방사선사 직무에서 조영제 정맥 주입 행위에 대한 인식도 분석 (Analysis Perceptions of Intravenous Injection Behavior of Contrast Medium in Radiological Technologists' Task)

  • 본 연구의 목적은 방사선사의 조영제 정맥확보 및 주입에 대한 직무 인식도를 분석하여 미래인력 대응안의 기초자료로 활용하고자 하였다. 임상의 방사선사 총 172명을 대상으로 인구학적 특성, 직무 우선 순위, 방사선사의 조영제 정맥확보 및 주입에 대한 인식도로 구성된 측정도구로 설문조사하였다. 통계분석은 기술통계, 빈도 분석, 독립표본 T-test 검정, ANOVA 분석으로 실시하였다. 그 결과, 첫째 현재 임상 방사선사는 방사선사의 미래인력 대응안으로 정맥확보 및 조영제 주입 행위에 대한 필요성을 높게 인지하고 있었으며, 이로 인한 업무 가중성은 낮게 평가하였다. 둘째, 정맥확보 및 조영제 주입 행위에 대한 두려움은 유의한 차이를 보여 모든 방사선사의 직무보다는 선택적인 직무 행위로 수행하는 것이 유용할 것으로 판단된다. 셋째, 방사선사의 정맥확보 및 조영제 주입 행위에 대한 교육과정 및 면허증 필요성이 제기되고 있어 이에 대한 구체적인 추가 연구가 요구된다. 넷째, 방사선사의 정맥확보 및 주입에 대한 긍정적 인식은 국민 의료서비스 향상과 방사선사 전문성 강화 및 직무 확장의 토대로 기대할 수 있다.

피부자극과 관심전환이 항암제 정맥주사 삽입시 통증감소에 미치는 영향 (The Effect of cutaneous Stimulation and Distraction on IV Injection Pain of Chemotherapy Patients)

  • Cutaneous stimulation and distraction are independent nursing interventions used in various painful conditions, which is explained by gate control theory. This study was aimed at identifying the effect of cutaneous stimulation, distraction and combination of cutaneous stimulation and distraction on the reduction of intravenous injection pain levels of chemotherapy patients. Repeated measurement post test research design was used for one group. Fifty-three cancer patients who received intravenous chemotherapy regulary in outpatient injection rooms of D medical center and Y medical center in Taegu were studied from June 23, 1997 to July 12, 1997. First the intravenous injection pain level of the control period was measured. Second, the intravenous injection pain level of the experimental period using cutaneous stimulation was measured. Third, the intravenous injection pain level of the experimental period using distraction was messured. Fourth, the intravenous injection pain level of the experimental period using a combination of cutaneous stimulation and distraction was measured. The instruments used for this study were a visual analogue pain scale as subjective pain measurement and an objective pain behavior checklist. Analysis of data was done by use of repeated measure ANOVA, bonferni, t-test, and F-test. The results of this study were summerized as follows : 1) The first hypothesis that the subjective pain score of intraveneous injection pain in the experimental period with cutaneous stimulation will be lower than in the control period was rejected. 2) The second hypothesis that the objective pain behavior score of intravenous injection pain in the experimental period with cutaneous stimulation will be lower than in the control period was accepted(F=24.23, p=0.0001, Bornferni p<.05). 3) The third hypothesis that the subjective pain score of intraveneous injection pain in the experimental period with distraction will be lower than in the control period was rejected. 4) The fourth hypothesis that the objective pain score of intravenous injection pain in the experimental period with distraction will be lower than in the control period was accepted(F=24.23, p=0.0001, Bornferni p<.05). 5) The fifth hypothesis that the subjective pain score of intravenous injection pain in the experimental period with combination of cutaneous stimulation and distraction will be lower than in the control period was accepted(F=3.04, p=0.031, Bonferni p<.05). 6) The sixth hypothesis that the objective pain score of intravenous injection pain in the experimental period with combination of cutaneous stimulation and distraction will be lower than in the control period was accepted(F=24.23, p=0.0001, Bonferni p<.05).

풍선 아트를 이용한 관심 전환이 학령전기 아동의 정맥 주사 삽입 시 통증에 미치는 효과 (Effects of Distraction using Balloon Art on Pain during Intravenous Injections in Preschool Children)

  • Purpose: This study was done to evaluate the effects of distraction using balloon art on pain experienced by preschool children when undergoing an intravenous injection. Methods: Participants were 40 hospitalized preschool children. Children in the experimental group (20) were distracted by balloon art while undergoing an intravenous injection. Children in the control group (20) received regular care. Pain experienced by the children was measured using self-report, observation tools, heart rate and oxygen saturation. The data were analyzed using the SAS program. Results: Compared with the control group, children in the experimental group showed significant difference in pain as shown by pulse rate. However, children in the experimental group did not show a decrease in pain in the self-report or behavior observation when compared with children in the control group. Conclusion: Distraction using balloon art can be used to reduce pain for preschool children when undergoing an intravenous injection. Further nursing interventions need to be developed and provided to preschool children when undergoing painful procedures.

Effects of the addition of low-dose ketamine to propofol anesthesia in the dental procedure for intellectually disabled patients

  • Background: This study aimed to examine whether the combination of low-dose ketamine and propofol in deep sedation is clinically useful in controlling the behavior in intellectually disabled patients who are typically extremely noncooperative during dental procedures. Methods: A total of 107 extremely noncooperative intellectually disabled adult patients were analyzed. In all patients, deep sedation was performed using either propofol alone (group P) or using a combination of propofol and 0.2 mg/kg or 0.4 mg/kg ketamine (groups PK0.2 and PK0.4, respectively). The procedures were performed in the order of insertion of nasal cannula into the nostril, attachment of mouth gag, and mouth cleaning and scaling. The frequency of patient movement during the procedures, mean arterial pressure, heart rate, peripheral oxygen saturation, recovery time, discharge time, and postoperative nausea and vomiting were examined. Results: The three groups were significantly different only in the frequency of patient movement upon stimulation during single intravenous injection of propofol and scaling. Conclusion: For propofol deep sedation, in contrast to intravenous injection of propofol alone, prior intravenous injection of low-dose ketamine (0.4 mg/kg) is clinically useful because it neither affects recovery, nor causes side effects and can suppress patient movement and vascular pain during procedures.

입원 아동의 말초정맥 주사시 통증 반응 (Peripheral Intravenous Injection Pain in Hospitalized Children)

  • Purpose: The purpose of the study is to offer necessary data to develop nursing interventions to reduce intravenous injection pain and uneasiness among hospitalized children. Methods: A total of 200 patients aged 1-72 months were selected. Pain during intravenous cannulation was assessed using the Procedural Behavior Checklist (PBCL) and the Faces Pain Rating Scale (FPRS). Data were analyzed by t-test and ANOVA using the SPSS/WIN 12.0. Results: Younger patients showed higher pain response than older patients (F=33.87, p<.001). Children with respiratory diseases showed higher responses in FPRS and PBCL than children without respiratory disease (F=4.17, p=.017; F=25.31, p<.001, respectively). Children of preschool age showed higher pain response during IV cannulation than the comparison group (t=2.04, p=.045). Children who had previous experiences with hospitalization and injections showed higher response to pain than those without these experiences (t=2.05, p=.045). In regards to FPRS, patients who were recannulated showed more painful restarts compared with patients injected just once (t=-3.60, p<.001). In regards to PBCL, infants and toddlers (t=-4.88, p<.001) and preschoolers (t=-3.86, p<.001) showed high pain scores during recannulation. Conclusion: A sick child's response to pain may be worse as they feel more pain over time. These characteristics should be considered for development of nursing interventions.

Vitamin E Potentiates the Anti-nociceptive Effects by Intraperitoneal Administration of Lidocaine in Rats

  • The present study was to evaluate effects of vitamin E on intravenous administration of lidocaine-induced antinociception. Experiments were carried out using male Sprague-Dawley rats. Orofacial formalin-induced nociceptive behavioral responses were used as the orofacial animal pain model. Subcutaneous injection of formalin produced significant nociceptive scratching behavior. Intraperitoneal injection of 5 and 10 mg/kg of lidocaine attenuated formalin-induced nociceptive behavior in the 2nd phase, compared to the vehicle-treated group. Intraperitoneal injection of 1 g/kg of vitamin E also attenuated the formalin-induced nociceptive behavior in the 2nd phase, compared to the vehicle-treated group. However, low dose of vitamin E (0.5 g/kg) did not affect the nociceptive behavioral responses produced by subcutaneous injection of formalin. The present study also investigated effects of intraperitoneal injection of both vitamin E and lidocaine on orofacial formalin-induced behavioral responses. Vehicle treatment affected neither formalin-induced behavioral responses nor lidocaine-induced antinociceptive effects. However, intraperitoneal injection of 0.5 g/kg of vitamin E enhanced the lidocaine-induced antinociceptive effects in the 2nd phase compared to the vehicle-treated group. Intraperitoneal injection of naloxone, an opioid receptor antagonist, did not affect antinociception produced by intraperitoneal injections of both vitamin E and lidocaine. These results suggest that treatment with vitamin E enhances the systemic treatment with lidocaine-induced antinociception and reduces side effects when systemically treated with lidocaine. Therefore, the combined treatment with vitamin E and lidocaine is a potential therapeutic for chronic orofacial pain.

비협조적인 아동의 치과 치료를 위한 정주 진정요법 (INTRAVENOUS SEDATION FOR UNCOOPERATIVE DENTAL CHILD PATIENTS)

  • 소아치과에서 진정요법은 치과 환경에 대하여 공포를 가진 아동, 협조 전 단계의 아동, 장애를 가진 아동 그리고 구토반사 등을 보이는 환자에게 사용될 수 있다. 본 증례에서는 다발성 치아 우식증을 주소로 본원에 내원한 환아에서 1.0mg/kg 케타민과 $0.03{\sim}0.06mg/kg$ 미다졸람 그리고 아트로핀 정주 병용투여 하에 수복치료를 시행하였다. 치료시간의 연장과 적절한 진정 상태 유지를 위하여 케타민(0.33mg/kg)과 미다졸람 $1mg(0.03{\sim}0.05mg/kg)$을 혼합하거나 각각 추가 투여하였다. 지속적으로 환자의 활력징후를 관찰하면서 약물의 추가투여 용량을 결정하였다. 모니터링 장비로 맥박 산소 측정법(pulse oximetry), 전자심전도(electrocardiogram), 비혈관식혈압(NIBP : Noninvasive Blood Pressure)을 사용하여 맥박, 호흡수, 혈압, 산소 포화도 등을 측정하였다. 진료 과정동안 안전을 위해 미국 소아치과 학회(American Academy of Pediatric Dentistry ; AAPD)의 권고 사항을 준수하였다.

말에서 detomidine과 tramadol의 정맥 투여에 의한 진정 및 진통효과 (Sedative and Analgesic Effects of Intravenous Detomidine and Tramadol on Horses)

  • 본 연구는 승용말에 detomidine과 tramadol을 정맥 투여한 후 진정 및 진통 효과를 평가하기 위하여 실시되었다. 여섯 마리의 승용말에 각각 detomidine (10 ${\mu}g/kg$), tramadol (2 mg/kg) 및 detomidine/tramadol 병용 (각 10 ${\mu}g/kg$과 2 mg/kg) 투여하였다. 심박수, 호흡수, 직장체온, 간접동맥혈압, 위장관 운동에서 detomidine과 detomidine/tramadol 병용은 큰 차이를 보이지 않았다. 진정효과는 detomidine, detomidine/tramadol 병용 투여 후 5분에 관찰되기 시작하였으나, detomidine 및 detomidine/tramadol 병용간 유의한 진정효과의 차이는 나타나지 않았다. Detomidine과 detomidine/tramadol 병용은 투여 후 50분까지 유사한 진통효과를 나타내었으나, detomidine/tramadol 병용이 더 긴 진통효과를 나타내었다. Detomidine의 투여 후 혈당 수치가 투여 후 60분까지 증가하였으나, detomidine/tramadol 병용 투여 후의 혈당 수치는 증가하지 않았다. Tramadol과 detomidine/tramadol 병용 투여된 한 마리가 투여 5분 이내에 흥분된 행동을 보였다. 이상의 결과로부터 detomidine/tramadol 병용 투여가 기립상태에서 말의 간단한 외과적 처치와 진단을 위하여 유용하게 이용될 수 있을 것으로 판단된다.

Peripheral metabotropic glutamate receptors differentially modulate mustard oil-induced craniofacial muscle pain in lightly anesthetized rats

  • The present study investigated the role of peripheral group I, II, and III metabotropic glutamate receptors (mGluRs) in mustard oil (MO)-induced nociceptive response in the masseter muscles of lightly anesthetized rats. Experiments were carried out on male Sprague-Dawley rats weighing 300-350 gm. After initial anesthesia with sodium pentobarbital (40 mg/kg, i.p.), one femoral vein was cannulated and connected to an infusion pump for intravenous infusion of sodium pentobarbital. The rate of infusion was adjusted to provide a constant level of anesthesia. MO (30 ${\mu}L$) was injected into the mid-region of the left masseter muscle via a 30-gauge needle over 10 seconds. After 30 mL injection of 5, 10, 15, or 20% MO into the masseter muscle, total number of hindpaw-shaking behavior was monitored. Intramuscular administration of MO significantly produced hindpawshaking behavior in a dose-dependent manner, as compared with the vehicle (mineral oil)-treated group. Intramuscular pretreatment with 10 or 100 ng DHPG, a group I mGluRs agonist, enhanced MO-induced hindpaw-shaking behavior, while APDC (20 or 200 ${\mu}g$), a group II mGluRs agonist, or L-AP4 (2 ${\mu}g$), a group III mGluRs agonist, significantly reduced MO-induced nociceptive behavior. The antinociception, produced by group II or III mGluRs agonists, was abolished by pretreatment with LY341495, a group II mGluRs antagonist, or CPPG, a group III mGluRs antagonist, res-pectively. Based on these observations, peripheral mGluRs differentially modulated MO-induced nociceptive behavior response in the craniofacial muscle pain and peripheral group II and III mGluRs agonists could be used in treatment of craniofacial muscle nociception.

행동조절장애 환자에서 치과치료를 위한 외래마취의 분석 (The Analysis of Outpatient Anesthesia for Dental Treatment in Handicapped Patients with Behavior Disorder)

  • Background: The aim of this study was to analyze outpatient anesthesia for dental treatment in handicapped patients with behavior disorder in order to use data for carrying out better and safe anesthetic management. Methods: The data were drawn from the 100 patients with behavior disorder who visited CNUDH dental clinic for disabled based on anesthesia record to investigate patient's systemic condition, cooperative level, anesthesia method according to patients cooperation, and side effects after recovery time. Results: Mental retardation (58%) is the most reason to choose general anesthesia. The methods of induction according to cooperative level are intravenous propofol injection in 22 cases and inhalation of sevoflurane in 78 cases. Induction time of anesthesia were within 10 seconds in cases of propofol induction and average $48.8{\pm}18.5$ seconds in cases of inhalation induction. The time spent on dental treatment was average $3.2{\pm}1.1$ hours. After the end of treatment, average time to move from unit chair to recovery bed, to recliner, and to discharge from hospital are $10.4{\pm}5.1$, $36.9{\pm}17.1$ and $72.4{\pm}16.0$ minutes, respectively. During recovery, there are nausea with 9%, vomiting with 4%, dizziness with 2%, finger injury with 1%. Conclusions: This study showed our successful anesthetic outcomes without any severe side effects or complications. Through this study, it will be used for safe anesthetic management as useful reference data.
