Internal Mammary Artery Grafting Without Intraluminal Dilatation - Measurement of Internal Mammary Artery Flow and Clinical Results - (내경 확장을 시행하지 않은 내유동맥을 이용한 관상동맥 우회로술의 임상적 결과[내유동맥 혈류량과 그임상적 결과])

  • The internal mammary artery has been advocated for use in bypass grafting owing to its superior long-term patency when compared to saphenous vein grafts. Concern exists that the flow through internal mammary artery may be inadequate during periods of peak myocardial demand when the internal mammary artery graft was used for proximal left anterior descending artery stenosis. This flow adequacy was investigated in 13 consecutive patients with a mean proximal left anterior descending artery stenosis of 84.2% who were selected for coronary bypass using internal mammary artery. We checked flow and diameter of left internal mammary artery without intraluminal dilatation just before anastomosis to left anterior descending artery during cardiopulmonary bypass. Clinical results were evaluated postoperatively with clinical symptoms, echocardiographies, stress tests, and coronary angiographies. The mean internal mammary artery flow measured just before anastomosis was 38ml/ min[range of 20 to 80ml /min] and its mean internal diameter 1.4mm. Maximal workload was improved from preoperative value of 6.3$\pm$2.5METS to postoperative value of 9.1$\pm$1.4 METS in 9 patients who Paired-test can be used. Cardiac symptoms recurred in two patients after bypass surgery, but they were not related to left internal mammary artery grafts. All patients were discharged in postoperatively 9.3 days[range of 7 to 20 days] after operation without mortality. Thus, on the basis of these findings, the internal mammary artery is a reasonable graft that we can routinely use for proximal left descending artery stenosis if internal diameter of the internal mammary artery is more than 1.0mm and its flow is more than 20ml /min at mean arterial pressure of 50 to 60mmHg during cardiopulmonary bypass.

  • Numerical analysis were performed to investigate the stability and internal movement of tunnel located beneath the base of abutment of bridge according to the method of supporting tunnel. Two supporting methods of the multi-staged grouting method with steel pipes and the large diameter of pipe supporting method were used in the centrifuge model tests. The slip form of model lining, specially built to simulate the process of tunnel excavating under the condition of accelerated g-level, was used in the centrifuge model tests. Four centrifuge model tests were performed, changing the supporting methods of the multi-staged grouting method with steel pipes and the large diameter of pipe supporting method and the location of model abutment base of bridge. For internal displacement of tunnel, movements of the crown. The left and the right sides of spring line were measured during the proceeds of excavating tunnel in centrifuge model tests. Test results were compared with numerically estimated values of internal displacement of tunnel by using the commercially available FEM software of PENTAGON-3D. It was found that they were in good agreements and the large diameter of pipe supporting method was more stable than the multi-staged grouting method with steel pipes with respect to the internal movement of tunnel.

  • Three dogs were presented with signs of recurrence of coughing, dyspnea and difficulty of barking after endotracheal stent placement. On the basis of history taking, physical examination, laboratory tests and radiography, complications of endotracheal stent were suspected. Dog 1 had unfitted stent diameter (stent diameter is larger than tracheal diameter) and getting disentangled of proximal tracheal stent wire. Dog 2 was suspected a foreign body reaction. Dog 3 had the migration of stent caudally. Based on these cases, the veterinarian should accurately evaluate the indicated patients before endotracheal stent and has to select the appropriate stent (diameter, length and location) to prevent complications after endotracheal stent and concern the regular follow-ups to assure proper endotracheal stent placement.

Factor analysis of subgrade spring stiffness of circular tunnel

  • This paper studied the subgrade spring stiffness and its influencing factors in the seismic deformation method of circular tunnel. Numerical calculations are performed for 3 influencing factors: stratum stiffness, tunnel diameter and burial depth. The results show that the stratum stiffness and tunnel diameter have great influence on the subgrade spring stiffness. The subgrade spring stiffness increases linearly with stratum stiffness increasement, and decreases with the tunnel diameter increasement. When the burial depth ratio (burial depth/tunnel diameter) exceeds to 5, the subgrade spring stiffness has little sensitivity to the burial depth. Then, a proposed formula of subgrade spring stiffness for the seismic deformation method of circular tunnel is proposed. Meanwhile, the internal force results of the seismic deformation method are larger than that of the dynamic time history method, but the internal force distributions of the two methods are consistent, that is, the structure exhibits elliptical deformation with the largest internal force at the conjugate 45° position of the circular tunnel. Therefore, the seismic deformation method based on the proposed formula can effectively reflect the deformation and internal force characteristics of the tunnel and has good applicability in engineering practice.

  • In this paper, a parametric study on ribs which are installed in an equilateral triangular internal cooling channel is presented. The numerical analysis of the flow structure and heat transfer characteristics is performed using three-dimensional Reynolds-averaged Navier-Stokes equations with the shear stress transport turbulence model. The numerical results are obtained at Reynolds number, 20,000. The parametric study is performed for the parameters, the angle of a rib, rib pitch-to-hydraulic diameter ratio, rib width-to-hydraulic diameter ratio, and rib height-to-hydraulic diameter ratio. The computational results are validated with the experimental data for area-averaged Nusselt number.

A Clinical Anatomic Study of Internal Mammary Perforators as Recipient Vessels for Breast Reconstruction

  • Background Partially resecting ribs of the recipient site to facilitate easy anastomosis of the internal mammary vessels to free flaps during breast reconstruction can cause chest wall pain or deformities. To avoid this, the intercostal perforating branches of the internal mammary vessels can be used for anastomosis. The purpose of this study was to investigate the location and size of the internal mammary perforator vessels based on clinical intraoperative findings and to determine their reliability as recipient vessels for breast reconstruction with microsurgical free tissue transfer. Methods Twelve patients were preoperatively screened for the presence of internal mammary perforators using Doppler tracing. After modified radical mastectomy was performed by a general surgeon, the location and size of the internal mammary perforator vessels were microscopically investigated. The external diameter was examined using a vessel-measuring gauge from a mechanical coupling device, and the distance from the mid-sternal line to the perforator was also measured. Results The largest arterial perforator averaged 1.5 mm, and the largest venous perforator averaged 2.2 mm. Perforators emerging from the second intercostal space had the largest average external diameter; the second intercostal space also had the largest number of perforators arising from it. The average distance from the mid-sternal line to the perforator was 20.2 mm. Conclusions Internal mammary perforators presented consistent and reliable anatomy in this study. Based on these results, the internal mammary perforators appear to have a suitable diameter for microvascular anastomosis and should be considered as an alternative recipient vessel to the internal mammary vessel.

  • I investigated experomentally the spray characteristics to the operating conditions and the shapes of internal mixing twin fluid atomizer. The wide variations of air per liquid ratio are conducted to predit the influences of the Sauter mean diameter(SMD), spray angle, distribution of drop size, the flowing condition of gas and length, flowing, area of gas and liquid, and diameter, number and place of the orifice. In this experiment, air per liquid raio, mixing chamber length per diameter, orifice diameter, and the flowing area ratio of gas and fluid influences greatly on SMD, spray angle, distribution of drop size and intermittent fluctuation region.

A study of internal wave influence on OTEC systems

  • Ocean Thermal Energy Conversion (OTEC) systems utilize the temperature difference between the surface water and deep ocean water to generate electrical energy. In addition to ocean surface waves, wind and current, in certain locations like the Andaman Sea, Sulu Sea and the South China Sea the presence of strong internal waves may become a concern in floating OTEC system design. The current paper focuses on studying the dependence of the CWP hydrodynamic drag on relative velocity of the flow around the pipe, the effect of drag amplification due to vortex induced vibrations and the influence of internal waves on the floating semi and the cold water pipe integrated OTEC system. Two CWP sizes are modeled; the 4m diameter pipe represents a small scale prototype and the 10m diameter pipe represents a full commercial size CWP. are considered in the study.

  • The measurements of velocities of internal flow in a scaled-up nozzle were made by laser Doppler velocimetry in order to clarify the effect of internal flow on the characteristics of fuel spray. The investigated length to diameter ratio(L/d) of the orifice were 1, 3, 4, 5 and 8, and inlet radius to diameter ratio(r0/d) were 0 and 0.5. Mean and fluctuating velocities and discharge coefficients were obtained at various Reynolds number ranging between 15,000 and 28,000, and L/d ranging between 1 and 8 in sharp and round inlet nozzle. The turbulent intensity and turbulent kinetic energy at exit in a sharp inlet nozzle were higher than that in a round inlet nozzle. For sharp inlet nozzle, fluctuating velocities near exit were decreased with increasing L/d.

Hydrodynamic Characteristics of the Rectangular Type Circulating Fluidized Bed (4각 순환유동층의 수력학적 특성)

  • A rectangular type circulating fluidized bed (CFB) with an internal nozzle and two partition walls was proposed. In this modified CFB, an internal nozzle and two slanted partition walls were additionally set in the riser. This cold mode apparatus was made of acrylic resin; the riser was 1500mm high and $1000{\times}1000mm^2$ in the cross sectional area, the internal nozzle was 200mm high and 10mm in the inner diameter, and the partition wall was 7mm thick. Glass beads of $91{\mu}m$ in the mean diameter were employed as bed materials. In the cold mode by using the proposed CFB with an internal nozzle, it was possible to change the particle hold up by changing the gas flow ratio of the nozzle to the total(Qn/Qt). It was found that the inflection point which devided the bed structure between the dense and the dilute phase in the riser varied with Qn/Qt.
