Fine Needle Aspiration Cytology of Hepatocellular Carcinoma - A Study on 247 Cases - (간세포암종의 세침흡인 세포학적 소견 - 247예에 대한 연구 -)

  • Hepatocellular carcinoma (HCC) is malignant tumor frequently occurring in Koreans. There have been few reports regarding the cytologic findings of fine needle aspiration (FNA) of HCC. Most have suggested a diagnostic problem in the cytology distinguishing HCC from some benign hepatic lesions-for example, a regeneration nodule in cirrhosis and liver cell adenoma. In spite of its high frequency in Korea, no cytologic study has been reported, concerning the FNA of HCC. In an attempt to achieve cytologic criteria for the diagnosis of HCC, the authors studied retrospectively cytopathologic findings of 247 cases of HCC. These cases were confirmed either by histoiogic examination including lobectomy, biopsy, or ceil block material, or, when tissue diagnosis was unavailable, by a high serum alpha-fetoprotein level (over 400 I. U.). All aspiration smears were stained by the Papanicolaou method. In each case, the smears were analyzed for cell patterns and various cytomorphology of the tumor cells. The smear background was assessed for the presence of tumor cell necrosis and inflammatory components and compared to that of metastatic carcinomas. The cell patterns were classified as trabecular, acinar, dispersed, and irregular. The cytologic parameters analyzed included the degree of nuclear atypia and the presence of mitoses, intranuclear cytoplasmic inclusions, nucleolar prominency, endothelial lining, multinucleated giant cells, eosinophiic globules, bile, and Mallory body. Most of the FNA of HCC showed markedly cellular smears. The tumor cells were most frequently arranged in a trabecular pattern (80.3%). The irregular (12.6%), the acinar (5.5%), and the dispersed patterns (1.7%) followed in decreasing frequency. Individual hepatoma cells were larger than normal liver cells. However, they had morphologic features characteristic of the hepatic cells the cells were round or polygonal, their cytoplasm was abundant and granular with eosinophilic or amphophilic stainability, and their nuclei were round to oval, located centrally, and tended to have prominent nucleoli. Anaplasia and pleomorphism of tumor cells were generally mild to moderate. These findings existed even in very well differentiated cases. Mitotic figures were present in about 85% of the cases. Prominent nucleoli were observed only in about half the cases. The frequency of other cytologic features was as follows intranuclear cytoplasmic inclusion in 86.8% : endothelial lining in 56.1% : bile in 19.8% : and giant cells in 60.1%. Clear cells were often present in 11.7%, Most aspiration smears of HCC displayed clean background without necrosis or inflammatory material in contrast to the dirty, necrotic background of metastatic cancers and cholangiocarcinomas. Based on the above mentioned features, it is suqqested that the cytologic critieria most important for the diagnosis of HCC include a markedly cellular smear, trabecular pattern, hepatocytoid appearance of tumor cells, endothelial lining, the presence of bile, giant cells, intranuclear cytoplasmic inclusions, and prominent nucleoli, Among these, trabecular pattern, endothelial lining, giant cells and clean smear background are points to be considered in differentiating HCC from metastatic and cholangiocellular carcinoma.

A Study on the Eating Habits of Obese Elementary School Students in a Rural Community (농촌 지역 고학년 비만 초등학생의 식습관에 관한 연구)

  • This study was conducted to determine the eating habits of obese elementary school students residing in Iksan city, in June 2006 using 1167 participants(obese: 233, normal weight: 934). The results are summarized as follows. The average age was 12 years-old in both the obese and normal weight groups. The average BMI, ideal body weight and obesity index in the obese and normal weight groups were $24.8kg/m^2$, 39.8 kg, and 32.5% and $18.2kg/m^2$, 37.7 kg and -1.4%, respectively. Regarding the number of subject who acknowledged their physical build, the obese group was highest(88.9%) in stating obese or extremely obese, while the normal group was highest(71.5%) in stating they were normal weight. For the frequency of eating breakfast, the obese group the results were: "eating it everyday"(69.1%) and $"3{\sim}4\;times/week"$ (13.7%), while in the normal weight group, the highest response was "eating it everyday"(75%) and the lowest response was "never eat it"(4.1%). In terms of having breakfast with someone, there were slight differences for eating alone between the obese group(17.4%) and normal group(12.4%) and also between the obese group(19.6%) and normal group (39.6%) for eating with parents. There were considerable gaps in the association of eating habits with weight such as having irregular meals in the obese group(52.4%) and having regular meals in the normal group(61.1%). The frequency of having snacks was different by the rate of obesity, where 65.9% of the obese group, and 57.8% of the normal group had snacks more than once. There were significant differences in the practice of controlling intake with the obese group at 77.3% and the normal group at 36.3%. There were high numbers regarding eating more than average in stressful times for both the obese group(10.3%) and normal group(7.7%). The results of this study show the prevalence of obesity in Iksan, and proper nutrition education is currently needed. Moreover, the greater the level of obesity, the higher the frequency of skipping breakfast, eating alone, and eating irregular meals, and the more experience in attempting to control weight. To resolve the problems triggered by inappropriate food habits, proper food and eating methods to control weight should be included in nutrition education programs for elementary students.

Differentiation of Adductor-Type Spasmodic Dysphonia from Muscle Tension Dysphonia Using Spectrogram (스펙트로그램을 이용한 내전형 연축성 발성 장애와 근긴장성 발성 장애의 감별)

  • Background and Objectives : Adductor type spasmodic dysphonia (ADSD) is neurogenic disorder and focal laryngeal dystonia, while muscle tension dysphonia (MTD) is caused by functional voice disorder. Both ADSD and MTD may be associated with excessive supraglottic contraction and compensation, resulting in a strained voice quality with spastic voice breaks. The aim of this study was to determine the utility of spectrogram analysis in the differentiation of ADSD from MTD. Materials and Methods : From 2015 through 2017, 17 patients of ADSD and 20 of MTD, underwent acoustic recording and phonatory function studies, were enrolled. Jitter (frequency perturbation), Shimmer (amplitude perturbation) were obtained using MDVP (Multi-dimensional Voice Program) and GRBAS scale was used for perceptual evaluation. The two speech therapist evaluated a wide band (11,250 Hz) spectrogram by blind test using 4 scales (0-3 point) for four spectral findings, abrupt voice breaks, irregular wide spaced vertical striations, well defined formants and high frequency spectral noise. Results : Jitter, Shimmer and GRBAS were not found different between two groups with no significant correlation (p>0.05). Abrupt voice breaks and irregular wide spaced vertical striations of ADSD were significantly higher than those of MTD with strong correlation (p<0.01). High frequency spectral noise of MTD were higher than those of ADSD with strong correlation (p<0.01). Well defined formants were not found different between two groups. Conclusion : The wide band spectrograms provided visual perceptual information can differentiate ADSD from MTD. Spectrogram analysis is a useful diagnostic tool for differentiating ADSD from MTD where perceptual analysis and clinical evaluation alone are insufficient.

  • A precedure for recovering surface displacement from a time series of pressure measured by a pressure gage in a shallow water (that is, FFTM, LCM. IWM) is investigated with respect to a proper cut-off-frequency of a frequency response function for the accurate recovery of wave height and period. The authors examined the applicability of above mentioned three transformation procedures through field observations and laboratory experiments and the following results are obtained. i) The cut-off-frequency of the frequency response function used in FFTM is deeply depend on both the frequency response of the pressure sensor and the water depth at the sensor. In this study, a relatively accurate surface displacement can be recovered when the frequency response function is cut off at the frequency corresponding to kh=3.0 where k is a wave number at the depth of h. The frequency response function in the region higher than the cut-off-frequency is set constant to be the value at the cut-off-frequency. ii) The transformed surface displacements by LCM are affected by the small waves of short periods included in the measured pressure. It is found that pressure variation whose local frequency is higher than kh=1.5 has to be neglected to recover surface displacement sufficiently. iii) In IWM, the linear pressure response function is usually utilized by multiplying a coefficient N which is a function of the frequency (or kh) and takes a value around unity. However, in this study, a constant value of N(=1.0) gives a relatively accurate recovery of surface displacements.

  • The specially orthotropic plate theory is used to analyse three-span continuous composite slab bridges with elastic intermediate supports. A method of calculating the natural frequency corresponding to the first mode of vibration of beams and tower structures, with irregular cross sections and with arbitrary boundary conditions, was developed and the result of application of this method to the three-span continuous composite slab bridges with elastic intermediate supports is presented. This type of bridge represents either concrete or sandwich type three-span bridge on polymeric supports for passive control or on actuators for active control. Any method may be used to obtain the deflection influence surfaces needed for this vibration analysis. The finite difference method is used for this purpose in this paper. The influence of flexural stiffnesses and the modulus of the foundation are studied.

  • The frequency distribution and morphological characteristics of tissue mast cells in the various tissues of normal domestic animals (bovine, swine, dog and chicken) and systematically infected (Hog cholera, Canine distemper and Newcastle disease) animals were studied. The results were as follows: 1 In cattle, density of mast cells was higher in foetus (bovine) and young animals than in adults Differences in frequency distribution among individual animals were also observed. 2. In chicken, the highest number of mast cells was found in age group of 15 to 40 days, the moderate number in age group of one to 10 days, and the lowest number in age group of 40 days or older. 3. The morphologically, mast cells were usually round, ovoid, spindle, oval and irregular, and particularly in ovary of bovine, it was usually round and ovoid forms. 4. The largest numbers of mast cells were shown in ovary of bovine, intestine of swine and dog, and proventriculum of chicken. 5. In the systemic infections, the number of mart cells usually tends to increase.

  • A series of laboratory experiments has been performed in order to investigate the behavior of water-air two-phase flow in a horizontal pipe. A conductivity meter has been applied to detect the irregular alternation of air at the specific points in flows. The experimental condition has been established according to the water and air flowrates. Passing time, which is the time length for a measuring probe to pass through the entire length of a specific bubble, has been defined to evaluate the size of bubbles in the flow. Passing length, which can be considered as the equivalent value to bubble size and determined from the product of passing time and cross-sectional averaged velocity, and its corresponding occurrence frequency have been analyzed to classify the air flow patterns according to the condition of air and water fluxes. From the result, the dependancy of flow patterns on the variation of air-water flux ratio has been investigated and the existence of thresholds also checked for classifying the behavior of air in the flow.

A Novel AE Based Algorithm for PD Localization in Power Transformers

  • In this paper, a novel algorithm for PD localization in power transformers based on wavelet de-noising technique and energy criterion is proposed. Partial discharge is one of the main failures in power transformers. The localization of which could be very useful for maintenance systems. Acoustic signals due to a PD event are transient, irregular and non-repetitive. So wavelet transform is an efficient tool for this signal processing problem that gives a time-frequency demonstration. First, different wavelet based de-noising methods are analyzed. Then, a reasonable structure for threshold value determining and applying manner on signals is presented. Evaluated errors are good evidences for choices. Next, applying the elimination low energy frequency bands is discussed and developed as a de-noising method. Time differences between signals are used for PD localization. Different ways in time arrival detection are introduced and a novel approach in energy criterion method is presented. At the end, the quality of algorithm is verified through the different assays in lab.

A study on Hair Bundle Feature Estimation Based on Negative Stiffness Mechanism Using Integrated Vestibular Hair Cell Model (전정 유모세포 통합 모델을 이용한 반강성 기전 기반 섬모번들 특성 추정에 관한 연구)

  • In this paper hair bundle feature model and integration method for hair cell models were proposed. The proposed hair bundle feature model was based on spring-damper-mass model. Input of integrated vestibular hair cell model was frequency and output was interspike interval of hair cell that was reflected the feature of hair bundles. Irregular afferents that had a great gain variation showed reduction of negative stiffness section. Regular afferents that had a small gain variation, however, showed same feature with base negative stiffness feature. As a result, integrated vestibular hair cell model showed almost the same modeling data with experimental data in the modeled eleven frequency bands. It is verified that the proposed model is a good model for hair bundle feature modeling.

Effect of Up-and-Down Torch Oscillation for Providing Uniform Heat Input along the Sidewall of Gap on Ultra Narrow Gap Welding (울트라 내로우 갭 용접에서 갭 내 고른 아크입열 분포를 위한 상ㆍ하 토치요동 효과)

  • Narrow gap welding has many advantages over conventional V-grooved butt welding such as high productivity, small deformation and improved mechanical property of joints. With narrower groove gap, less arc heat input is expected will all the other advantages of narrow gap welding. The main defects of narrow gap welding include the lack of root fusion, convex bead surface and irregular surface, all of which have negative effects on the next welding pass. This paper suggests an up-and-down torch oscillation for ultra narrow gap welding with gap size of 5mm and investigates the proper welding conditions to fulfill the reliable and high welding quality. First, GMA welding model was suggested for ultra narrow gap welding system with Halmoy's model referenced for wire melting modeling. And the arc length in ultra narrow gap was defined. Secondly, based on the experimental results of up-and-down torch oscillation welding, phase shift of current and wire extension length were simulated for varying oscillation frequency to show that weld the bead shape in ultra narrow gap welding can be predicted. As the result, it was confirmed that reliable weld quality in ultra narrow gap welding can be achieved with up-and-down torch oscillation above 15Hz due to its ability to provide uniform heat input along the sidewall of gap.
