL, rehabilitation nurses should ensure that these patients receive training in ADL. Also, it was identified that informational support is important, that is, nurses should help to provide these patients with Self-Care Agency training for ADL.

A Study of Family Support, Pain, Self-esteem, ADL in Patients with Ankylosing Spondylitis (강직성 척추염환자의 가족지지, 통증, 자아존중감, 일상생활 활동에 관한 연구)

  • Purpose: The purpose of this study was performed to understanding of family support, pain and self-esteem, activities of daily living(ADL) in patient with ankylosing spondylitis(AS) and to identify the relationship among variables. Method: This study was conducted on 55 adults diagnosed with AS in C hospital in Daejeon during January 15th to June 20th, 2004. Instruments in this study were pain scale (VAS), family support scale, self-esteem scale, ADL scale. The data were analyzed mean, standard deviation, one-way ANOVA, Pearson's Correlation Coefficient using SPSS WIN(ver 10.0) program. Results: 1) 83.6% of the subjects was men, 30.9% of the subjects' age was below 29. And the 90.9% of the subjects was no experience of education about AS, 41.8% of the subjects was doing exercise. 2) The pain score was 48.36, family support score was 3.98, self-esteem score was 3.52 and ADL score was 81.41. 3) Family support was positively correlated with self-esteem and ADL and negatively correlated with pain. self-esteem was negatively correlated with pain and ADL. Conclusion: The results in this study can help nurse who care patients with AS understanding the relationships among family support, self-esteem, ADL, pain. And this findings showed that the patients with AS have few experience for disease-related education to manage and to understand AS. Therefore disease-related educational nursing program based on family support, pain, ADL of AS is needed to understand and manage AS. Because family support, pain, ADL of AS were significant correlated.

A Study on Clinical Characteristics of Patients with Mild Cognitive Impairment at Convalescent Hospitals (요양병원에 내원한 경도인지장애환자의 진료특성에 관한 연구)

  • Dementia among the diseases occurring in the old age is recognized as a disease to be solved at the national level and systematic management is being carried out. However, the awareness and response of the mild cognitive disorder, which is the middle stage of normal aging and dementia, are insufficient and social costs are increasing every year. The subjects of the study were 303 hospitalized patients whose primary diagnosis was mild cognitive impairment (F067) in a nursing hospital located in one district in 2016. The number of days of hospitalization was shorter in autumn and longer in winter or in hospital discharge. The cost of treatment for patients with mild cognitive impairment is lower in autumn and higher in winter or in 'discharge' of medical treatment results. The total cost of medical treatment was high in the winter by season in the ADL 6-12 and ADL 13-20 groups. Difference in the cost of medical treatment according to the characteristics of medical treatment, the medical expenses were the highest when the number of corporal disease was 5 ~ 8. In the medical treatment results, the ADL 6-12 point group and the ADL 13-20 point group had high medical expenses in 'continue'. Considering the clinical characteristics of mild cognitive impairment and considering that 10-15% of the patients with mild cognitive impairment progress to dementia every year, early detection and treatment of mild cognitive impairment is an effective and potentially important task for reducing potential social and dementia patients.
