Trends of Tongue Features in Functional Dyspepsia Patients (기능성 소화불량 환자에서 설 지표의 경향성 파악)

  • Objectives: In this study, the tongue features of patients with functional dyspepsia (FD) were compared with those of healthy controls. Methods: This prospective, case-control study was conducted on patients with FD and controls recruited at a single center. After screening, the subjects were allocated to the patient or control groups (patients=42, controls=40). Tongue images were acquired using a computerized tongue image acquisition system (CTIS). An independent t-test was conducted to compare the measurements from patients and controls. Binary logistic regression was performed to determine significant differences between the two groups after adjusting for age and sex. Results: The CIE $a^*$ color value in the tongue coating area was significantly lower in the patients with FD than in the controls (p=0.001). The tongue coating ratios were also significantly higher in the FD group than in the control group (p=0.003). We found that the CIE $a^*$ color value in the tongue coating area and the tongue coating ratios were significant predictive factors in both groups, based on binary regression analysis (p=0.016, 0.044, respectively). Conclusions: This study found that FD was significantly associated with CIE $a^*$ color value in the tongue coating area and tongue coating ratios. We suggest that these factors could be used as objective indicators of FD.

  • As the increase in the use of computers and rapid development of information technology exacerbate auditors' judgmental process, achieving objectives(effectiveness) of control and auditing with limited resources requires auditors to identify relative importance and priority of controls. Consequently, auditors' review and evaluation of internal control is been highlighted in modrn auditing process. Especially, as more organizations adopted LAN(Local Area Network) systems. LAN environment is facing with problems arising from characteristics of theLAN and the evaluation of LAN controls. The modern in this study is LAN control evaluation model with information system auditing objectives. This study concludes that auditors' proper evalution of the importance of LAN controls contributes to the effective achievement of IS auditing objectives.

Prevalence and Risk Factors of Osteoporosis in Patients with Chronic Obstructive Pulmonary Disease (만성폐쇄성폐질환 환자에서 골다공증의 유병률과 위험인자)

  • Background: Osteoporosis is a significant comorbidity in patients with chronic obstructive pulmonary disease (COPD). This study examined the prevalence and risk factors associated with osteoporosis in patients with COPD. Methods: The bone mineral densities (BMDs) of the lumbar spine and femoral bone were measured in 53 patients with clinically stable COPD and 41 age- and gender-matched control subjects showing a normal lung function. Osteoporosis was defined as a T-score $\leq$-2.5. The subjects' clinical characteristics and laboratory data were reviewed, and multiple logistic regression analysis was used to identify the risk factors associated with osteoporosis in COPD patients. Results: The prevalence of osteoporosis was 47% and 32% in the COPD patients and controls, respectively. In particular, using the femoral neck T-score, the prevalence of osteoporosis in COPD patients was higher than that in the controls (26% vs. 5%; p=0.006). The average T-score of the lumbar spine (p=0.025) and femoral neck of COPD patients were significantly lower than those of the controls (p=0.001). The forced expiratory volume in the 1 second ($FEV_1$) % predicted (p=0.019; odds ratio [OR], 0.955; 95% confidence interval [CI], 0.919-0.993) and age (p=0.024; OR, 1.144; 95% CI, 1.018-1.287) were independently associated with osteoporosis in patients with COPD. Conclusion: Using the femoral neck T-score, the prevalence of osteoporosis in patients with COPD was higher than the age-and gender-matched controls. A lower $FEV_1$ and older age further increase the risk of osteoporosis in patients with COPD.

A Study on the Clinical Significance of Serum Tissue Polypeptide Antigen Levels in Patients with Malignancy (악성종양환자(惡性腫瘍患者)에서 혈청(血淸) Tissue Polypeptide Antigen 치(値)의 임상적(臨床的) 의의(意義)에 관(關)한 연구(硏究))

  • To evaluate the clinical significance of serum tissue polypeptide antigen (TPA) levels in patients with malignancy, serum TP A levels were measured by radioimmunoassay in 49 normal controls, 9 patients of postoperative colon cancer without recurrence and 68 patients with various untreated malignancy, who visited Seoul National University Hospital from February, 1983 to September, 1983. The results were as follows; 1) Serum TPA levels in 49 normal controls were in the range of 22-135 U/L $(74{\pm}28U/L,\;mean{\pm}S.D.)$. There was no sex or age difference. Normal upper limit of serum TPA was defined as 130 U/L (mean+2S.D.). 2) Serum TPA levels in 68 patients with various untreated malignancy (stomach cancer 33 cases, colon cancer 11 cases, lung cancer 10 cases, primary liver cancer 9 cases and metastatic cancer of unknown primary site 5 cases) were in the range of 10-800 U/L $(189{\pm}170U/L,\;mean{\pm}S.D.)$ and significantly elevated, compared with those of normal controls (p<0.005). 3) The sensitivities of serum TPA in various untreated malignancy were 39% in stomach cancer, 55% in colon cancer, 50% in lung cancer, 67% in primary liver cancer and 80% in metastatic cancer of unknown primary site respectively. 4) The sensitivities of serum TPA related to resectability in stomach and colon cancer were 32% in resectable stomach cancer, 50% in unresectable stomach cancer, 29% in resectable colon cancer and 100% in unresectable colon cancer respectively. 5) The mean value of serum TPA levels in 9 patients of postoperative colon cancer without recurrence was $70{\pm}39U/L$ and significantly decreased, compared with that of untreated colon cancer, $180{\pm}150U/L$ U/L (p<0.05). 6) In patients with stomach or colon cancer, there was no significant correlation between serum TP A and serum CEA levels, but simultaneous measurement of serum TPA and serum CEA levels increased sensitivities. From above results, we concluded that serum TPA level is a useful indicator reflecting tumor activity and responses to anticancer treatment in patients with malignancy.

Clinical Study on Risk Factors of Acute Brain Infarction And NIH stroke scale (급성기 뇌경색의 위험인자와 NIH stroke scale에 관한 임상적 연구)

  • This study was done to examine the risk factors between controls group and acute brain infarction patients group, and to compare high risk group with low risk group through NIHSS(National Institutes of Health stroke scale). We compared the risk factors between acute brain infarction patients group(N=180) and controls group(N=93). And according to risk factors, we analyzed 1st NIHss, after 3 weeks improved extent within acute brain infarction patients group. The results were as follows. 1. Among the risk factors, HTN, DM, the blood levels of HCY were significantly higher and the blood levels of HDL-C was significantly lower in patients group than controls group. 2. In scale analysis according to risk factors, 1st NIHss were significantly higher in Hypo-HDL-cholesterolemia, Obesity, High-Homocysteine, HTN, DM, previous CVA history group than low risk group within acute brain infarction patients group. 3. In after 3 weeks improved extent analysis according to risk factors, improved extent were lower in Hyperlipidemia, HTN, DM, previous cardiac history, older age group than low risk group within acute brain infarction patients group. The above results suggest that significant risk factors of acute brain infarction, and shows the High risk group that had risk factor of brain infarction recognized from the former research tends to have higher 1st NIHss. Also the High risk group tends to have lower improved extent, but the results are not statistically significant. Furthur research on subject is needed.

Correlation Analysis between Gastric Emptying Measurement by Ultrasonography and Korean Medical Questionnaires (초음파 위배출 측정과 한의설문 간의 상관성 분석)

  • Objective: This study was designed to investigate the correlation between gastric emptying measurement by ultrasonography and the findings of various questionnaires of functional dyspepsia (FD) or in Korean medicine to explore the possibility of quantification of questionnaires. Methods: Eighty-eight patients (44 patients with FD and 44 healthy controls) were recruited from October 2016 to November 2017. Gastric emptying half-time (T1/2) was measured by ultrasonography at fasting, and at 0, 15, 30, 45, 60, 90, and 120 min after meals (apple juice, 500 ml, 224 kcal). The average half-time (average T1/2) was calculated by averaging all measured half-times. A visual analogue scale (VAS), food retention questionnaire (FRQ), damum questionnaire (DQ), cold and heat questionnaire (CHQ), deficiency and excess questionnaire (DEQ), spleen-qi deficiency questionnaire (SQDQ), Nepean dyspepsia index-Korean version (NDI-K), and quality of life of NDI (NDI-QoL) were filled out by all participants. Correlation between the average T1/2 and questionnaires were analyzed for 79 patients (37 patients with FD and 42 healthy controls) after excluding missing and erroneous data. Results: The FRQ, DEQ, SQDQ, NDI-K, and NDI-QoL scores, especially for some gastrointestinal symptoms and general weakness-related symptoms, showed statistically significant correlations with average T1/2. Conclusions: Our results suggest that gastric emptying measurement by ultrasonography may be a quantitative substitute method for some Korean medical questionnaires or for some dyspepsia-related and deficiency-related items among them. However, further studies using various measurements of gastric emptying will be needed.

Clinical Significance of Autoantibodies in Some Thyroid Disorders (갑상선질환(甲狀腺疾患)에서의 자가항체(自家抗體)의 임상적(臨床的) 의의(意義))

  • Clinical measurement of thyroid autoantibodies in sera of some thyroid disorders have been widely applied since about twenty years ago. We investigated the incidence and titers of both anti microsomal and antithyroglobulin antibodies in forty eight cases with controls and one hundred and thirty three patients with some form of thyroid disorders. The results were as follows; 1) In controls, antimicrosomal antibodies were positive in 2% but anti thyroglobulin antibodies were all negative. 2) In a series of one hundred and thirty three patients with thyroid disease, anti microsomal antibodies were positive in 44% but antithyroglobulin antibodies were positive in only 15%. 3) The rate disclosing the positive results of antimicrosomal antibodies were 71 % in Hashmoto's disease, 60% in Graves' disease, and 38% in primary hypothroidism, respectively. On the other hand, the positive results of antithyroglobulin antibodies showed 21 % in Graves' disease, 19% in primary hypothyroidism, and 18% in Hashmoto's disease, respectively. Though there were relatively high rate of both antimicrosomal and anti thyroglobulin antibodies in patients with nodular goiter, they were only seven cases in our series. 4) The rate with the extremely high titers of antimicrosomal and antithyroglobulin antibodies$(>1:160^2)$ was 83% and 67% in Hashmoto's disease, 50% and 67% in primary hypothyroidism, and 41% and 18% in Graves' disease. Accordingly, the thyroid autoantibodies were commonly found higher positive rate in patients with Hashmoto's disease, primary hypothyroidism, and Graves' disease. Among these disorders, the extremely high positive rate of the thyroid autoantibodies was found in patients with Hashmoto's disease.

The Effects of Aconiti Radix on Thyroid Function in Hypothyroidism Rat Model Induced by 6-propyl-2-thiouracil(PTU). (부자(附子)가 6-propyl-2-thiouracil(PTU)로 유발된 rat의 갑상선 기능저하증에 미치는 영향)

  • Objective : Although hypothyroidism is a common disease in the endocrinology system, it is reported that there are a lot of difficulties in treating it effectively. Aconiti Ra얕 traditionally has been used in treatment of coldness, fatigue, and bradycardia. In this study, we investigated the therapeutic effects of Aconiti Radix on hypothyroidism rat model induced by 6-propyl-2-thiouracil (PTU). Methods : Two-month-old rats were used with administration of PTU which induced hypothyroidism in the rats. After 2 weeks, Aconiti Radix and thyroxine were administered, respectively. The body weights were measured every week. After 4 weeks, the blood samples of all rats were taken from their hearts. They were analyzed biochemically and $T_4$ (thyroid hormone) & TSH (thyroid stimulating hormone) was measured by ELISA kits. Results : In comparison with normals, controls showed hypothyroidism with significantly low $T_4$ and high TSH the statistics. In Aconiti Radix administration groups significantly increased $T_4$ was observed in the statistics and its effects were dose-dependent. There was no difference statistically in TSH of Aconiti Radix treatment groups from controls, nor were statistical differences observed significantly in biochemical labs and weight of each group. Conclusions : These findings suggest that Aconiti Radix protects thyroid cells and makes thyroid cells produce thyroid hormones. It is also very safe in the view of liver, kidney function, and other metabolism. It may be a useful agent for treating hypothyroidism.

Red blood cell distribution width is useful in discriminating adult onset Still's disease and sepsis within 24 hours after hospitalization

  • Background/Aims: Red blood cell distribution width (RDW) is a value representing the heterogeneity in the size of red blood cell, and it is usually used in distinguishing types of anaemia. Recently, it was reported that it could reflect the burden of inflammation in diverse diseases and their prognosis. Hence, in this study, we investigated whether RDW may contribute to discriminating adult onset Still's disease (AOSD) from sepsis in serious febrile patients within 24 hours after hospitalization. Methods: We reviewed the medical records and enrolled 21 AOSD patients, 27 sepsis patients and 30 matched healthy controls. We collected at least two laboratory results of variables including RDW within 24 hours after hospitalization, and we calculated their mean values. Results: Sepsis patients showed the significantly increased median white blood cell count, compared to AOSD patients ($14,390.0/mm^3$ vs. $12,390.0/mm^3$, p = 0.010). The median RDW in sepsis patients was higher than that in AOSD patients (15.0% vs. 13.3%, p = 0.001), and furthermore, the median RDW in both patient-groups was significantly higher than that in healthy controls. In contrast, the median ferritin level in sepsis patients was lower than that in AOSD patients (544.0 mg/dL vs. 3,756.6 mg/dL, p = 0.001). In multivariate analysis, RDW ${\geq}14.8%$ (odds ratio, 17.549) and ferritin < 2,251.0 mg/dL (odds ratio, 32.414) independently suggested sepsis more than AOSD in patients initially presenting with fever requiring hospitalization. Conclusions: RDW might be a rapid and helpful marker for a differential diagnosis between AOSD from sepsis at an early phase.

Continuous Audits Using Decision Support Systems

  • Purpose - This article's aim is to examine how the utilization of existing and future decision-support systems will lead to a change in the auditing process. Research design, data, and methodology - An information system is a special decision-support system that combines information obtained from various sources and communicates among them to help in assessing appropriate complex financial decisions. This paper analyzes techniques such as data and text mining as components of decision-support systems to be used in the auditing process. Results - We present views about how existing decision-support systems will lead to a change in audits. Auditors, who currently collect significant data manually, will in the future move towards management through complex decision-support systems. Conclusions - Although some internal audit functions are integrated into systems of continuous monitoring, the use of such systems remains limited. Thus, instead of multiple decision-support systems, a unified decision-support system can be deployed for this that includes sensors integrated within a company in different contexts (e.g., production, sales, and accounting) that continually monitors violations of controls, unusual patterns, and unusual transactions.
