Structural Analysis on the Path of Problem Gambling among College Students - Using Jacob's General Theory of Addiction- (대학생의 문제도박 발달경로 구조분석 -Jacob의 중독의 일반이론 적용-)

  • The growth of legal and illegal gambling industry in Korea has increased the gambling accessibility. Actually, the prevalence of gambling addiction in Korea is significantly higher than those in western countries. The problems related to gambling make negative consequences in the individual, familial and socio-economical aspect. Previous studies showed that the prevalence of problem gambling by college students was much higher than that by adolescence and adult due to the nature of psychosocial developmental stage. The purpose of this study is to explore the path of problem gambling among college students based on the Jacob's general theory of addiction. Participants are 366 male college students who have experienced on gambling behavior. The main results are as follows. First, the problem gambling prevalence of participants is 14%. Second, statistical analysis using structural equation modeling shows the significant pathway among psychological vulnerability, needs to escape and problem gambling. Instead, the physiological vulnerability is not significant on the path to problem gambling. On the basis of these findings, the implications for social welfare prevention and intervention to college students are discussed.

Effects of Oral Care with 0.1% Chlorhexidine and Tantum Solution on Oral Status and Incidence of Oral Pathogens among Intubated ICU Patients: A Pilot Study (0.1% 클로르헥시딘과 탄툼액을 이용한 구강간호가 기관내관을 삽입한 중환자실 환자의 구강상태 및 구강내 세균 발생에 미치는 효과)

  • Purpose: The purpose of this pilot study was to investigate the effect of oral care using 0.1% chlorhexidine and tantum solution on oral status and incidence of oral pathogens among intubated ICU patients. Methods: The study subjects were 26 intubated patients of MICU in a university hospital. Oral care was provided with 0.1% chlorhexidine solution to the experimental group (n=12) and with tantum solution to the control group (n=14), 3 times a day for 6 days. After the intervention, the oral status of each subject was checked using structured method, and oral swab culture was done to count colonized bacterial pathogens. Results: The mean oral status score of experimental group was significantly higher than that of control group (U=42.50, p=.031). The mean incidence of oral pathogens in experimental group was smaller than that of control group, but the difference was not statistically significant. Conclusion: The oral care with 0.1% chlorhexidine solution is more effective than with tantum solution for improving oral status of intubated ICU patients. However, additional studies with larger sample size will be needed to figure out it's effects on the incidence of oral pathogens.

  • Purpose: The purpose of this study was to analyze the published research in the Journal of Muscle and Joint Health and to suggest the future perspectives. Methods: In this study, we reviewed the contents of 195 articles published in this journal from the first issue of the year 1994 to 2007. Results: The majority of subjects were adult, mostly with arthritis. The position of the authors was mostly professor (73.3%). Most of studies did not clearly describe about ethical consideration. The data collection was mostly conducted in the hospital (53.8%) or community (36.9%), with the questionnaire (62.6%). The types of research design were correlational study (36.4%), experimental study (31.8%), survey (18.5%), and qualitative research (4.6%). Physical variables included pain, fatigue, activities of daily living, joint flexibility, muscle strength, etc., psychosocial variables were measured by self-efficacy, depression, quality of life, etc., and cognitive variables by knowledge, education, etc. The most utilized nursing intervention for experimental study were exercise, self-help, and education. Conclusion: Further research is needed to extend the target population research design from multidisciplinary perspectives.

Job Stress and Its Related Factors in South Korean Doctors (일부 의사들의 직무스트레스와 관련 요인)

  • Objective : To investigate the sources, extent and related factors in South Korean doctors. Methods : The study subjects were 934 doctors in Taegu, Kyungpook Province, Korea(540 independent practitioners, 105 employed at hospitals and 289 residents in training). Information concerning job stress was obtained using a 9-item questionnaire. Information regarding related factors such as demographic characteristics(age, sex, marital status), perceptions on the socioeconomic status of doctors and working conditions(work time, on-call days per week) was also obtained by self-administered questionnaire during April and May, 2000. Results : Major sources of job stress included clnical responsibility/judgement factor, patient factor and work loading factor. The job stress score of residents was the highest among three groups. The score was lower in older doctors. The score was low among those who thought doctors' socioeconomic status was not good. The longer the work time, the higher the job stress score was. Multiple regression analysis was conducted to control for the mutual influence of independent variables. In regression analysis, the score of residents was higher than practitioners. Work time and socioeconomic status perception had negative effects on job stress score. Conclusion : The average job stress score of the doctors was high. Age, work type, working conditions and perceptions of socioeconomic status were found to besignificantly related to job stress score. Although the job stress of doctors is somewhat inevitable due to the nature of the doctor's job, control of work time, development of coping tools and other intervention methods are needed to reduce job stress of doctors. Further studies are required to understand the characteristics of iob stress and reduce the job stress of doctors.

An Analysis of Effects of Water Perturbation Exercise on Physiological Cost Index and Gait Ability in Stroke Patients (수중 동요 훈련이 뇌졸중 환자의 생리학적 소비지수와 보행 능력에 미치는 효과 분석)

  • PURPOSE : This study attempts to find the effects of water perturbation exercise performed on stroke patients in their physiological cost index and gait ability tests. METHOD : The subjects were 30 stroke patients, water perturbation exercise group was performed 3 day per week, for 40 minutes a day, for a period of eight weeks. The physiological cost index and gait of all subjects were assessed by using the polar, 6 Minute Walk Test (6MWT), and 10 meter Walk Test(10mWT) at pre training and post training. Paired t-test was used to analyze change before and after intervention in group. Pearson's correlation was used to analyze correlation of all variables. RESULT : Water perturbation exercise group showed increased physiological cost index. Water perturbation exercise increased gait ability, showing a significant difference. Showing the correlation between the relatively high amount between physiological cost index and 6 minutes walking test. CONCLUSION : From the result of the study, we found that water perturbation exercise was effective in improving physiological cost index and gait ability. The patient is considered to be used by itself to involve the treatment and the risk of falling from the lowered state into the treatment method for the intensive treatment of stroke patients to be useful in improving the cardiovascular system and ability to walk. Through underwater training for stroke patients in the future on the basis of this study it is considered to require additional clinical studies on the impact on daily living and quality of life of stroke patients.

Medical Treatment of Laryngopharyngeal Reflux (인후두역류의 약물치료)

  • Otolaryngological manifestations of acid reflux include a wide range of pharyngeal and laryngeal symptoms ; and the constellation of symptoms has been called laryngopharyngeal reflux (LPR). In the absence of definite diagnostic criteria, LPR disease remains a subjective entity. A diagnosis of LPR is usually based on response of symptoms to empirical treatment. Investigative modalities such as pH monitoring and, more recently, impedance studies are generally reserved for treatment failures. LPR usually requires more aggressive and prolonged treatment to achieve regression of both symptoms and laryngeal findings. The suppression of gastric acid and secretion with anti-secretary agents has been the mainstay of medical treatment for patients with acid-related disorders. The suppression of gastric acid secretion achieved with Hz-receptor antagonist $(H_2RA)$ has proved suboptimal for relief of reflux symptoms. The rapid development of tolerance and rebound acid hypersecretion after the with-drawal of $H_2RA$ limit their clinical use. Proton pump inhibitors (PPI) have been proved to be very effective for suppressing intragastric acidity, but the optimal dose and duration is unknown. Current evidence indicates that pharmacologic intervention should include, at a minimum, a 3 month trial of twice daily PPI. Symptoms of LPR improve over 2 months of therapy. The physical findings of LPR resolve more slowly than the symptoms and this continues through out at least 6 months of treatment. For most patients with LPR, twice daily dosing with a PPI is usually recommended for an initial treatment for a period of no less than 6 months treatment, and lifetime treatment may be required.

Development and Validation of a Survey Form for Adverse Events Associated with Acupuncture and Moxibustion (침구 치료 이상반응 설문지 개발과 타당도 평가)

  • Objectives : To develop and validate a survey form for adverse events(AEs) associated with acupuncture and moxibustion treatment. Methods : The 1st version of a survey form consisting of 9 categories was developed and modified based on the analysis of questionnaires from previous studies on AEs. To evaluate content validity of the 1st version, a Delphi survey where experts in acupuncture and moxibustion, and research methodology were invited, was conducted alongside a separate content validity questionnaire. The content validity indices for each item and total scale were calculated as the proportion of the number of experts judging each item or scale valid to the number of total experts. Results : After the Delphi round 1, the content validity of 2 items and total scale was rated as inadequate. The 1st version was modified accordingly, and the 2nd version of the survey form was developed. The content validity of the 2nd version was assessed in the Delphi round 2 and the final version was completed after incorporating the expert comments. The final version consists of 13 questions regarding practitioner/patient information, condition/disease, intervention, treated body parts, safety issue and AE occurrence related with practitioner's behavior or instruments, AEs associated with treatment and emergency actions taken if necessary, time to AE occurrence, outcomes, serious AEs or death, and time to AE disappearance. Conclusions : This newly developed and validated survey form will serve to collect AEs associated with acupuncture and moxibustion treatments in Korea and contribute to establishing the evidence on safety of Korean acupuncture treatment.

Motherhood and Role Confidence in the Immediate Postpartum Period (산욕초기 어머니 됨과 돌봄 자신감)

  • Being a mother is motherhood which means maternal role attainment and has to meet maternal identity and role confidence of caring infant. This study was designed to examine maternal identity as a motherhood and role confidence in the immediate postpartum period, and then explain the correlations of them. Data were collected for 2 months by questionnaires from 64 mothers in the hospital. The scales were to measure motherhood questionnaire which consisted of 11 items and role confidence questionnaire which consisted of 13 items. The analysis of data was done with SPSSWIN program for descriptive statistics and t-test, chi-square, ANOVA, Multiple Regression Analysis. The results were as follows: 1. Mean score of maternal identity of primipara was 55.41 and multipara was 53.81. In this outcome primipara's score was higher than multipara's. 2. Mean score of role confidence of primipara was 44.0 and multipara was 46.81. Multipara's score was higher than primipara's. 3. Correlation between maternal identity and role confidence was r=0.29(P=0.03). This was a interesting result. Because many studies reported that primipara had a lower score of maternal identity and role confidence than multipara's. But this study showed that primipara's score of maternal identity was higher than multipara's. So nurse has to support primipara's identity as a mother, taking advantage of nursing intervention from prenatal to postnatal periods. Also nurse has to offer many opportunities to experience premiparas and multiparas how to care their infants.

Dendrosomal Curcumin Suppresses Metastatic Breast Cancer in Mice by Changing M1/M2 Macrophage Balance in the Tumor Microenvironment

  • Curcumin, a lipid-soluble compound extracted from the plant Curcuma Longa, has been found to exert immunomodulatory effects via macrophages. However, most studies focus on the low bioavailability issue of curcumin by nano and microparticles, and thus the role of macrophages in the anticancer mechanism of curcumin has received little attention so far. We have previously shown the potential biocompatibility, biodegradability and anti-cancer effects of dendrosomal curcumin (DNC). In this study, twenty-seven BALB/c mice were equally divided into control as well as 40 and 80 mg/kg groups of DNC to investigate the involvement of macrophages in the antitumor effects of curcumin in a typical animal model of metastatic breast cancer. At the end of intervention, the tumor volume and weight were significantly reduced in DNC groups compared to control (P<0.05). Histopathological data showed the presence of macrophages in tumor and spleen tissues. Real-time PCR results showed that DNC increased the expression of STAT4 and IL-12 genes in tumor and spleen tissues in comparison with control (P<0.05), referring to the high levels of M1 macrophages. Furthermore treatment with DNC decreased STAT3, IL-10 and arginase I gene expression (P<0.05), indicating low levels of M2 macrophage. The results confirm the role of macrophages in the protective effects of dendrosomal curcumin against metastatic breast cancer in mice.

Anticancer Effects of Curcuma C20-Dialdehyde against Colon and Cervical Cancer Cell Lines

  • Background: Recent attention on chemotherapeutic intervention against cancer has been focused on discovering and developing phytochemicals as anticancer agents with improved efficacy, low drug resistance and toxicity, low cost and limited adverse side effects. In this study, we investigated the effects of Curcuma C20-dialdehyde on growth, apoptosis and cell cycle arrest in colon and cervical cancer cell lines. Materials and Methods: Antiproliferative, apoptosis induction, and cell cycle arrest activities of Curcuma C20-dialdehyde were determined by WST cell proliferation assay, flow cytometric Alexa fluor 488-annexin V/propidium iodide (PI) staining and PI staining, respectively. Results: Curcuma C20 dialdehyde suppressed the proliferation of HCT116, HT29 and HeLa cells, with IC50 values of $65.4{\pm}1.74{\mu}g/ml$, $58.4{\pm}5.20{\mu}g/ml$ and $72.0{\pm}0.03{\mu}g/ml$, respectively, with 72 h exposure. Flow cytometric analysis revealed that percentages of early apoptotic cells increased in a dose-dependent manner upon exposure to Curcuma C20-dialdehyde. Furthermore, exposure to lower concentrations of this compound significantly induced cell cycle arrest at G1 phase for both HCT116 and HT29 cells, while higher concentrations increased sub-G1 populations. However, the concentrations used in this study could not induce cell cycle arrest but rather induced apoptotic cell death in HeLa cells. Conclusions: Our findings suggest that the phytochemical Curcuma C20-dialdehyde may be a potential antineoplastic agent for colon and cervical cancer chemotherapy and/or chemoprevention. Further studies are needed to characterize the drug target or mode of action of the Curcuma C20-dialdehyde as an anticancer agent.
