The Effects of Lower Limb Training Using Sliding Rehabilitation Machine on the Foot Motion and Stability in Stroke Patients

  • Purpose: The purpose of this study was to investigate the effect of lower limb training using a sliding rehabilitation machine on the foot motion and stability in stroke patients. Methods: Thirty participants were allocated to two groups: Training group (n=15) and Control group (n=15). Subjects in the control group received physical therapy for 30 minutes, five times per week, and those in the training group received lower limb training using a sliding rehabilitation machine for 30 minutes, five times per week, with physical therapy for 30 minutes, five times per week, during a period of six weeks. Heel rotation, hallux stiffness, foot balance, metatarsal load, toe out angle, and subtalar joint flexibility were measured by RS-scan. Results: Significant improvement of the foot motion (hallux stiffness, meta load) and the foot stability (toe out angle, subtalar joint flexibility) was observed in the training group. Conclusion: This study demonstrated that lower limb training using a sliding rehabilitation machine is an effective intervention to improve the foot motion and stability.

The effects of the 4-weeks visual biofeedback training in individuals with hyperextended knee

  • 본 연구의 목적은 4주 동안의 시각 바이오피드백 훈련이 무릎관절 각도와 하지 근활성도에 미치는 영향을 조사하는 것이다. 15명의 과도한 무릎 젖힘을 가진 자원자가 본 연구에 참가하였다. 과도한 무릎 젖힘을 향상시키기 위해 4주 동안의 시각 바이오피드백 훈련을 사용하였다. 이 훈련은 발바닥의 앞 체중부하와 뒤 체중부하 사이의 균형을 유지하는 운동이다. 시각 바이오피드백 훈련 후, 무릎 관절 각도는 유의하게 증가하였으며, 앞정강근의 근전도는 유의하게 감소하였다. 본 연구의 결과를 통해 발바닥 압력 분산의 정보를 이용한 시각 바이오피드백 훈련은 과도한 무릎 젖힘에 대하여 치료효과가 있음을 확인하였다.

재활운동에 참가한 뇌졸중환자의 운동과학적 연구 (The Scientific Research of Rehabilitation Training Program Participants in Stroke Patients)

  • 본 연구는 뇌졸중 발병 후 재활운동에 참가하는 환자들을 대상으로 재활운동 3개월이 지난 시점에서 1차 실험을 하였고, 6개월이 지난 시점에서 2차 실험을 하여 운동역학적 비교분석을 하였다. 실험에 사용된 장비는 영상분석기, 족저압분석기, 근전도분석기를 사용하였다. 대상자는 7명으로 하였으며 통계방법은 t-test분석 이용하였다. 결과는 엉덩관절의 최대신전 피크 값과 최대신전 피크 모멘트 값에서 통계적으로 유의한 차이를 나타내었다(p<0.05). 족저압의 비교에서는 환측 다리의 족저압에서 일반적인 걷기 동작시에 족저압의 중심이동거리에서 통계적으로 유의한 차이를 나타내었다(p<0.05). 근전도 분석에서는 대퇴사두근 중의 대퇴직근과 외측광근의 근력에서 통계적으로 유의한 차이를 나타내었다(p<0.05).

시각되먹임 균형훈련이 낙상을 경험한 노인의 균형에 미치는 효과 (Effects of Visual Feedback-Based Balance Training on Balance in Elderly Fallers)

  • Purpose: The purpose of this study was to investigate the effects of a visual feedbackbased balance training, using force platform biofeedback, on the postural balance of elderly faller. Methods: Fifty one community-dwelling older adults (aged 66-88 years) with a recent history of fall participated in the study. Participants were randomized to an experimental group (EG, n=25) and to a control group (CG, n=26). The EG participated in training sessions three times/week for 6 weeks. Visual feedbackbased balance training with the a computerized force platform with visual feedback screen was used in the experimental group. Static balance (center of gravity) and dynamic balance (Functional reach test, Timed "Up & Go" test, Berg balance scale) were assessed before and after end of training. Results: A significant improvement in static balance and dynamic balance were demonstrated within the EG (p<.05), but not in the CG. Conclusion: Visual feedback-based balance training may be an effective intervention to improve postural balance of elderly fallers.

Effect of Treadmill Training With Eyes Open and Closed on Knee Proprioception, Functional Balance and Mobility in Children With Spastic Diplegia

  • Objective To investigate the effect of treadmill training with eyes open (TEO) and closed (TEC) on the knee joint position sense (JPS), functional balance and mobility in children with spastic diplegia. Methods Forty-five children with spastic diplegia aged 11-13 years participated in this study. They were randomly assigned to three groups of equal number. The control group (CON) underwent designed physical therapy program whereas, the study groups (TEO and TEC) underwent the same program, in addition to treadmill gait training with eyes open and closed, respectively. Outcome measures were the degree of knee joint position error, functional balance and mobility. Measurements were taken before and after 12 weeks of intervention. Results After training, the three groups showed statistically significant improvement in all measured outcomes, compared to the baseline with non-significant change in the knee JPS in the CON group. When comparing posttreatment results, the TEC group showed greater significant improvement in all measured outcomes, than the TEO and CON groups. Conclusion Treadmill training with eyes open and closed is effective in rehabilitation of children with diplegia, but blocked vision treadmill training has more beneficial effect.

Effect of Uneven Surface Gait Training on Ankle Muscle Activation and Balance in Stroke Patients

  • Purpose: The purpose of this study was to investigate the effects of regular training on the uneven surface that stroke patients encounter in their daily life on their ankle joint muscle activity and balance ability. They were divided into two groups: the gait training group on uneven surfaces and the gait training group on normal surfaces. Methods: In this study, 30 patients diagnosed with stroke and undergoing rehabilitation were selected. 15 people in the uneven surface gait training group and 15 people in the flat gait training group were selected. The muscle activation of the ankle muscles was measured when walking again on a even surface after walking on an uneven surface and on a flat ground. After each gait training, the limit of stability and Romberg test were performed to evaluate the balance ability. Results: As a result of the experimental results before and after walking by group, the tibialis anterior muscle activity of the paralyzed side was significantly decreased in the uneven surface walking group. As a result of measuring balance ability after training, the limit of stability in all directions was significantly increased in the uneven surface gait training group, and the area and length moved significantly decreased in the uneven surface gait training group in the Romberg test as well (p<0.05). Conclusion: After walking on uneven surface, it was confirmed that the muscle activity of the ankle joint decreased in normal flat walking, and thus the efficiency of muscle activity was increased. In addition, it was possible to confirm the improvement of the balance ability of the gait training on the uneven surface, and in conclusion, it could be confirmed that it had an effect on the improvement of the walking ability.

교각운동시 엉덩관절 초기 굽힘 각도에 따른 체간 및 하지의 근활성도 분석 (Analysis of the Muscle Activity of the Trunk and the Lower Extremities in Relation to the Initial Bending Angle of the Hip Joint During Bridge Exercise)

  • Background: The present study was conducted with 30 adult males in order to examine the muscle activity of the trunk and the lower extremities at diverse initial bending angles of the hip joint during bridge exercise on a stable surfaces and on an unstable surface that is widely performed for stabilization. Methods: The initial angles of the hip joint used were $0^{\circ}$, $45^{\circ}$ and $90^{\circ}$ and the subjects were divided into a matt experimental group and a balance training group. Results: In maximum values of muscle activity at different exercise methods and angles, the matt experimental group showed statistically significant differences in the muscle activity values of the rectus abdominis muscle, the erector spinae muscle, the rectus femoris muscle and the peroneus muscles between different angles while the balance training group showed significant differences only in the muscle activity values of the erector spinae muscle between different initial angles of the hip joint. The matt experimental group showed significant differences in muscle activity between initial angles $0^{\circ}$ and $90^{\circ}$, between $45^{\circ}$ and $90^{\circ}$ in the rectus abdominis muscle, between $0^{\circ}$ and $90^{\circ}$ in the erector spinae muscle, between $45^{\circ}$ and $90^{\circ}$ in the rectus femoris muscle and between $0^{\circ}$ and $90^{\circ}$ in the peroneus muscles while the balance training group showed significant differences between $0^{\circ}$ and $90^{\circ}$ in the erector spinae muscle. Conclusions: Therefore, it is thought that bridge exercises should be applied to patients using diverse methods.

Evaluation of Biomechanical Movements and Injury Risk Factors in Weight Lifting (Snatch)

  • Objective: The purpose of this study was to investigate the possibility of injuries and the types of movement related to damage by body parts, and to prepare for prevention of injuries and development of a training program. Method: For this study, the experiment was conducted according to levels of 60 percentages (ST) and 85 percentages (MA) and 10 subjects from the Korean elite national weightlifting team were included. Furthermore, we analyzed joint moment and muscle activation pattern with three-dimensional video analysis. Ground reaction force and EMG analyses were performed to measure the factors related to injuries and motion. Results: Knee reinjuries such as anterior cruciate ligament damage caused by deterioration of the control ability for the forward movement function of the tibia based on the movement of the biceps femoris when the rectus femoris is activated with the powerful last-pull movement. In particular, athletes with previous or current injuries should perceive a careful contiguity of the ratio of the biceps femoris to the rectus femoris. This shows that athletes can exert five times greater force than the injury threshold in contrast to the inversion moment of the ankle, which is actively performed for a powerful last pull motion and is positively considered in terms of intentional motion. It is activated by excessive adduction and internal rotation moment to avoid excessive abduction and external rotation of the knee at lockout motion. It is an injury risk to muscles and ligaments, causing large adduction moment and internal rotation moment at the knee. Adduction moment in the elbow joint increased to higher than the injury threshold at ST (60% level) in the lockout phase. Hence, all athletes are indicated to be at a high risk of injury of the elbow adductor muscle. Lockout motion is similar to the "high five" posture, and repetitive training in this motion increases the likelihood of injuries because of occurrence of strong internal rotation and adduction of the shoulder. Training volume of lockout motion has to be considered when developing a training program. Conclusion: The important factors related to injury at snatch include B/R rate, muscles to activate the adduction moment and internal rotation moment at the elbow joint in the lockout phase, and muscles to activate the internal rotation moment at the shoulder joint in the lockout phase.

인체주행 시 중족지절 관절 보조 강성에의 적응에 따른 동역학적 변화 고찰 (Dynamic Changes depending on Adaptation to Assistive Joint Stiffness in Metatarsophalangeal Joint during Human Running)

  • Recently, several studies have been conducted to lower the cost of transport of human by adding external joint stiffness elements. However, it has not been clearly elucidated whether adaptation time is required for human subjects to adapt to the added external joint stiffness. In this study, carbon plates in the form of shoe midsoles were added to the metatarsophalangeal joint, and the lower limb joint torque and mechanical energy consumption were compared before and after a total of 5 sessions (2.5 weeks) of running. A total of 11 young healthy participants exhibited higher elastic energy storage in carbon plates in the fifth session compared to the first session, and lower power in the ankle joint. This suggests that a single training session may be insufficient to validate the efficiency effect of added joint stiffness, and the human body seems to increase the elastic energy stored in the assistive joint stiffness and its reutilization.

관절각도에 따른 근 피로도와 등척성 훈련 전이효과 (Muscle Fatigue according to Joint Angle and the Transfer Effect with Isometric Training)

  • This study examined whether there is a transfer effect to other joint angles and the angular specificity of muscle fatigue after 6 weeks of isometric training of the vastus medialis. Twenty subjects were randomly assigned to 30° and 90° knee flexion groups and were trained at 80% maximal voluntary isometric contraction(MVIC) three times a week for 6 weeks. The pre-and post-training values of the 80% holding time(endurance time) of MVIC, the Fatigue Index(FI), and the MVIC at 30°, 60°, and 90° were compared. After isometric training for 6 weeks, in the 30° knee flexion group, FI decreased significantly(p<0.05) only at 30°, which was the training angle; there was no change at other angles. By contrast, in the 90° knee flexion group, FI decreased significantly(p<0.05) at both the trained angle and at the other angles, indicating a transfer effect of training. MVIC did not increase significantly(p<0.05) at any trained angle in either the 30° or 90° knee flexion groups after 6 weeks of isometric training, neither did the 80% holding time of MVIC differ significantly compared with pre-training in either group. These findings suggest that training at 90° of knee flexion is more effective than training at 30° of knee flexion for obtaining a training transfer effect on muscle fatigue in the vastus medialis.
