Effects of an Integrated Case Management Program on Medication Adherence, Pain, Physical Function and Depression among Korean Medical Aid Beneficiaries with Osteoarthritis (의료급여 관절염환자의 복약순응향상을 위한 통합중재프로그램의 효과)

  • Purpose: The purpose of this study was to test the effectiveness of an eight session integrated case management program for improvement of medication adherence, physical function, pain, and depression among medical aid beneficiaries with osteoarthritis. Method: A nonequivalent control group pre-posttest design was employed. Participants were 55 medical aid beneficiaries who agreed to participate in this study, and were assigned to an experimental group (n=28) or control group (n=27). The framework of this research derived from Cox's Interaction Model of Client Health Behavior guided the overall intervention and the components. The program led by a case manager with a medication calendar, motivating interviewing and coaching strategies and collaboration with a pharmacist. Analysis included change in scores, ${\chi}^2$-test, and t-test. Result: The results showed significant increase in medication adherence, physical function and decrease joint pain, joint stiffness and depression in the experimental group compared to the control group. Conclusion: The eight session integrated case management program indicated an effect on medication adherence, pain, physical function, and depression. Partnership with a pharmacist is recommended for medication adherence and conduct of further studies will be needed in order to determine the long-term effect of an extended integrated program on health outcomes.

Lessons of Goseong Abandoned Metal Mine Accident (고성 폐금속 광산 오염 사건의 교훈)

  • The Goseong abandoned mine accident was a typical case of long-term sequela resulting from environmental disruption during the rapid economic development of Korea. While the final conclusion of epidemiologic investigation was that residents surrounding the abandoned mine were simply exposed to cadmium from the abandoned mine without any patient contracting 'itai-itai' disease, not only did residents around the abandoned mine suffer enormous psychological and economic damage, but people in neighbouring communities did as well when the entire area was stigmatized as an contaminated area. Environmental civic groups and the government worked together to solve the problem by forming a joint committee which held the right of decision in any matter to be determined. By inviting all the stakeholders to participate in the joint committee, a transparent investigation was guaranteed and the results of investigation and recommendations to solve the problem were accepted by all involved. Even though the environmental health division in Ministry of Environment has developed considerably and built up a capacity to deal with environmental accidents, the process which was adopted to solving the Goseong abandoned mine accident can be useful to settle environmental health issues with severe conflicts among stakeholders.

The Effect of Self-Management Course on Pain, Flexibility of Lumbar Spine, Uncertainty and Self-efficacy in patients with Ankylosing Spondylitis (자기관리과정이 강직성 척추염환자의 요통, 허리유연성, 불확실성과 자기효능감에 미치는 효과)

  • Purpose: To investigate the effects of self-management course in patients with ankylosing spondylitis. Method: Forty-nine subjects were sampled according to research criteria, and divided into two groups: 24 of the experimental group and 25 of control group. To the experimental group, self-management course which developed by the investigators was applied for 6 weeks, each session adopted for two hours per week and additional 30-minutes exercise was carried out two times a week. Result: After participating the self-management course in experimental group, 1) the degree of pain was significantly decreased 2) significant improvement in the flexibility of lumbar spine, 3) uncertainty was significantly decreased. 4) Self-efficacy was not changed between the two groups after self-management course. Conclusions: These findings indicate that the self-management course could be effective in improving the flexibility of lumbar spine and decreasing pain, and uncertainty in patients with ankylosing spondylitis.

Analysis of Health Counseling by the Internet - on the Home page of Korean Rheumatology Health Professions Society (인터넷을 이용한 관절염 환자의 건강상담 내용분석 -대한류마티스 건강전문학회 홈페이지를 대상으로-)

  • Recently with the development in computer technology and its communication system, many changes in medical informatics enable us to use various medical information regardless of time or place. There are many home pages on the web, which provide medical counseling and hospital information. On May 11th 1999, Korean Rheumatology Health Professions Society began its new service as a home page on the web with various rheumatologic health information, questioning/ answering, and so on. This study was undertaken to examine the content and the purpose of health counseling on the web. The data was collected from 173 questioners who visited questioning/ answering site in the KRHPS home page for May 11th, 1999 through November 10th, 1999. Most of the questioners consulted the health problems of their or their families. Over two thirds of them were already diagnosed medically. Rheumatoid arthritis was the most frequent one. Other diseases, such as, osteoarthritis, ankylosing spondylitis, Still's disease were also on the list. Most of the questioners wanted to know treatment strategies, to consult about their symptoms, and to make diagnosis. And many questioners wanted detailed explanations about their diseases or the informations regarding the hospital. These findings suggest that the health counseling on the web may be used to supplement the lack of direct medical interviews with doctors. It also is expected to guide the patients to the right direction.

Factors Influencing Illness Intrusiveness of the Sexual Life in Women with Overactive Bladder (과민성방광 여성 환자의 성생활에 관한 지각된 질병의거변화의 영향요인)

  • Purpose: This study was conducted to explore factors that influence illness intrusiveness of the sexual life in women with overactive bladder. Methods: Ninety-two women diagnosed with overactive bladder, from D University Hospital and three private urology hospitals in B region, participated in a survey between April 7 and August 7, 2012. The Overactive Bladder-questionnaire (OAB-q), the Marital Intimacy Tool, the Center for Epidemiologic Studies Depression (CES-D), and 1 question to ask illness intrusiveness of the sexual life were used in the study. Results: The illness intrusiveness score of the sexual life in women with overactive bladder was 2.22 out of 5. There were no significant differences of illness intrusiveness by demographic and disease related characteristics. The predictor were the clinical symptom score and depression, accounting for 13% of the variance of the illness intrusiveness of the sexual life in the women with overactive bladder. Conclusion: Effective nursing intervention to relieve clinical signs of overactive bladder and reduce depression can improve quality of sexual life in women with overactive bladder.

A Predictors of Exercise Participation in Rheumatoid Arthritis Patients (류마티스 관절염 환자의 운동참여 예측요인)

  • The purposes of this study were to examine and to predict the affecting factors on exercise participation of Rheumatoid arthritis Patients. The subjects were 161 adult out-patients who visited the hospital for rheumatic disease in H-university. Data were composed of self-reported questionnaire. The conceptual model of this study consisted of that personal characters(age, marriage, education, income), situational characters(pain intensity, fatigue, IADL, depression), behavioral characters(formerly exercise behavior, life-style), and cognitive-perceptional characters(perceived health status, perceived benefit, perceived barrier, perceived self-efficacy) affected directly to exercise participation. Logistic regression analysis was applied for testing model of this study. The results were as follows : 1. Personal characters(education), situational characters(pain intensity), behavioral characters(formerly exercise behavior, life-style), and cognitive characters(perceived barrier, perceived self-efficacy) were significant difference between current exercise participants(127subjects) and non-exercise participants(34). 2. Personal characters(income), situational characters(pain intensity), behavioral characters(life-style), and cognitive-perceptional characters(perceived barrier, perceived self-efficacy) were correlated to exercise participation. 3. Formerly exercise behavior, perceived barrier, and perceived self-efficacy were significant predictor of exercise participation. The logistic equation predicted overall 81.94% of this study subjects 161.

  • Temporomandibular joint disorders (TMD) often show muscular disorder, internal derangement and degenerative change of articular surface of the condyle. It is important to choose adequate diagnostic method for obtaining data for the diagnosis and treatment. Magnetic Resonance Imaging (MRI) has some advantages, because it shows good resolution image for the soft tissue and it is not invasive to the human body. We have performed serial research about the retrodiscal tissue of the TMJ according to the progress of the disease, using T2 weighted image, since 2002. In this study, we applicated resized observation area to compare signal intensity between the retrodiscal tissue of the TMJ and cerebral gray matter, using T2 weighted image.

Temporomandibular joint disorder from skull-base osteomyelitis: a case report

  • Skull-base osteomyelitis is a rare disease affecting the medulla of the temporal, sphenoid, and occipital bones. In general, it occurs due to external ear canal infections caused by malignant external otitis. Skull-base osteomyelitis usually affects elderly diabetic patients. The patient, a 58-year-old man, was referred for evaluation and management of the left jaw. Clinical examination of the patient revealed pain in the left jaw and mouth-opening deflection to the left. The maximum active mouth opening was measured to about 27 mm. Panoramic, CT, and CBCT revealed bone resorption patterns in the left condyle. Through control of diabetes, continued pharmacological treatment, arthrocentesis, and occlusal stabilization appliance therapy were carried out. The extent of active mouth opening was increased to 45 mm, and pain in the left jaw joint was alleviated. This was a case wherein complications caused by failure to control diabetes induced skull-base osteomyelitis. There is a need for continued discussion about the advantages and disadvantages of arthrocentesis with lavage for patients with skull-base osteomyelitis and other treatment options.

Comparison of Movement Axis Change during Cervical Flexion and Extension according to Cervical Lordosis Angle to Scoliosis Patients (척추옆굽음증 환자에서 목뼈 앞굽음각 감소에 따른 목뼈 굽힘과 폄시 운동축의 변화 비교 연구)

  • Background: The purpose of this study is to compare axis change during cervical flexion and extension according to lordosis angle to patients with scoliosis. Methods: Movement axis change was estimated during cervical flexion and extension in twenty-four scoliosis patients with hypolordosis using radiography. Subjects were divided into mild lordotic curve group (MLCG, n=12, $34{\sim}25^{\circ}$) and severe lordotic curve group (SLCG, n=12, less $25^{\circ}$) according to cervical lordosis angle. Results: During cervical flexion, both group showed movement axis change to upper part of cervical vertebra and SLCG showed greater than MLCG but there is no significant difference. During cervical extension, SLCG showed greater than MLCG and there is significant difference. Conclusion: It is considered that cervical hypolordosis acts as important factor to scoliosis and degenerative joint disease because it leads to change of movement axis and central route of joint.

Physical Function of Patients with Amyotrophic Lateral Sclerosis (근위축성측삭경화증 환자의 신체적 기능 상태)

  • Purpose: This study was designed to present preliminary data for the development of appropriate nursing care system for the patients with ALS by analyzing their physical function. Method: The clinical data of 36 ALS patients, who visited ALS Clinic of H University Hospital in Seoul, were collected from January, 2006 to August, 2006. To determine the physical function, Norris ALS scale and Appel ALS Rating Scale were used. The data were analyzed by frequency, percentage, mean, standard deviation, range, t-test, ANOVA, using SPSS PC program. Results: The mean score of physical activity, muscle strength, upper extremity function, lower extremity function was 18.08, 27.72, 25.94, 25.19 respectively. There were significant differences in physical activity, muscle strength, and upper extremity function according to sender and comorbid disease(diabetes). Although sites of symptom onset were not statistically significant with all physical function, patients with bulbar onset showed relatively severe physical disabilities. Conclusion: The preliminary data on physical function of patients with ALS would be helpful for the development of ALS nursing guideline system.
