Metabolic Changes in Patients with Parkinson's Disease after Stereotactic Neurosurgery by Follow-up 1H MR Spectroscopy

  • Authors investigated neuronal changes of local cellular metabolism in the cerebral lesions of Parkinsonian symptomatic side between before and after stereotactic neurosurgery by follow-up 1H magnetic resonance spectroscopy (MRS). Patients with Parkinson's disease (PD) (n = 15) and age-matched normal controls (n = 15) underwen MRS examinations using a stimulated echo acquisition mode (STEAM) pulse sequence that provided 2${\times}$2${\times}$2 ㎤ (8ml) volume of interest in the regions of substantia nigra, thalamus, and lentiform nucleus. Spectral parameters were 20 ms TE, 2000 ms TR, 128 averages,2500 Hz spectral width, and 2048 data points. Raw data were processed by the SAGE data analysis package (GE Medical Systems). Peak areas of N-acetylaspartate (NAA), creatine (Cr), choline-containing compounds (Cho), inositols (Ins), and the sum (Glx) of glutamate and GABA were calculated by means of fitting the spectrum to a summation of Lorentzian curves using Marquardt algorithm. After blindly processed, we evaluated neuronal alterations of observable metabolite ratios between before and after stereotactic neurosurgery using Pearson product-moment analysis (SPSS, Ver. 6.0). A significant reduction of NAA/Cho ratio was observed in the cerebral lesion in substantia nigra of PD patient related to the symptomatic side after neurosurgery (P : 0.03). In thalamus, NAA/Cho ratio was also significantly decreased in the cerebral lesion including the electrode-surgical region (P : 0.03). A significant reduction of NAA/Cho ratio in lentiform nucleus was not oberved, but tended toward significant reduction after neurosurgery (P = 0.08). In particular, remarkable lactate signal was noted from the surgical thalamic lesions of 6 among 8 patients and internal segments of globus pallidus of 6 among 7 patients, respectively. Significant metabolic alterations of NAA/Cho ratio might reflect functional changes of neuropathological processes in the lesion of substantia nigra, thalamus, and lentiform nucleus, and could be a valuable finding fur evaluation of Parkinson's disease after neurosurgery. Increase of lactate signals, being remarkable in surgical lesions, could be consistent with a common consequence of neurosurgical necrosis. Thus, IH MRS could be a useful modality to evaluate the diagnostic and prognostic implications fur Parkinsons disease after functional neurosurgery.

  • In recent high speed rail way, the operating speed of train is enhanced and the introduction of EMU train vehicles is increased. In addition, as expected the demand of the concrete slab track and the trend of design cross-section reduction, the clear behavior of evaluation of internal slab layer is demanded about the variation of design load and speed. The purpose of this study is to evaluate and identify the mechanical behavior pattern of concrete slab track and track-road bed with the variation of axle load and train speed. To this end, the behavior of TCL and HSB was evaluated in according to the variation of axle load and speed. And the analysis results and the data measured TCL strain sensor, which was embedded in TCL slab under installation on Honam high speed railway, was analyzed. The analysis result shows that the strain are increasing in according to the speed-up of train, and line regression was obtained from measured data. Analysis data of the state of bonding condition of slab layer and measured data was analyzed. It is conducted that the TCL layer stress of HEMU 430X, which of axle load, is lighter was similar to the stress of KTX-Honam, the standard deviation of measured stress is dramatically increased.

A Study on the Diffuser Design of Exhaust Pipes for the Infra-Red Signature Reduction of Naval Ship (함정 적외선 신호 감소를 위한 폐기관의 디퓨져 설계에 관한 연구)

  • In modern naval ships, an infrared signature suppression (IRSS) system is used to reduce the metal surface temperature of the heated exhaust pipe and high-temperature exhaust gases generated from the propulsion system. Generally, the IRSS systems used in Korean naval ships consist of an eductor, mixing tube, and diffuser. The diffuser reduces the temperature of the metal surface by creating an air film due to a pressure difference between the internal gas and the external air. In this study, design variables were selected by analyzing the shapes of a diffuser designed by an advanced overseas engineering company. The characteristics of the design variables that affect the performance of the IRSS were investigated through the Taguchi experimental method. A heat flow analysis technique for IRSS systems established in previous studies was used analyze the performance of the diffuser. The performance evaluation was based on the area-averaged value of the metal surface temperature and exhaust gas temperature at the outlet of the diffuser, which are directly related to the intensity of the infrared signature. The results show that the temperature of the exhaust gas was significantly affected by changes in the diameter of the diffuser outlet, and the temperature of the diffuser's metal surface was significantly affected by changes in the number of diffuser rings.

Top Management's Human and Social Capital Effect on Governmental R&D Support System Utilization and Success (최고경영진의 인적 및 사회적 자본이 정부의 R&D 지원제도 활용과 초기 성과에 미치는 영향)

  • Purpose - This study attempts to analyze whether or not there are characteristics among the top management of companies that promote corporate performance at venture companies. It investigates the characteristics of the human and social capital that are inherent in top management at a venture company and conducts an empirical analysis of hypotheses examining if these characteristics will affect utilization of the governmental R&D support system as well as affect the firm's initial success. Research design, data, and methodology - This study conducted theoretical and empirical research together to accomplish the goal of the study. The pilot study researched human capital and social capital as the independent variables; the governmental R&D support system as the parameter; and, the initial success as the dependent variable. The empirical study carried out research on the model, establishment of hypotheses, and the statistical treatment. A survey was conducted targeting top management of high-tech venture companies in Daedeok Innopolis; 500 questionnaires were distributed; and, 222 were collected. Results - The human and social capital inherent in top management at venture companies in the early stages of their existence become good evaluation data for those who are invested in similar resources. If top management includes strong human and social capital, access to external resources will be easier; these will have a positive influence on the selection of overnmental support systems; and, this proper support will also have a positive influence on the initial success of the venture company. The results revealed the following. First, it was found that when the educational level and functional background, (the top management human capital), are the output function, top management human capital had a significant influence on selection of governmental R&D support funds. Second, it was found that the internal social capital and external social capital, (the top management social capital), had a significant influence on selection of governmental R&D support tasks. Third, it was found that selection of the governmental R&D support tasks at the start of the venture company had a positive influence on the corporate financial performance such as sales, business profits, and the increase in workers; and, had a significant influence on nonfinancial performance such as market share, competitive position, product competitiveness, and the future product development. Conclusions - Selection of the governmental R&D support system is not recognized as part of the direct sales of a venture company in its early stages, but as it can reduce costs for technical development and helps significantly in creating test products and mass production, it has a positive influence on the company's financial performance and nonfinancial performance as a result. Therefore, companies should take great efforts to frequently be selected as a candidate in the governmental R&D support system, as it can help facilitate R&D that requires extensive funds. As a result, companies can expect effects such as job creation and patent applications and they can advance future product sales.

Durability Assessment of High Strength Concrete with High Volume Mineral Admixture (다량의 광물질 혼화재를 사용한 고강도 콘크리트의 내구성 평가)

  • The purpose of this study was to assess the durability of high-strength concrete with high volume mineral admixture (HVMAC) derived from previous studies within ternary blended concrete (TBC) and normal concrete (NC). Four durability evaluation types such as chloride penetration resistance, freezing and thawing resistance, carbonation resistance in two pre-treatment conditions, and sulfuric acid and sulfate resistance using 5% sulfuric acid ($H_2SO_4$), 10% sodium sulfate ($Na_2SO_4$), and 10% magnesium sulfate ($MgSO_4$) solution were selected and performed in this study. HVMAC showed the excellent chloride penetration resistance in any age and the freezing and thawing durability close to 100%. In addition, HVMAC affected more reduction in carbonation resistance than TBC. When the curing time was increased, to create a concrete internal organization densely improved resistance to carbonation. HVMAC also showed the most superior in sulfuric acid and sulfate resistance. As the reduction of calcium hydroxide and $C_3A$ to apply a large amount of admixture reduced the swelling and cracking of concrete, the strength reduction and mass change of concrete was found to be small indicated.

Evaluation of the Probability of Failure in Rock Slope Using Fuzzy Reliability Analysis (퍼지신뢰도(fuzzy reliability) 해석기법을 이용한 암반사면의 파괴확률 산정)

  • Uncertainties are pervasive in engineering geological problems. Therefore, the presence of uncertainties and their significance in analysis and design of slopes have been recognized. Since the uncertainties cannot be taken into account by the conventional deterministic approaches in slope stability analysis, the probabilistic analysis has been considered as the primary tool for representing uncertainties in mathematical models. However, some uncertainties are caused by incomplete information due to lack of information, and those uncertainties cannot be handled appropriately by the probabilistic approach. For those uncertainties, the theory of fuzzy sets is more appropriate. Therefore, in this study, fuzzy reliability analysis has been proposed in order to deal with the uncertainties which cannot be quantified in the probabilistic analysis due to the limited information. For the practical example, a slope is selected in this study and both the probabilistic analysis and the fuzzy reliability analysis have been carried out for planar failure. In the fuzzy reliability analysis, the dip angle and internal friction angle of discontinuity are considered as triangular fuzzy numbers since the random properties of the variables cannot be obtained completely under the conditions of limited information. In the study, the fuzzy reliability index and the probabilities of failure are evaluated from fuzzy arithmetic and compared to those from the probabilistic approach using Monte Carlo simulation and point estimate method. The analysis results show that the fuzzy reliability analysis is more appropriate for the condition that the uncertainties arise due to incomplete information.

Uncertainty evaluation for the determination of creatinine in urine by LC-MS/MS (LC-MS/MS를 이용한 소변 중 크레아티닌 분석의 측정불확도 평가)

  • The objective of the study was to estimate the measurement uncertainty associated with determination of creatinine (Cr) in urine samples by liquid chromatography-tandem mass spectrometry (LC-MS/MS). Centrifuged urine samples (10 ${\mu}L$) were diluted with 390 ${\mu}L$ of distilled water. To 20 ${\mu}L$ aliquots of diluted urine samples, 30 ${\mu}L$ of internal standard solution (Cr-$d_3$, 5 ${\mu}g/mL$) and 10 ${\mu}L$ of acetonitrile were added and filtered. The samples (1 ${\mu}L$) were introduced into LC-MS/MS with no further pretreatment. Cr was separated on a multi-mode ODS column (Scherzo SM-C18, 75 ${\times}$ 2.0 mm I.D., 3 ${\mu}m$) and quantified by LC-MS/MS operating in MRM mode (Cr, m/z 114.0${\rightarrow}$ 86.0; Cr-$d_3$, m/z 117.0${\rightarrow}$ 89.1). The four factors that contribute uncertainty to the final result were extracted and evaluated. The principal factors of contribution to combined standard uncertainty were sample dilution, calibration curve and repeatability, while the preparation of standard solution was only a minor factor. Relative extended uncertainty of the measured concentration was 14.2% in a real urine sample.

The metabolic syndrome and body composition in childhood cancer survivors

  • Purpose: Long-term survivors of childhood cancer appear to have an increased risk for the metabolic syndrome, subsequent type 2 diabetes and cardiovascular disease in adulthood compared to healthy children. The purpose of this study was to investigate the frequency of the metabolic syndrome and associated factors in childhood cancer survivors at a single center in Korea. Methods: We performed a retrospective review of medical records of 98 childhood cancer survivors who were diagnosed and completed anticancer treatment at Samsung Medical Center, Seoul, Korea between Jan. 1996 and Dec. 2007. Parameters of metabolic syndrome were evaluated between Jan. 2008 and Dec. 2009. Clinical and biochemical findings including body fat percentage were analyzed. Results: A total of 19 (19.4%) patients had the metabolic syndrome. The median body fat percentage was 31.5%. The body mass index and waist circumference were positively correlated with the cranial irradiation dose (r=0.38, P<0.001 and r=0.44, P<0.00, respectively). Sixty-one (62.2%) patients had at least one abnormal lipid value. The triglyceride showed significant positive correlation with the body fat percentage (r=0.26, P=0.03). The high density lipoprotein cholesterol showed significant negative correlation with the percent body fat (r=- 0.26, P=0.03). Conclusion: Childhood cancer survivors should have thorough metabolic evaluation including measurement of body fat percentage even if they are not obese. A better understanding of the determinants of the metabolic syndrome during adolescence might provide preventive interventions for improving health outcomes in adulthood.

Clinical Evaluation of Pleural Fluid (늑막강내 저류액의 임상적 고찰)

  • Author made a clinical study of 248 cases of pleural effusion patients who were diagnosed and treated at departments of chest surgery and internal medicine, Pusan National University Hospital, during the period from Jan. 1983 to Dec. 1985. The age distribution ranged from 1 to 76 years old and the ratio of male to female was 1.38:1. The cardinal symptoms were chest pain[69.4%], dyspnea[66.1%], cough[57.7%], fever[37.1%], sputum[26.2%], general malais[13.7%] and cyanosis[1.6%] in this order. The causes of pleural effusion were pulmonary tuberculosis[42.4%], pneumonia[23.0%], malignancy[16.5%], congestive heart failure[9.3%], liver cirrhosis[2.8%] and nephrosis[2.0%] in this order. The protein in the pleural effusions was 1.61*0.90[mean*SD] gm% in transudate and 5.05*1.10[Mean*SD] gm% in exudate. In 34 cases[89.5%]out of 38 transudates, the protein was under 3 gm% and in 201 cases [95.7%] out of 210 exudates, the protein was over 3 gm%. The protein ratio of pleural effusion to serum was 0.2650.11[Mean LSD] in transudates and 0.73*0.12[Mean LSD] in exudate. The ratio under 0.5 was in 36 cases[94.8%] out of 38 transudates and over 0.5 was in 206 cases[98.1%] out of 210 exudates. The LDH in the pleural effusion was 114.7550.3[mean*SD] units / ml in transudate and 627.05325.9[mean*SD] units / ml in exudate. The LDH less than 200 units / ml was in 36 cases[94.6%] out of 38 transudates and more than 200 units / ml was in 199 cases[94.7%] out of 210 exudates. The LDH ratio of pleural effusion to serum was 0.34k 0.11[mean*SD] in transudate and 1.15*1.12[mean*SD] in exudate. The LDH ratio of pleural effusion to serum was less than 0.6 in 36 cases[94.8%]out of 38 transudates and more than 0.6 in 200 cases[95.2%] out of 210 exudates. Etiologic organisms were confirmed in 78 cases[48.1%] among the requested 162 cases. In the 78 cases of etiologic organisms, staphylococcus was 33 cases[20.3%], streptococcus 24 cases[14.8%], Klebsiella pneumonia 7 cases[4.3%], pseudomonas 6 cases[3.7%], E. coli[3.1%], enterobacter 3 cases[1.9%]. 43 patient of pleural effusion from malignancy were undergone three or more thoracenteses. In 13 cases[31.7%], three specimen were negative and in 7 cases[17.1%], three specimens were positive for malignancy. In the remaining of 21 cases[51.2%], malignant cells were found in one or more of the specimens but not in all. Methods of treatment of pleural effusion by closed thoracotomy was 188 cases[75.8%], thoracentesis 27 cases[10.9%], decortication 16 cases[6.5%], thoracoplasty 6 cases[2.4%] and decortication with thoracoplasty 3 cases[1.2%].

Quality Characteristics of White Pan Bread Containing HPMC, MC, and Sodium Alginate (HPMC, MC, sodium alginate 등의 검류가 식빵의 품질 특성에 미치는 영향)

  • This study was carried out to evaluate the quality characteristics of white pan breads containing 0.5% of gums, including HPMC, MC and SA. Moisture content, water activity, cooling loss, bread volume, rheological, and sensory evaluations were performed to examine the bread quality characteristics. Moisture content and water activity were highest in the bread with added HPMC. HPMC showed the smallest cooling loss among the breads compared to the other added gums. The bread with added SA had the thelargest volume at $2,560{\pm}24$ mL. In terms of rheological properties, the hardness of the bread containing HPMC was lowest and the springness of the bread with added SA was the highest. In sensory evaluations, the bread containing HPMC was evaluated as the most preferred product by acquiring the highest scores in internal and external evaluations. Consequently, the bread containing 0.5% SA showed better volume and springiness values. However, the bread containing 0.5% HPMC showed greater moisture content, greater water activity, lower cooling loss, and better sensory evaluation scores. Based on the overall results, HPMC was considered to be the most effective hydrocolloid to increase bread quality.
