만성폐쇄성폐질환의 보완대체의학 임상연구 동향 (Research Trends for Chronic Obstructive Pulmonary Disease in Complementary and Alternative Medicine)

  • Objectives : This study analyzed the contents of research papers of complementary and alternative medicine (CAM) concerning chronic obstructive pulmonary disease (COPD) published in PubMed during the last 5 years. This study was conducted to help clinical studies for treating COPD with Oriental medicine. Materials and Methods : We inspected 31 theses and scrutinized their objectives, periods, participants, materials and methods, methods of assessment, results and Jadad score. Results : The treatments in the studies were supplements, physical training, acupuncture, acu-TENS, Bojungikgi-tang (Buzhongyiqi-tang), counseling, breathing training, osteopathic manipulative treatment, reflexology, and distractive auditory stimuli. The aims of treatment were improvement of exercise capacity, lung function, quality of life, oxidative status, nutrient status, systemic inflammation, and cessation of smoking. The median for treatment period of study was 8 weeks, the median number of participants was 35 and the differences between mean $FEV_1$ from groups were less than 10% in 22 studies. The methods of assessment were lung function test, exercise capacity test, muscle strength test, questionnaire of QoL, laboratory studies, and measurement of nutrient state. The mean of Jadad score was $2.4\;{\pm}\;1.03$, and 24 treatment were assessed as effective. Conclusion : Recent CAM studies of COPD have focused on various topics in alternative and complementary medicine, and it is necessary to provide objective studies for treatment of this disease with Oriental medicines.

무작위대조군연구(RCT)를 중심으로 한 근.골격계 질환의 약침 치료에 관한 고찰 (Review of Randomized Controlled Trials on Pharmacopuneture Treatment for Musculoskeletal Diseases)

  • Objectives : The purpose of this study is to review RCTs on pharmacopuncture treatment for musculoskeletal diseases and to establish standards of pharmacopuncture treatment model. Methods : We searched articles up to date of March 2009 via computerized databases of Pubmed, The Journal of Korean Acupuncture & Moxibustion, Journal of Korean institute of Herbal Acupuncture, Journal of Oriental Rehabilitation and Journal of Korean Oriental Medicine. Only Randomized Controlled Trials (RCT) concerning the effects of pharmacopuncture on musculoskeletal diseases. The pharmacopuncture treatment methods were assessed based on STRICTA and items considering the feature of pharmacopuncture. And the methodological quality of the trials was assessed by FEAS and modified Jadad score. Results : Eighteen trials of pharmacopuncture on musculoskeletal diseases were analyzed. Except for 4 trials comparing the effect of SBV and BV, positive outcome was reported in ten trials. Among eighteen trials; most of the trials were about Bee Venom acupuncture, and most of the trials used about five acupuncture points, mainly local acupuncture points. But, the amount of injection to each point and total injection were various. And most of trials were lack in the information about method of stimulation. The adjusted FEAS score ranged from 0 to 12, and modified Jadad scoreranged from 1 to 5. Conclusions: To standardize pharmacopuncture treatment, we need more well-designed, high quality clinical trials. And methodological assessment tools designed for pharmacopuncture treatment are also needed.

경피적외선요법의 경혈(經穴) 조사(照射) 치료법에 대한 고찰 (An Effect of Infrared Therapy on Acupoints ; Randomized Controlled Trials Review)

  • Objectives : The aim of this study is to establish evidence-based guideline on infrared therapy on acupoints. Methods : We investigated all the articles in websites including Pubmed, China national knowledge Infrastructure(CNKI) and medical journals authorized by national research foundation of Korea. The key words are acupoints and infrared. For the first time, we withdrew some papers no relationship to key words by reading titles and abstracts. In the second, we only selected randomized clinical trials(RCT) published from 2000 to 2010. Results : Out of 62 papers, only 2 RCT were analyzed. One (RCT 1) was about depression patients with insomnia(jadad score: 1). The other(RCT 2) was about urimic pruritus patients(jadad score: 5). In RCT 1, experimental group was significantly better than control group. In RCT 2, there was not statistical signification between two groups. Conclusions : In reality, there were not many papers on infrared therapy applications. Further study just like RCTs may be needed to establish evidenced-based guideline on infrared.

감기 임상연구의 최신 동향 및 평가도구에 관한 연구 (Study of Instruments for Assessment and Clinical Research Trends in Common Cold)

  • Objectives: This study was conducted to help clinical studies for treating the common cold with Oriental medicine. We inspected tendencies of clinical studies of the common cold in western medicine and analyzed the assessing instruments for it by scrutinizing PubMed publications for the last five years. Materials and Methods: We inspected 32 theses and scrutinized their objectives, periods, participants, materials and methods, the instrument of assessment for the studies, and validity, reliability, criteria and symptom score for questionnaires, results and JADAD score. Results: 1. The median for total period of study was 7 months, the median for days of treatment was 7.5, and most studies were carried out during the winter/spring period, because of the enhanced risk of cold infections. 2. The studies focused on unorthodox medicines or new drug products, such as antipyretic analgesics (e.g. paracetamol), decongestant and bronchodilator (e.g. ephedrine), echinacea, antioxidants (e.g. zinc), probiotic bacteria and so on. 3. All theses except one had subjectscomposed of cold patients who were infected naturally. The median number of subjects for final analysis was 187. 4. 27 studies used survey as the instrument of assessment. Among these, only one thesis was verified for validity. 5. The mean of JADAD score was 4.41, suggesting most of theses of worth. 7 studies were assessed as ineffective. Conclusion: Recent studies of the common cold are focused on unorthodox medicines or new drug products, and it is necessary to provide an objective instrument for assessing common cold treatments.

국외 가족간호중재 연구의 현황과 질 분석 (Research Trends and Quality Evaluation of Family Nursing Interventions Outside Korea)

  • Purpose: This study investigated trends in family nursing intervention studies and evaluated the quality of studies using the RCT design. Methods: This study included a total of 898 abstracts published from 1977 to 2007 in order to describe trends in family nursing intervention studies. Out of 898 studies, 153 RCT studies were selected for quality evaluation. The criteria of Jadad et al. (1996) were employed for the quality evaluation. Results: The number of RCT studies on family nursing interventions increased since 2001. Only 6.8% of the studies were published in the area of nursing. Most of the family interventions (27.1%) focused on individual family members and only 8.2% of the studies provided interventions to family as a whole unit. Nine different modes of family nursing intervention could be categorized, but none of the studies used the double blind design. Few studies utilized protocols for interventions. Only 17.5% of the studies reported the rationale for sample size. The mean score was 1.6 out of 5 according to the criteria of Jadad et al. (1996). Conclusion: Refined definitions and attributes of family nursing intervention modes are needed. Most of the studies did not meet the expectations of RCT. Thus, it is needed to improve the quality of design. More RCT studies should be conducted to provide evidence-based practice of family nursing interventions.

다낭성 난소증후군의 난임치료에 대한 한약임상연구 경향 분석 -중국의 임상연구를 중심으로- (A Review on Clinical Studies of Herbal Treatment for Infertility Caused by Polycystic Ovary Syndrome -Focusing on Chinese Clinical Trials-)

  • Objectives: The purpose of this study is to review the trend of chinese clinical studies of herbal treatment for infertility caused by Polycystic Ovary Syndrome (PCOS). Methods: We searched the clinical studies published from 1979 to 2013 through CAJ (Chinese Academic Journal) with keywords of "PCOS and 不孕" or "多囊卵 巢綜合證 and 不孕". We analyzed the intervention type of clinical studies. In addition, We researched randomized controlled trials (RCTs) published from 2009 to 2013, assessed the quality of RCTs using the Jadad Scale and reanalyzed the intervention type of RCTs. Results: One hundred and seventeen clinical studies were finally selected. Clinical studies of herbal treatment for infertility caused by PCOS increased remarkably after the 2000s in China. Combined herbal medicine and western medicine was used the most frequently. RCTs published from 2009 to 2013 were fifty. Average of Jadad score of RCTs was 1.2. Combined herbal medicine and western medicine was also the most frequently used as intervention in RCTs. Conclusions: Clinical studies on effect of combined herbal medicine and western medicine for infertility caused by PCOS were mostly published in China. But Quality of studies was generally low. Further research on effect of herbal treatment for infertility caused by PCOS is needed.

RCT논문을 중심으로 한 퇴행성 슬관절염 침구 치료 방법에 대한 고찰 (Review of Randomized Controlled Trials on Ideal Acupuncture Treatment for Degenerative Knee Osteoarthtritis)

  • Objectives : To analyze the inclusion criteria of participants, control group interventions, and the results of prior studies of acupuncture for OA. Also to identify aspects of the procedure that are associated with positive outcomes in order to establish ideal acupuncture treatment model. And to assess the methodological quality of the trials with modified Jadad score and FEAS in order to evaluate the quality of prior studies and find out whether or not acupuncture has a positive effect in treating OA. Methods : Articles up to the date of November 2008 were searched via computerized databases of PubMed, Journal of Korean Oriental Medicine, The Journal of Korean Acupuncture & Moxibustion Society and Journal of Oriental Rehabilitation Medicine. Bibliographies of reviewed papers were also searched and reviewed. Only randomized controlled trials (RCTs) and systematic reviews concerning the effects of acupuncture or electroacupuncture (EA) on symptoms of osteoarthritis of the human knee, published in English and Korean were included. The acupuncture treatment methods of the reviewed trials were assessed based on STRICTA. And the methodological quality of the trials was assessed by modified Jadad score and FEAS. Results : Twenty one trials of acupuncture for OA were analyzed. Based on the results of this review the following factors might contribute to optimal results from acupuncture treatment. 1) Usage of $ST_{35}$, $GB_{34$, $EX_{32}$, $ST_{36}$ and $SP_9$ acupuncture points. 2) More than four acupuncture points should be used. 3) More than 15 minutes of needle retention time. 4) Needle length-40mm and diameter-0.30mm 5) Usage of EA 6) more than 10 times treatment 7) Treatment frequency of more than once a week, 8) Treatment duration longer than 6 to 8 weeks. Conclusions : High quality clinical trials of Acupuncture for OA is still in lack. Future investigators must concentrate their attentions on the quality of acupuncture treatment itself used in the trials as well as the methodological quality of trials.

중풍(中風) 재활(再活)의 침치료(鍼治療) 효과(效果)에 대한 고찰(考察) -최근 RCT(Randomized controlled trial) 논문을 중심으로- (The Study on the Effectiveness of Acupuncture in Stroke Rehabilitation)

  • 본 논문은 지금까지의 침 치료와 중풍재활에 관해 쓰인 논문들을 살펴보고 그들의 연구방법, 실험과정, 결과들을 분석 평가하여 침 치료가 중풍 재활 치료에 유의함을 가지는지 알아보고자 하였다. 이를 위해 pubmed와 science direct, EBSCO를 검색하고 경희의료원 의학도서관에 소장된 비전산화 저널 자료에 대한 수기 조사를 병행하여 논문 원문 16편을 얻을 수 있었으며 다음과 같은 결론을 얻었다. 1. 검색을 통해 원문을 얻은 논문은 모두 16편이었으며, 그중 9편의 논문이 RCT논문이었다. 2. 중풍 재활치료로써 침 치료가 환자의 전반적 운동기능향상 및 일상생활과 생활의 질의 향상에 유의한 효과를 보인다고 보고한 논문이 3편이었으며, 다른 세 논문은 침 치료가 유의한 효과를 보이지 않는다고 보고하였다. 3. 중풍 후 증상인 spasticity에 대한 두 논문 중 상지의 spasticity에 관련한 논문은 침 치료가 유의하다고 보고하였으며, 하지의 spasticity에 관련한 논문은 이와 반대로 유의하지 않다고 보고하였다. 4. 침 치료와 자세조절에 관한 논문 1편은 침 치료가 중풍 후 자세 조절의 정상화를 촉진시키는데 유의하다고 보고하였다. 5. 9편의 논문 중 침 치료가 유의한 효과를 보고한 논문의 Jadad score는 2점, 2점, 1점, 1점, 1점으로 낮게 나타났으며 유의한 효과를 보이지 않는다고 보고한 논문의 Jadad score는 5 점, 5점, 3점, 1점으로 높게 나타났다.

동종 조혈모세포이식을 받은 환자에서 운동중재의 효과 (Effectiveness of Physical Exercise in Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation)

  • Purpose: To summarize and review the methodological quality of the evidence from trials examining the effectiveness of physical exercise in patients undergoing allogeneic hematopoietic stem cell transplantation (Allo-HSCT). Methods: Six randomized clinical trials (RCTs) were identified, reviewed for substantive results, and assessed for methodological quality. Results: Six trials met all methodological criteria on the modified Jadad score above 3 out of 5 points. Failure to blind the outcome assessor, and failure to describe the method of blinding of outcome assessor appropriately were the most prevalent methodological shortcomings. Various exercise modalities have been applied, differing in content, frequency, intensity, and duration. Positive results have been observed in part for a diverse set of outcomes, including physical and psychological performance. Conclusion: The trials reviewed in this study were of moderate methodological quality. They suggest that exercise in patients undergoing Allo-HSCT may be safe and feasible, and in part patients benefit from increased physical performance both during and after transplantation. Future RCTs should use larger samples, appropriate comparison groups, and a standard of outcome measures, and examine what kind of exercise intervention (aerobic vs. resistance vs. combined) is the most effective for Allo-HSCT patients. It would be necessary to define contraindication for exercise to guarantee its safety.

뇌졸중 후 배뇨장애 침치료의 임상연구에 대한 고찰 (Review of Clinical Research on Acupuncture Treatment of Voiding Difficulty in Stroke Patients)

  • Objectives: The aim of this study was to collect and analyze studies on acupuncture therapies for treatment of voiding difficulty in stroke patients and to suggest methods of study for acupuncture therapies. Methods: Electronic searches were performed on search sites including NDSL, 국회도서관, RISS4U, DBPIA, KISS, KMBASE, KoreaMed, 한국전통지식포탈, OASIS, Pubmed, CNKI, using the search words ‘중풍’, ‘뇌졸중’, ‘stroke’, ‘배뇨장애’, ‘voiding’, ‘urinary’, ‘incontinence’, ‘retention’, ‘ ’, ‘침치료’, and ‘acupuncture’ as single or combined keywords, from January 1990 to August 2013 with language limitation, Korean, English, Chinese. Also, quality of the studies were evaluated using Jadad score. Results: The searches identified 16 studies for selection and analysis. In the present study, voiding difficulty includes urinary incontinence and urinary retention. The acupuncture points CV4, SP6, and CV3 were the most frequently used. Treatments were most frequently applied daily. The times and terms of acupuncture treatment, were most frequently 30 minutes per treatment for 4 weeks Conclusions: The acupuncture therapies were effective in the treatment of voiding difficulty in stroke patients, but the study of acupuncture therapy as a treatment for this issue in stroke patients needs to be standardized and high-quality study models should be introduced.
