상지말초신경손상에 대한 침술치료의 효과: 체계적 문헌고찰 (Acupuncture for Upper Extremity Peripheral Nerve Injury: A Systematic Review)

  • Objectives The purpose of this study is to systematically explore the effects and safety of acupuncture treatment for upper extremity peripheral nerve injury and to review the methodology of clinical trials. Methods We searched 9 electronic databases(3 international, 1 Chinese, 5 Korean) including English, Korean and Chinese, up to December 2017 for randomized controlled trials which evaluated the effects of the acupuncture in patients with upper extremity peripheral nerve injury. We abstracted the designs of the randomized clinical trials and the method of acupuncture treatment according to the Standards for Reporting Interventions in Clinical Trials of Acupuncture(STRICTA). Results A total of 8 papers were reviewed. All randomized clinical trials were conducted in China. Of them, five studies(62.5%) were electro-acupuncture as intervention. All randomized clinical trials reported favorable effects of acupuncture treatments compared to baseline or control group with outcomes of efficacy rate. However risk of bias seemed high. LI4, LI11, SI3, PC3, PC6 were most frequently used for acupoints to treat upper extremity peripheral nerve injury. Conclusions These results suggest that it is recommended to develop more detailed reporting standards for acupuncture treatment method. In the future, well designed randomized clinical trials which evaluate the effects and safety of acupuncture treatment for upper extremity peripheral nerve injury is highly needed.

맞춤형 암성통증관리 개별교육 및 코칭의 효과 (Effects of a Tailored Education and Coaching Program to Enhance Care of Cancer-related Pain)

  • Purpose: The study was to develop a tailored education and coaching program (TECP) for cancer patients, and to identify the effects of TECP on pain severity, daily living impairment, barriers on pain management, self-efficacy, and pain management satisfaction. Methods: A randomized controlled trial was used. Patients referred to an oncology nurse specialist from oncologists for pain management were randomly assigned to TECP (25) or usual education program (UEP, 22). The intervention was offered by educating the method of taking opioid medication, managing opioid-induced side effects, reducing pain misconceptions and enhancing self-efficacy for communicating with a medical team on cancer pain severity, and pain-related impairment. Patients completed questionnaires before the education program and on the next visit 3~4 weeks later. Data were analyzed by SPSS 19.0 program using percentage, frequency, mean, standard deviation, x2 test and independent t-test. Results: At all levels, pain severity improved significantly in the experimental group-worst pain (8.16 to 3.80, p<.001), average pain (6.16 to 2.52 p=.008), and least pain (3.32 to 0.96, p=.038)-but not in the control group. Pain management satisfaction also showed significant differences (t=2.93, p=.005) between experimental (4.70±0.49) and control (4.17±0.73) groups. Interference with daily living, barriers to managing cancer pain, and self-efficacy for managing pain improved in both groups but there were no significant differences. Conclusion: The findings suggest that TECP should be considered for outpatients who need cancer pain management.

The effects of daily intake timing of almond on the body composition and blood lipid profile of healthy adults

  • BACKGROUND/OBJECTIVES: Timing of almond intake during a day may result differently in the perspectives of body composition and changes of lipid profile. The current study was conducted to compare the effects of daily almond intake as a preload versus as a snack on body composition, blood lipid profile, and oxidative and inflammation indicators among young Korean adults aged 20-39 years old. SUBJECTS/METHODS: Participants were randomly assigned to one of three groups: a pre-meal almond group (PM), a snack almond group (SN) in which participants were instructed to consume 56 g of almonds either as a preload before meals or as a snack between meals, respectively, and a control group (CL) in which participants were provided high-carbohydrate iso-caloric control food. Measurements were performed at baseline, weeks 8 and 16. RESULTS: A total of 169 (M 77 / F 92) out of the 227 participants completed the study between June 2014 and June 2015 (n = 58 for PM; 55 for SN; and 56 for CL). A significant decrease in body fat mass was observed in the PM group at both weeks 8 and 16 compared with the CL. There were significant intervention effects on changes of body fat mass (P = 0.025), body fat percentages (P = 0.019), and visceral fat levels (P < 0.001). Consuming almonds as a daily snack reduced the levels of total cholesterol (P = 0.043) and low-density lipoprotein (LDL) cholesterol (P = 0.011) without changing high-density lipoprotein (HDL) cholesterol compared with the CL. CONCLUSION: Almond consumption as a preload modified body fat percentages, whereas snacking on almonds between meals improved blood lipid profiles. This trial was registered at ClinicalTrials.gov as NCT03014531.

Comparative evaluation of virtual reality distraction and counter-stimulation on dental anxiety and pain perception in children

  • Background: This study evaluated the efficacy of virtual reality (VR) distraction and counter-stimulation (CS) on dental anxiety and pain perception to local anesthesia in children. Methods: A prospective, randomized, single-blinded interventional clinical trial with a parallel design was used. Seventy children 7-11 years old who required local anesthesia (LA) for pulp therapy or tooth extraction were recruited and allocated to two groups with equal distribution based on the intervention. Group CS (n = 35) received CS and Group VR (n = 35) received VR distraction with ANTVR glasses. Anxiety levels (using pulse rate) were evaluated before, during, and after administration of local anesthesia, while pain perception was assessed immediately after the injection. Wong-Baker faces pain-rating scale (WBFPS), visual analog scale (VAS), and Venham's clinical anxiety rating scale (VCARS) were used for pain evaluation. Student's t-test was used to test the mean difference between groups, and repeated measures ANOVA was used to test the mean difference of pulse rates. Results: Significant differences in mean pulse rates were observed in both groups, while children in the VR group had a higher reduction (P < 0.05), and the mean VCARS scores were significant in the VR group (P < 0.05). Mean WBFPS scores showed less pain perception to LA needle prick in the CS group while the same change was observed in the VR group with VAS scores. Conclusions: VR distraction is better than CS for reducing anxiety to injection in children undergoing extraction and pulpectomy.

치매위험노인의 한국형 지중해식 식단의 적용 및 중재평가 (Practicability of Six Weeks of Korean-Style Mediterranean Diet for Elderly Koreans with High Risk for Dementia)

  • The purpose of this study was to verify whether Mediterranean diet, which proved to have a significant effect on preventing dementia for people aged 65 or older, could be well modified to be a Korean-style Mediterranean diet. This study was performed as a randomized-controlled trial for 6 weeks. Functional physical changes, cognitive scores, depression scores and dietary changes were all assessed. The walking speed (P<0.001) and the cognitive scores were statistically improved in only the experimental group (P<0.001), and the depression scores were also significantly improved in only the experimental group (P<0.01). The dietary intake showed a 30% improvement for consuming more than 7 cups per day of vegetables and fruits. When the participants were contacted four months after the end of the study, 90% of them said that the Korean-style Mediterranean diet was feasible and 100% said the nutrition interventions helped them maintain the diet during their daily lives. The results suggest that although the Mediterranean diet can be difficult to apply, any limitations of this healthy diet can be overcome.

머리-목 굽힘 기반 몸통 안정화 운동이 경직성 뇌성마비 아동의 대동작기능과 앉은 자세정렬에 미치는 효과 (The Effects of Cranio-Cervical Flexion based Trunk Stabilization Exercise on Gross Motor Function and Posture Alignment Change in Children with Spastic Cerebral Palsy)

  • Purpose: This study was to evaluate the effect of cranio-cervical flexion based trunk stabilization exercise on gross motor function and posture alignment change in children with spastic cerebral palsy. Design: Randomized Controlled Trial. Methods: Twenty-six children participated in this study. All subjects were randomly assigned to either the Cranio-Cervical Flexion Based Trunk Stabilization Exercise (CCFTS) group (n=13) or the Trunk Stabilization Exercise (TS) group (n=13). In both groups were trained general physical therapy for 10 minutes, in the CCFTS group was trained cranio-cervical flexion based trunk stabilization exercise for 20 minutes and in the TS group was trained trunk stabilization exercise for 20 minutes. The training was provided 2 times a week during 8 weeks. All subjects were measured with the Gross Motor Function Measure (GMFM) and Cranio-Vertebral Angle (CVA) before and after intervention. Results: The results showed that the CCFTS have increased significantly in GMFM (B, C, D and E-dimension) and CVA, and the TS group have increased significantly in GMFM (B, C D and E-dimension). In particular, the CCFTS group improved significantly than TS group in GMFM (B, C and D-dimension) and CVA. Therefore, the cranio-cervical flexion based trunk stabilization exercise improved gross motor function and posture alignment in children with spastic cerebral palsy. Conclusion: These results suggest that cranio-cervical flexion based trunk stabilization exercise is feasible and suitable for individuals with a spastic cerebral palsy and can be used in addition to conventional physical therapy.

후각자극이 노인의 균형에 미치는 영향 (The Effects of Olfactory Stimulation on the Balance Ability of the Elderly)

  • Purpose: The purpose of this study was to investigate the effect of olfactory stimulation on the balance ability of the elderly. Design: Randomized controlled trial. Methods: A total of 20 subjects were randomly assigned to the olfactory stimulation group (2 males and 8 females) and deception olfactory stimulation group (3 males and 7 females). Subjects were subjected to 30 second 3 times olfactory stimulation and deception olfactory stimulation, and the assessment measured pre and post postural distance and postural speed. Results: In the elderly, the olfactory stimulation group was significantly improved the postural distance and postural speed in the eyes closed state than the deception stimulation group (p<.05). Conclusion: As the information is provided through the olfactory stimulation, the postural distance and postural speed are significantly improved. therefore, it is expected that the olfactory stimulation in the elderly will be an effective intervention method to prevent fall.

고압산소치료가 지연성근육통의 통증, 관절운동범위 및 근피로 회복에 미치는 영향 (Effect of Hyperbaric Oxygen Therapy on the Pain, Range of Motion and Muscle Fatigue Recovery of Delayed Onset Muscle Soreness)

  • Background: The purpose of this study was to investigate the effects of HBOT (hyperbaric oxygen therapy) on the pain, ROM (range of motion) and muscle fatigue recovery of DOMS (delayed onset muscle soreness). Design: Randomized Controlled Trial. Methods: Twenty-six subjects who are student in their 20s at a university participated in this study, these subjects were assigned into two groups, a control group (n=12) and an experiment group (n=14). The subjects in experimental group were intervened by HBOT (40 minutes, 1.3 ATA), while ones on control group weren't by any intervention after induced DOMS. Results: First, in the comparison of VAS (visual analog scale), there were significant variations with the period (p<0.001), interaction of period (p<0.05) and group (p<0.05). In the comparison of PPT (pressure pain threshold), there were significant variations with the period (p<0.001) and interaction of period (p<0.05). Second, in the comparison of ROM, there were significant variations with the period (p<0.001), interaction of period (p<0.001) and group (p<0.01). Third, in the comparison of CK (creatine kinase) and LDH (lactate dehydrogenase), there no signigicant variations with all measure variables. Conclusion: The above results indicated that HBOT were effective to decrease the pain and improve the ROM in DOMS. Also the statistical significant variations of blood factors of muscle fatigue were not found in this.

만성 골반통의 한의약 치료 : 무작위 대조군 연구에 대한 문헌 고찰 (Interventions of Korean Medicine for Chronic Pelvic Pain: a Literature Review of Randomized Controlled Trials)

  • Objectives: This study aimed to review randomized controlled trials (RCTs) on the effects of interventions of Korean medicine for chronic pelvic pain (CPP). Methods: We searched eleven electronic databases from inception up to Apr 2018. RCTs evaluating the effects of Korean medicine interventions for CPP were retrieved. Results: 11 RCTs were included and total number of experimental group was 390 cases. A total of 4 types of interventions were used, of which acupuncture (45.5%), electro-acupuncture (18.2%), electro-acupuncture + auricular acupuncture + moxibustion (18.2%) were the most frequently utilized. 關元 (CV4) (100.0%), 會陰 (CV1) (80.0%), 三陰交 (SP6) (80.0%), 陰陵泉 (SP9) (80.0%) were most frequently used acupoint in acupuncture treatment. Conclusions: For evidence-based treatment of korean medicine intervention for CPP, high quality RCTs must be conducted.

Eye Movement Program Consisting of Saccadic Eye Movement and Pursuit Eye Movement Improved Visual Memory in Institutionalized Elderly Person: Randomized controlled pilot study

  • Background: Aging reduces cognitive abilities, including visual memory (VM) and visual discrimination (VD). Since common cortical networks subserve eye movement and attention, voluntary eye movement may improve visual attention. Visual selective attention was major role for memory, and visual memory and visual attention are intimately related. Objective: To identify the improvement in VD and VM, after implementing the eye movement program consisting of saccadic eye movement (SEM) and pursuit eye movement (PEM) in the institutionalized healthy elderly. Design: Randomized controlled trial. Methods: The study involved a sample of 36 participants, and the mean age was 79.03 years (range 76~84 years). They were randomly allocated to the experimental group (n=16) and control group (n=20). Participants in the experimental group performed SEM 5 times per week for 4 weeks: twice daily at the same time in the morning and afternoon. The program was carried out for 3 minutes, and it consisted of SEM and PEM. The target's moving frequency was set at 0.5 Hz. VM and VD at the baseline and post-intervention were measured using Motor-Free Visual Perception test-4 (MFVPT-4). Results: VM significantly improved in the experimental group (p < .01), and significant differences were observed compared to the control group (p < .01). There was no significant change in VD. Conclusion: The eye movement program consisting of SEM and PEM increased VM more than VD. Therefore, eye movement program was feasible interventions for improving VM in institutionalized elderly persons.
