PNF를 이용한 하지근력 강화운동이 뇌졸중 환자의 보행 및 균형 능력에 미치는 영향 -단일사례연구- (Effect of a Lower Extremity Strengthening Exercise Using Proprioceptive Neuromuscular Facilitation on the Gait and Balance Ability of a Stroke Patient -A Single Case Study-)

  • Purpose: This study seeks to determine the effect of a lower extremity strengthening exercise that uses proprioceptive neuromuscular facilitation (PNF) on the gait and balance ability of a stroke patient. Methods: In this case study, a lower extremity strengthening exercise that used PNF was performed by the subject for 30 minutes 4 times per week for a 4-week period alongside typical rehabilitation therapy. The lower extremity pattern used flexion-abduction-internal rotation with knee flexion, extension-abduction-internal rotation, bilateral leg extension pattern. The 10 m walk test and 6 minute walk test were used to assess gait ability. The Berg balance scale was used to assess balance ability. Results: After the intervention, the time for the 10 m walk test decreased by 5.72 sec, the distance for the 6 minute walk test decreased by 20 meters, and the score on the Berg balance scale improved by 7 points, which indicates the effectiveness of this therapy for stroke patients. Conclusion: The results of this study indicate that a lower extremity strengthening exercise using PNF can improve the gait ability and balance ability of stroke patients.

슬개대퇴동통증후가 성인 여성의 드롭랜딩 시 하지 주요관절의 운동역학적 변화에 미치는 영향 (Effects of Kinematics and Kinetics of the Lower Extremities Joint during Drop Landing in Adult Women with Patellofemoral Pain Syndrome)

  • Objective: This study investigated the different in isokinetic peak strength of the knee joint, and kinetics and kinematics in drop landing pattern of lower limb between the patellofemoral pain syndrome (PFPS) patients and normal. Method: 30 adult females were divided into the PFPS (age: 23.13±2.77 yrs; height: 160.97±3.79 cm, weight: 51.19±4.86 kg) and normal group (age: 22.80±2.54 yrs, height: 164.40±5.77 cm, weight: 56.14±8.16 kg), with 15 subjects in each group. To examine the knee isokinetic peak strength, kinematics and kinetics in peak vertical ground reaction force during drop landing. Results: The knee peak torque (Nm) and relative strength (%) were significantly weaker PFPS group than normal group. In addition, PFPS group had significantly greater hip flexion angle (°) than normal group. Moreover, normal group had significantly greater moment of hip abduction, hip internal rotation, and left ankle eversion than PFPS group, and PFPS group had significantly greater moment of knee internal rotation. Finally, there was significant differences between the groups at anteroposterior center of pressure. Conclusion: The PFPS patients had weakened knee strength, and which can result in an unstable landing pattern and cause of more stress in the knee joints despite to effort of reduce vertical ground reaction force.

달리기 속도에 따른 인체 안정성의 생체역학적 분석: 리아프노프 지수와 변이계수 방법의 비교 분석 (Biomechanical Analysis of Human Stability According to Running Speed: A Comparative Analysis of Lyapunov Exponent and Coefficient of Variation Methods)

  • Objective: The purpose of this study was to examine the effects of increasing running speed on human stability by comparing the Lyapunov Exponent (LyE) and Coefficient of Variation (CV) methods, with the goal of identifying key variables and uncovering new insights. Method: Fourteen adult males (age: 24.7 ± 6.4 yrs, height: 176.9 ± 4.6 cm, weight: 74.7 ± 10.9 kg) participated in this study. Results: In the CV method, significant differences were observed in ankle (flexion-inversion/eversion; p < .05) and hip joint (internal-external rotation; p < .05) movements, while the center of mass (COM) variable in the coronal axis movements showed a significant difference at the p < .001 level. In the LyE method, statistical differences were observed at the p < .05 level in knee (flexion-extension), hip joint (internal-external rotation) movements, and COM across all three directions (sagittal, coronal, and transverse axis). Conclusion: Our results revealed that the stability of the human body is affected at faster running speeds. The movement of the COM and ankle joint were identified as the most critical factors influencing stability. This suggests that LyE, a nonlinear time series analysis, should be actively introduced to better understand human stabilization strategies.

학제간 융합연구를 위한 테니스 백핸드 스트로크 동작의 운동역학적 비교 분석 (Kinetic comparative analysis of tennis backhand stroke for interdisciplinary convergence research)

  • 본 연구는 테니스 한손과 양손 백핸드 스트로크 동작에서 하지관절 움직임의 차이를 확인하여 유형별 특성을 밝히는데 그 목적이 있으며 그 결과는 다음과 같다. 볼의 속도를 결정하는 중요한 요인인 라켓헤드의 합성 속도는 양손 백핸드 스트로크 동작이 한손보다 빠른 속도를 나타냈다. 양손 백핸드 스트로크는 하체의 움직임을 최소화시키고 몸통 회전을 통한 스트로크를 하는 반면 한손 백핸드 스트로크는 몸통을 이용한 스트로크를 하기 보다는 공을 쫓아가듯이 스트로크 하는 것으로 나타났다. 슬관절의 신전모멘트는 한손 백핸드 스트로크가 큰 것으로 나타났지만, 내번모멘트와 회내모멘트 그리고 굴곡모멘트는 양손 스트로크에서 크게 나타났다. 고관절의 경우 신전, 내번, 회내 모멘트가 양손 백핸드 스트로크가 한손 보다 모두 큰 것으로 나타났는데 특히 내번모멘트의 경우 큰 차이를 나타난 반면, 외번모멘트는 한손 백핸드 스트로크가 큰 것으로 나타났다.

한국인의 고관절 회전 가동 범위의 측정 및 비교 고찰 (A Comparative Study and Measurement of R.O.M. in the Hip joint of Korean's)

  • The test for internal and external rotation of hip joint, was donet from September 2 to 9, 2002, with the participation of 50 male/female university students who are in a normal health condition. Results are : 1. When applying the external rotation of 45 degrees angle ; 1) In a sitting position, for male it results in an increase of 5 degrees on right and left sides each. 2) In a sitting position, for female there is no change on both right and left sides. 3) In a supine position, for both male and female it results in an increase of 5 to 10 degrees on right and left sides each. 4) In a prone position, for male it results in an increase of 5 degrees on the left side only. 5) In a prone position, for female it results in an increase of 5 to 10 degrees on right and left sides each. 2. When applying the internal rotation of 45 degrees angle ; 1) In a sitting position, for male it results in a decrease of 5 degrees on right and left sides each. 2) In a sitting position, for female there is no change on both right and left sides. 3) In a supine and prone position, there is no change for male. 4) In a supine position, for female it results in an increase of 5 to 10 degrees on right and left sides each. 5) In a prone position, for female it results in an increase of 5 degrees on right and left sides each.

Comparative clinical and radiologic evaluation between patients undergoing standard reversed shoulder arthroplasty or bony increased offset

  • Background: Modifications of the medialized design of Grammont-type reverse shoulder arthroplasty (RSA) using a bony increased offset (BIO-RSA) has shown better clinical results and fewer complications. The aim of this study is to compare the clinical results, complications, and radiological outcomes between patients undergoing standard RSA and BIO-RSA. Methods: A retrospective review was performed of 42 RSA procedures (22 standard RSA and 20 BIO-RSA). With a minimum of 1 year of follow-up, range of motion (ROM), Constant shoulder score (CSS), visual analog scale (VAS), and subjective shoulder score (SSS) were compared. Radiographs and computed tomography (CT) scan were examined for scapular notching, glenoid and humeral fixation, and graft healing. Results: At a mean follow-up of 27.6 months (range, 12-48 months), a significant difference was found for active-internal rotation (P=0.038) and for passive-external rotation (P=0.013), with better results in BIO-RSA. No other differences were found in ROM, CSS (P=0.884), VAS score, and SSS. Graft healing and viability were verified in all patients with CT scan (n=34). The notching rate was 28% in the standard RSA group and 33% in the BIO-RSA group, but the standard RSA had more severe notching (grade 2) than BIO-RSA (P=0.039). No other significative differences were found in glenoid and humeral fixation. Conclusions: Bone-graft lateralization is associated with better internal and external rotation and with less severe scapular notching compared to the standard RSA. Integration of the bone graft occurs effectively, with no relevant changes observed on radiographic evaluation. Level of evidence: III.

Glenohumeral versus subacromial steroid injections for impingement syndrome with mild stiffness: a randomized controlled trial

  • Background: The subacromial (SA) space is a commonly used injection site for treatment of impingement syndrome. For shoulder stiffness, glenohumeral (GH) injections are commonly performed. However, in cases of impingement syndrome with mild shoulder stiffness, the optimal site of steroid injection has yet to be identified. Methods: This prospective, randomized study compared the short-term outcomes of ultrasound-guided GH and SA steroid injections in patients who were diagnosed with impingement syndrome and mild stiffness. Each group comprised 24 patients who received either a GH or SA injection of 40 mg of triamcinolone. Range of motion and clinical scores were assessed before and 3, 7, and 13 weeks after the injection. Results: GH and SA injections significantly improved the range of motion and clinical scores after 13 weeks of follow-up. Notably, targeting the GH joint resulted in an earlier gain of forward elevation, external rotation, and internal rotation in 3 weeks (P<0.001, P=0.012, and P=0.002, respectively) and of internal rotation and a Constant-Murley score in 7 weeks (P<0.001 and P=0.046). Subsequent measurements were similar between the groups and showed a steady improvement in all ranges of motion and clinical scores. Conclusions: GH injections may be more favorable than SA injections for treatment of impingement syndrome with mild stiffness, especially in improving the range of motion in the early period. However, the procedures showed similar outcomes after 3 months. Level of evidence: I.

Reverse total shoulder replacement for patients with "weight-bearing" shoulders

  • Background: Reverse total shoulder arthroplasty (rTSA) has gained popularity in recent years and is indicated for a wide variety of shoulder pathologies. However, use of rTSA in patients with "weight-bearing" shoulders that support wheelchair use or crutches has higher risk. The aim of this study was to assess the results of rTSA in such patients. Methods: Between 2005 and 2014, 24 patients (30 shoulders) with weight-bearing shoulders were treated with rTSA at our unit. Patients had cuff arthropathy (n=21), rheumatoid arthritis (n=3), osteoarthritis (n=1), acute fracture (n=3), or fracture sequela (n=2). Postoperatively, patients were advised not to push themselves up and out of their wheelchair for 6 weeks. This study was performed in 2016, and 21 patients (27 shoulders) were available for a mean follow-up of 5.6 years (range, 2-10 years). The mean age at surgery was 78 years (range, 54-90 years). Results: Constant-Murley score improved from 9.4 preoperatively to 59.8 at the final follow-up (P=0.001). Pain score improved from 2/15 to 13.8/15 (P=0.001). Patient satisfaction (Subjective Shoulder Value) improved from 0.6/10 to 8.7/10 (P=0.001). Significant improvement in mean range of motion from 46° to 130° of elevation, 14° to 35° of external rotation, and 29° to 78° internal rotation was recorded (P=0.001). Final mean Activities of Daily Living External and Internal Rotation was 32.4/36. Only three patients showed Sirveaux-Nerot grade-1 (10%) glenoid notching and three grade 2 (10%). Conclusions: rTSA can be used for treatment of patients with weight-bearing shoulders. Such patients reported pain free movement, resumed daily activities, and high satisfaction rates.

상완골 후경각이 가동역에 따른 견관절 근력에 미치는 영향 (Effects of Shoulder Muscle Strength on Terminal Range by Humeral Head Retroversion)

  • 어깨 관절의 외선의 증대와 내선의 감소는 야구 투수들의 어깨에 주의 깊게 발생한다는 것을 알아야한다. 본 연구의 목적은 MRI 진단을 통해 상완골 후경각을 판단하고, 야구 투수들에게 정해진 각도에서의 길항근의 신장성 수축과 주동근의 단축성 수축을 통한 근력을 비교 분석하는데 있다. 대상자는 어깨관절의 통증을 경험하지 않은 9명의 투수들을 선정하였고, 투구측과 비투구측을 어깨관절을 대상으로 등속성 시스템을 이용하여, $90^{\circ}/s$$180^{\circ}/s$의 속도로 어깨관절의 내선 $90^{\circ}$와 외선 $20^{\circ}$의 범위에서 근력측정을 하였다. 모든 데이터는 대응 t검정을 사용해 분석 및 평가를 하였다(p<0.05). 그 결과 투구측의 총 가동역은 $180.1^{\circ}$로 나타났으며, 비투구측은 $183.7^{\circ}$로 나타났다. 상완골 후경각은 투구측이 $47.6{\pm}6.1^{\circ}$로 나타났으며, 비투구측은 $37.8{\pm}5.3^{\circ}$로 유의한 차이를 보였다(p<0.05). 투구측의 내선근 단축성 수축력(IR-Con)은 $31.5{\pm}5.1\;Nm$로 나타났으며, 비투구측의 $38.7{\pm}5.2\;Nm$와 비교해 유의한 차이를 보였다(p<0.05). 투구측의 외선근 신장성 수축력(ER-Ecc)도 $20.3{\pm}4.7\;Nm$로 나타났으며, 비투구측의 $25.1{\pm}3.7\;Nm$에 비교해 통계학적으로 유의한 차이를 보였다(p<0.05). 결론으로는 본 연구의 검토를 통해 투수들의 어깨관절에 있어서 외선의 증대 및 내선의 감소는 상완골 후경각의 골성의 변화가 중요한 요인으로 나타났으며, 근력의 변화에도 중대한 영향을 미치는 것으로 나타났다.

중심성 교뇌수초용해 환자의 치료 경과에 따른 보행 기능의 정량적 평가 (Case Report: Quantitative Evaluation of Gait Function Following Treatment Progression in a Patient with Central Pontine Myelinolysis)

  • Introduction: Central pontine myelinolysis (CPM) is a rare neurological disorder marked by symmetric nerve fiber damage, commonly following rapid hyponatremia correction, with no established treatment and a poor prognosis. The condition, often linked to alcoholism, malnutrition, and various diseases, lacks comprehensive studies on its impact on gait. This research aims to quantitatively analyze gait changes in CPM patients receiving both traditional Korean and conventional rehabilitation treatments, addressing a gap in current understanding of CPM management. Case presentation: A 56-year-old male diagnosed with CPM following an initial misdiagnosis underwent combined electoracupuncture and rehabilitative treatment at ○○ university Korean medical center, resulting in significant gait improvements. A treadmill gait analysis system was used to measure changes in key gait parameters at 2-week intervals, and the patient's progress was documented. Conclusions: The quantitative analysis revealed significant gait improvements. Foot rotation decreased from 8.9° to 6.4° (right) and from 11.1° to 7.2° (left); lateral symmetry improved from -7.8 to 0.8; step length increased from 21 cm to 44 cm (right) and from 19 cm to 44 cm (left); and velocity increased from 1.2 m/s to 2.7 m/s. These findings highlight decreased foot rotation and lateral symmetry, along with increased step length and velocity, suggesting a positive outcome of the treatment regimen. Notably, the patient experienced no adverse effects related to the treatments. Despite limitations, including the singe case focus and lack of prior gait-focused CPM research, this case report provides valuable insights into effective CPM management strategies, paving the way for future research in this domain.
