Research about the Effect of Sopungsoongiwon on Constipated Stroke Patients (중풍환자의 변비에 대한 소풍순기원의 효능에 관한 고찰)

  • Object : This study aims to assess the efficacy of Sopungsoongiwon(疏風順氣元) in constipated stroke patients and to find out the hepatotoxicity and nephrotoxicity of Sopungsoongiwon. Methods : Subjects were 111 patients who were admitted to tile hospital. The informations about the constipation and other factors were taken through medical records as follows : time until defecation, amount of Sopungsoongiwon until defecation, amount of stool, AST, ALT, BUN, Creatinine. We get the means and standard deviation from the various factors obtained from medical records. Results : 1. The mean time until defecation after taking Sopungsoongiwon was $20.919{\pm}17.885$ hours. 2. The dose of Sopungsoongiwon until defecation was $3.126{\pm}1.624$ packs. 3. The mean amount of stool was $135.360{\pm}63.288g$. 4. AST, ALT, BUN, Creatinine was maintained in normal range after taking sopungsoongiwon(p<0.0001).

  • This study has been carried out to investigate the cause and originative mechanism of vertigo by referring to 46 literatures. The results were as follows; 1. The 1st factors causing vertigo are exuberance of fire in the Liver (肝火偏亢). and ascending of Yang of Wind (風陽升動) resulting from thought excess (思慮太過) and melancholy (憂鬱). 2. The 2nd factors causing vertigo is a malnurtrient of the brain resulting from dispersion of the Liver function (肝血虛) and failure in ascending of the Clear Yang (淸陽不升) due to hemorrhage and so on. 3. The 3rd factors causing vertigo are failure in ascending to the Brain and deficiency of blood of the Liver (肝血虛) resulting from the injury of the essence of the Kidney (肝精虧損). 4. The 4th factors causing vertigo is a ascending of exogenous pathogenic factors (外邪) to the Brain on deficiency state. 5. The 1st factors causing vertigo are Stagnatum of clear Yang (淸陽不振) and pershing of Yang (亡陽) resulting from loss of water and damage of active thin body fluid (津氣虧損). 6. The obesity is beonged to excessiveness Symptom-Complex (實證) and the thin to deficiency Symptom-complex (虛證). 7. The vertigo is connective with the Live (肝), Spleen (脾) and the Kidney (腎), but among those, most intimative viscera is the Liver (肝).

BASIC RESEARCH OF SUB-PACKAGE PROBLEM IN KOREAN CONSTRUCTION INDUSTRY

  • In the building construction, the specialist contractors play the important roles in the point of the quality securing. Therefore, it is very important for the construction industry to study the sub-package problem. The sub-package problem includes two problems which should be solved. One is to decide the scope of works of each specialist contractor, and another one is to decide the particular specialist contractor which carries out the work. However, the sub-package problem in Korea is not clarified yet, although the circumstance around it has changed rapidly. Many factors influence to the sub-package problem regardless of internal factors or external factors of the project. The general contractor usually decides the sub-package under considering the project conditions. In case of the internal factors, each general contractor manages the organization and materials. But the external factors are relatively more difficult to control and predict than internal ones. But out of the external factors, the legal system has very close relationship with a sub-package problem especially in Korean construction system. So, this paper clarifies relationship between the legal system relating subcontracting and the state of sub-package.

Is Season a Prognostic Factor in Breast Cancer?

  • Background: Some studies have indicated an inverse relationship between cancer risk and sunlight exposure. Others have reported that the prognosis of some cancers such as prostate, colon, ovarian and non melanoma skin cancer, were affected by the season in which the cancer was diagnosed. In our study, we evaluated whether season is prognostic in Turkish patients with breast cancer. Materials and Methods: A total of 517 patients from Kayseri Training and Research Hospital were analysed retrospectively. Patients were divided into 4 groups according to season of cancer diagnosis: winter, spring, summer and autumn. The prognostic factors for disease free survival and overall survival were investigated. Results: No significant differences were found among groups regarding prognostic factors overall. Only estrogen receptor status and lymphovascular invasion were independent prognostic factors (p=0.001 and p=0.001 respectively). We found significantly differences for mean disease free survival among groups (p=0.019). Winter group had better mean DFS while summer group had worse DFS. Mean overall survival was similar in the four groups (p=0.637). Conclusions: The season is not an independent predictive factor. However, due to interaction with other factors, we think that the season of cancer diagnosis is important for cancer prognosis.

Risk factors affecting amputation in diabetic foot

  • Background: A diabetic foot is the most common cause of non-traumatic lower extremity amputations (LEA). The study seeks to assess the risk factors of amputation in patients with diabetic foot ulcers (DFU). Methods: The study was conducted on 351 patients with DFUs from January 2010 to December 2018. Their demographic characteristics, disease history, laboratory data, ankle-brachial index, Wagner classification, osteomyelitis, sarcopenia index, and ulcer sizes were considered as variables to predict outcome. A chi-square test and multivariate logistic regression analysis were performed to test the relationship of the data gathered. Additionally, the subjects were divided into two groups based on their amputation surgery. Results: Out of the 351 subjects, 170 required LEA. The mean age of the subjects was 61 years and the mean duration of diabetes was 15 years; there was no significant difference between the two groups in terms of these averages. Osteomyelitis (hazard ratio [HR], 6.164; 95% confidence interval [CI], 3.561-10.671), lesion on percutaneous transluminal angioplasty (HR, 2.494; 95% CI, 1.087-5.721), estimated glomerular filtration rate (eGFR; HR, 0.99; 95% CI, 0.981-0.999), ulcer size (HR, 1.247; 95% CI, 1.107-1.405), and forefoot ulcer location (HR, 2.475; 95% CI, 0.224-0.73) were associated with risk of amputation. Conclusion: Osteomyelitis, peripheral artery disease, chronic kidney disease, ulcer size, and forefoot ulcer location were risk factors for amputation in diabetic foot patients. Further investigation would contribute to the establishment of a diabetic foot risk stratification system for Koreans, allowing for optimal individualized treatment.

Risk Factors Influencing Rebleeding after Bronchial Artery Embolization on the Management of Hemoptysis Associated with Pulmonary Tuberculosis

  • Background: Hemoptysis due to pulmonary tuberculosis (TB) frequently develops in Korea where the prevalence of TB is intermediate. The effect of bronchial artery embolization (BAE) on the control of massive hemoptysis has been well known. This study is designed to identify the risk factors contributing to rebleeding after BAE in patients with TB. Methods: We retrospectively evaluated risk factors and the time for rebleeding after BAE in 72 patients presenting with hemoptysis. Results: The overall immediate success rate of BAE was 93.1% (67 of 72 patients). Of the 29 patients (40.3%) who showed rebleeding after BAE, 13 patients experienced rebleeding within 1 month, and 14 patients between 1 month to 1 year. The existence of a shunt in angiographic finding, aspergilloma, and diabetes mellitus were risk factors of rebleeding after BAE in multivariate analysis. Conclusion: BAE was very effective for obtaining immediate bleeding control in hemoptysis associated with active TB or post-TB sequelae. It is important to observe whether or not rebleeding occurs up to 1 year of BAE especially in TB patients with aspergilloma, DM, or a shunt. Even rebleeding can be managed well by second BAE.

Formation of Internal Wind Paths of Open Space and Its Effect on Meteorological Factors and the Generation of Negative Air Ions (녹지 내부 바람통로가 기상요소와 음이온 발생량 변화에 미치는 영향)

  • The purpose of this study is to understand the formation of internal wind paths of open space and its effect on meteorological factors and the generation of negative air ions. Various types of internal wind paths of open space were formed. Subsequently, changes in meteorological factors in each type were measured and the generated negative air ions were analyzed. The four key findings of the study are summarized as follows. First, the average wind speed formed inside the open space was analyzed such that the difference in wind speed was dependent on the difference in the composition of the wind path. Second, the negative air ion generation was observed to have the same trend as the average wind speed difference. Third, changes to the meteorological factors were more evident depending on the difference in wind path formation patterns. Solar radiation was expected to be highly affected by the physical structure (direction) of the target site. The relative humidity was found to show large difference depending on the different wind path type; however, this difference was significantly reduced when converting to absolute humidity. Fourth, it was found that the wind path formation type of open space affects meteorological factors through path analysis, and the changed meteorological factors affect the amount of generated negative air ions. Two conclusions can be obtained based on these results. First, the changes in internal wind speed formation of open space directly reduced the amount of generated negative air ions. Second, the changes in wind speed affect meteorological factors as well as the amount of generated negative air ions.

  • The strength of concrete is affected significantly not only by the internal influence factors of cement, water, sand, aggregate, and admixture, but also by the external influence factors of concrete placement delay and curing temperature. The objective of this research was to predict the concrete strength considering both the internal and external influence factors when concrete is placed at the construction site. In this study, a concrete strength test was conducted on the 24 combinations of internal and external influence factors, and a neural network model was constructed using the test data. This neural network model can predict the concrete strength considering the external influence factors of the concrete placement delay and curing temperature when concrete is placed at the construction site. Contractors can use the concrete strength prediction neural network model to make concrete more robust to external influence factors during concrete placement at a construction site.

A Study on the Effects of Internal Marketing Factors to Job Satisfaction, Emotional Labor, and Organizational Commitment of the Elderly Care Facility Employee (노인요양시설 종사자들의 내부마케팅 요인이 직무만족, 감정노동 및 조직몰입에 미치는 영향에 관한 연구)

  • The purpose of this study is to analyze the effects of internal marketing factors on organizational commitment and emotional labor in the elderly Care Worker. The data are collected through structured questionnaires. The Subjects of the final analysis is 191 persons working in the elderly care facilities in Gyeongnam and Gyeongbuk provinces. Hypothesis testing of the research model is performed using AMOS 18.0. The main results of this study are as follows. Internal marketing factors influenced only internal communication and management support for job satisfaction, internal support for managerial support for emotional labor, internal communication and management support for organizational commitment. The results of this study suggest that in order to enhance the organizational commitment of long-term care workers, it is important not to strengthen internal marketing capacity but to increase job performance first, and secondly, to improve job performance, It is necessary to establish a more diversified research on the variables of service quality of job satisfaction and emotional labor.

Clinical Features and the Risk Factors of Distant Metastasis in Locally Advanced Advanced Head and Neck Cancer Patients after Induction Chemotherapy Followed Locoregional Control Therapy (유도화학요법 및 국소 치료 후 원격전이를 보인 국소 진행성 두경부암 환자군의 임상 특징 및 위험인자에 관한 연구)

  • Backgrounds : Head and neck cancer is one of the most prevalent cancers in the world. It tends to remain localized at the primary site and regional lymph nodes, but if distant metastasis occurs, it has a poor prognosis. This study was performed to evaluate the prevalence of distant metastasis and to determine the risk factor in locally advanced head and neck cancer after induction chemotherapy followed locoregional control therapy. Methods : A retrospective review was performed in 420 patients with locally advanced head and neck cancer who treated with induction chemotherapy followed locoregional control therapy from January 2001 to December 2010. Among them, 31 patients who had distant metastasis as first relapse within 2 years after termination of therapy were analyzed for clinical features and the risk factors of distant metastasis. Results : The overall incidence of distant metastasis was 7.3%. The bone, lung, and liver were the most frequent metastatic organs. In univariate analysis, nodal stage, nasopharyngeal cancer, laryngeal cancer, G3/G4 neutropenia during induction chemotherapy, and concurrent chemoradiotherapy were the influencing factors for distant metastasis. In multivariate analysis, advanced N stage and nasopharynx were the risk factors of distant metastasis, and grade 3/4 neutropenia during induction chemotherapy was considered to decrease distant metastasis. Conclusion : This study suggests that the advanced N stage is the risk factor of distant metastasis and Grade 3/4 neutropenia during induction chemotherapy can be beneficial against distant metastasis in locally advanced head and neck cancer patients treated with induction chemotherapy followed locoregional control therapy.
