The Analysis of Risk Factor Management Programs for Patients with Coronary Artery Disease (관상동맥질환자를 위한 위험요인관리 프로그램 분석)

  • Purpose: This study was performed to analyse the trends of risk factor management programs for patients with coronary artery disease. Methods: Using PubMed, 35 intervention studies related to risk factor management programs among randomized controlled trials searched with the key words of coronary artery disease and coronary heart disease. Collected studies were analysed according to the characteristics of studies and participants, method and content of intervention, and outcome indicator and its effects. Results: The mean period of intervention was $28.7{\pm}26.8$ weeks, the mean frequency was $3.0{\pm}2.0$ times per week, and the duration of one session was below 60 minuets in 65.8% of the reviewed studies. The interventions were counselling, exercise, education, and cognitive behavior therapy. Counselling was applied most frequently in previous studies. The outcomes of intervention had been measured with anthropometric, physical, physiological, psychological, behavioral, and cognitive aspects, but the effect of the intervention was inconsistent among the studies. Conclusion: Based on the results of this study, systematic and comprehensive cardiac rehabilitation program consisted of counselling, exercise, and education should be developed and performed for health management and relapse prevention of patients with coronary artery disease.

Analysis of Obesity Intervention Programs in Adolescents: Focused on Endothelium Functions (비만 청소년의 비만 중재 프로그램에 대한 국내외 연구 분석: 혈관내피 기능을 중심으로)

  • Purpose: The purpose of this study was to determine the current status of obesity intervention programs, and its effects on endothelium function in adolescents. Method: This was a descriptive research study, that investigated domestic and international literature 2009-2018. Using databases inside and outside of Korea to search for 'adolescent', 'obese adolescent', 'exercise', 'program', or 'intervention', and 'endothelium function', a total of 14 literature have been selected for 6 domestic research and 8 international research, excluding overlapping, case studies, literature reviews, and unrelated studies. Results: Interventions for enhancing endothelium function were exercise program, diet, vitamin D3 treatment, and Orlistat intake relative to international research, and combined exercise and purple sweet potato intake, in most domestic research. Dependent variables included FMD, RHI, NMD, EPCs, and EMPs, for measuring endothelium function in international research, and PWV, FMD and RHI, in domestic research. Conclusion: Results of this study confirmed that application of obese intervention programs, may improve endothelium function in adolescents. Further studies are required, to develop nursing intervention, that would enhance endothelium function in adolescents.

A Systemic Review of Self-Esteem Programs for Schizophrenic Individuals in South Korea (조현병 환자의 자아존중감을 위한 국내 프로그램 체계적 고찰)

  • Purpose : This study systematically assessed programs self-esteem programs for schizophrenic individuals that have been implemented over the past 10 years in South Korea, with the objective of providing clinical grounds and data for further programs that can be implemented in the Korean environment. Methods : For this study, a data search was conducted from 2012 to 2021, using search terms 'schizophrenia' AND 'self-esteem' among the studies registered in the RISS, KISS, Google Scholar, and National Library of Korea databases. A total of 988 studies were searched, and among them, 40 studies meeting the selection criteria had been taken by reviewing the titles and abstracts. After, 33 studies were excluded according to the exclusion criteria, and finally, the 7 studies were selected. Results : As a result, research into self-esteem programs for schizophrenic individuals has progressed steady, and most studies tend to be non-randomized controlled studies. All studies used the Self-Esteem Scale to evaluate self-esteem. All studies used the Self-Esteem Scale to evaluate self-esteem. The interventions of the selected studies were classified as were social skill programs, cognitive behavioral therapy programs, art therapy programs, and emotional management programs. And among them, the cognitive behavioral therapy programs were the most frequently used (42.8 %). Typically, the majority of the intervention sessions consisted of 11~15 sessions, the frequency of intervention was twice a week, and each session lasted 40~80 minutes were the most (each 57.1 %). In the programs for self-esteem, significant effects were reported for cognitive behavioral and art therapy programs, and there is no significant effect on social skill programs and emotional management programs. Conclusion : Future studies should expand the research scope, both domestically and internationally, and suggest more comprehensive conditions for a literature search by widening the research subject coverage.

A Systemic Review of Community-Based Programs for the Quality of Life of the Elderly (노인의 삶의 질을 위한 지역사회 프로그램에 관한 체계적 고찰)

  • Purpose : The purpose of this study was to identify the clinical effect, assessment, method, and recent trend of the programs by systematically reviewed community-based programs for the elderly's quality of life conducted in the last ten years, and to present comprehensive basic data for occupational therapists who want to apply community-based programs improving quality of life. Methods : This study identified 811 studies from among those registered in the RISS, KISS, DBpia, NDSL, and KCI databases from January to September 2020, using "elderly," "community elderly," "65 aged," "quality of life," "occupational therapy," "occupational therapy program," "intervention," and "intervention program" as keywords. From these results, 11 documents were selected. Results : It was found that studies of occupational therapy programs for the quality of life of the elderly have been continuously conducted since 2010, and most of them were non-random control studies. The quality of life evaluation tool used most frequently was SF-36, and WHOQOL-BREF and SF-8 were also used. The selected programs were classified as were exercise programs and complex programs. For most of the exercise programs, interventions were carried out for 12 weeks, three times a week for 30~60 minutes. In the case of the complex programs, the duration of interventions was nine weeks, and the frequency and time per session varied. All of the exercise programs showed significant effects. Among the complex programs, two studies showed significant effects, but the remaining one study did not. Conclusion : Through this study, the types and effects of occupational therapy programs for the quality of life of the elderly were examined. Hereafter, studies of the development and to verify the effects of these programs should be actively carried out.

Analysis of Worksit Health Promotion Programs (우리나라 사업장의 건강증진 프로그램 유형 분석 - 사업장 건강증진운동 우수사례집을 중심으로 -)

  • Purpose: The purpose of this study is to identify the types of worksite health promotion programs. Method: Data were collected from the excellent 35 cases chosen at the contest for worksite health promotion held by Korea Occupational Safety and Health Agency. Result: Out of all the health promotion programs, the exercise program recorded 35.0%, the nutrition program 29.4%, the smoking cessation program 28.0%, and the alcohol reduction program 7.6%. The major element of worksite health promotion programs were awareness raising intervention. Behavior change intervention and supportive environment intervention occupied a small portion of the health promotion programs. Evaluation of health promotion programs was made mainly by indicators of health behavior change and clinical symptom. Yet economical indicator was not used at all. Conclusion: Use of various evaluation indicator and development of various interventions including behavior change and supply of supportive environment are required to encourage worksite health promotion program.

A Survey on Visiting Nurses' Management for Elders with Cognitive Impairment Living in a Community: Focused on Health Centers in a Metropolitan and Medium-sized Cities (방문간호사의 재가노인 인지기능장애 관리에 대한 실태조사: 광역시보건소와 중소도시보건소를 중심으로)

  • Purpose: The purpose of this study was to investigate the status and characteristics of visiting nurses' management for elders with cognitive impairment living in a community focused on health centers in a metropolitan city and five medium-sized cities. Methods: Data were collected from 47 visiting nurses working in a metropolitan city and 47 visiting nurses working in five medium-sized cities from November to December 2012. Results: There were no statistically significant differences in knowledge, attitudes, and nursing behaviors of cognitive impairment between two groups (F=2.13, p=.148; F=3.64, p=.060; F=0.28, p=.595). Among the elders referred to a physician in a metropolitan city by visiting nurses, 42.4% were diagnosed as mild cognitive impairment and 15.2% were diagnosed as severe dementia. The major intervention programs which visiting nurses currently applied for elders were medication and exercise intervention programs, and the intervention programs which they would want to apply in the future were playing, music and recall intervention programs. Conclusion: The cognitive impairment screening test can be done effectively by visiting nurses. This study also suggests to develop various kinds of intervention programs to improve cognitive function for elders living in a community.

The Literature Review on Life style Intervention Program for the Prevention of Cardiovascular Disease (심뇌혈관질환 예방을 위한 생활습관 개선 프로그램에 관한 문헌분석 연구)

  • This study is a literature analysis study that analyzed program intervention research papers to understand the characteristics of lifestyle intervention programs to prevent cardiovascular disease. The final 18 papers were selected by applying search terms such as "cardio-cerebrovascular disease" and "intervention" through electronic databases such as Pubmed. As for the criteria for selecting program participants in the study, physiological criteria were mainly used, and there were many programs that mediated physical activity and eating habits. Education was the most widely used intervention type, and there were many studies that combined and applied several interventions. During the intervention period, long-term interventions averaged more than 22 weeks, and biophysical indicators were most frequently used as effect indicators for measuring the intervention effect. Through this study, it is suggested that environmental and social support is needed to develop intervention programs considering the interrelationship of various lifestyle habits and maintain healthy lifestyle habits.

A Systematic Review on Oral Health Care Programs for the Elderly in Korea (2009~2020)

  • Background: Various oral health management programs in Korea affect the oral health improvement in the elderly Several studies have been conducted to date; however, those studies have not shown uniform results due to the differences in research methods or designs. Hence, this study aimed to review the overall research trends of the reported oral health care programs for the elderly in Korea, verify their effects, and clarify them based on the systematic literature review. Methods: The literature search selected intervention studies that applied the oral health care program for the elderly in Korea from 2001 to 2020. Following the COre, Standard, and Ideal (COSI) models presented by the US National Library of Medicine, we selected databases including Korean studies Information Service System (KISS), ScienceOn, Research Information Sharing Service (RISS), DBpia, PubMed, and Google Scholar. Of the 1,335 studies searched using keywords, titles, and abstracts, 21 were finally selected based on primary and secondary exclusion criteria. Results: The most frequent intervention period was 4 weeks, and the number of interventions varied between 2 and 90 times. As for the type of intervention, 14 studies that conducted both theory and practice were the most frequent. Significant differences in the clinical indicators, such as calculus, halitosis, salivation rate, swallowing function, and dry mouth, were found in most oral health care programs. Conclusion: Based on the results of this study, the intervention program needs further verification using multiple indicators in future studies. In addition, a study extending the intervention period and the number of samples is considered necessary for verifying continuous effectiveness of the intervention program.

Effectiveness of Obesity management programs: systematic review and meta-analysis (메타분석을 이용한 비만관리 프로그램의 효과분석)

  • As overweight and obese people have increased, obesity management programs have generated much concern in Korea. Many types of obesity management programs were operated, aimed at reducing weight, BMI, body fat percentage and so on. This study was conducted to review the features of studies in obesity management programs and systematically assess the effects which were published from 1995 to 2006 in Korea. Databases were systematically searched for published data in Korea. It were KISS (Korean studies information services system) and KERIS (Korean Education research information system), which were major literature search systems for all academic fields in Korea. Total 114 studies were initially identified, of which 21 satisfied our inclusion data. The effects of obesity management programs were assessed on the results in weight, BMI (body mass index), HDL (high density lipids), body fat percentage, self-efficacy. Effect size estimated on the equation of M1 - M2 / Sp, where M1 was mean of the experimental group, M2 was the mean of control group, and Sp was the pooled standard deviation. Magnitude of effect size was interpreted by using Cohen's definition. Cohen described small, medium, and large effect sizes as 0.2, 0.5, and 0.8 respectively. The studies about obesity management programs were the most published in 2005(26.1%). Obesity management programs were popularly operated as the type of 12 weeks intervention, exercise intervention(52.4%), quasi-experimental study(85.7%). Ten studies reported significant effects on weight, and nine studies reported significant effects on BMI and HDL. Only five and three studies reported significant effects on body fat percentage and self-efficacy respectively. The effects of each outcome were generally significant when the studies included these elements, intervention over 10 weeks, evaluation over 3 times, a comprehensive program (exercise, nutrition education, behavior modification) and reinforcing factors (self-monitoring, group discussion, one's goal setting, and etc.). Effective obesity management programs should contain these essential elements and objectives of obesity management programs must be set out evident at the beginning. The participants should be registered, educated and evaluated by continuous obesity management programs.

Changes in HbA1c According to Education & Consultation Intervention of Type 2 Diabetes Workers (교육 및 상담 중재에 따른 제2형 당뇨병 근로자의 당화혈색소 변화)

  • Objectives: The purpose of this study was to investigate changes in HbA1c through an education and consultation intervention with diabetic workers. Methods: The participants were 65 workers with Type 2 diabetes. The data were collected from May to October 2014 using questionnaires. The intervention program included six monthly sessions on exercise, dietary habits, stress management, and diabetes knowledge. Changes in HbA1c were evaluated. The data were analyzed using descriptive statistics and paired t-tests with SPSS WIN23.0. Results: The mean of HbA1c was 7.67% before intervention and 7.28% after intervention, and this difference was statistically significant (P<0.01).The mean level of HbA1c was significantly different depending on job position, working hours, work duration, and smoking habit. Conclusion: In conclusion, these findings support the usefulness of intervention programs in reducing HbA1c. Therefore, appropriate intervention programs involving exercise, dietary habits, stress management and diabetes knowledge should be developed and provided to diabetic workers.
