When the complete denture is made, the record base for jaw relation is divided into temporary record base and permanent record base. However, The temporary record base include some disadvantages such as, the lackness of intimate contact between model and base, the lackness of retention during the jaw relation registration, When we obtained jaw relation the permanent record bases made from heat curing resin were utilized. We could get several advantages as follows : 1. The permanent record base provided intimate contact between the model and record base. 2. In fabricating occlusal rim on record base, the dimensional change of record base was little because the permanent record base was lesser influenced to thermal change of occlusal rim than the temporary record base. 3. At the stage of jaw relation, the retention of final denture could be early checked. 4. It could be able to get more accurate registration of jaw relation because all procedure were done on the same base during the jaw relation, artificial teeth arrangement, try-in, and final denture construction. 5. Although there was an inconvenience due to double curing procedure, the shrinkage rate in resin polimerization was relatively reduced so that more dimensional stability could be taken.

The purpose of this study was to show occlusion on the simple hinge articulator optionally mounted. Modelling of upper-lower jaw and simple hinge articulator were developed. This modelling of upper-lower jaw inserting wax bite was mounted imaginary on the modelling of simple hinge articulator by use of the computer simulation. From changes of THA(transverse horizontal axis)-incisor distance, Balkwill angle and THA deviation, eight types were mounted respectively. After removal of wax bite, upper-lower jaw position changing were compared with centric jaw relation. The results were as follows: 1. The change of THA-incisor distance had influence on mostly a vertical shift of upper jaw. 2. The change of Balkwill angle had influence on mostly a horizontal shift of upper jaw. 3. Inferior type in the THA deviations was the least shift of upper jaw. The above results suggest that the simple hinge articulator optionally mounted effect a shift of upper jaw.

The growth changes in position of upper and lower jaws, incisal inclination in relation to inferior cranial base have been described. Twenty five males was studied quantitavely by means of serial cephalometric reontgenogram from seven to thirteen years of age. The findings seem to warrant the following conclusions: 1. Growth change in anteroposterior relationship of upper and lower jaws to the anterior cranial base showed very little change before eleven years of age but axial inclination of incisal teeth tended to become labiaization in relation to the anterior cranial base. 2. kiter eleven years of age, there wasn't nearly labialization of incisal teeth but jaw prognathism occurred a little in relation to the anterior cranial base.

After making the surgical treatment plan, the surgical movements are duplicated in the model surgery. During this procedure, reference points and lines are drawn on the base of the models over the dental arch, and sawcuts are made according to these marked osteotomy lines. This method requires more accuracy for better postsurgical results in that the surgical splint which enables the surgeon to position the jaws intraoperatively is made from the casts as repositioned by the model surgery, and finally it will define the postsurgical results. This technique, however, has been found to be inexact, especially when the jaws are moved in several dimensions simultaneously. To overcome this, different methods have been developed for an accurate repositioning of the jaws as planned. A new appliance, Surgical Jaw Relator, was devised by the author for the simple 3-dimensional relocation of the upper and lower models, resulting in the easy construction of the splints such as centric relation splint, intermediate and final splint. This article describes an introduction and a clinical application of this appliance.

최근 다양한 증례 보고에서 소개되고 있는 jaw motion tracking은 환자의 안궁 이전 및 개별화된 하악 운동 경로를 기록한 후, 이를 computer-aided-design/computer-aided-manufacturing (CAD-CAM) 소프트웨어의 가상공간상에 재현하는 방법이다. 본 증례의 환자는 오랜 기간 구치부의 상실로 인해 교합평면의 붕괴가 관찰되었기에, 수직 교합 고경의 증가를 동반한 완전 구강 회복술을 계획하였다. 우선 jaw motion tracking을 진행하여 새로운 중심위 상에서 환자의 하악 운동을 기록한 후, 이 정보를 환자의 초진 구내 데이터 및 3차원 안면 스캔 데이터와 조합하여 가상 환자를 생성하였다. 가상 환자 상에서 진행한 디지털 왁스업과 임플란트 식립 계획을 바탕으로 임시 보철물을 제작하였다. 새롭게 설정된 수직 교합 고경 상에서 적절한 견치 유도를 보이는 임시 보철물의 검증을 통해, 최종 보철물로 이행하였다. 이처럼 디지털 치의학 기술의 장점을 활용하여 환자는 저작능과 심미성의 개선에 만족하였기에 본 증례를 보고하고자 한다.

악골부조화가 심할수록 기능향상을 위해 악골 및 치아 치조부에서의 보상반응이 현저해진다. 대표적인 현상으로 하악과두의 변위와 상악교합평면의 변화 등을 들 수 있다. 이러한 상하악골에서의 적응성 변화는 안모의 심미성과 교합의 기능 향상을 위해, 술후 안정성을 높이기 위해서 악교정수술을 통해 반드시 수정해야 할 요소이다 적절한 상악골 수술을 위해서 통상적으로 model surgery를 시행하여 왔으나 이의 단점을 보완하기 위해 Surgical Jaw Relator를 고안하였으며 이는 교합기의 mounting plate 대신에 부착함으로써 상하악 모형의 삼차원적 이동과 회전이 가능하도록 설계되어 있으며 기존의 모든 종류(Panadent. Sam, Denar, Hanau 등)의 교합기에 손쉽게 부착하여 수술용 스프린트의 제작 시간을 대폭 줄일 수 있는 장점이 있다 상악골 상방 이동이 필요한 하악전돌 증례와 상악골 하방 이동이 요구되는 하악후퇴 증례 등에 본 기구를 적용한 결과 양호한 수술 결과를 얻었으며, 이로써 정교한 상악 수술을 요하거나 명확한 수술 계획이 설정된 증례 등에서 본 기구를 적절하게 사용할 경우 수술용 스프린트를 효율적으로 제작하는데 다소 도움이 될 것으로 생각된다.

구치부 교합지지 상실과 비정상적 악간 관계는 교합 부조화로 이어지며 다양한 병적 징후를 동반한다. 저작, 심미 및 발음 기능을 회복하기 위해 교합고경과 교합평면 재설정에 따른 완전구강회복이 필요하게 된다. 본 증례는 75세의 여성으로 교합지지 상실과 불균일한 교합평면 및 II 급 부정교합을 가지고 있었다. 안모를 관찰하고 악간관계를 평가한 결과 상하악의 최대교두간접촉위가 상실되면서 수직교합고경이 감소된 것으로 진단되었다. 상악 국소의치와 하악 고정성보철물로 상실된 교합수직고경을 회복하고 조화로운 교합평면을 형성하였다. 수직고경 거상량은 7.5 mm로 하였고, 임시보철물을 이용하여 8 주간 경과 관찰 후 최종 보철물을 제작하였으며 저작효율과 심미성은 개선되었다.

무치악 환자에서 기능적으로 우수하고 심미적으로 만족스러운 의치 제작을 위해서는 상, 하악 교합관계, 즉 교합수직 고경과 중심위를 정확하게 결정해야 한다. 수직관계를 결정할 때에는 근육, 상, 하악 치열궁간 거리와 상, 하악 치조제의 평행도 등 여러 요소들이 고려되어야 한다. 또한, 중심위는 모든 하악 운동이 시작되는 재현 가능한 위치로서 기록하는 과정 역시 의치 제작에 매우 중요하다. 만일 이러한 악간관계 채득시 오차가 생기면 결과적으로 제작한 의치가 불편하게 된다. Gothic arch tracer와 같은 트레이싱을 이용하는 방법은 술자가 쉽게 교합수직고경과 중심위를 찾아내고 기록할 수 있는 좋은 방법으로 안정적인 교합수직고경에서 하악의 운동위를 얻기에 유리하다.
