A Case of Dominantly Inherited β Thalassemia Due to Hb Dieppe (Hb Dieppe에 의한 우성유전 베타 지중해빈혈 1례)

  • ${\beta}$ thalassemias are usually transmitted as autosomal recessive traits. However, some dominant forms of ${\beta}$ thalassemia have been identified in individuals who have inherited a single copy of an abnormal ${\beta}$ globin gene. Thalassemia intermedia with mild anemia, jaundice, and splenomegaly has been observed in these patients. Electrophoresis has shown elevated Hemoglobin(Hb) $A_2$ and Hb F levels. In particular, there are inclusion bodies in the erythroid precursors and peripheral red blood cells after splenectomy. The molecular basis of these dominant ${\beta}$ thalassemias is heterogeneous. The authors studied the first Korean case of dominantly inherited ${\beta}$ thalassemia due to Hb Dieppe. Hb Dieppe is a missense mutation of ${\beta}$ codon $127(CAG{\rightarrow}CGG)Gln{\rightarrow}Arg$. The patient in this case was characterized by moderate anemia, hypochromia, microcytosis, elevated Hb $A_2$ levels, elevated Hb F levels and splenomegaly. The father of the patient also has the same disease. We report this case and review related literature.

The Molecular Phylogenetic Study of Filipendula (Rosaceae) (터리풀속(Filipendula)의 분자계통학적연구)

  • 터리풀속(Filipendula)은 장미과(Rosaceae), 장미아과(Rosoideae)에 속하는 다년생 초본이며, 북반구 온대지역의 산지지역에 서식하며 15-20여 종이 보고되어 있고, 이 중 10여종이 한국, 중국, 일본, 타이완 등의 동아시아 지역에 분포한다. 본 연구의 목적은 DNA 염기서열 자료를 이용하여 터리풀속(Filipendula)내 종들간의 계통관계를 규명하기 위하여 본 연구를 수행하였다. 이를 위해서 11종 29개체의 터리풀속(Filipendula)샘플과 외군인 산딸기나무속(Rubus)에 속하는 3종 5개체의 샘플을 이용하였다. 추가로 Genbank에서 3속 10종 18개의 염기서열을 다운받아 비교분석에 이용하였다. 계통연구를 위하여 엽록체에 존재하는 atpF-atpH, psbK-psbI, psbA-trnH, matK, rbcL, 5개 마커와 핵에 존재하는 ITS, 총 6개 마커의 염기서열을 생산하였다. 총 52개의 샘플에 대하여 엽록체유전체 5개 마커지역은 염기서열 길이가 3,485bp였고 핵 ITS지역은 631bp였으며, 이들을 합한 염기서열 길이는 4,116bp였다. 계통분석결과, 터리풀속(Filipendula)은 단계통군을 이루었다. F. occidentalis와 F. vulgaris가 기저분류군을 이루었고 이들은 각각의 아속에 해당한다. 그리고 나머지 종들은 모두 하나의 단계통군을 이루었다. 위의 결과들은 1961년 시미즈가 본 속을 Hypogyna아속, Filipendula아속, Ulmaria아속으로 나눈 분류시스템과 일치한다. 나아가 분자계통수에서 Ulmaria아속은 크게 4개의 subclade로 구분되었다. 먼저 subclade I에는 F. vestita, F. kiraishiensis, F. tsuguwoi, F. multijiuga, F. purpurea 등 5개 종으로 구성되었다. Subclade II는 F. ulmaria 한 종으로만 구성되었다. Subclade III에는 F. glaberrima, F. koreana, F. formosa, F.camtschatica 로 구성되었으며 subclade III에는 한국에 서식하는 3종이 포함되었다. Subclade IV에는 F. rubra, F. angustiloba, F. palmata, F. intermedia 4종으로 구성되었다. 이번연구에서는 Ulmaria아속내에 4개의 subclade가 존재함이 처음으로 확인되었다.

Antimicorbial effect of Zea Mays L. and Magnoliae cortex extract mixtures on periodontal pathogen and effect on human gingival fibroblast cellular activity (옥수수 불검화 추출물(Zea Mays L.)과 후박(Magnoliae cortex) 추출물 혼합물의 치주질환원인균에 대한 항균작용 및 치은섬유아세포 활성도에 미치는 영향)

  • Zea Mays L. has been known to be effective for improving tissue health and Magnoliae cortex to have effective antibacterial and antimicrobial activity against pathogenic microbes. The purpose of this study was to examine the antimicrobial effects of Zea Mays L. and Magnoliae cortex extract mixtures on periodontal pathogens(Prevotella intermedia, Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis, Streptococcus mutans )and to examine the effects on human gingival fibroblast cellular activity. Zea Mays L. and Magnoliae cortex extracts and their mixtures were prepared with various mixing ratios (0.5:1, 1:1, 1.5:1, 2:1). These extracts were loaded to periodontal pathogen cultured petri dish for antimicrobial test and also loaded to cultured human gingival fibroblast for cellular activity test. Each test was repeated 3 times and data were analyzed by one-way ANOVA with 95% confidence level. Mixture of these two extracts showed greater amount of inhibition area on periodontal pathogen and more improved gingival fibroblast activity as Zea Mays L. ratio reduced. So, mixture ratio 0.5:1 (Zea Mays L. : Magnoliae cortex) group showed statistical significance in antimicrobial activity and cellular activity among various mixtures(p < 0.05). In conclusion, 0.5:1 (Zea Mays L. : Magnoliae cortex) mixture possessed best gingival fibroblast cellular activity and antimicrobial activity toward periodontal pathogens.

Identification of immunological parameters associated with the alveolar bone level in periodontal patients

  • Purpose: The present study was performed to clarify the relationship between periodontal disease severity and selected immunological parameters consisting of serum IgG titer against periodontopathogenic bacteria, the expression of the helper T-cell cytokine by gingival mononuclear cells, and patients' immunoreactivity to cross-reactive heat shock protein (HSP) epitope peptide from P. gingivalis HSP60. Methods: Twenty-five patients with moderate periodontitis had their gingival connective tissue harvested of gingival mononuclear cells during an open flap debridement procedure and peripheral blood was drawn by venipuncture to collect serum. The mean level of interproximal alveolar bone was calculated to be used as an index for periodontal disease severity for a given patient. Each of selected immunologic parameters was subject to statistical management to seek their correlations with the severity of periodontal disease. Results: A significant correlation could not be identified between serum IgG titers against specific bacteria (Porphyromonas gingivalis, Prevotella intermedia, Fusobacterium nucleatum, Actinobacillus actinomycetemcomitans, and Streptococcus mutans) and the severity of periodontal disease. Expression of interleukin (IL)-10 by gingival mononuclear cells was statistically significant in the group of patients who had higher levels of alveolar bone height. However, a similar correlation could not be demonstrated in cases for IL-4 or interferon-$\gamma$. Patients' serum reactivity to cross-reactive epitope peptide showed a significant correlation with the amount of alveolar bone. Conclusions: It was concluded that expression of IL-10 by gingival mononuclear cells and patients' sero-reactivity to the cross-reactive HSP peptide of P. gingivalis HSP60 were significantly correlated with alveolar bone height.

Antibacterial Effects and Cytotoxicity of Crassirhizomae Rhizoma (관중의 항균작용 및 세포독성에 관한 연구)

  • The purpose of this study was to evaluate the antimicrobial activity of Crassirhzimae rhizoma and its possible use as an oral antiseptics for prevention of periodontitis. Its antibacterial activity against periodontopathic microorganisms including Actinobacillus actiomycetem - comitans, Capnocytophaga ochracea, Streptococcus mutans, Porphyromonas gingivalis, Prevotella intermedia, Actinomyces viscosus, Fusobacterium nucleatumwas evaluated via modified stab culture method. The cytotoxicity against gingival fibroblasts and rat osteoblasts was investigated via [$^3H$]thymidine incorporation and cellular activity was investigated via MTT assay. Chlorhexidine was used as control group. Crassirhizomae rhizoma was prepared at concentrations of 0.2, 0.15, 0.1, 0.05%. Chlorhexidine was also prepared at the same concentration. Crassirhizomae rhizoma showed lower antimicrobial antivity against these microorganism than chlorhexidine, but this difference was not significant. And, Crassirhzomae rhizoma showed more cellular activity and less cytotoxicity than chlorhexidine on human gingival fibrablast and rat osteoblast. This study suggests that Crassirhzomae rhizoma might be a candidate for a safe oral antiseptic for the prevention and treatment of periodontal disease.

Influence of Roasting Time on Antibacterial and Antioxidative Effects of Coffee Extract (배전시간에 따른 커피 추출물의 항균 및 항산화 효과)

  • The influence of roasting time on antibacterial and antioxidative effects of methanol and water coffee extracts was investigated. Extract yield differed with roasting time. The maximum yield of methanol extract was 20.02% and 24.00% at respective roasting times of 12 and 20 min. The maximum yield of water extracts was 2.70% and 18.58% at 5 and 25 min roasting time, respectively. Antibacterial effects of each extract were determined by the classical minimal inhibitory concentration (MIC) paper disc diffusion method. Methanol extracts of different coffee samples inhibited growth of various strains except Escherichia coli. Extracts obtained following roasting times of 12, 14, 16, 20, and 25 min in particular displayed the most potent activity against Staphylococcus aureus. Among these extracts, that obtained from 12 min roasted coffee samples produced a MIC of $16.125{\mu}g$/mL against S. aureus. Water extracts applied at $1,000{\mu}g$/mL were growth inhibitory except against Salmonella choleraesuis and Prevotella intermedia. However, growth inhibition by water extracts was weak, with inhibitory zones of only 6-8 mm diameter produced. Determinations of free radical elimination for the different coffee extracts using 1,1-diphenyl-2-picrylhydrazyl were compared with ascorbic acid and butylated hydroxytoluene positive controls. Methanol and water extracts of different coffee samples ($100{\mu}g$/mL) showed $67.1{\sim}92.3%$ and $66.4{\sim}93.3%$ radical scavenging activity, respectively. However, longer roasting time (especially >20 min) tended to somewhat lower free radical elimination using both extracts. Total phenol in different coffee samples measured by the Folin-Denis method revealed the highest level of phenol contents with non-roasted coffee, whereas phenol content differed with different roasting time, ranging from $87.{\sim}126.5\;mg/g$ in methanol extracts. In water extracts, the phenol content was maximum at 8 min roasting time, whereas in other samples the content was varied from $95.0{\sim}199.1\;mg/g$.

A Study on the Self-Healing Mechanism using Spanning Tree Algorithm for PLC-based Home Network (전력선 홈 네트워크를 위한 스패닝 트리 알고리즘 기반의 자동 중계 기술 연구)

  • The home network environment based on the power line communication has some unexpected events. The noise source from the home appliances makes the network impedance increased, and the transmission packets distorted. Therefore the packet transmission from the source node may not be delivered to the destination node directly. In this paper we propose a new self-healing mechanism which relays the packet to the directly unreachable destination automatically with the help of intermediate nodes. In order to choose the minimum required intermediate nodes our mechanism makes a spanning tree with both the directly reachable nodes and indirectly reachable nodes via the intermedia nodes. We developed the network simulator in order to compare its performance to the Echelon's repeater selecting mechanism. Though it required more packet relations than Echelon's but the reachable probability to the destination was highly increased about two times.

Prevalence and abundance of 9 periodontal pathogens in the saliva of periodontally healthy adults and patients undergoing supportive periodontal therapy

  • Purpose: This study aimed to examine the prevalence and abundance of 9 representative periodontal pathogens in the saliva samples of periodontally healthy subjects (PH) and patients with periodontitis who underwent supportive periodontal therapy (SPT). The age-specific distribution of these pathogens in periodontally healthy individuals was also analyzed. Methods: One hundred subjects (aged >35 years) were recruited (50 each in the PH and SPT groups) between August 2016 and April 2019. The prevalence and abundance of periodontal pathogens in the PH group were compared with those in periodontally healthy young subjects (94 subjects; aged <35 years), who were included in our previous study. DNA copy numbers of Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythia (Tf), Treponema denticola (Td), Prevotella intermedia (Pi), Fusobacterium nucleatum (Fn), Campylobacter rectus (Cr), Peptostreptococcus anaerobius (Pa), and Eikenella corrodens (Ec) were analyzed using real-time polymerase chain reaction. Results: The detection frequencies of all pathogens, except Aa, were high in the PH and SPT groups. The ranking order of pathogen DNA copy numbers was similar in both groups. In both groups, Fn had the highest abundance, Aa had the lowest abundance. Additionally, Td was significantly more abundant in men than in women in both groups (P<0.05). Compared with the PH group, the SPT group exhibited significantly lower total bacteria and Fn abundance and higher Pg abundance (P<0.05). The age-specific pathogen distribution analysis revealed a significantly low Aa abundance and high Tf and Cr abundance in the PH group. Conclusions: The clinical parameters and microbial profiles were similar between the SPT and PH groups. However, patients with periodontitis require supportive care to prevent recurrence. As the abundance of some bacteria varied with age, future studies must elucidate the correlation between age-related physiological changes and periodontal bacterial composition.

Efficacy of salivary versus subgingival bacterial sampling for the detection and quantification of periodontal pathogens

  • Purpose: The aim of this study was to investigate the efficacy and validity of subgingival bacterial sampling using a retraction cord, and to evaluate how well this sampling method reflected changes in periodontal conditions after periodontal therapy. Methods: Based on clinical examinations, 87 subjects were divided into a healthy group (n=40) and a periodontitis group (n=47). Clinical measurements were obtained from all subjects including periodontal probing depth (PD), bleeding on probing (BOP), clinical attachment loss (CAL), and the plaque index. Saliva and gingival crevicular fluid (GCF) as a subgingival bacterial sample were sampled before and 3 months after periodontal therapy. The salivary and subgingival bacterial samples were analyzed by reverse-transcription polymerase chain reaction to quantify the following 11 periodontal pathogens: Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythus (Tf), Treponema denticola (Td), Prevotella intermedia (Pi), Fusobacterium nucleatum (Fn), Pavimonas micra (Pm), Campylobacter rectus (Cr), Prevotella nigrescens (Pn), Eikenella corrodens (Ec), and Eubacterium nodatum (En). Results: Non-surgical periodontal therapy resulted in significant decreases in PD (P<0.01), CAL (P<0.01), and BOP (P<0.05) after 3 months. Four species (Pg, Tf, Pi, and Pm) were significantly more abundant in both types of samples in the periodontitis group than in the healthy group. After periodontal therapy, Cr was the only bacterium that showed a statistically significant decrease in saliva, whereas statistically significant decreases in Cr, Pg, and Pn were found in GCF. Conclusions: Salivary and subgingival bacterial sampling with a gingival retraction cord were found to be equivalent in terms of their accuracy for differentiating periodontitis, but GCF reflected changes in bacterial abundance after periodontal therapy more sensitively than saliva.

A Convergence Implementation of Realtime Traffic Shaping and IPS on Small Integrated Security Router for IDC (IDC용 소형 통합보안라우터의 실시간 트래픽쉐이핑과 IPS의 융합 구현)

  • Various server-based services such as big data, IoT and artificial intelligence have been made online. As a result, the demand for IDC to support stable server operation is increasing. IDC is a server-based facility with a stable line and power supply facility that manages 20 to 30 servers in an efficiently separated rack-level subnetwork. Here, we need a way to efficiently manage servers security, firewall, and traffic on a rack-by-rack basis. Including traffic shaping capabilities that control routers, firewalls, IPS, and line speeds, as well as VPN technology, a recent interest. If three or five kinds of commercial equipment are adopted to support this, it may be a great burden to the management cost as well as the introduction cost. Therefore, in this paper, we propose a method to implement the five functions in one rack-unit small integrated security router. In particular, IDC intends to integrate traffic shaping and IPS, which are essential technologies, and to propose the utility accordingly.
