Peripheral Neuropathy in the Orofacial Region after Third Molar Extraction as an Initial Manifestation of Anemia: Two Case Reports

  • Third molar extraction, one of the important surgical treatments commonly practiced in dentistry, presents various symptoms after surgery ranging from temporary or mild symptoms to permanent or severe complications. However, oral burning pain, dysesthesia, parageusia, dry mouth, headache and pain in multiple teeth are not the common symptoms that patients often complain after dental extraction. Here, the authors report two cases who presented acute neuropathic symptoms mentioned above in the orofacial regions following third molar extraction. At the initial examination, the healing of the tooth sockets of two patients was normal. One patient was diagnosed as megaloblastic anemia associated with Vitamin $B_{12}$ deficiency and was referred to the Department of Hematology for assessing the underlying etiology of anemia. The laboratory test for the other patient revealed microcytic anemia related to iron deficiency. The patient with iron deficiency anemia was successfully treated with iron supplement. These two cases suggest that anemia, as an underlying systemic disease, may be a rare etiology explaining acute onset of peripheral neuropathy in the orofacial regions after third molar extraction and should be considered in the assessment of patients who report neuropathic symptoms after dental extraction.

철분과 운동이 흰쥐의 헴 생합성과 지질 대사에 미치는 영향 (Effect of Dietary Iron and Exercise on Heme Biosynthesis and Lipid Metabolism of Rats)

  • 본 연구는 식이 철과 운동이 흰쥐의 헴 생합성과 지질대사에 미치는 영향을 살펴보고자 수행되었다. 쥐는 식이 철의 수준에 따라 철결필군(Fe-, 5 ppm)과 철 충분군(Fe+, 50 ppm)으로 나누고 규칙적인 운동 여부에 따라 운동군과 대조군으로 나누었다. 실험 식이의 공급과 운동을 시킨 기간은 8주였으며, 운동군은 경사 6도에서 28 m/min의 속도로 1회 30분씩 일주일에 5회씩 트레이드밀 운동을 시켰다. 그 결과 횐쥐의 헴 생합성 정도는 운동을 한 군에서 유의적으로 증가하는 것으로 나타났다. 운동은 또한 체중 증가량을 낮추고, 부고환지방량을 감소시켰으며, 혈중 중성지방과 콜레스테롤 수치를 낮추고 HDL-콜레스테롤의 수치를 증가시켰다. 한편, 식이 철의 부족은 헤마토크리트와 헤모글로빈 농도를 감소시켰으며, 간 콜레스테롤의 축적을 증가시켰다. 앞으로 인체를 대상으로 철분 수준이 다른 상태에서의 운동이 헴의 생합성, 지질대사 및 젖산 농도 등에 어떠한 영향을 미치는지에 대한 연구가 더 많이 이루어져야 할 것으로 사료된다.

철보충이 납중독된 흰쥐의 ${\delta}-Aminolevulinic$ Acid Dehydratase 활성과 해독에 미치는 영향 (Effects of Dietary Irons Supplementation on ${\delta}-Aminolevulinic$ Acid Dehydratase Activity and Detoxification of the Pb-administered Rats)

  • 철보충이 납종독된 흰쥐의 체내 무기질 함량과 DALAD activity 및 해독에 미치는 영향을 관찰하기 위해, 흰쥐에게 납 2,000ppm을 함유한 식수를 제한없이 주면서, 철분 100ppm, 500ppm을 보충한 식이로 5주동안 사육하여 얻을 결과는 다음과 같다. 체중증가량, 식이섭취량 및 식이효율은 납급여군이 대조군보다 낮게 나타났으며, 철보충납급여군은 납단독급여군보다 높은 것으로 나타났다. 간과 심장의 무게는 차이를 보이지 않았으나, 신장은 납급여군이 대조군보다 증가를 보였다. 비장은 납단독급여군에서만 증가를 보였고, 철보충납급여군에서는 대조군과 차이를 보이지 않았다. 헤마토크릿치는 철보충납급여군에서는 대조군과 차이를 보이지 않았으나, 납단독급여군은 유의적인 감소를 보였다. DALAD activity는 혈액과 간 모두 납급여군에서 대조군보다 유의적인 감소를 보였으나, 납단독급여군보다 철보충납급여군이 높게 나타났고, 철보충수준이 높을수록 높게 나타났다. 혈청과 간의 amino-transferase activity는 철보충납급여군에서는 대조군과 비슷한 경향을 보였으나 납단독급여군에서는 유의적인 증가를 보였다. 혈청 납함량은 납단독급여군과 철보충납급여군 사이에 차이를 보이지 않았으나, 간의 납함량은 철보충납급여군이 납단독급여군보다 높게 나타났다. 혈청 철함량은 납급여군이 대조군보다 감소했으나, 간의 철함량은 납급여군에서 높게 나타났으며, 철보충납급여군이 납단독급여군보다 증가를 보였다. 혈청과 간의 구리함량은 납단독급여군은 대조군과의 차이를 보이지 않았으나, 철보충납급여군은 감소를 보였다.

골다공증 모델 흰쥐에서 칼슘과 철 보충제의 과다섭취가 골격손실과 신석회침착 및 신장기능에 미치는 영향 (Effect of Excess Calcium and Iron Supplement on Bone Loss, Nephrocalcinosis and Renal Function in Osteoporotic Model Rats)

  • This study examined the effects of excess intake of calcium (Ca) and iron (Fe) supplement on bone loss, nephrocalcinosis and renal function in osteoporotic model rats. Seven-week-old female rats were first fed a Ca-deficient diet for four weeks after ovariectomy operation, and then one of nine experimental diets for additional eight weeks, containing three levels of Ca, normal (0.5%) or high (1.5%) or excess (2.5%) and three levels of Fe, normal (35ppm) or high (210ppm) or excess (350ppm). The osteoporotic model rats showed a remarkable increase in body weight, serum alkaline phosphatase (ALP) and decrease in breaking force, Ca, P, Mg contents of femur. Serum Ca concentration was not significantly affected by dietary Ca and Fe levles. Liver Ca content increased in rats fed a high-and excess-Ca diet. Kidney Ca content and microscopic Ca deposition remarkably increased in osteoporotic model rats compared to control group, and showed a tendency to decrease in rats fed a excess-Ca diet. Breaking force of femur increased with increasing dietary Ca levels, but Ca, P contents of femur and serum ALP were not significantly affected by dietary Ca and Fe levels. Serum total protein decreased in rats fed a excess-Ca diet, BUN increased in rats fed a excess-Ca diet, while serum uric acid and creatinine were not significantly affected by dietary Ca levels. Urinary creatinine, GFR increased in rats fed a high-and excess-Ca, diet, and GFR was highest in rats fed a excess-Ca/excess-Fe diet. These results suggest that excess intake of Ca may increase breaking force of femur, but not increase mineral contents of femur, and decrease kidney function. Furthermore, excess intake of Fe and Ca concurrently may aggravate kidney function leading to potential health problems in ovariectomized osteoporotic model rats.

영양강조표시제품 중 비타민 C 함량 조사 (A Study on Vitamin C Content of Nutrition Emphasized Products)

  • Objectives: Vitamin C has various functions such as antioxidative effect and supporting absorption of iron (Fe). Aim of this present study was to provide vitamin C nutrition information and to briefly evaluate absorption interaction of vitamin C and Fe content of vitamin C emphasized products. Methods: Vitamin C emphasized foods including beverages, cereal, snacks, chocolate products, other cocoa products, and sugary products were examined by HPLC. Fe contents in samples after dry-ashing were examined by ICP. Results: Vitamin C content ranges in various products tested were the following: beverages (n=11) $20.15{\pm}0.08{\sim}845.41{\pm}6.07mg$, cereal (n=11) $52.50{\pm}0.23{\sim}262.50{\pm}0.07mg$, snacks (n=1) $50.00{\pm}0.25mg$, chocolate products (n=1) $311.73{\pm}2.44mg$, other cocoa products (n=1) $311.73{\pm}2.44mg$, other sugary products (n=2) $52.50{\pm}0.23{\sim}262.50{\pm}0.07mg$. Vitamin C (n=27) analysis values ranged from 82 to 450% of the labeled value. Vitamin C content in vitamin C emphasized food (n=6) was estimated 7.7 times~56.6 times more than Fe content. Conclusions: Analyzed samples ranged more than 80% of the labeled value in vitamin C emphasized products, which complied with food labeling regulation. But, beverages (n=3), cereal (n=4), chocolate products (n=1) were 2 times more than the labeled value. To provide accurate nutrition information, food manufactures should supervise nutrition labeling and understand the interactions between nutrients. Also, consumer should decide about the adequate amount of nutrient intake by thoroughly checking nutrition labeling.

Suppressive Effect of Zinc on the Formation of Colonic Preneoplastic Lesions in the Mouse Fed High Levels of Dietary Iron

  • We investigated the effect of zinc on the formation of colonic aberrant crypt foci induced by azoxymethane (AOM) followed by dextran sodium sulfate (DSS) in mice with high iron diet (HFe; 450 ppm iron). Sixweek old ICR mice were fed on high iron diets with combination of three different levels of zinc in diets, low-zinc (LZn; 0.01 ppm), medium-zinc (MZn; 0.1 ppm), and high-zinc (HZn; 1 ppm) for 12 weeks. Animals were received weekly intraperitoneal injections of AOM (10 mg/kg B.W. in saline) for 3 weeks followed by 2% DSS (molecular weight 36,000~50,000) in the drinking water for a week. To confirm the iron storage in the body, the hepatic iron concentration has been determine chemically and compared with histological assessment visualized by Prussian blue reaction. Aberrant crypt (AC) and aberrant crypt foci (ACF) were analyzed in the colonic mucosa of mouse fed high dietary iron. Superoxide dismutase (SOD) activity and thiobarbituric acid-reactive substances (TBARS) level were also investigated. Apoptosis in the preneoplastic lesion was determined by terminal deoxynucleotidyl transferase-mediated dUTP nickend labeling (TUNEL). In addition, immunohistochemistry of ${\beta}$-catenin was also performed on the mucous membrane of colon. The number of large ACF (${\geq}4$ AC/ACF), which possess greater tumorigenic potential, was significantly lower in MZn and HZn groups compared with LZn group. Cytosolic SOD activity in the liver was significantly higher in HZn group compared with LZn group. Hepatic MDA level was decreased significantly in HZn group compared with MZn and LZn groups. Apoptotic index was significantly higher in HZn group. Taken together, these findings indicate that dietary zinc might exert a protective effect against colonic preneoplastic lesion induced by AOM/DSS in ICR mice with high iron status, and suggest that dietary supplement of zinc might play a role in suppressing colon carcinogenesis in mice.

재즈댄스와 철분섭취가 성인여성의 신체 조성, 혈액 성분 및 골 대사에 미치는 영향 (Effect of Jazz Dance and Iron Intake on Body Composition, Blood Components and Bone Metabolism in Adult Women)

  • 본 연구는 성인여성을 대상으로, 재즈댄스+철분섭취군(7명), 재즈댄스군(7명), 대조군(7명) 총 21명으로 하였다. 재즈댄스와 철분섭취가 성인여성의 신체조성, 혈액성분 및 골 대사에 미치는 영향을 구명하기 위하여 운동 강도 RPE (9$\sim$15), 운동 빈도 주3회, 1회 60분의 지속시간으로 12주간 실시하였다. 철분은 1일 1회 식후 80 mg을 섭취시켜, 비교 분석한 결과 다음과 같은 결론을 얻었다.신체조성은 집단내 변화에서 재즈댄스+철분섭취군은 체중, 체지방량은 유의하게 감소하였고, 제지방은 유의하게 증가 하였으며, 재즈댄스군에서 체지방량이 유의하게 감소한 것으로 나타났다. 집단 간 변화에서 대조군보다 재즈댄스+철분섭취군과 재즈댄스군이 유의한 차이로 긍정적인 변화가 있는 것으로 나타났다.혈액성분은 집단 내 변화에서 재즈댄스+철분섭취군은 RBC, WBC, Hb는 유의하게 증가 하였고, MCV는 유의하게 감소하였으며, 재즈댄스군은 WBC, MCHC가 유의하게 증가하였다. 대조군은 MCHC 유의하게 감소를 하였다. 집단 간 변화에서 RBC는 재즈댄스+철분섭취군이 대조군보다 유의한 차이가 있었고, WBC, MCV, MCHC, Hb는 재즈댄스+철분섭취군, 재즈댄스군이 대조군보다 유의한 차이가 있었다. 골대사는 집단 내 변화에서 재즈댄스+철분섭취군, 재즈댄스군은 DPD, OC는 유의하게 증가하였고 대조군은 유의하게 감소하였다. 집단간 변화에서 대조군보다 재즈댄스+철분섭취군과 재즈댄스군이 유의하게 증가하였다. 이상의 연구 결과를 통해 재즈댄스가 성인여성들에게 여가시간과 건강에 도움이 되는 댄스프로그램으로서 바람직하고, 다양한 연령층에서 신체기능을 향상시키는 과학적인 연구가 필요하며, 보다 효율적이고 규칙적인 운동프로그램 개발과 영양섭취에 대한 지속적인 연구가 요구된다.

Prevalence and risk factors for vitamin D deficiency in children with iron deficiency anemia

  • Purpose: The increasing prevalence of breast feeding has led to concerns about vitamin D deficiency (VDD) and iron deficiency anemia (IDA) in children. We evaluated the prevalence of VDD in a population of Korean children with IDA and assessed the risk factors for VDD in these children. Methods: A total of 79 children who were diagnosed with IDA were prospectively surveyed from April 2010 to March 2011. Data were collected by questionnaire, medical assessment, and laboratory tests, including measurement of 25-hydroxyvitamin D (25OHD), hemoglobin, and wrist radiography. Results: The median age was 22 months and 30% of the subjects were female. Over a half of subjects (58%) had subnormal vitamin D level (25OHD<30 ng/mL), and VDD (25OHD<20 ng/mL) was present in 39% of children. There was no difference in serum hemoglobin level between IDA patients with VDD and those without VDD. Most subjects (89%) were currently or had recently been breastfed and almost all subjects (97%) who had VDD received breastfeeding. Children with VDD were more likely to be younger than 2 years, to have been breastfed, and to have been tested in winter or spring. Multivariable analysis indicated seasonal variation was a significant independent risk factor for VDD in our IDA patients. Conclusion: Our results demonstrated that VDD has a high prevalence in Korean children with IDA. Primary care physicians should be aware of the possibility of VDD in children with IDA and should supplement the vitamin D as well as iron.

  • In this study, we selected supplementary vegetable resources to develpe nutritious supplementary model of korean formula for infants and children, purchased and analyzed it to use with a model. We also referred to an old book about Korean vegetable resources and searched a form of vegetable resources. There are a lot of vegetable resources of various kinds which lied scattered in mountains and fields of our country. As for these, a beyond compare menstruation activity component is becoming clear with a lot of things. It is thought that depth search and study of these vegetable resources and practical application to a infant and child supplemen is desirable. Therefore, we choosed vegetable resources ; a Capsella brusapastoris, a Codonopsis lanceolata, Dandelion, a wild plant, a Tractylis ovata(dried), a Tractylis ovata(native), a Pine plant, a burdock, a JanDae, a Plantain, a HollpDae, which gathered in our country, and we analyzied priximate aomposition of sample (moisture, ash, total carbohydrate, crude fat, crude protein, dietary fiber) and micronutrient contents of sample (calcium, Iron, flavonoids, vitamin A, vitamin C, vitamin E).

광주지역 일부 저소득층 여자노인에서 지역사회 영양중재 프로그램 적용에 따른 식행동 변화 및 영양상태 개선 효과 (The Effect of a Community-Based Nutrition Intervention Program on Dietary Behavior and Nutritional Status of Low-Income Elderly Women in Gwangju City)

  • This study was conducted to measure the effect of a community-based nutritional intervention program on dietary behaviors and nutrient intakes of low-income elderly women in Korea. The subject of the study was a group of 88 elderly women using the free meal service in Dong-gu, Gwangju city in Korea. The study was carried out from April to July 2009, and the pretest, application of nutrition education, and post-test were applied in each stage. This community-based nutrition intervention program consisted of snack supplement and nutrition education provided three times a week and 36 times in total. The nutrition education was led by the nutritionists and professionals using various educational media. Snack supplement included were milk, soymilk, carrot juice, bread, yogurt, and bananas. The result of pretest showed that the nutrient intakes of the subjects were much below the Recommended Nutrient Intake (RNI) level of Dietary Reference Intake for Koreans. Comparison of the dietary behavior score and nutrient intakes before and after the nutritional intervention program indicated that the food behavior score increased from 3.1 to 5.6 (P<0.001) and intake of energy, protein, carbohydrate, thiamin, niacin, vitamin C, iron, and potassium of the subjects all increased significantly (P<0.05). Mean Adequacy Ratio (MAR) of their nutrient intakes was also improved from 0.53 to 0.64 (P<0.01). These results indicated that the nutrition intervention program conducted in this study was effective in improving dietary behavior and nutrient intakes of elderly women in local community.
