Objective : Aneurysm volume quantification (AVQ) using the equation of ellipsoid volume is widely used although it is inaccurate. Furthermore, AVQ with 3-dimensional (3D) rendered data has limitations in general use. A novel universal method for AVQ is introduced for any diagnostic modality and application to any shape of aneurysms. Methods : Relevant AVQ studies published from January 1997 to June 2019 were identified to determine common methods of AVQ. The basic idea is to eliminate the normal artery volume from 3D model with the aneurysm. After Digital Imaging and Communications in Medicine (DICOM) data is converted and exported to stereolithography (STL) file format, the 3D STL model is modified to remove the aneurysm and the volume difference between the 3D model with/without the aneurysm is defined as the aneurysm volume. Fifty randomly selected aneurysms from DICOM database were used to validate the different AVQ methods. Results : We reviewed and categorized AVQ methods in 121 studies. Approximately 60% used the ellipsoid method, while 24% used the 3D model. For 50 randomly selected aneurysms, volumes were measured using 3D Slicer, RadiAnt, and ellipsoid method. Using 3D Slicer as the reference, the ratios of mean difference to mean volume obtained by RadiAnt and ellipsoid method were -1.21±7.46% and 4.04±30.54%, respectively. The deviations between RadiAnt and 3D Slicer were small despite of aneurysm shapes, but those of ellipsoid method and 3D Slicer were large. Conclusion : In spite of inaccuracy, ellipsoid method is still mostly used. We propose a novel universal method for AVQ that is valid, low cost, and easy to use.
Objective : Distinguishing between an infundibulum and a true aneurysm is clinically important. This study aimed to evaluate whether using source image based new three-dimensional rotational angiography (S-n3DRA) can increase the rate of aneurysm detection and improve distinction between a true aneurysm and an infundibulum. Methods : Twenty-two consecutive patients with 23 lesions, were evaluated by time-of-flight (TOF) magnetic resonance angiography (MRA), S-n3DRA, and digital subtraction angiography (DSA). The data were retrospectively and independently reviewed by two neurointerventionists, and the diagnoses based on TOF MRA, S-n3DRA, and DSA were compared. The diagnostic efficacy (interobserver agreement and diagnostic performance) of S-n3DRA was compared with that of TOF MRA. Results : S-n3DRA showed higher interobserver agreement (κ=0.923) than TOF MRA (κ=0.465) and significantly higher accuracy than MRA in distinguishing an aneurysm from an infundibulum (p=0.0039). Conclusion : Compared to MRA, S-n3DRA could provide better screening accuracy and information for distinguishing an aneurysm from an infundibulum. Therefore, S-n3DRA has the potential to reduce the need for DSA.
본 연구는 20세에서 50세까지 건강한 남성 지원자를 대상으로 내경동맥과 내경정맥의 혈류속도를 측정하기 위하여 2D-PC MRA를 촬영했다. 2D-PC MRA는 제4-5경추디스크를 지나는 FOV 24cm 넓이로 횡단 촬영하여 내경동맥과 내경정맥의 혈류속도를 넥타이를 매지 않은 상태와 세게 맨 상태에서 각각 측정하였다. 넥타이를 매지 않았을 때 내경동맥의 평균 혈류속도 최고치는 우측 72.13cm/sec, 좌측 74.96cm/sec (양측 평균 73.54cm/sec)이였고 내경정맥의 평균 혈류속도 최고치는 우측 -34.45cm/sec, 좌측 -24.99cm/sec(양측평균 -29.72cm/sec)였다. 그러나 넥타이를 세게 매었을 때 내경동맥의 평균 혈류속도 최고치는 우측 61.35cm/sec, 좌측 65.19cm/sec(양측 평균 63.27cm/sec)이며 내경정맥의 혈류속도 최고치는 우측 -22.14cm/sec, 좌측 -17.93cm/sec (양측 평균 20.03cm/sec)였다. 목에 세게 넥타이를 맨 경우가 넥타이를 매지 않은 상태 보다 양쪽 내경동맥의 평균 혈류속도가 86% (63.27/73.54cm/sec)로 약간 감소하는 반면 양쪽 내경정맥의 평균 혈류속도는 67%(-20.03/-29.72cm/sec)로 유의성 있게 감소하였다(P<0.05). 따라서 넥타이를 세게 매었을 때 내경정맥의 순환에 미치는 영향(33%의 혈류속도감소)이 내경동맥(14%의 혈류속도감소) 보다 유의성 있게 큰 것을 알 수 있었다(P<0.05). 결론적으로 본 연구결과는 2D-PC MRA를 이용하여 넥타이를 목에 세게 매었을 때 순간적으로 양쪽 내경정맥이 내경동맥보다 상대적으로 압박되어 혈류속도가 의미 있게 감소되며 일시적 폐쇄가 되는 소견을 보였다. 이 경우 넥타이를 목에 세게 매고 생활을 하는 경우에 대변을 보거나 또는 세면을 하는 등 일상적인 생활에서도 예기치 않게 일시적으로 내경정맥혈관이 압박되어 폐쇄되며 이차적으로 두개강 내의 정맥성고혈압을 일으켜 뇌졸중의 요인이 될 수 있는 것으로 유추된다.
목 적 : 원위부 전대뇌 동맥에 발생하는 동맥류는 모동맥의 확보가 힘들고 재출혈 및 수술중 조기출혈의 빈도가 높으며, 익숙치 않은 수술적 접근법이 필요하다든지 하는 몇 가지 독특한 문제점이 있다. 따라서 전대뇌 반구간열 주변의 해부학적 구조물과 박리 시작점에 대한 수술전 지식을 요한다. 저자들은 박리 시작점에 대한 일관된 외부표식자를 이용하여 전두 기저부 대뇌반구간열 접근법 수술을 시행하였고 이를 심부 구조물들에 대한 접근 지표로 삼고자 하였다. 대상 및 방법 : 1995년 11월부터 1999년 6월까지 동맥류경을 결찰한 총 131명의 뇌동맥류파열 환자 중에서 원위부 전대뇌 동맥류 9명에 대하여 혈관조영술, 의무기록지, 수술소견을 통해 임상적, 수술소견을 조사하였다. 결 과 : 원위부 전대뇌동맥류의 빈도는 6.3%였고, 6례에서 뇌량 연변 동맥이 기시하는 뇌량 주위 동맥에서 발생하였고, 3례에서는 전두극동맥이 기시하는 뇌량 주위 동맥에서 발생하였다. 동반된 다른 혈관기형 및 다른 동맥류는 각각 3예에서 발견되었다. 술전 환자 상태는 일반적으로 불량했다. 조기수술은 7예에서 실시되었으며, 전두 기저부 대뇌반구간열 접근법으로 동맥류 결찰을 시행한 경우도 7예였다. 술후 심한 혈관연축과 흡인성 폐렴으로 인한 사망이 각각 1례씩이었으며 그 이외에는 신경학적 소견은 정상이었다. 결 론 : 원위부 전대뇌 동맥류의 조기수술시에 전두 기저부 대뇌반구간열 접근법이 유용하였다. 이 방법으로 뇌견인을 최소화할 수 있었으며 동맥류의 조기파열을 방지할 수 있었다. 저자들은 대뇌반구 간열의 박리 시작점으로 첫번째 교정맥(전두극 정맥)이 상시상 정맥동으로 유출되는 지점을 기준으로 하여 뇌량 및 뇌량 주변부 원위부 전대뇌동맥류에 접근할 수 있었다.
목 적 : 파열된 뇌동맥류 수술후 경련발작은 잘 알려진 합병증으로 항경련제 치료가 요구된다. 저자들은 뇌동맥류 파열에 의한 지주막하출혈 환자에서 수술후 경련발작의 위험인자를 조사하여 경련발작 치료에 임상적 의의를 비교 하고자 한다. 방 법 : 1990년부터 1996년까지 파열된 뇌동맥류로 개두술을 시행한 321명의 환자중 1년 이상 추적관찰(1~4.6년)하였던 206명을 대상으로 후향적 방법으로 조사하였다. 모든 환자는 수술후 3~18개월 동안 예방적 항경련제 치료를 받았다. 경련발작에 기여하는 요인으로서 연령, 성별, 내원 당시 신경학적 상태, 수술전후 전산화단층촬영소견, Hunt-Hess grade, Fisher grade, 뇌동맥류의 위치, 고혈압의 기왕력, 수술시기, 그리고 지연성 신경학적 결손 등을 조사하여 비교 분석하였다. 통계적 검증분석 방법으로 chi-square test와 Fisher's exact test를 이용하였다. 결 과 : 평균 추적관찰기간은 1.8년으로 206명중 18명이 경련발작 소견을 보여 8.7%의 발생률을 보였다. 연령에서 나이가 적을수록, 파열된 동맥류의 위치가 중뇌동맥인 경우에서 경련발작이 높게 나타났다. 지연성 신경학적 결손을 보인 42명의 환자에서 8명(19%)이 경련발작이 발생하여 높은 통계학적 유의성을 보였다. 또한 고혈압의 기왕력, 수술후 뇌전산화촬영상 뇌경색등도 이와 관련성이 있었고, 수술전후 뇌내혈종의 소견이 관찰되면 또한 경련발작이 높게 발생되었다. 그러나 Hunt-Hess grade, Fisher grade, Glasgow Outcome Scale, 그리고 출혈후 수술의 시기 등은 경련발작의 발생빈도와는 통계학적 유의성을 보이지 않았다. 결 론 : 뇌동맥류 파열에 의한 수술후 경련발작의 위험인자로 낮은 연령, 중뇌동맥류, 지연성 신경학적 결손, 수술후 뇌전산화촬영상 뇌경색소견, 고혈압, 그리고 수술전후 뇌내혈종의 유무 등이 연관되었고, 이러한 경련발작의 유발인자를 인지함으로서 수술후 경련발작에 대한 예방과 치료에 도움을 줄 수가 있으며 이에 대한 전향적 연구가 요구된다.
Objective : Subarachnoid hemorrhage(SAH) is still one of the most serious disease with high morbidity and mortality in the neurosurgical field. Clipping of the aneurysmal neck is the gold standard of the surgical treatment of aneurysmal SAH. The purpose of this study was to investigate the role of the postoperative angiography and to assess the risk factors related to the incomplete clipping. Materials and Methods : From July 1995 to June 1998, the pre- and postoperative angiography were performed in 50 patients among total 81 patients who have underwent the aneurysmal surgery. We reviewed the various contributing factors including age, sex, Hunt-Hess grade, Fisher grade and the premature rupture of aneurysm during operation retrospectively. Careful evaluation of pre- and postoperative angiography focusing on the size, shape, and remnant neck of the aneurysms and vasospasm was performed. According to the angiographic findings, the patients were divided into two groups ; a complete clipping group and an incomplete clipping group. The data were analyzed by using unpaired independent sample t test after F-test to compare the significance between two groups. Results : Incomplete clipping of aneurysms was found in 6(12%) patients through the evaluation of postoperative angiography. Among them, three cases were located on the middle cerebral artery territory. Whereas the patient age, sex, Hunt-Hess grade, and Fisher grade were not significant(p>0.05), an intraoperative premature rupture had a statistical significance(p<0.05). A severe vasospasm occurred in 24(48%) cases and one patient with anterior communicating aneurysm was reoperated due to residual sac. Conclusion : According to our experience, the surgeons' judgement is the most reliable factor in deciding the postoperative angiography. During the aneurysmal surgery, the premature rupture always disturbs a complete clipping of aneurysms. Therefore, the temporary clipping of parent arteries is considered essential for a successful clipping. We believe that the postoperative angiography has a role in decreasing the re-bleeding risk due to clip migration and an inaccurate clipping only in the selected cases.

Objective : Paraclinoid aneurysms are a group of aneurysms arising at the distal internal carotid artery. Due to a high incidence of small, wide-necked aneurysms in this zone, it is often challenging to achieve complete occlusion when solely using detachable coils, thus stent placement is often required. In the present study, we aimed to investigate the effect of stent placement in endovascular treatment of paraclinoid aneurysms. Methods : Data of 98 paraclinoid aneurysms treated by endovascular approach in our center from August 2005 to June 2016 were retrospectively reviewed. They were divided into two groups : simple coiling and stent-assisted coiling. Differences in the recurrence and progressive occlusion between the two groups were mainly analyzed. The recurrence was defined as more than one grade worsening according to Raymond-Roy Classification or major recanalization that is large enough to permit retreatment in the follow-up study compared to the immediate post-operative results. Results : Complete occlusion was achieved immediately after endovascular treatment in eight out of 37 patients (21.6%) in the stent-assisted group and 18 out of 61 (29.5%) in the simple coiling group. In the follow-up imaging studies, the recurrence rate was lower in the stent-assisted group (one out of 37, 2.7%) compared to the simple coiling group (13 out of 61, 21.3%) (p=0.011). Multivariate logistic regression model showed lower recurrence rate in the stent-assisted group than the simple coiling group (odds ratio [OR] 0.051, 95% confidence interval [CI] 0.005-0.527). Furthermore there was also a significant difference in the rate of progressive occlusion between the stent-assisted group (16 out of 29 patients, 55.2%) and the simple coiling group (10 out of 43 patients, 23.3%) (p=0.006). The stent-assisted group also exhibited a higher rate of progressive occlusion than the simple coiling group in the multivariate logistic regression model (OR 3.208, 95% CI 1.106-9.302). Conclusion : Use of stents results in good prognosis not only by reducing the recurrence rate but also by increasing the rate of progressive occlusion in wide-necked paraclinoid aneurysms. Stent-assisted coil embolization can be an important treatment strategy for paraclinoid aneurysms when considering the superiority of long term outcome.
목적: 이 연구의 목적은 소아 수막종의 임상적, 영상의학적, 그리고 병리학적 특징을 기술함에 있다. 대상 및 방법: 병리학적으로 진단된 수막종을 가진 16예의 소아환자를 대상으로 임상기록과 자기공명영상을 후향적으로 분석하였다. 평균연령은 14세 (3-18세)였다. 자기공명영상은 병변의 크기, 신호강도, 경계부의 특징, 내부 구조, 조영증강, 그리고 경막과 뇌실질의 변화를 검토하였고 다른 영상 방법 및 병리소견을 함께 분석하였다. 결과: 종양의 평균 크기는 5.24 cm (1.3-18.1 cm)였으며 12예는 천막상부에 4예는 천막하부에 위치하였다. T2 강조 영상에서 높은 신호강도를 보이는 종양이 9예 있었고 4예에서 동신호강도를, 3예에서 낮은 신호강도를 보였다. T1 강조 영상에서는 낮은 신호강도 11예, 동신호강도 4예, 그리고 높은 신호강도 1예가 있었다. 모든 종양의 경계가 좋았고 조영증강을 보였다. 5예에서 종양은 균질한 양상을 보였으며 나머지에는 낭종이나 괴사에 의해 비균질한 양상을 보였다. 경막 부착 소견이 11예에서, 뇌실질 부종이 10예에서 동반되었다. 컴퓨터 단층촬영 소견은 6예에서 밝은 음영을 보였으며 5예에서 석회화를 동반하였다. 혈관 조영술 상 3예는 내경동맥에서, 4예는 외경동맥으로부터 혈액 공급이 이루어졌다. 병리검사결과 종양의 아형은 이행성 (4예), 수막세포성 (4예), 척삭성 (2예), 섬유성 (2예), 투명세포성 (1예), 유리질화성 (1예), 횡문근양 유두모양 (1예), 그리고 비정형(1예) 수막종으로 진단되었다. 결론: 소아의 수막종은 주로 10대에 발생하며 다양한 병리학적 아형을 보일뿐만 아니라 비정형적인 영상 소견으로 인해 축내 종양으로 오인될 수 있다.
Objective : The objective of this study was to evaluatelong-term radiologic prognosis and characteristics of in-stent stenosis (ISS) after stent assisted coiling (SAC) for cerebral aneurysm and analyze its risk factors. Methods : Radiological records of 362 cases of SAC during 10 years were retrospectively reviewed. Patients were included in this study if they had follow-up angiogram using catheter selected angiography at least twice. All subjected were followed up from 12 months to over 30 months. Of 120 patients, 123 aneurysms were enrolled. Patient data including age, sex, aneurysm size, neck size, procedural complication, kinds of stent, ISS associated symptom, ruptured state, location of ISS, degree of ISS, radiologic prognosis of ISS, follow-up period of time, and medical comorbidities such as hypertension, diabetes mellitus (DM), dyslipidemia, and smoking were collected.Statistical comparisons of group clinical characteristics were conducted for the total population. Results : Among 123 casesof aneurysm, 22 cases (17.9%) of ISS were revealed on follow-up angiography. Multiple stenting was performed in three cases and intra-procedural rupture occurred in two cases. Most cases were asymptomatic and symptomatic stenosis was identified in only one case. Sixteen cases were ruptured aneurysm. Mild stenosis was observed in 11 cases. Moderate stenosis was found in eight cases and severe stenosis was identified in three cases. Mean timing of identification of ISS was 8.90 months. The most common type was proximal type. Most cases were improved or not changed on follow-up angiography. Only one case was aggravated from mild stenosis to occlusion of parent artery. Mean follow-up period was 44.3 months. We compared risk factors and characteristic between ISS group and non-ISS group using univariate analysis. Multiple stenting was performed for three cases (13.6%) of the ISS group and four cases (4.0%) of the non-ISS group, showing no statistical difference between the two groups (p=0.108). Additionally, the proportion of patients who had more than two risk factors among four medical risk factors (hypertension, DM, dyslipidemia, and smoking) was higher in the ISS group than that in the non-ISS group, the difference between the two was not statistically significant either (31.8% vs. 12.9%, p=0.05). Conclusion : Clinical course and long-term prognosis of ISS might be benign. Most cases of ISS could be improved or not aggravated. Control of medical co-morbidity might be important. To the best of our knowledge, our study had more cases with longer follow-up period of time than other reports.

저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에

제 5 장 계약 해지 및 이용 제한

② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
