Preliminary Validation Study of the Korean Version of the DSM-5 Level 2 Cross-Cutting Symptom Measure: Depression and Irritability for Parents of Children Aged 6-17 Years

  • Objectives: This study investigated the reliability and validity of the Korean version of the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) Level 2 Cross-Cutting Symptom Measure-Patient-Reported Outcomes Measurement Information System (PROMIS)-Depression and the Irritability for parents of children aged 6-17 years. Methods: Participants were 190 children diagnosed with depressive disorder (n=14), anxiety disorder (n=21), attention-deficit/hyperactivity disorder (ADHD; n=111), ADHD with anxious depression (n=13), and tic disorder with somatic symptoms (n=31). Patients were 8-15 years of age. The participants' mothers completed the Korean versions of the DSM-5 Level 2 Cross-Cutting Symptom Measure-PROMIS Depression and Irritability (Affective Reactivity Index, ARI), and the Korean Child Behavior Checklist (K-CBCL). Using these data, we calculated the reliability coefficient and examined the concurrent and discriminant validity of the PROMIS Depression and the Irritability (ARI) scales for assessing depression and irritability in children. Results: The reliability coefficient of the PROMIS Depression scale (Cronbach's α) was 0.93. The correlation coefficient with the K-CBCL DSM emotional problem score was 0.71. The PROMIS Depression scale significantly discriminated children with depressive disorders from those with other conditions. The reliability coefficient of the Irritability (ARI) scale was 0.91, suggesting its high reliability. Conclusion: Our results suggest that the Korean version of the DSM-5 Level 2 Cross-Cutting Symptom Measure for Depression and Irritability Scales for parents of children aged 6-17 years is reliable and valid and may be an efficient alternative to the K-CBCL.

A Study on the Property and the Skin Irritability of the Reclaimed Soap (폐유을 이용한 재생비누의 특성 및 피부자극도에 대한 연구)

  • The purpose of this study is to find troubles in producing and using the reclaimed soaps to get the solutions of their troubles. Five kinds of reclaimed soaps were taken from religious or civic organizations and nine kinds of soaps were made by adding sodium hydroxide of different concentrations to the used vegetable oil in our laboratory. Washing efficiency, pH test, BOD, and skin irritability of the reclaimed of manufactured soaps were examined. The washing efficiency of the manufactured soaps were ±80%. The percent of free-alkali tended to increase as the amount of NaOH increased. The skinirritability was proportional to the amount of NaOH added. To make the reclaimed soap, the best percent of NaHO to used oil is 17% or 18% and this ratio(17%∼18%) should be kept. Manufactured soaps by this ratio have good apperance condition and washing efficiency. And also they have low skin irritability and free-alkali. This study has the limitations that the skin irritability test can have individual and the collected soaps couldn't represent all kinds of recaimed soap.

A Study on the Subjective Symptoms of the Printers in Chunbuk Area (전북지역 인쇄공의 심신 자각증상 조사연구)

  • The purpose of this study was to investigate the psychosomatic health status of printers. The 77 printers and 24 control group were analysed about salary, drinking, smoking, education, sex, marriage, age and working age by the THI (Todai Health Index) questionaire. THI was modified from CMI(Cornell Medical Index) and developed by Tokyo University Research Team in Japan. The resuts obtained were summarized as follows. 1. The printers, who get more salary showed high score about mental conplaints, especially, mental irritability(j), nervusness (E), lie Scale(L), aggressiveness(F) and irregualr life(G) and lower salary showed generally high score about physical complaints, especially, mouth and anus (D), digestive symptom(C) multiple subjective symptom(I). 2. According to the printers drinking amount shows the difference, eg nondrinker scored higher on mouth and anus(D), 90mg/week drinker scored higher on multiple subjective symptom(I), digestive symptom(C), depression(K), nervousness(E), and irregular life(G), 91~179mg/week drinker scored higher on impulsiveness(H), mental irritability(J), 270~359mg/week drinker scored higher on respiratory(A), lie scale (L) and aggressiveness (F). 3. The nonsmoker scored high level on mouth and anus(D), mental irritability(J). The previous smoker scored on multiple symptom(I), eyes and skin(B), digestive(C), lie scale(L), and depression(K). The present smoker scored on respiratory(A), impulsivehess(H), aggressiveness(F), nervousness(E), and irregular life(G). 4. According to the printers working age showed almost high score about subjective symptoms on 1~3 year. 5. Men printers high scored on respiratory(A). lie scale(L), aggressiveness(F), women printers scored about mental complaints, especially, impulsiveness(H), mental irritability(J), depression (K), nervousness (E). 6. According to the printers age showed high scored about, below 20 years were lie scale(L). aggressiveness(F), irregular life(G) 21~30years were multiple subjective symptom(I) respiratory (A), eyes and skin(B), mouth and anus(D), impulsiveness(H), mental irritability(J), depression (K), nervousness(E), and over 41 years were digestive(C). 7. Married printers scored high level on eyes and skin(B), digestive(C) and impulsivehess(H), and single printers on respiratory(A), mouth and anus(D), lie scale(L), mental irritability(J). 8. According to education shows the difference, eg high school scored higher on eyes and skin (B), mental irritability(J), depression(K), nervousness(E), collage and over scored higher on multiple subjective symptom(I ), respiratory (A), mouth and anus (D), lie scale (L), aggressiveness (F), irregular life (G), and middle school scored high on digestive (C), impulsiveness (H).

The Effects of Electrical Stimulation Therapy with Microchip for the Treatment of Bladder Irritability Symptoms in Stress Urinary Incontinent Women (방광 자극증상을 호소하는 복압성 요실금 환자에서 마이크로칩을 이용한 전기자극치료의 효과)

  • Purpose: This study was carried out to evaluate the efficacy of intravaginal pelvic floor electrical stimulation (FES) therapy on bladder irritability symptoms in stress urinary incontinent women. Materials and Methods: The evaluation before and after treatment included the Blaivas's female Bladder Questionnaire, the quality of life questionnaires and the overall satisfaction with present voiding state using visual analogue test (VAT). All patient were treated for 20 minutes, twice a week for 6 weeks, using a new intravaginal electrical stimulator with microchip (PIC16C74). Results: After the FES therapy, the overall success rate of the SUI was 50.4.% at 9 months. The bladder irritability symptoms such as frequency, nocturia, urgency, residual urine sensation and lower abdominal discomfort were improved. In particular, the symptoms of frequency, urge incontinence, dysuria were significantly improved after the FES therapy at 9 months. The VAT score of the overall satisfaction with the present voiding state was significantly lower after the FES therapy. 73.7% of patients were satisfied with the FES therapy and complications such as hemorrhage, vaginitis, urinary tract infection and pain were not encountered. Conclusion: These results suggest that FES therapy with microchip improves some bladder irritability in SUI women. Therefore, electrical stimulation therapy should be considered as a first line therapy in SUI women with bladder irritability symptoms.

The impact of Digital Video Effects and subtitles on evaluation and agenda recognition in TV News (TV뉴스의 어깨걸이와 자막이 뉴스에 대한 평가와 의제 인식에 미치는 영향)

  • An experiment was conducted to investigate the relationship between the DVEs and subtitles provided with anchor speech in TV news and the news evaluation, trust and agenda recognition. 120 university students were asked to watch four types of news that differed in the contents of their DVEs and subtitles, and then they evaluated the fairness, sensibility, and irritability of the news. The content of DVEs and subtitles were related to the irritability evaluation and trust of the news, and it was not related with the fairness and sensibility evaluation. When the DVEs and subtitles emphasizing the stimulating aspects of the issue were given, the irritability was highly evaluated and the trust was low. The evaluation on news fairness affected the news trust. The more they rated news as fair, the greater the trust in news. Unlike the assumption in this study, DVEs and subtitles were not the main factors influencing the news agenda perception, and the viewers tended to perceive the agenda based on the news content itself.

Study of on Academic Stress Responses According to Sasang Constitutions of Oriental Medicine College Students (한의과대학생의 사상체질별 학업스트레스 연구)

  • Objectives : This study was founded to identify the differences of stress responses, according to Sasang Constitutions (the Taeyangin, Taeumin, Soyangin and Soumin) of highly stressed oriental medicine college students. Methods : The subjects were 76 students who majored in oriental medicine. We processed 'University students Stress Scale', and 'Medical Stress Scale'. We selected 30 students who stressed higher than average. We then processed 'QSCC II (Questionnare for the Sasang Constitution Classification II)' and 'Scale for the Stress Response'. Collected data was analyzed with the SPSS 19.0 for windows statistical program. Results : The university students stress score of oriental medicine students was 1.97 that was higher than the other college students. The medical stress score was 2.77, which was similar with other medical college students. The symptoms of stress score was 2.53, which was higher than the others. Particularly, the depressive syndrome and rage syndrome were highly ranked. The frequently appearing symptoms of each type of Sasang Constitutions (the Taeyangin, Taeumin, Soyangin and Soumin) showed as follows. The Soyangin's symptoms of stress showed in order of Anxiety, Depression, and Emotional irritability. The Soumin's symptoms of stress showed in order of Depression, Emotional irritability, and cognitive disorganization. The Taeumin's symptoms of stress showed in order of Anxiety, Depression, and Emotional irritability. However, there was were no significant differences among the results of each constitution. Conclusions : The results show that oriental medicine students have as much stress as other medical college students and there was tendency of showing different patterns of stress response, according to Sasang Constitution, but it's not significantly different.

The Clinical Usefulness of Halo Sign on CT Image of Trauma Patients (2세 미만 소아의 경도 두부 외상 후 두개골 골절 및 두개내 병변의 위험 인자)

  • Purpose: This research was performed to determine which clinical signs and symptoms of brain injury are sensitive indicators of skull fracture (SF) and intracranial injury (ICI) in head injured children. Methods: We conducted a prospective study of minor head trauma in children younger than 2 years of age for a 1-year period. Skull radiographs, brain computed tomography (CT), and data forms, including mechanism of injury, symptoms, physical findings, and hospital course, were completed for each child. Results: Of 137 study subjects, 17 (12.4%) had SF/ICI. Falls were the most common mechanism of injury, and heights of fall above 1 meter were associated with incidence of SF/ICI (p<0.05). Scalp abnormalities were not associated with incidence of SF/ICI. As for clinical symptoms, lethargy and a grouping of features (irritability & vomiting) were associated with incidence of SF/ICI (p<0.05). The incidence of seizure, loss of consciousness, vomiting, irritability, and scalp abnormality did not differ significantly between those with normal radiologic findings and those with SF/ICI. Among asymptomatic patients, 11 (14.5%) patients had SF/ICI, and among patients with normal scalp findings, 9 (12.7%) patients had SF/ICI. Conclusion: Clinical signs and symptoms, except for lethargy and a grouping of features (irritability & vomiting), were not sensitive predictors of SF/ICI. Nevertheless, SF/ICI occurred among normal children. In such a case, a liberal policy of CT scanning is warranted.

A Study on the Differences of Stress Responses According to Sasang Constitutions (사상체질에 따른 스트레스 반응의 차이에 관한 연구)

  • This study was founded to identify the differences of the stress responses according to Sasang Constitutions(the Taeyangin, Taeumin, Soyangin and Soumin) of female college students. The subject were 226 the regular female students who majored in nursing in Kyungbuk, Kimcheon. Tools used in this study were the Questionnare for the Sasang Constitution Discrimination and the Scale for the Stress Response which were revised in some parts. The values of Cronbach's alpha for the Questionnare for the Sasang Constitution Discrimination and the Scale for the Stress Response were $0.45{\sim}0.73$ and $0.66{\sim}0.85$ respectively. The collected data was anaylyzed with the SPSS 10.0 windows statistical program. The results of this study were as follows; 1. The constitutions were shown in the order of Taeumin(45%), Soyangin(28%), Soumin(18%) and Taeyangin(9%). 2. There were significant difference in the stress response for Depression(p<.05), Anxiety(p<.05), Emotional irritability(p<.01) and the Total stress response(p<.05) according to the Sasang Constitutions (the Taeyangin, Taeumin, Soyangin and Soumin). In conclusion, it is needed to find out an effective ways of treating stress individually to apply Sasang Constitutions to a new esthetic treatment.

Comparison between adverse effects of low and usual doses of intravenous aminophylline (정주 아미노필린의 투여 농도에 따른 부작용에 관한 연구)

  • Purpose : Some patients develop side effects from theophylline even at low serum concentrations. We designed a prospective study to evaluate the side effects of theophylline. Methods : A Prospective, controlled trial study was conducted. The low-dose group received an intravenous continuous aminophylline dose of 5 mg/kg/day on the first day and subsequently 10 mg/kg/day on the following two days. The usual-dose group received 10 mg/kg/day for three days and the control group received normal saline for three days. Heart rate, respiratory rate, serum concentration of theophylline, and four adverse events (irritability, sleep disturbance, jitter, and vomiting) were checked at the time of admission and at 2, 12, 24, 48, and 72 h after the start of aminophylline infusions. Results : Nine patients out of 37 in the low-dose group and six of 21 in the usual-dose group dropped out because of uncontrolled irritability. The serum concentrations of theophylline in dropouts ($3.68{\pm}1.93$ ig/mL) and participants ($4.47{\pm}2.45$ ig/mL) were not significantly different. Irritability was a more frequent side effect in the usual-dose group at 12 h, but there was no difference between the low-dose and usual-dose groups in terms of vomiting, sleep disturbance, and jitter. Most of the severe adverse effects were observed in children below two years of age. Conclusion : Some patients dropped out regardless of the initiating aminophylline dose, especially patients under the age of two years.
