Using ICF model Rehabilitation Management Case Report on Patients with Cerebellum Disorder (ICF 모델을 적용한 소뇌손상환자의 재활관리 사례보고)

  • Purpose : Currently, ICF to describe the functions and disability in the world has been used as a universal language. ICF tools based on ICF, the rehabilitation management of clients have been developed to be efficient. This study was designed to describe clinical decision for functional goal of clients to used ICF tools. Methods : In the following the utilization of all developed ICF tools will be described within a case example of a 53-year-old women, suffering from cerebellum disorder. As problems in the subject's functional activities was difficulties in changes sitting postures, standing postures and maintaining standing postures. Activity limitation was determined change sitting, standing posture as a goal through discussion with the patient. Results : After setting the identified problems as the purpose of intervention through the assessment, we find out the outcomes using the ICF evaluation display. Consequently, with functional activities limitation that discovered from assessment(categorical profile, assessment sheet), sitting postures to standing postures and maintaining standing postures were improved. Conclusion : This study was showed ICF tools based on Rehab-cycle for the patient's functional goals clinical practice. The future study, the ICF in clinical practical tools for effective use will require more attempt.

First Record of Ptereleotris evides (Gobiiformes: Microdesmidae) from the Southern Jejudo Island, Korea (제주도 남부 해역에서 채집된 Ptereleotris evides (망둑어목: 청황문절과) 한국 첫기록)

  • A single specimen (30.35 mm standard length) of Ptereleotris evides was collected from the southern coast of Jejudo Island, Korea, in September 2017 during a skin diving outing. Ptereleotris evides is characterized by having a slightly elongated body, six spines on the 1st dorsal fin, one spine and 25 soft rays on the 2nd dorsal fin, 22 pectoral fin rays, one spine and four soft rays on the pelvic fin, one spine and 25 soft rays on the anal fin, and no disc in pelvic fin. This species differs from the congeneric species, P. hanae and P. heteroptera in having no elongated rays on the caudal fin and a black spot at the ventral margin of the caudal peduncle, respectively. Molecular analysis results based on mitochondrial DNA cytochrome c oxidase subunit I (COI) sequences showed that our specimen almost corresponded to P. evides (99.82%) sequences from registered at the NCBI. As this is the first record of P. evides in Korean waters, we propose a new Korean name for it, "Ga-mak-cheong-hwang-mun-jeol".

Fabrication of Optically Active Nanostructures for Nanoimprinting

  • Optically active nanostructures such as subwavelength moth-eye antireflective structures or surface enhanced Raman spectroscopy (SERS) active structures have been demonstrated to provide the effective suppression of unwanted reflections as in subwavelength structure (SWS) or effective enhancement of selective signals as in SERS. While various nanopatterning techniques such as photolithography, electron-beam lithography, wafer level nanoimprinting lithography, and interference lithography can be employed to fabricate these nanostructures, roll-to-roll (R2R) nanoimprinting is gaining interests due to its low cost, continuous, and scalable process. R2R nanoimprinting requires a master to produce a stamp that can be wrapped around a quartz roller for repeated nanoimprinting process. Among many possibilities, two different types of mask can be employed to fabricate optically active nanostructures. One is self-assembled Au nanoparticles on Si substrate by depositing Au film with sputtering followed by annealing process. The other is monolayer silica particles dissolved in ethanol spread on the wafer by spin-coating method. The process is optimized by considering the density of Au and silica nano particles, depth and shape of the patterns. The depth of the pattern can be controlled with dry etch process using reactive ion etching (RIE) with the mixture of SF6 and CHF3. The resultant nanostructures are characterized for their reflectance using UV-Vis-NIR spectrophotometer (Agilent technology, Cary 5000) and for surface morphology using scanning electron microscope (SEM, JEOL JSM-7100F). Once optimized, these optically active nanostructures can be used to replicate with roll-to-roll process or soft lithography for various applications including displays, solar cells, and biosensors.

Ultrastructural Changes in the Neuropil of the Anterior Thalamic Nucleus following the Lesion in the Mamillary Body (유두체 손상이 시상전핵 조직상의 미세구조에 미치는 영향)

  • Degeneration of the axon terminals of mamillo-thalamic tract following the electrical coagulation of mamillary body is well known. In this study, the author investigated the ultrastructural alterations of neuropil components, initiated by terminal degenerations. Rats weighing approximately 250 gm were fixed on the stereotaxic instrument(David Kopf Inc., Heavy duty model), and NE 300 active electrode(Rhodes Med. Instr. Inc.) was introduced to the mamillary position of anterior 3.8 mm, lateral 0.5 mm, height 3.8 mm and lateral angle of $23^{\circ}$ according to De Groot's Atlas. Electric current of 20 mA was applied during 1 minute between active and inactive electrodes with Radio Frequency Lesion Generator(RFG 4, Radionics Inc.). Two hours, 2 days, 1 week and 2 weeks following the electrical coagulation of mamillary body, ipsilateral anterior thalamic nucleus was fixed in 1% glutaraldehyde-l% paraformaldehyde and 2% osmium tetroxide, embedded in Araldite mixture, cutted with LKB ultra tome V, stained with uranyl acetate-lead citrate and observed with JEOL 100 CX electron microscope. Observed results were as follows; 1. Degenerated mamillo-thalamic synapses were observed to form asymmetric axospinous or axo-dendritic types. 2. Terminal degeneration was not easily discernible at 2 hours interval after mamillary lesion, but following 2 days the terminal degeneration was apparent. 3. Postsynaptic spines, dendrites and even their cell bodies show edematic changes caused by the degeneration of postsynaptic counterpart. 4. Astrocytic territories, including perivascular processes forming glial limitans of blood-brain barrier, exhibit remarkable expansion. 5. Oligoglia and astroglia are actively engaged in the removal of degenerated elements. 6. Active forms of microglia were increased. 7. The observed results may represent typical ultrastructural alteration pattern within neuropil following the degeneration of certain input axon terminals.

A Study on Treatment of Sleep Disturbance in the Books on Cold Damage (상한문헌에 나타난 수면장애 치료법에 대한 연구)

  • Purpose : There are two types of sleep disturbance: inability to sleep (不眠) and somnolence (多眠). This study is to examine treatments of the two types of sleep disturbance in the Books of Cold Damage and how those theories were established and formed. Methods : 1. Verses including the words related to inability to sleep and somnolence were extracted from "Treatise on Cold Damage Diseases" (傷寒論). 2. Among the Books on Cold Damages that are classified according to the symptoms, 17 books with contents related to inability to sleep and somnolence were selected to collect and classify data in three perspectives: mechanism of disease (病機), method of treatment (治法) and disease pattern (病證). 3. Data collected through the above methods were compared and diagramed. Results & Conclusions : 1. On Cold Damage, inability to sleep is mostly rooted when human body lacks yin energy while having excessive yang energy (陽盛陰虛) due to fire and heat (火熱). The reason could be misuse of perspiration inducing method (汗法) or purgation (下法) on the doctors' part. 2. On Cold Damage, somnolence is rooted when pathogen (邪氣) is spread to yin meridians (陰經) and the human body lacks yang while having excessive yin energy (陰盛陽虛) or when heat (熱邪) is invaded into interior parts (裏部). 3. Many scholars of Cold Damage in later periods had copied the "Treatise on Cold Damage Diseases" for treatments of sleep disturbance but many others have applied the verses from "Treatise on Cold Damage Diseases" or added new treatments. Do jeol-am (陶節庵) and Wang Geung-dang (王肯堂) particularly had deep understanding on "Treatise on Cold Damage Diseases" and utilized the content freely or suggested new remedies because they had thorough knowledge on relating formula as well.

Consideration of the Comparing the Original Texts with Quotations in Edema Chapter of Dong-Yi-Bo-Gam (동의보감 부종편의 원문과 인용문헌에 대한 비교고찰)

  • We studied on the purpose of comparing the original texts with quotations in edema chapter of Dong- Yi-Bo-Gam. The research mainly depended on compare and consider these original text and quote. In Pyong Yeol Byong chapter of SoMoon, Su Yeol Hyul chapter of SoMoon, Pyong Yin Ki Sang chapter of SoMoon, Ja Jeol Jin Sae chapter of YoungChu, Emperor's Classic of Internal Medicine(黃帝內經;ECIM), We were able to find that some sentences about edema were quoted. Drug arrangement order that construct herb medicine is disposed depends on drug's weight, and all unit was concorded by Jeon and Pun. Quoted words in Emperor's Classic of Internal Medicine(黃帝內經;ECIM) does not perfectly match with original text. However, edema chapter of Dong- Yi-Bo-Gam is identical with original text, because only few words were deleted or inserted.

Transmission electron microscopic ultrastructures of the male germinal cells of Fibricola seoulensis (Fibricola seoulensis 웅성생식세포의 투과전자현미경적 미세구조)

  • A transmission electron microscopic study was performed to observe the ultrastructures of the male germinal cells and spermatozoa of Fibricola seoulensis. Spermatogonia were found in the periphery of the testis and characterized by large nuclei and comparatively little cytoplasms. Spermatocytes contained an oval to spherical nucleus. Their nuclear volume was little larger in comparative to that of cytoplasm, and the chromatin was comparatively little. The early spermatids were characterized by a great amount of cytoplasm, and numerous mitochondria encircled the nucleus. In a more advanced spermatids the electron-dense strands of chromatin appeared in the nucleus, and a pair of rootlet of the axoneme and a microtubule-organizing center (MTOC) were observed near the nucleus. The sectioned spermatozoa were found in the testis and the seminal vesicle. Their cross sectional views were divided into 6 types when they were distinguished on the basis of the morphology and components. The spermatozoa of F. seoulensis showed two flagella of 9+1 type axoneme.

AN ELECTROCHEMICAL STUDY ON SURFACE FINISH OF DENTAL AMALGAM (아말감의 표면연마에 관한 전기화학적 연구)

  • The purpose of this study was to observe characteristic properties of amalgam through the polarization curves and SEM images from 4 type amalgams (Amalcap, Shofu spherical. Dispersalloy and Tytin) with 3 different surface finish procedures (polishing, burnishing and carving) by using the potentiostats (EG & GPARC) and SEM (Jeol JSM-35). After each amalgam alloy and Hg was triturated as the direction of the manufacturer by means of mechanical amalgamator (Samki), the triturated mass was inserted into the cylndrical metal mold which was 12 mm in diameter and 10 mm in height and was pressed with $100kg/cm^2$. 4 specimens of each type amalgam were burnished with egg burnisher and another 4 specimens of each type amalgam were carved with Hollenback carver. Above 8 specimens and remaining untreated 4 specimens were stored at room temperature for about 7 days. Untreated 4 specimens of each type amalgam were polished with abrasive papers (Deer) from #400 to #1200 and finally on the polishing cloth with $0.5{\mu}m$ and $0.06{\mu}m$ $Al_2O_3 $ powder suspended water. Anodic polarization measurements was employed to compare the corrosion behaviours of the amalgams in 0.9% saline solution at $37^{\circ}C$. The open circuit potential was determined after 30 minutes immersion of specimen in electrolyte. The scan rate was 1 mV/sec and the surface area of amalgam exposed to the solution was $0.64cm^2$ for each specimen. All the potentials reported are with respect to a saturated calomel electrode (SCE). SEM images of each specimen were taken after + 800 mV (SCE) polarization. The results were as follows: 1. The corrosion potential of high copper amalgam was more anodic than that of low copper amalgam. 2. The polished amalgam were more resistant to corrosion than any other burnished and carved amalgam. 3. In the case of polishing, current density of high copper amalgam was lower than that of low copper amalgam.

SURFACE ANALYSIS OF CERCE RESTORATIONS POLISHED BY DIFFERENT TECHNIQUE (연마방법에 따른 Cerec block의 표면 거칠기 비교)

  • This study was performed to investigate the surface roughness of the Cerec Vita Mark II polished by various polishing techniques, compare with that of the Vintage enamel porcelain glazed by high temperature glazing technique. All of the Cerec specimen were finished with sequential use of high speed diamond burs(grit 45, 30 and $15{\mu}m$). The groups were divided into 5 groups : Group I : Cerec Vita Mark II block specimens polished with Sof-lex discs. Group II : Cerce Vita Mark II block specimens polished with Two Striper MPS. Group III : Cerce Vita Mark II block specimens polished with Enhance. Group IV : Cerce Vita Mark II block specimens polished with Porcelain laminate polishing FG kit. Group V : Vintage enamel porcelain glazed by high temperature glazing Technique. Each group was consisted of 10 specimens. The surfaces produced were examined quantitatively using a laser specular reflectance machine(Perthen RM600-s, Feinpruf Perthen GmbH., Germany) and qualitatively under SEM(JSM-5400, JEOL, Japan). The Results were as follows : 1. The arithmetic mean roughness value(Ra) in groups 1, 2, 3 and 4 was higher than that of group5. There was statistically significant difference(P<0.05). 2. The arithmetic mean roughness value(Ra) decreased in the following orders : group 1, group 2, group 4, group 3 and there was no statistically significant difference between group 1 and group 2, group 3, and group 4. There was statistically significant difference among group 1, 2, and group 3, 4 and group 5(P<0.05). 3. The maximum individual peak-to-valley-height(Rmax) decreased in the following orders : group 2, group 1, group 4, group 3, group 5 and there was no statistically significant difference between group 1 and 2, group 1 and group 4, group 3 and group 5. There was statistically significant difference among group 1, 2, and group 1, 4 and group 3, 5(P<0.05). 4. The treated surfaces of group 5 had smoother surface than that of groups 1, 2, 3, 4 with SEM.

  • Background and purpose: Gin Ga Chu Yo(該家樞要) was written in the year 1359 by Hwal Su(滑壽) who was a famous oriental doctor between Won(元) and Myeong(明) dynasty. As specialized in sphygmology(脈學), this book was brought together most things about sphygmology(脈學) before Won(元) dynasty by the author. Moreover he added the self-realized things to this book. Methods: In this book, there are all 20 chapters; Chu Yo Hyeon Eon(樞要玄言), Jwa U Su Bae Jang Bu Bu Wi (左右手配臟腑部立), O Jang(五臟), Sa Si peong Maek(四時平脈), Nae Gyeong Sam Bu Maek Beop(內經三部脈法), Ho Heup Chim Bu Jeong O Jang Beop(呼吸沈浮定五臟法), In Ji Ha Gyeong Jung I Jeong O Jang Beop(因指下輕重以定五臟法), Sam Bu So Ju(三部所主), Ji Maek Su Beop(持脈手法), Maek Gwi Yu Sin(脈實有神), Maek Eum Yang Yu Seong(脈陰陽類成 ), Gyeom Hyeon Maek Ryu(兼見脈類), Je Maek Ui Gi Ryu(諸脈宜忌類), Heom Je Sa Jeung Ryu(驗諸死症類) , Sa Jeol Maek Ryu(死絶脈類), O Jang Dong Ji Maek(五藏動止脈), Bu In Maek Beop(婦人脈法), So A Maek Beop(小兒脈法), Maek Sang Tong Hoe(脈象統會), Maek Sang Ga(脈象歌) and an epilogue in the end. This treatise is made up of principal, notes, study and conclusion, we tried to make a translation faithful to the original. Results and Conclusion: Looking into this book, we can know that it is very detail in analyzing item by item, bright and simple in explaining and it is very useful in studying pulse evaluation(脈診). In his preface, Hwal Su(滑壽) expresses his opinion with firm confidence that 'Do(道) of the World scattered in several scholarships and books of method and technique(方術). Nothing of method and technique(方術 ) is more important than medicine. Nothing of medicine goes before pulse evaluation(脈診).' With such good reasons, as a student of sphygmology(脈學), I think this book is very valuable in studying pulse evaluation. So I add Hangul suffixes(吐) to a part of this book and translate it into Korean.
