In vitro test using chorioallantoic membrance vascular assay to assess the irritancy potential of surfactants (CAMVA(Choriollantoic Membrane Vascular Assay)를 이용한 계면활성제의 자극 평가에 관한 연구)

  • Skin irritations accompany the series of complex, physical and chemical changes. Alternative methods which reflect the biological phenomenon more sensitively are necessary for the purpose of reducing the animal tests and improving the accuracy and reliability. Chorioallantoic membrane (CAM) vascular assay is a in vitro method which utilizes the chorioallantoic membrane of hen's fertilized egg of about 10 days old. Test substances are placed directly onto the surface of the CAM and 1 hour later the CAM vasculature is subjectively evaluated to determine with a chemically related-injurious reponse including the appearance of haemorrage, congestion, coagulation, and so on. In this research, using the various surfactants, the correlations of CAMVA with in vivo models (intradermal safety test and human primary irritation test) were investigated. And CAMVA closely correlated to intradermal safety test (r=0.89) and human primary irritation test (r=0.90). From the result, it seems that CAMVA can also be used as a method for predicting the skin irritaions.

A Study on Oriental Medical Treatment Programs for Adolescents with Rhinitis in Collaboration with Community Health Center (청소년 비염 환자에 대한 보건소 연계 한방치료 효과에 관한 연구 - 피내침과 한약을 이용한 치료 효과 -)

  • Objectives : The purpose of this study is to investigate for rhinitis of middle school students and to confirm the effect of oriental medical treatments on rhinitis. We proceeded with this oriental medical treatment programs in collaboration with community health center. Methods : We have given oriental medical treatment, such as herbal medicines(pillet type Gamisinicheongpye-um and ointment) and intradermal acupuncture therapy at Hapgok (LI4), Yeonghyang (LI20), Indang, Bi-ik points, to patients for 9 weeks. Results of this treatment program were evaluated on a 4-point-scale depends on symptoms ; 'severe ()', 'moderate (++)', 'mild (+)' and 'no symptom (-)'. Results : After oriental medical treatments, rhinitis symptoms (sneezing, rhinorrhea, nasal obstruction, nasal itching and dysosmia) were improved significantly compared to before treatments (p<0.05). Conclusions : We confirmed rhinitis treated with oriental medicine should significant improvement and suggest that it is necessary to expand oriental medical treatment programs in collaboration with community health center.

Development of a New Approach to Determine the Potency of Bacille Calmette-Guérin Vaccines Using Flow Cytometry

  • Objectives: To circumvent the limitations of the current golden standard method, colony-forming unit (CFU) assay, for viability of Bacille Calmette-$Gu{\acute{e}}rin$ (BCG) vaccines, we developed a new method to rapidly and accurately determine the potency of BCG vaccines. Methods: Based on flow cytometry (FACS) and fluorescein diacetate (FDA) as the most appropriate fluorescent staining reagent, 17 lots of BCG vaccines for percutaneous administration and 5 lots of BCG vaccines for intradermal administration were analyzed in this study. The percentage of viable cells measured by flow cytometry along with the total number of organisms in BCG vaccines, as determined on a cell counter, was used to quantify the number of viable cells. Results: Pearson correlation coefficients of FACS and CFU assays for percutaneous and intradermal BCG vaccines were 0.6962 and 0.7428, respectively, indicating a high correlation. The coefficient of variation value of the FACS assay was less than 7%, which was 11 times lower than that of the CFU assay. Conclusion: This study contributes to the evaluation of new potency test method for FACS-based determination of viable cells in BCG vaccines. Accordingly, quality control of BCG vaccines can be significantly improved.

Spinal orexin A attenuates opioid-induced mechanical hypersensitivity in the rat

  • Background: Repeated administration of opioid analgesics for pain treatment can produce paradoxical hyperalgesia via peripheral and/or central mechanisms. Thus, this study investigated whether spinally (centrally) administered orexin A attenuates opioid-induced hyperalgesia (OIH). Methods: [D-Ala2, N-Me-Phe4, Gly5-ol]-enkephalin (DAMGO), a selective µ-opioid receptor agonist, was used to induce mechanical hypersensitivity and was administered intradermally (4 times, 1-hour intervals) on the rat hind paw dorsum. To determine whether post- or pretreatments with spinal orexin A, dynorphin A, and anti-dynorphin A were effective in OIH, the drugs were injected through an intrathecal catheter whose tip was positioned dorsally at the L3 segment of the spinal cord (5 ㎍ for all). Mechanical hypersensitivity was assessed using von Frey monofilaments. Results: Repeated intradermal injections of DAMGO resulted in mechanical hypersensitivity in rats, lasting more than 8 days. Although the first intrathecal treatment of orexin A on the 6th day after DAMGO exposure did not show any significant effect on the mechanical threshold, the second (on the 8th day) significantly attenuated the DAMGO-induced mechanical hypersensitivity, which disappeared when the type 1 orexin receptor (OX1R) was blocked. However, intrathecal administration of dynorphin or an anti-dynorphin antibody (dynorphin antagonists) had no effect on DAMGO-induced hypersensitivity. Lastly, pretreatment with orexin A, dynorphin, or anti-dynorphin did not prevent DAMGO-induced mechanical hypersensitivity. Conclusions: Spinal orexin A attenuates mechanical hyperalgesia induced by repetitive intradermal injections of DAMGO through OX1R. These data suggest that OIH can be potentially treated by activating the orexin A-OX1R pathway in the spinal dorsal horn.

Tramadol as a Voltage-Gated Sodium Channel Blocker of Peripheral Sodium Channels Nav1.7 and Nav1.5

  • Tramadol is an opioid analog used to treat chronic and acute pain. Intradermal injections of tramadol at hundreds of millimoles have been shown to produce a local anesthetic effect. We used the whole-cell patch-clamp technique in this study to investigate whether tramadol blocks the sodium current in HEK293 cells, which stably express the pain threshold sodium channel Nav1.7 or the cardiac sodium channel Nav1.5. The half-maximal inhibitory concentration of tramadol was 0.73 mM for Nav1.7 and 0.43 mM for Nav1.5 at a holding potential of -100 mV. The blocking effects of tramadol were completely reversible. Tramadol shifted the steady-state inactivation curves of Nav1.7 and Nav1.5 toward hyperpolarization. Tramadol also slowed the recovery rate from the inactivation of Nav1.7 and Nav1.5 and induced stronger use-dependent inhibition. Because the mean plasma concentration of tramadol upon oral administration is lower than its mean blocking concentration of sodium channels in this study, it is unlikely that tramadol in plasma will have an analgesic effect by blocking Nav1.7 or show cardiotoxicity by blocking Nav1.5. However, tramadol could act as a local anesthetic when used at a concentration of several hundred millimoles by intradermal injection and as an antiarrhythmic when injected intravenously at a similar dose, as does lidocaine.

Comparative Literature Review of Floating Acupuncture: Compared to Meridian Muscle, Myofascial Pain Syndrome and Wrist-Ankle Acupuncture (부침 이해를 위한 문헌 비교: 경근, 근막동통이론, 완과침 그리고 부침)

  • Objectives : Floating acupuncture (FA) is a kind of newly developed acupuncture technique that contains its own apparatus. The technique has unique points that the body of the acupuncture needle stays intradermal space and manipulation is performed by shaking the needle horizontally; after manipulation, part of the needle remained in the intradermal space for 2~3 days. FA is not a common acupuncture methodology while various clinical study claims its efficacy on musculoskeletal disorders. In this study, the authors aimed to enhance the use of FA by comparing related theories. Methods : The authors reviewed classics, books, and articles related to FA, Meridian Muscle (MM), Myofascial Pain Syndrome (MPS), and Wrist-Ankle Acupuncture (WAA), and compared its characteristics by related theory, related symptoms, apparatus, and performing procedures. Results : FA was related and had various common parts with MM, MPS, and WAA, however, there were unique parts in the manipulation, apparatus, and stimulation location. FA is based on MM and MPS which pointing 'ashi points' or 'trigger points' as a treating target while FA does not stimulate the points directly. FA also targets subcutaneous space by inserting the needle horizontally as WAA does, while FA remains the needle handle part in the subcutaneous area for 2~3 days for more stimulation. Conclusions : FA has a unique manipulation procedure and potential benefit for musculoskeletal disorders despite the crude theological base written by Fu himself. Thus, developing a new explanation and patient-friendly methodology/apparatus is required for further down-to-earth studies.

The Effects of Dimethyl Sulfoxide and Sodium Thiosulfate for the Prevention of Tissue Necrosis due to Extravasaion of Mitomycin-C (혈관외로 유출된 Mitomycin-C에 의한 조직괴사 예방을 위한 Dimethyl Sulfoxide와 Sodium Thiosulfate의 효과)

  • Extravasation of toxic chemotherapeutic agents cause severe skin ulceration and necrosis which often need secondary surgical intervention. Still, there were not established antidote agent in case of extravasation with mitomycin-c. Dimethyl sulfoxide is known as an effective chemical scavenger of toxic hydroxyl free radical and sodium thiosulfate also was demonstrated significant protector from mitomycin-c induced ulceration by a few experimental studies. Author investigated necrotic area of mitomycin-c injected site and compare to the effectiveness of topical treatment with dimethyl sulfoxide and intradermal injection of sodium thiosulfate according to starting times, forty five mice were divided into 3 groups. Control group(n=5) had no treatment after subcutaneous injection of mitomycin-c. Experimental group I and II were 20 mice treated dimethyl sulfoxide and sodium thiosulfate, respectively. Depending on the starting time of treatment, group I and II were subdivided into 1, 2, 3 and 4 as immediate, 6 hours, 12 hours and 24 hours after mitomycin-c injection. Histologic studies of the necrotic area and survival area after treatment were performed using hematoxylin-eosin staining. The mean necrotic area of group I was significantly decreased depending on the starting time of treatment compared with control group(p<0.01). The results means there was no necrosis area which was treated with topical sodium thiosulfate within 6 hours, and it showed also significant decrease of necrosis area within 24 hours. There was also no necrosis area in group II-1 and significant decrease of necrosis area II-2 and III-3. But, effctiveness of intradermal injection of sodium thiosulfate was not found in group II-4 which was started after 24 hours. Hisotolgic findings showed a bland coagulative necrosis without inflammatory changes and no granulation tissue. The significant difference that cytoplasmic loss of subcutaneous fat and decrease number of hair follicles between two groups resulted from the methods of treatment by topical application and intradermal injection. In conclusion, immediate treatments with topical dimethyl sulfoxide or intradermal injection of sodium thiosulfate signifcantly prevents necrosis by extravasation of mitomycin-c.

Delayed Allergic Reaction to Secondary Administrated Epidural Hyaluronidase

  • We are reporting a rare case of a delayed hypersensitivity reaction caused by hyaluronidase allergy following a lumbar transforaminal epidural block. Using an intradermal skin test, we have provided evidence that the systemic allergic reaction resulted from hypersensitivity to hyaluronidase. To our knowledge, this is a rare case of a delayed hypersensitivity reaction to epidural hyaluronidase, comprised of an initial exposure to hyaluronidase with no subsequent allergic response in prior block followed by a subsequent delayed reaction to hyaluronidase during a second epidural block.

A comparative study on the diagnosis of ELISA test and PPD test of the bovine tuberculosis (ELISA를 이용한 우결핵검사 결과에 대한 PPD 접종법 결과 분석 비교)

  • On the basis of the 2009 business plan, 20,394 Korean native cattle and beef cattle were carried examination of bovine tuberculosis by using ELISA technique from March to December. As a result, 66 cattle tested positive for tuberculosis and showed 0.32% positive ratio. Intradermal tuberculin test about 66 cases of ELISA positive cattle was carried out, and all of 66 cattle were confirmed as negative. However, when 7 PPD-positive cattle derived from slaughterhouse were tested by 20k ELISA kit and MS ELISA kit, 3 (2 suspect) cattle and 5 cattle showed positive results, respectively. As compared to the results of PPD test, the concordance rates were 43% (71% included suspect) with 20k ELISA kit and 71% with MS ELISA kit.

Studies on the Anti-allergic Effect of Biahwan and Sutodan in Rats and Mice (비아환(肥兒丸) 및 수토단(水土丹)의 항(抗)알레르기에 관(關)한 실험적(實驗的) 효과(?果))

  • Experimental studies were done to research the clinical effects of Biahwan and Sutodan on the Anti-allergic Effect in Rats and Mice. The results were summerized as follows; 1. In the 48hours homologous passive cutaneous anaphylaxis in rats provoked by the IgE-like antibody against egg white albumin, Biahwan and Sutodan revealed significant effects. 2. In the effects of Biahwan and Sutodan on vascular permeability responses to intradermal histamine and serotonin in rats, Biahwan revealed significant effect, but Sutodan revealed none significant effect. 3. In the delayed type hypersensitivity responses to Picryl Chloride in mice, Biahwan and Sutodan revealed none significant effects. 4. In the delayed type hypersensitivity responses to SRBC (Sheep Red Blood Cell) in mice, Biahwan and Sutodan revealed significant effects.
