전치부 개교합을 동반한 골관절염 환자에 대한 악간견인장치의 응용 (A Case Report on the Treatment of A TMJ Osteoarthritis Patient with Anterior Open Bite Using An Intermaxillary Traction Device)

  • A patient with TMJ osteoarthritis and anterior open bite was treated with an intermaxillary traction device. Pretreatment examination revelaed a pain in both TMJ during mouth opening, moderate tendernesso f left sternocleidomastoid and right trapezius muscles. Anterior open Bite was aobserved with interincisal distance of 2mm. Tomograms and MRI showed anterior disc displacement withouit reductoin of both temporomandibular joints, and the condyles were flattened and slightly eroded. A pair of full-coverage occlusal appliances was made on both maxillary and mandibular dentition, with pivoting fulcrum on the site of the second moalr. Traction force was gained by the intermaxillary orthodontic elastics which were hooked by orthodontic brackets on the labial surfaces of the upper and lower anterior and premolar teeth. After 8 weeks of traction treatment, the joint pain was subsided completely and the anterior open bite was closed to get an edge to edge relationship of anterior teeth.

Anterior open bite with temporomandibular disorders treated with intermaxillary traction using skeletal anchorage system

  • Objectives: The anterior open bite with temporomandibular disorders (TMD) is one of the most challenging cases both orthodontically and surgically. We introduce an intermaxillary traction treatment for patients with anterior open bite and TMD using a skeletal anchorage system (SAS). Materials and Methods: This study was comprised of 52 patients with anterior open bite and TMD. A total of four mini-screws were inserted, two screws each into the maxilla and mandible, to obtain a class II pattern of elastic application with 120-200 g force. Adjunctive muscle relaxation treatments, such as splint therapy, medication, and botulinum toxin injection were applied during or before intermaxillary traction. At least one treatment among adjunctive muscle relaxation treatment, mentioned above, was applied to 96.2% of patients. We evaluated the clinical characteristics of patients, TMD symptom changes, amount of open bite improved. The degree of open bite improvement was compared between the open bite-reduced group (21 patients) and not-reduced group (5 patients). Results: TMD symptoms (muscle/joint pain, joint sound, mouth opening) remained or improved in most patients, and worsened in about 10% of patients for each items. Anterior open bite was improved by a mean of 1.75 mm (P<0.01) during treatment. The open bite-reduced group exhibited a significant open bite improvement compared to the not-reduced group (P<0.05), with 37% of open bite improvement occurring during the first 3 months of treatment. Conclusion: The intermaxillary traction technique using SAS is a valid modality for correction of anterior open bite and improvement of TMD symptoms.

  • Temporomandibular joint osteoarthritis (TMJ OA) causes destruction of the temporomandibular joint (TMJ) and can lead to occlusal changes such as anterior open bite in some patients. Consequently, these patients may experience difficulty in chewing food items and exhibit abnormal functional habits such as bruxism, preventing healing of the TMJ condyles. Treatment protocols include the use of traction appliances to reduce stress on the condyles. Unstable occlusions can lead to weakness of the masticatory muscles which, in turn, worsen the occlusal changes and complicate pain management. Therefore, the current study evaluated the condition of the masseter muscle using ultrasonography and educated patients on the execution of gum-chewing exercises for muscle strengthening. It also aimed to assess the effects of traction appliances and strengthening exercises on the masticatory muscles of patients with occlusal changes caused by TMJ OA.

  • The new type of orthodontic mini-implant named C-implant can be an effective alternative to conventional one-component mini-implant in the intermaxillary fixation (IMF) cases because of its particular design. The small size, two-part design, efficiency, and low cost of the C-implant make it applicable to various types of IMF cases easily such as fracture reduction and orthognathic surgery. The two part design resists highly to the fracture or deformation during implantation and removal. The long span head allows the patient to easily attach intermaxillary elastics, so that the patient can apply intermaxillary elastics for traction easily. Through this article, we tried to show the possibility of this appliance as a good adjunct for the IMF screw.

  • Temporomandibular joint disorder(TMJD) was mainly characterized with joint pain, motion limitation, joint sound, resulted from pathologic conditions in temporomandibular joint and around tissue. As temporomandibular joint is one of decisive factors determining the occlusion, disorders in temporomandibular joint may cause the occlusal changes. The causes of occlusal changes related with TMJD can be classified into 2 categories; (1) those related to progression of disorder, 2) those related to treatment of the disorder. The clinical manifestation of occlusal changes depend on their causes and affected site. Therefore, whenever possible, treatment should be directed to the relief of the underlying causes, However, it is not always possible to relieve the underlying conditions. Moreover, some occlusal changes may remain irreversible even after the considerable improvement in clinical symptoms. Regarding the treatment of the permanent occlusal changes, it has been reported that the extensive occlusal treatment including occlusal adjustment, prosthodontic treatment, orthodontic treatment should be applied. Here, we present with a case report of occlusal change caused by the progressive temporomandibular joint disorder, together with introducing the intermaxillary traction appliance as the possible treatment option.

급성 비정복성 관절원판 변위에 따른 급성 교합변화의 증례 (Acute Occlusal Change Following Acute Anterior Disc Displacement without Reduction: A Case Report)

  • 35세 여자환자가 갑작스런 개구장애와 함께 발생한 급성 교합변화를 주소로 내원하였다. 환자는 이전 수년 동안의 관절잡음의 병력을 가지고 있었으며 관절잡음의 소실과 동시에 개구제한이 발생하였음을 보고하였다. 환자는 개구제한과 함께 개구시 우측 악관절의 통증을 호소하였으며 구강내 교합 검사 시 전치부의 개교합과 함께 하악이 우측으로 전위된 양상이 관찰되었다. 또한 좌측으로의 측방운동량 또한 감소된 것으로 관찰되었다. 자기공명영상에서 우측 악관절에서 비정복성 관절 원판 전방 변위가 관찰되었으며 후방 관절강의 협착이 관찰되었다. 이 환자는 우측 악관절의 비정복성 관절원판 전방변위로 진단되었으며, 급성 교합변화는 변위된 관절원판에 의해 야기된 과두위치의 변화로 초래된 것으로 추정하였다. 처음 3개월간의 물리치료와 약물치료와 병행된 안정교합장치요법에서는 만족할 만한 치료결과를 얻지 못하였으나, 그 다음 약 8개월간 사용된 악간견인장치 치료를 통해서 통증 및 교합변화가 해소되었으며 정상적인 개구량으로 회복되었다. 술후 촬영된 자기공명영상에서 우측 과두가 재형성된 양상을 관찰할 수 있었다.

  • Treatment of the mandibular fracture consists of reduction and fixation. The apparatus that is used to keep the jaws together during healing will often reduce the fracture as well. When the jaws are brought together and intermaxillary elastic rubber traction is placed, the occlusion of the teeth will help to orient the fractured parts into good position. Intermaxillary fixation, that is, fixation obtained by elastic bands between the upper & lower jaws to which suitable anchoring devices have been attached, will successfully treat most fractures of the mandible. Arch bars are perhaps the ideal method for intermaxillary fixation. Several types of ready-made arch bars are used. But, daily occupational life and oral hygiene is difficult to maintain during the period of longterm immobilized intermaxillary fixation (commonly 6-8 weeks), owing to malnutrition and emotional disorders in a position of the patient with mandibular fractures. Most mandibular fractures heal well enough to allow removal of fixation in about 6 weeks. Though there are many complications of mandibular fracture, such as infection, hemorrhage, trismus, paresthesia and nonunion, it is favorable to attain the short-term removable intermaxillary fixation care by use of an additive incision & drainage establishment on the oral lacerated wounds of adjacent mandibular compound fractures. The purpose of an additive incision & drainge establishment is the prevention of wound infection & nonunion by removing the hematoma & seroma in the fracture sites.

안면골 골절에서 상하악 치열궁 복원을 위한 양측 대구치간 철사견인술의 유용성 (The usefulness of intermolar traction wiring for restoration of maxillary & mandibular dental arch in facial bone fracture)

  • Purpose: Palatal fracture and mandible fracture result in instability of dental arch. Because they divide the maxillary and mandibular alveolus sagittally and / or transversely and comminute the dentition, they permit rotation of dental alveolar segments and significantly increase the potential for fracture malalignment, complicating fracture treatment. Previous treatment of palatal fracture consisted of palatal splint application and rigid palatal vault stabilization. This procedure result in patient's oral discomfort and removal of palate and screw. Mandible fracture often results in malocclusion due to widening of posterior aspect of dental arch. So we introduce more simple method using intermolar traction wiring, which can protect the widening of dental arch and rotation of dental alveolar segment. Methods: Arch bar and intermolar traction wiring with wire 1 - 0, or 2 - 0 was applied. After exposure of fracture line, neutrooclusion was maintained with intermaxillary fixation. And then open reduction & internal fixation on maxillary fracture line, commonly maxillary buttress, alveolar ridge, pyriform aperture except palatal vault or mandibular fracture line. After 1 week, intermolar traction wiring was removed. We checked occlusion and postoperative radiologic finding. Results: From June of 2007 to October of 2007, 10 patient, who have maxillary fracture with palatal fracture and mandible fracture, underwent open reduction & internal fixation with intermolar traction wiring. All have satisfactory occlusion and there were no complication, like gingiva disease, mouth opening impairment and nonunion. Conclusion: The intermolar traction wiring accompany open reduction and internal fixation can be alternative method for restoration of dental arch in facial bone fracture.

혼합치열기 반대교합자에서 III급 악간견인력의 효과에 관한 연구 (THE EFFECTS OF CLASS III INTERMAXILLARY TRACTION IN MIXED DENTITION WITH ANTERIOR CROSSBITES)

  • The purpose of this study is to observe how the Class III intermaxillary elastics act upon the craniofacial structures of mixed dentition with anterior crossbites. The cephalometric headplafes of 16 childrens treated only with Class III elastics (C III elastics group) and 23 childrens treated with Class III elastics and Reverse pull headgear simultaneously (C III elastics + RPHG group) were traced, digitized and statistically analyzed. The results were as follows. 1 . Anterior displacement of maxilla was observed in both groups. 2. Simultaneous Class III elastics and reverse pull headgear group showed the counterclockwise rotation of maxilla, however Class III elastics group manifested no rotational change of maxilla. 3. Simultaneous Class III elastics and reverse pull headgear group showed the downward and backward rotation of mandible, however Class III elastics group manifested no rotational change of mandible. 4. Counterclockwise canting of occlusal plane, labial tipping of upper incisor and lingual tipping of lower incisor and mesial displacement of upper molar to pterygoid root vertical were observed in both groups.
