THE CHANGES IN PRACTICE PATTERN AND PATIENT DISTRIBUTION FOR LAST 5 YEARS(2000-2005) IN THE DEPARTMENT OF PEDIATRIC DENTISTRY AT SEOUL NATIONAL UNIVERSITY DENTAL HOSPITAL (최근 5년간(2000-2005) 서울대학교 치과병원 소아치과의 진료현황 변화 및 신환분포에 대한 조사)

  • The spectrum of pediatric dentistry has been changing recently, due to many socioeconomic changes and developments in dental materials. The purpose of this study was to recognize the changing pattern in pediatric dental treatment and to present the direction for pediatric dentistry in the future. Patient distribution and practice trends were reviewed based on the patient records of the department of pediatric dentistry, Seoul National University Dental Hospital, from 2000 to 2005. Results were as follows ; 1. In 2000, preventive treatments comprised 14.5%, restorative treatments 52.6%, surgical treatments 11.4% and orthodontic treatments 21.5%. In 2005, the percentages were changed to 12.1%, 38.3%, 13.7%, and 35.9% respectively, showing a decline in preventive and restorative treatments and an increase in surgical and orthodontic treatments. 2. In restorative treatments, the proportion of amalgam, stainless steel crown restoration and pulp treatments decreased, and that of glass ionomer and resin restoration increased. 3. The number of out-patients increased from 2000 to 2002, and decreased thereafter. 4. In 2005, the average age of patients decreased from that of 2000.

Prostatic Artery Embolization for Lower Urinary Tract Symptoms via Transradial Versus Transfemoral Artery Access: Single-Center Technical Outcomes

  • Objective: To evaluate the safety and feasibility of prostatic artery embolization (PAE) via transradial access (TRA) compared with transfemoral access (TFA). Materials and Methods: This retrospective study included 53 consecutive men with lower urinary tract symptoms (LUTS) who underwent PAE between September 2018 and September 2021. Thirty-one patients (mean age ± standard deviation: 70.6 ± 8.4 years) were treated with TFA, including 14 patients treated before adopting TRA. Since December 2019, TRA has also been attempted with the procedure's selection criteria of patent carpal circulation and a height ≤ 172 cm, with 22 patients treated via TRA (69.1 ± 9.6 years). Parameters of technical success (defined as successful bilateral embolization), clinical success (defined as LUTS improvement), procedural time, radiation dose, and adverse events were compared between the two groups using the Fisher's exact test, independent sample t test, Wilcoxon signed-rank test, or Mann-Whitney test. Results: All patients received at least one-side PAE. Technical success of PAE was achieved in most patients (TRA, 21/22; TFA, 30/31; p > 0.999). No technical problem-related conversion from TRA to TFA occurred. The clinical success rate was 85% (11/13) in patients with TRA, and 89% (16/18) in patients with TFA for follow-up > 2 weeks post-PAE (median, 3 months) (p > 0.999). The median procedure time was similar in both groups (TRA, 81 minutes vs. TFA, 94 minutes; p = 0.570). No significant dose differences were found between the TRA and TFA groups in the dose-area product (median Gycm2, 95 [range, 44-255] for TRA and 84 [34-255] for TFA; p = 0.678) or cumulative air kerma (median mGy, 609 [236-1584] for TRA and 634 [217-1594] for TFA; p = 0.551). No major adverse events occurred in either of the groups. Conclusion: PAE via TRA is a safe and feasible method comparable to conventional TFA. It can be safely implemented by selecting patients with patent carpal circulation and adequate height.

Physiological Evaluation of Korea Ginseng, Deoduk and Doragi Pickles (인삼, 더덕 및 도라지 피클의 품질 특성 연구)

  • The principal objective of this study was to conduct a physiological evaluation of Korea Ginseng, Deoduk and Doragi pickles. Prior to the processing of the 3 kinds of pickles, total phenolic acid contents, lecithin oxidation inhibitory effect, SOD -linked activity and hydroxyl radical scavenging activity of Korea Ginseng, Deoduk and Doragi water extracts were assessed. After the processing of 3 kinds of pickles, we conduct a sensory evaluation and color values assessment. The total phenolic acid contents of Korea Ginseng, Deoduk and Doragi water extracts were $1.66{\sim}1.70mg/m{\ell}$, levels which were similar to that of tocopherol($1.81mg/m{\ell}$) but significantly lower than that of BHT($4.06mg/m{\ell}$)(p<0.05). The lecithin oxidation inhibitory effects of the Ginseng extract(98.86%) were similar to those of BHT(98.90%), but were significantly higher than those of Deoduk(35.70%), Doragi(78.07%) and tocopherol(65.91%). SOD-linked activity of Korea Ginseng water extract (42.58%) was similar to those of BHT(47.86%) and tocopherol(50.47%), but significantly higher than those of Deoduk (17.98%) and Doragi(20.75%). The hydroxyl radical scavenging activity of the Ginseng water extract(87.85%) was similar to that of BHT(8.58%), but significantly higher than that of Deoduk(79.51%), Doragi(77.62%) and tocopherol(78.95%). In the results of our sensory evaluations of the 3 kinds of pickles, the Ginseng pickle evidenced significantly lower acceptance scores in taste, color, flavor, texture, and overall quality. The luminance of the Ginseng pickle was significantly higher than the Deoduk pickles, the value of the Doragi pickle was significantly higher than those of the Ginseng and Deoduk pickles, and the b value of the Deoduk pickle was significantly higher than that of the Ginseng pickle.

Imported Malaria in Korea: a 13-Year Experience in a Single Center

  • The incidence of imported malaria has been increasing in Korea. Were viewed data retrospectively to evaluate the epidemiology, clinical features, and outcomes of imported malaria from 1995 to 2007 in a university hospital. All patients diagnosed with imported malaria were included. Imported malaria was defined as a positive smear for malaria that was acquired in a foreign country. A total of 49 patients (mean age, 35.7 year; M: F = 38 : 11)were enrolled. The predominant malarial species was Plasmodium falciparum (73.5%), and the most frequent area of acquisition was Africa (55.1%), followed by Southeast Asia (22.4%) and South Asia (18.4%). Fourteen-patients (30.6%) suffered from severe malaria caused by P. falciparum and 1 patient (2.0%) died of multiorgan failure. Most of the patients were treated with mefloquine (79.2%) or quinine (10.2%); other antimalarial agents had to be given in 13.2% treated with mefloquine and 44.4% with quinine due to adverse drug events (ADEs). P. falciparum was the most common cause of imported malaria, with the majority of cases acquired from Africa, and a significant number of patients had severe malaria. Alternative antimalarial agents with lower rates of ADEs might be considered for effective treatment instead of mefloquine and quinine.

The State of the Art of Preventive Medicine in North Korea With Reference to the Content Analysis of a Medical Textbook (의학교과서를 통해 본 북한예방의학 내용분석에 관한 연구)

  • Objectives : The most frequently cited health related slogan in North Korea is that socialistic medicine is preventive medicine. It implies that North Korea puts preventive medicine at the operational center of its national health care system. This study aimed at examining and comparing preventive medicine practices in North Korea with those of South Korea. Methods : Efforts have been made to obtain a textbook for analysis its contents. Many people have iassisted in the study by joining the interview. Some of these people are as follows : a former professor of PyongYang Medical School, NK physicians living in South Korea, WHO staffs, diplomatic officials, etc. The major items of analysis consisted of industrial medicine and hygiene, nutrition, school health, epidemiology, health statistics and heath policy & management. Results : Public health philosophy is finely noted and well integrated in the operation of the North Korean national health care system, particularly in the area of industrial medicine and hygiene. Preventive medicine with a strong health surveillance system spanning a number of broad social organizations is a major tool to improve the health of the people in North Korea. The emphasis on preventive medicine has a close relationship with the 'Juche Philosophy' and the shortage of pharmaceuticals and medical equipment. To cope with the shortage problem, North Korean health workers are encouraged to grow medicinal herbs. We have found that they put little effort into teaching newly emerging diseases, such as AIDS, VDT syndrome, hazards of EMF, and agricultural chemical poisonings. Of the subjects of the preventive medicine text, 78.9% coincide with those of South Korean industrial health manuals and 34.2% with South Korean epidemiology texts. However, an absolute difference was found to exist between the health policies and management systems. Conclusion : In North Korea, the concept of preventive medicine functions as the basic philosophic strategy of the national health care system. It differs greatly from the South Korean system in both practice and educational content. Its contribution to society is simply incomparable to that of South Korea. More communication and further study is called for in order to improve the preventive medicine practices in the future.

Adaptive Reference Structure Decision Method for HEVC Encoder (HEVC 부호화기의 적응적 참조 구조 변경 방법)

  • This paper proposes adaptive reference structure decision method to improve the performance of HEVC (High Efficiency Video Coding) encoder. When an event occurs in the input sequence, such as scene change, scene rotation, fade in/out, or light on/off, the proposed algorithm changes the reference structure to improve the inter prediction performance. The proposed algorithm divides GOP (Group Of Pictures) into two sub-groups based on the picture that has such event and decides the reference pictures in the divided sub-groups. Also, this paper proposes fast encoding method which changes the picture type of first encoded picture in the GOP that has such event to CRA (Clean Random Access). With the statistical feature that intra prediction is selected by high probability for the first encoded picture in the GOP carrying such event, the proposed fast encoding method does not operate inter prediction. The experimental result shows that the proposed adaptive reference structure decision method improves the BD-rate 0.3% and reduces encoding time 4.9% on average under the CTC (Common Test Condition) for standardization. In addition, the proposed reference structure decision method with the picture type change reduces the average encoding time 12.2% with 0.11% BD-rate loss.

  • Flywheel energy storage system (FESS) is defined as a high speed rotating flywheel system that can save surplus electric power. The FESS is proposed as an efficient energy storage system because it can accumulate a large amount of energy when it is operated at a high rotating speed and no mechanical problems are encountered. The FESS consists of a shaft, flywheel, motor/generator, bearings, and case. It is difficult to simulate rotor dynamics using common structure simulation programs because these programs are based on the 3D model and complex input rotating conditions. Therefore, in this paper, a program for the FESS based on the 2D FEM was developed. The 2D FEM can model easier than 3D, and it can present the multi-layer rotor with different material each other. Stiffness changing of the shaft caused by shrink fitting of the hub can be inputted to get clear solving results. The results obtained using the program were compared with those obtained using the common programs to determine any errors.

Differences in Larvicidal Activity of Complement and Chemiluminescent Response of Phagocytes in Carp (Cyprinus carpio), Crucian carp (Carassius auratus) and False Dace (Pseudorasbora parva) against Excysted Metacercariae of Clonorchis sinensis (간흡충 탈낭유충에 대한 잉어, 붕어, 참붕어의 보체 살충능 및 식세포 Chemiluminescent 반응 차이)

  • Differences in larvicidal activity of complement and chemiluminescent response of phagocytes in carp (Cyprinus carpio), crucian carp (Carassius auratus) and false dace (Pseudorasbora parva) against excysted metacercariae of Clonorchis sinensis were investigated. The prevalence of C. sinensis metacercariae of false daces caught at Nakdong river (NR) was 100%. However, no C. sinensis metacercariae were found in false daces, which were collected at a reservoir in Chinyoung (CY), crucian carps and carps. The sera of false dace, which were intensively infected with C. sinensis metacercariae, killed excysted metacercariae of C. sinensis more readily than that of carp. However, the serum obtained from C. sinensis metacercariae-free false dace showed the lowest larvicidal ability. The larvicidal ability of sera collected from each fish species completely disappeared when the complement was inactivated by heating. When supernatant of excysted metacercariae homogenate were added to phagocytes of each species, the chemiluminescent responses were significantly (p<0.05) diminished in false dace and carp. The inhibition ratio of chemiluminescent responses by the supernatant was 22.9% in false dace, 9.6% in crucian carp and 12.4% in carp.

Analysis of General Characteristics and Structural Characteristics Centering on Offline Board Games (오프라인 보드게임을 중심으로한 일반적 특성과 구조적 특성 분석)

  • This study selected 60 board games that have a high sales rate and awareness in the board game market, and analyzed 37 board games that were selected through the advice of experts. According to the analysis of general characteristics, there was the highest number of board games that more than 2 people could participate and people from ages 7 to 11 could use, and the most common play time was from 20 minutes to 30 minutes. Also, there was the largest number of board games produced in 2000s, and Germany was the most common producer of board games. Next, the content analysis showed that abstract strategy was the most common game genre, and cognitive domain was the most common in the development area. The analysis based on how to play the game showed that games that had to go through 4 stages were the most common. Card games were the most common form of game, group was the most common in organization form, and reaching goal was most common among result analysis method. According to the analysis of correlation by items, the number of people and work analysis had a statistical correlation, and the playing age, time, and genre had a correlation. The origin of the game and game genre were also correlated to each other, and game form, game genre, organization form, and result analysis had a statistical correlation as well. The purpose of this study is to analyze the board game in terms of structural characteristics and to provide a foundation for future research.

Comparison of the Korean Activity Status Index with cardiopulmonary exercise test in patients with acute myocardial infarction

  • This study aimed to compare the Korean Activity Status Index (KASI) with the cardiopulmonary exercise test (CPET) among patients with acute myocardial infarction. A total of 2,268 patients (85.4% male; mean age, 59.3±10.2 years; range, 23-90 years) diagnosed with acute myocardial infarction were enrolled in the Regional Center Myocardial Infarction Registry between July 2016 and June 2019. The KASI is a tool used to measure functional capacity by asking patients about their ability to perform specific activities and then scoring their responses. In contrast, CPET is the gold standard for assessing the objective functional capacity in patients undergoing cardiac rehabilitation. Peak oxygen uptake (VO2peak) was used to analyze the correlation. Patients who completed two consecutive KASI and CPET evaluations during their first (KASI_1, VO2peak_1) and second visits (KASI_2, VO2peak_2) for cardiac rehabilitation were included in the study. The mean KASI_1 and KASI_2 scores were 43.3±14.3 and 49.8±13.9, respectively, and the mean VO2peak_1 and VO2peak_2 scores were 25.9±8.0 and 28.5±8.3, respectively. Both the KASI scores were significantly correlated with the measured VO2peak during each visit, with correlation coefficients of 0.385 (P<0.001) and 0.346 (P<0.001), respectively. Moreover, the KASI score and VO2peak had a linear relationship (VO2peak_1=0.22×KASI_1+16.5, P<0.001; VO2peak_2=0.21×KASI_2+18.2,VO2peak_2=0.21×KASI_2+18.2, P<0.001). This study revealed that the KASI is a valid measure for the follow-up evaluation of the functional capacity of patients. These findings suggest that VO2peak can be predicted using the KASI score in patients who do not undergo CPET.
