A Case of Ileocolic Type Intussusception Associated with Hematemesis (토혈을 동반한 회맹장형 장중첩증 1례)

  • Intussusception is a common cause of lower intestinal bleeding from five to 11 months of age. Typical symptoms are paroxysmal colicky pain, irritability, vomiting, abdominal mass and currant jelly stool, but hematemesis is rare. Though the most common type of intussusception is ileocecal type, it is rarely accompanied with hematemesis. Up until the present, hematemesis has been observed rarely but only in ileoileal type intussusuption for children and gastrojejunal or jejunojejunal type intussuception for adults who went through gastrectomy. We report a case of the ileocolic type intussusception associated with hematemesis in a 18 months old child.

A Cabe of Congenital Malaria (선천성 말라리아 1례)

  • A case of congenital malaria infection has been studied in a 46-day old female Korean infant. Her mother suffered from malaria infection during pregnancy in Uppervolta, Africa, and returned to Korea at the 9th month of gestation for delivery. At 39 days of age, the clinical features characterized by fever, irritability, pallor, jaundice and hepatosplenomegaly were developed. The laboratory data revealed a hemolytic anemia with thronbocytopenia, hyperbilirubinemia and increased hepatic enzyme values. A peripheral blood smear demonstrated intraerythrocytic malarial parasites and gametocytes of Plasmodium falciparum. She was successfully treated with quinine solfate (25mg/kg/day in three doses for 5 days) and trimethoprimejsulfamethoxazole (8mg/kg/day in two doses for 5 days) orally, and repeated blood smear had been negative for malaria. This report also signifies the first description of congenital malaria in Korea imported from Uppervolta in Africa. A brief review of related literature was made.

Two Cases of Alocasia Intoxication (알로카시아 중독 2례)

  • Alocasia was originally distributed throughout subtropical and tropical areas. Recently, in Korea, it has been used in air cleaners and in control of humidity. Despite easy access in Korea, there are few reports on Alocasia toxicity. We report on two cases of Alocasia intoxication. One patient was a 16-month-old male, who was admitted with a complaint of irritability after biting leaves of Alocasia. Four hours later, he was discharged without any symptoms. Another patient, a 52-year-old female, complained of oral pain, numbness on the perioral area, dysphonia, swallowing difficulty, and chest and abdominal pain after eating root stuck of alocasia odora. She underwent gastrointestinal fibroscopy (GIF) due to lasting chest and abdominal pain. Finding on GIF showed erythema and swelling in the aryepiglottic fold and larynx. Her symptoms lasted 13 days; she was then discharged without any complications or sequelae.

The Correlation between Problematic Behaviors and Activities of Daily Living of Elderly People with Dementia in Patients in a Geriatric Hospital

  • The purpose of this study is to determine the correlations between problematic behaviors and activities of daily living(ADL) targeting 106 demented elderly people hospitalized in a geriatric hospital. To examine the cognitive function of the subjects, the study used Korean Mini Mental State Examination(K-MMSE). For problematic behaviors and ADL, the study collected data based on Patient Assessment Forms in the geriatric hospital. Among problematic behaviors, apathy/indifference had the highest correlation with the items of ADL. Irritability/lability, agitation/aggression, depression/ dysphoria, night-time behavior and wandering also showed to be correlated to items of ADL(p<.05). This study demonstrated that cognitive function, problematic behaviors and ADL of the demented elderly hospitalized in the geriatric hospital are correlated to each other.

Clinical Study of Midgut Volvulus (중장염전에 관한 임상적 고찰)

  • Intestinal malrotation is usually asymptomatic and most often is found during abdominal exploration for other surgical diseases. However, a serious complication of intestinal malrotation, midgut volvulus is a true surgical emergency of childhood. The clinical findings, diagnostic procedures, treatment, and prognosis of midgut volvulus were reviewed by a retrospective study. Between 1980 and 2005, 29 patients with midgut volvulus ranging in age from 1 day and 15 years were treated at HanYang University Hospital. Seventy-nine percent of the patients presented before 1 month of age. Midgut volvulus occurred 2 times more frequently in male. The clinical findings were bilious vomiting (96.6 %), irritability (34.5 %), abdominal distention (13.8 %), abdominal pain (10.3 %), and palpable abdominal mass (6.9 %). The diagnosis was made by abdominal simple x-ray (17.2 %), upper gastro-intestinal contrast study (37.9 %), abdominal sonogram (20.7 %), abdominal CT (3.4 %), and abdominal exploration (20.7 %). Among the 29 patients, 5 patients developed gangrene of small intestine due to strangulation and underwent resection of bowel. Two patients died due to sepsis.

  • The neuropsychiatric sequelae of traumatic brain injury are effects on complex aspect of cognition, emotion and behavior. They include problems with attention and arousal, concentration, executive function, intellectual changes, memory inpairments, personality changes, affective disorders, anxiety disorders, psychosis, apathy, aggression, and irritability. There are many useful therapeutic approaches available for people who have been brain injuries. Although a multifactioral, multidisciplinary, collaborative approach to treatment is proposed, for purposes of exposition the author have divided treatment into psychopharmacological, cognitive, behavioral, psychological, and social interventions.

  • Extracts of Black cohosh (Cimicifugae rhizoma) have been used for the treatment of climacteric complaints for decades. A significant number of woman entering menopause exhibit the following symptoms: getting hot flushes, night sweats, irritability, depression, and anxiety, A reduction of the frequency of hot flushes equivalents and hints on the antidepressant activity of Cimcifuga extracts. In the present work, we have screened several 80% ethanol extracts from medicinal plants and found that the extracts from Cimicifugae Rhizoma(Black cohosh:승마) have inhibitory effect on catecholamine secretion in bovine chromaffin cell. Since this extract inhibited 1,1-dimethyl-4-phenylpiperazinium iodide (DMPP)-induced catecholamine secretion, but did not inhibit KCl, bradykinin, and veratridine-evoked case, this inhibitory effect is mediated by nicotinic acetylcholine receptors with noncompetitive manner.

  • Attention-deficit hyperactivity disorder (ADHD) is characterized by inattention, hyperactivity, impulsiveness and problems in other higher cognitive processes such as executive function deficits. Currently, there are many treatment modalities, of which pharmacotherapy is the most strongly supported by scientific and clinical evidence. Stimulants, which are first choice in the pharmacological treatment of ADHD, block dopamine reuptake by binding the dopamine transporter and so increasing the concentration of dopamine in synaptic clefts. Stimulants are effective in improving core ADHD symptoms, as well as the nonspecific symptoms, such as aggressiveness and oppositional behavior. Frequently reported short-term adverse effects are decreased appetite, sleep disturbance, headache, dizziness and irritability. Although questions have been raised about the long-term side effects of stimulants, including growth suppression, cardiovascular events, and abuse potential, there is no clear evidence to support these concerns.

A Case of 24-Year-Old Woman with Recurrent Hypersomnic Complaint (수면과다증을 반복적으로 보이는 24세 여자환자)

  • A 24-year-old woman complained of recurrent episodes of hypersomnia lasting on the average about 15 days with mild mood alternation such as depression and irritability. During interepisode interval, she was free of any symptoms. Depending on the absence of excessive eating and hypersexuality, she was clinically diagnosed as recurrent monosymptomatic hypersomnia or the incomplete form of Kleine-Levin syndrome. When nocturnal polysomnography and multiple sleep latency test were performed 10 days after her recovery from a hypersomnic episode, reduced slow wave sleep % and pathologic daytime sleepiness were still noted. The authors suggest that the clinical recovery in recurrent monosymptomatic hypersomnia precede electrophysiological normalization by several days.

Case Reports on Two Patients with Dysfunctional Uterine Bleeding (기능성 자궁출혈 치험 2례 증례보고)

  • The purpose of this study is to present the two treatment case of patient with dysfunctional uterine bleeding. Each patient has received the Korean Medicinal treatment including acupuncture, moxibustion, and herbal medication twice a week for a month or six months. This efficacy of treatment was evaluated with the degree of bleeding by the number of pad used per day. After finishing treatment, two patients had stopped bleeding, and their menstrual cycle returned to the normal range. Moreover, additional clinical manifestations including leukorrhea, hot flush, bloating, feeling tired and irritability which refers to the premenstrual syndrome were also alleviated. This case report showed that the acupuncture, moxibustion, and the herbal medicinal treatment can be effective for the treatment of the dysfunctional uterine bleeding.
