Diagnostic Performance of a New Convolutional Neural Network Algorithm for Detecting Developmental Dysplasia of the Hip on Anteroposterior Radiographs

  • Objective: To evaluate the diagnostic performance of a deep learning algorithm for the automated detection of developmental dysplasia of the hip (DDH) on anteroposterior (AP) radiographs. Materials and Methods: Of 2601 hip AP radiographs, 5076 cropped unilateral hip joint images were used to construct a dataset that was further divided into training (80%), validation (10%), or test sets (10%). Three radiologists were asked to label the hip images as normal or DDH. To investigate the diagnostic performance of the deep learning algorithm, we calculated the receiver operating characteristics (ROC), precision-recall curve (PRC) plots, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) and compared them with the performance of radiologists with different levels of experience. Results: The area under the ROC plot generated by the deep learning algorithm and radiologists was 0.988 and 0.988-0.919, respectively. The area under the PRC plot generated by the deep learning algorithm and radiologists was 0.973 and 0.618-0.958, respectively. The sensitivity, specificity, PPV, and NPV of the proposed deep learning algorithm were 98.0, 98.1, 84.5, and 99.8%, respectively. There was no significant difference in the diagnosis of DDH by the algorithm and the radiologist with experience in pediatric radiology (p = 0.180). However, the proposed model showed higher sensitivity, specificity, and PPV, compared to the radiologist without experience in pediatric radiology (p < 0.001). Conclusion: The proposed deep learning algorithm provided an accurate diagnosis of DDH on hip radiographs, which was comparable to the diagnosis by an experienced radiologist.

  • Cloud computing is now used by most companies, business centres and academic institutions to embrace new computer technology. Cloud Service Providers (CSPs) are limited to certain services, missing some of the assets requested by their customers, it means that different clouds need to interconnect to share resources and interoperate between them. The clouds may be interconnected in different characteristics and systems, and the network may be vulnerable to volatility or interference. While information technology and cloud computing are also advancing to accommodate the growing worldwide application, criminals use cyberspace to perform cybercrimes. Cloud services deployment is becoming highly prone to threats and intrusions. The unauthorised access or destruction of records yields significant catastrophic losses to organisations or agencies. Human intervention and Physical devices are not enough for protection and monitoring of cloud services; therefore, there is a need for more efficient design for cyber defence that is adaptable, flexible, robust and able to detect dangerous cybercrime such as a Denial of Service (DOS) and Distributed Denial of Service (DDOS) in heterogeneous cloud computing platforms and make essential real-time decisions for forensic investigation. This paper aims to develop a framework for digital forensic for the detection of cybercrime in a joined heterogeneous cloud setup. We developed a Digital Forensics model in this paper that can function in heterogeneous joint clouds. We used Unified Modeling Language (UML) specifically activity diagram in designing the proposed framework, then for deployment, we used an architectural modelling system in developing a framework. We developed an activity diagram that can accommodate the variability and complexities of the clouds when handling inter-cloud resources.

The cooperative regulatory effect of the miRNA-130 family on milk fat metabolism in dairy cows

  • Objective: There is a strong relationship between the content of beneficial fatty acids in milk and milk fat metabolic activity in the mammary gland. To improve milk quality, it is therefore necessary to study fatty acid metabolism in bovine mammary gland tissue. In adipose tissue, peroxisome proliferator-activated receptor gamma (PPARG), the core transcription factor, regulates the fatty acid metabolism gene network and determines fatty acid deposition. However, its regulatory effects on mammary gland fatty acid metabolism during lactation have rarely been reported. Methods: Transcriptome sequencing was performed during the prelactation period and the peak lactation period to examine mRNA expression. The significant upregulation of PPARG drew our attention and led us to conduct further research. Results: According to bioinformatics prediction, dual-luciferase reporter system detection, real-time quantitative reverse transcription polymerase chain reaction and Western blotting, miR-130a and miR-130b could directly target PPARG and inhibit its expression. Furthermore, triglyceride and oil red O staining proved that miR-130a and miR-130b inhibited milk fat metabolism in bovine mammary epithelial cells (BMECs), while PPARG promoted this metabolism. In addition, we also found that the coexpression of miR-130a and miR-130b significantly enhanced their ability to regulate milk fat metabolism. Conclusion: In conclusion, our findings indicated that miR-130a and miR-130b could target and repress PPARG and that they also have a functional superposition effect. miR-130a and miR-130b seem to synergistically regulate lipid catabolism via the control of PPARG in BMECs. In the long-term, these findings might be helpful in developing practical means to improve high-quality milk.

RC 슬래브교의 신축이음 손상과 바닥판 응답과의 상관관계 분석 (Correlation Analysis between Damage of Expansion Joints and Response of Deck in RC Slab Bridges)

  • 우리나라 노후화 교량 중 많은 부분을 차지하는 RC 슬래브 교량은 대부분이 교량 안전 관리 및 유지관리의 대상인 1, 2종 교량에 포함되지 않는다. 또한 고속도로 교량에서 가장 높은 손상율을 차지하는 부속시설은 신축이음과 교량 받침부이다. 특히, 신축이음부를 통한 누수는 콘크리트 교량의 열화를 유발하고 균열 및 철근부 노출 등의 원인이 된다. 따라서 본 연구에서는 RC 슬래브교의 신축이음부 유간 밀착 손상이 바닥판 응답에 미치는 영향을 분석하였다. 양단 신축이음의 유간이 밀착된 경우 바닥판의 교축방향 변위가 발생되지 못해, 양쪽 단부 콘크리트에서 균열이 발생하였다. 또한, 응답 결과를 바탕으로 손상시나리오 별 신축이음부의 교축방향 변위와 바닥판 양단 콘크리트 응력과의 상관관계를 분석하고, 손상발생이 교량 거동에 미치는 영향을 파악하였다. 신축이음 장치 양면손상이 발생한 경우 변위와 응력과의 상관관계는 차량이 모든 차선을 지날 때 0.18로 낮은 상관성을 보였다. 무손상 상태와 비교했을 때도 바닥판 처짐은 차량이 지나는 동안 0.34~0.53, 차량이 지나간 후 0.17~0.43으로 낮은 상관성을 보였다. 이는 단부 콘크리트의 균열 발생으로 바닥판 전체 거동에 변화가 생긴 것을 의미한다. 따라서 바닥판 응답 데이터 기반 상관관계 분석과 이를 활용한 데이터 기반의 손상탐지가 가능하며, 이를 통해 바닥판 손상 및 열화의 원인이 되는 신축이음부의 조기 보수 및 보강, 사전관리에 기여하여 많은 도움이 될 것으로 기대된다.

황철석 광산 광미댐에서의 지하수흐름 경로탐지를 위한 물리탐사 적용 (Application of Geophysical Methods to Detection of a Preferred Groundwater Flow Channel at a Pyrite Tailings Dam)

  • 호주 남부 부룩캉가지역의 폐광된 황철석광산의 광미댐 (Tailings Dam)에서는 광미와 지하수의 반응으로 황산이 형성, 배출되어 주변의 토양, 하천 및 지하수를 오염시키고 있다. 이와 같은 산성 폐수에 의한 환경오염 확산 방지를 위해서는 광미댐을 통하여 흐르는 지하수의 흐름경로를 파악하고 이를 감소시키는 노력이 필요하다. 본 논문에서는 지하수면 깊이의 변화를 파악하고 이를 통하여 지하수의 주 흐름경로를 확인하기 위하여 기존 유용광물 탐사에 주로 사용된 물리탐사방법들이 사용되었다. 본 연구에서 물리탐사방법으로는 시간영역 전자탐사 (TEM), DC 전기비저항탐사 그리고 자연전위 (SP)탐사법들이 사용되었다. SP법에 의하여 측정된 자연전위들은 매우 작았으며, SP자료해석만으로 주 지하수흐름의 경로규명이 어려웠다 TEM과 DC전기비저항 자료해석을 통하여 지하수면의 깊이를 정확하게 결정할 수 있었으며, DC 전기비저항법은 주로 10m내의 천부 대수층 구분에 유용한 반면에 TEM법은 성부구조 결정에 더욱 좋은 분해능을 보였다 TEM과 DC탐사자료들의 복합역산 (Joint Inversion)은 각각의 역산 (Inversion)보다 정확한 지전기적 (Geoelectric) 단면도를 제공하였으며, 물리탐사에 의하여 결정된 지하수면 깊이들은 각 측점에 설치된 관측공에서 피에조미터 (piezometer)에 의하여 측정된 지하수 수위와 거의 일치하였다. 모든 TEM 및 DC탐사 자료해석을 통하여 산출된 지하수 수위도는 탐사지역의 남동부에서 약 1m의 가장 얄은 수위를 나타나며, 반면에 가장 깊은 심도 (약 17 m)의 지하수 수위는 탐사지역의 북서부에서 위치하였다. 또한, TEM 및 DC탐사 자료해석에 의하여 산출된 탐사지역내의 대수층의 전기비저항 분포도에서는 높은 전기전도도의 대수층은 광미댐의 북동부에 존재하며, 가장 낮은 전기전도도를 갖는 대수층은 댐의 남동부에 분포하였다. 이와 같은 결과들로부터 오염이 되지 않은 신선한 (전기비저항이 높은) 지하수 유입의 근원은 광미댐의 남동부에 있음을 시사한다.

Expression and Significance of ER, PR, VEGF, CA15-3, CA125 and CEA in Judging the Prognosis of Breast Cancer

  • Objective: To explore the expression and significance of estrogen receptor (ER), progestrone receptor (PR), vascular endothelial growth factor (VEGF), CA15-3, CA125 and carcinoma embryonic antigen (CEA) expression in judging the prognosis of breast cancer. Materials and Methods: Sixty-five patients with breast cancer undergoing operations in the general surgery department were considered as the observation group, and 50 healthy outpatients of our hospital as the control group. Cubital venous blood was drawn in the morning from fasting patients in the two groups and chemiluminescence immunoassays were used to detect the levels of CA15-3, CA125 and CEA in serum. The follow-up duration was from 4 months to 2 years, and change in levels of the indicators was detected by dynamically drawing blood. After surgery, cancer tissue samples of patients in observation group remained on file (the non-recurrent patients were biopsied). Immunohistochemistry was applied to determine the expression of ER, PR and VEGF in tissue. Results: The effective rate of 12 patients with negative ER and PR expression was 33.3% in the observation group, being associated with prognosis to varying extents. Serum CA15-3, CA125 and CEA in the observation group were all significantly higher than in control group (p<0.01). With increase in pathological staging, levels of serum CA15-3, CA125 and CEA gradually increased (p<0.01). Levels in patients with lymph node metastasis were markedly higher than in those without (p<0.01). In addition, values with distal lymph node metastasis were notably higher than with adjacent lymph node metastasis (p<0.01). The postoperative follow-up results revealed that positive VEGF and levels of serum VEGF, CA15-3, CA125 and CEA in recurrence group were obviously higher than in non-recurrence group (p<0.01). Conclusions: Joint detection of ER and PR expression as well as levels of serum VEGF, CA15-3, CA125 and CEA is meaningful and can guide the diagnosis and treatment for breast cancer.

Experimental and numerical disbond localization analyses of a notched plate repaired with a CFRP patch

  • Through the use of finite element analysis and acoustic emission techniques we have evaluated the interfacial failure of a carbon fiber reinforced polymer (CFRP) repair patch on a notched aluminum substrate. The repair of cracks is a very common and widely used practice in the aeronautics field to extend the life of cracked sheet metal panels. The process consists of adhesively bonding a patch that encompasses the notched site to provide additional strength, thereby increasing life and avoiding costly replacements. The mechanical strength of the bonded joint relies mainly on the bonding of the adhesive to the plate and patch stiffness. Stress concentrations at crack tips promote disbonding of the composite patch from the substrate, consequently reducing the bonded area, which makes this a critical aspect of repair effectiveness. In this paper we examine patch disbonding by calculating the influence of notch tip stress on disbond area and verify computational results with acoustic emission (AE) measurements obtained from specimens subjected to uniaxial tension. The FE results showed that disbonding first occurs between the patch and the substrate close to free edge of the patch followed by failure around the tip of the notch, both highest stress regions. Experimental results revealed that cement adhesion at the aluminum interface was the limiting factor in patch performance. The patch did not appear to strengthen the aluminum substrate when measured by stress-strain due to early stage disbonding. Analysis of the AE signals provided insight to the disbond locations and progression at the metal-adhesive interface. Crack growth from the notch in the aluminum was not observed until the stress reached a critical level, an instant before final fracture, which was unaffected by the patch due to early stage disbonding. The FE model was further utilized to study the effects of patch fiber orientation and increased adhesive strength. The model revealed that the effectiveness of patch repairs is strongly dependent upon the combined interactions of adhesive bond strength and fiber orientation.

융기성 피부섬유육종의 치료 및 예후 (Dermatofibrosarcoma Protuberans ; Treatment and Prognosis)

  • 목적 : 융기성 피부섬유육종은 피부에 생기는 드문 종양으로서 부적절한 절제가 잦은 재발과 예후에 영향을 미치는 것으로 보고되고 있다. 저자들이 경험한 환자를 대상으로 치료와 예후의 관계를 연구하여 적절한 치료법을 제시하고자 하였다. 대상 및 방법 : 1990년 3월부터 본원 정형외과에서 치료한 융기성 피부섬유육종 환자중 추시 기간이 최소 12개월 이상인 14례를 대상으로 치료 방법 및 최종 추시까지의 상태를 점검하였다. 전례 모두 병리학적 진단하에 치료되었으며, 재발 후 전원된 9례를 포함한 13례에서 광범위 절제술이 시행되었다. 1례는 서혜부의 신경, 혈관이 종양과 인접하여 종양내 전절제술을 시행하였고 술후 항암요법을 추가한 경우였다. 결과 : 재발후 전원되었던 9례의 평균 재발 회수는 1.3 (1~2)회였으며, 처음 수술 후 첫번째 재발까지의 기간은 평균 11.8(2~24)개월이었다. 술후 최종 추시까지(평균 추시기간 50.8개월)에서 광범위절제술로 치료한 13례에서는 재발을 볼 수 없었으나, 병소내 전절제술과 항암제치료를 하였던 예에서는 반복된 재발을 보였고, 결국 폐전이로 사망하였다. 결론 : 융기성 피부섬유육종은 반드시 생검을 통한 확진 후 초기에 광범위 절제연 이상으로 절제를 시행하여야 할 것으로 사료된다. 재발은 최소한 2년까지의 추시가 있어야 그 여부를 알 수 있을 것으로 사료된다.

천추 척색종의 수술적 치료 후 장기 추시 결과 (Long-Term Results of Surgical Treatment for Sacral Chordoma)

  • 목적: 천추에 발생한 척색종에 대해 수술적 치료를 시행했던 환자들의 장기 추시 결과를 알아보고자 하였다. 대상 및 방법: 천추 척색종으로 수술을 시행받았던 4예를 대상으로 하였으며, 평균 추시 기간은 8.3년(3~11)이었다. 3예에서 후방 도달법을, 1예에서는 전후방 도달법을 통해 종양의 완전 절제를 시행하였으며, 1예에서 제 2천추 신경근 이상을, 3예에서 제 3천추 신경근 이상을 보존하였다. 술 후 배뇨, 배변 장애 유무를 포함한 하지의 운동, 감각 장애 유무를 평가하였고 수술 부위 감염 등의 합병증과 추시상 국소적인 재발 및 원거리 전이 유무를 조사하였다. 결과: 전 예에서 술 후 하지의 운동 장애는 없었으나 1예에서 우하지의 방사통이 심하였고, 1예에서 수술창의 감염이 있었다. 술 후 배뇨 기능은 2예에서 정상이었고 1예에서는 간헐적 요실금을, 1예에서는 지속적인 자가 도뇨를 시행하였다. 배변 기능은 1예에서 정상이었고, 2예에서는 변비를 보였다. 추시상 1예에서 재발 및 전이가 없었고, 1예에서 수술 부위의 재발이, 1예에서 원거리 전이 및 수술 부위의 재발이 있었다. 결론: 천추에 발생한 척색종에 대하여 수술적 절제 후 평균 8년 이상의 생존으로 만족할 만한 결과를 보였으며, 조기 발견을 통한 적극적인 수술적 치료가 상부 천추 신경근의 보존으로 기능 소실의 최소화와 함께 생존을 연장할 수 있는 방법으로 생각된다.

일부 남성근로자의 3년간 고혈압 발생률과 위험요인 (The Incidence and Risk Factors of Hypertension that Developed in a Male-workers' Cohort for 3 Years)

  • Objectives: Cardiovascular disease is one of the main causes of death and morbidity in Korea. In this study, the prevalence and incidence of developing hypertension in a male-workers' cohort were investigated during 3-years follow-up with a view to find the risk factors that affected the development of hypertension. Methods: Among the 5,374 people who participated in a routine health check up, 3,852 people with normal blood pressure and who had no history of hypertension were prospectively followed up for 3 years. The classification of hypertension was based on the JNC7 report (the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure). Life style factors and underlying diseases that were related to the risk factors of hypertension were collected by using a self-report questionnaires via the internet. Results: The prevalence of hypertension was 28.3% (1,520/5,374) at the first screening (2001). It was found that the incidence in 2004 of hypertension for the follow-up subjects (3,711) who had normal blood pressure in 2001 was 7.6 per 100 person-year. Multiple logistic regression analysis of the variables related to the risk factors of hypertension was carried out. The relative risks were 1.037 (95% CI=1.022-1.053) as the age increased 1 year and 1.039 (95% CI=1.023-1.055) as the body mass index increased $1kg/m^2$. The relative risk for the prehypertensive group was 2.501 (95% CI=1.986-3.149) compared to the normotensive group. These results showed that age, body mass index and the baseline blood pressure were significantly related to the incidence of hypertension. Conclusions: The incidence of hypertension was 7.6 per 100 person-year during follow-up. It was concluded that the risk factors for developing hypertension in the short-term were age, BMI, and prehypertension; Especially, this showed that it is necessary for prehypertensives to manage their body weight and blood pressure to prevent hypertension in middle-age by modifying their life style.
