A Clinical Study for Promoting Quality Nusing Care in a University Hospital (질적 간호제공을 위한 간호단위 시범 운영 효과에 관한 임상적 연구)

  • The purpose of this study was to develop a new nursing unit which can meet changing health care needs, enhance patients' satisfaction and nurses' job satisfaction, and finally guarantee quality nursing care with present manpower. For this, one medical unit was selected as a unit for quality care. And one medical unit which is similar in staffing and patients' characteristics was selected as a control unit. To assess present problems and identify the remedies to the problems a hospital-wide survey and a workshop were performed. According to the survey results, educational programs and improvement of the facilities and equipment supply system, managereal support for interdepartmental cooperation and intensification of bed-side nursing care were adopted as main principles for operating model unit, This model unit was operated for 3 months from Sep. 1, 1992 to Nov. 30, 1992. To evaluate the effectiveness of the model unit, derect/indirect nursing care hours, patients' satisfaction to nursing care, nurses' job satisfaction, and quality care index were measured. Direct/indirect nursing care hours were compared with that of the control unit, and patients' and nurses' satisfaction and quality care index were measured before and after operating model unit and compared with each other. The results of the study were as follows; 1. In the model unit mean direct nursing care hours per cach shift was 146.88 minutes and indirect nursing care hours was 354.72 minutes. The ratio of the direct nursing care hour to indirect nursing hour was 29.6 ; 70.4 and that of the control unit was 26.9 : 73.1. Direct nursing care hour in model unit was longer than that of the control unit. But, the difference was not significant. In subcategories of direct nursing care, the time spent in mobility and exercise, conservation of body temperature, hygiene, and communication and health education were longer than that of the con" trol unit. 2. Indirect nursing care hour in model unit was shorter than that of the control unit. But, the difference was not significant. In subcategories of indirect nursing care, the time spent in drug management and ward arrangement was shorter than that of the control unit. 3. Patients' satisfaction to nursing care was increased significantly after operating the model unit (T=-3.48, P=-0.002) and satisfaction to subcategories of physical comfort measure, psychological cate, and unit management components were significantly higher than before. 4. In the model unit, nurses' total job satisfaction was increased significantly after operating the model unit(Z=2.1004, P=.0357) and satisfaction to subcategory of satisfaction to administration was significantly higher than before (Z=-2.0732, P=.0382). 5. After operating the model unit, quality care index was increased from 89 to 93. With this results, it can be summarized that all the measures tried for quality care, such as educational programs, managereal support for interdepartmental cooperation, and improvement of the equipment and facility provision resulted in partial increase in direct nursing care hours, nurses satisfaction to their job and patients' satisfaction to nursing care. In can be postulated that managereal support and motivation without proper staff supplementation is not enough for increasing direct nursing care hours. And for the enhancement of the level in clinical nursing, and staff supplement must be considered sincerely and the measures for reducing indirect nursing care hours, such as computerization of nursing care activities, improvement of facilities and equipment and facilities supply system, must be instituted in addition.

Study of Chinese Propaganda Paintings from 1949 to 1966: Focusing on Oil Paintings and Posters (1949년~1966년 시기 중국 선전화 연구 - 유화와 포스터를 중심으로)

  • The propaganda paintings in oil colors or in forms of posters made from 1949 to 1966 have gone through some changes experiencing the influence of the Soviet Union Art and discussion of nationalization, while putting political messages of the time in the picture planes. The propaganda paintings which have been through this process became an effective means of encouraging the illiterate people in political ideologies, production, and learning. Alike other propaganda paintings in different mediums, the ones which were painted in oil colors and in the form of posters have been produced fundamentally based on Mao Zedong's intensification of the literary art on the talks on literature at Yenan. Yet, the oil paintings and posters were greatly influenced by the socialist realism and propaganda paintings of the Soviet Union, compared to other propaganda paintings in different mediums. Accordingly, they were preponderantly dealt in the discussions of nationalization of the late '50s. To devide in periods, the establishment of People's Republic of China in 1949 as a diverging point, the propaganda paintings made before and after 1949 have differences in subject matters and styles. In the former period, propaganda paintings focused on the political lines of the Communists and enlightenment of the people, but in the latter period, the period of Cultural Revolution, the most important theme was worshiping Mao Zedong. This was caused by reflection of the social atmosphere, and it is shown that the propaganda painters had reacted sensitively to the alteration of politics and the society. On the side of formalities, the oil paintings and posters made before the Cultural Revolution were under a state of unfolding several discussions including nationalization while accepting the Soviet Union styles and contents, and the paintings made afterwards show more of unique characteristics of China. In 1956, the discussion about nationalization which had effected the whole world of art, had strongly influenced the propaganda paintings in oil colors more than anything. There were two major changes in the process of making propaganda paintings in oil colors. One was to portray lives of the Chinese people truthfully, and the other was to absorb the Chinese traditional styles of expression. After this period, the oil painters usually kept these rules in creating their works, and as a result, the subject matters, characters, and backgrounds have been greatly Sinicized. For techniques came the flat colored surface of the new year prints and the traditional Chinese technique of outlining were used for expressing human figures. While the propaganda paintings in oil colors achieved high quality and depth, the posters had a very direct representation of subject matters and the techniques were unskilled compared to the oil paintings. However, after the establishment of People's Republic of China, the posters were used more than any other mediums for propagation of national policy and participation of the political movements, because it was highly effective in delivering the policies and political lines clearly to the Chinese people who were mostly illiterate. The poster painters borrowed techniques and styles from the Soviet Union through books and exhibitions on Soviet Union posters, and this relation of influences constantly appears in the posters made at the time. In this way, like the oil paintings, the posters which have been made with a direct influence of the Soviet Union had developed a new, sinicised process during the course of nationalization. The propaganda paintings in oil colors or in forms of posters, which had undergone the discussion of nationalization, had put roots deep down in the lives of the Chinese people, and this had become another foundation for the amplification of influences of political propaganda paintings in the following period of Cultural Revolution.

  • Agricultural sciences field in South Korea has many strong points such as numerous researchers, establishment of research infra-structure, excellence in research competitiveness and high technological level. However, there are also many weaknesses including insufficient leadership at related societies and institutes, deficiency of the next generation research group, and insufficiency in research productivity. There are many opportunities including increasing the importance of the biotechnological industry, activating international cooperation researches, and exploring the multitude of possible research areas to be studied. However, some threats still exist, such as pressure from the government of developed countries to open the agricultural market, the decrease of specialized farms, and intensification for researches to gratify economic and social demands. To encourage research activities in the agricultural sciences field in Korea, the following actions and systems are required: 1) formulation of a mid- and a long-term research master plan, 2) development of a database on the man power in related fields, 3) activation of top-down research topics, and associated increase of individual research grants, 4) development of special national programs for basic researches in agricultural sciences, 5) organization of a committee for policy and planning within the related societies, and 6) system development for the fair evaluation of the research results.

The Effect of Combined Estrogen/calcium Therapy on Bone Metabolism in Ovariectomized Rats I. A Study on Biochemical Parameters in Ovariectomized Rats (에스트로겐/칼슘 혼합요법이 난소절제 쥐의 골대사에 미치는 영향 I. 생화학적 변화에 관한 연구)

  • This study was implemented in order to investigate the effects of dietary calcium-salt, estrogentreatment, and estrogen/calcium treatment on bone metabolism. Ovariectomized rats were used as animal models. Female Sprague-Dawley rats with a body weight of 250~280g were underwent ovariectomy or sham-operation. The ovariectomized rats were divided into 9 different experimental groups including the saline-treated group, the estrogen-treated group, the high calcium salt-treated group, and the estrogen/calcium treated groups and fed for 6 weeks. Creatinine and hydroxyproline in urine were analyzed. Creatinine, calcitonin, osteocalcin, alkaline phosphatase and parathyroid hormone in plasma were also determined. The results of the experiment are as follows : The ovariectomy caused a significant increase in the level of food intake, food effciency ratio and body weight gain in comparison with sham-operation. The overall food intake, food effciency ratio and body weight gain were significantly decreased by estrogen. The ovariectomized animals developed obesity as a result of increased food intake. In addition, estradiol injections suppressed food intake with a concomitant loss in body weight. The level or urinary hydroxyproline, as an indicator or bone resorntion, was higher in the ovariectomized rats compared to sham-opertion, while these decresed in the estrogen/calcium treated group. Parathyroid hormone and calcitonin in the plasma, that were used as the indicator of calcium homeostasis, parathyroid hormone higher in the ovariectomized rats compared to sham-operation. It was lowered by estrogen and high calcium treated groups; thus, estrogen and estrogen/calcium treated groups were decreased by 32% compared to saline treated group. Osteocalcin and alkaline phosphatase which are indicators of bone formation, were significantly higher in ovariectomized group, while this showed to be decreased in the estrogen and the ostrogenicalcium treated groups. Estrogen and estrogen/calcium in ovariectomized rats resulted in lower bone loss. However, estrogen treated group its gradual reduction showed little effect on bone loss, while the gradual reduction of estrogen had a preventive effect on bone loss when the treatment was combined with calcium intensification.

Optimal Spatial Scale for Land Use Change Modelling : A Case Study in a Savanna Landscape in Northern Ghana (지표피복변화 연구에서 최적의 공간스케일의 문제 : 가나 북부지역의 사바나 지역을 사례로)

  • Land Use and Land Cover Changes (LUCC) occur over a wide range of space and time scales, and involve complex natural, socio-economic, and institutional processes. Therefore, modelling and predicting LUCC demands an understanding of how various measured properties behave when considered at different scales. Understanding spatial and temporal variability of driving forces and constraints on LUCC is central to understanding the scaling issues. This paper aims to 1) assess the heterogeneity of land cover change processes over the landscape in northern Ghana, where intensification of agricultural activities has been the dominant land cover change process during the past 15 years, 2) characterise dominant land cover change mechanisms for various spatial scales, and 3) identify the optimal spatial scale for LUCC modelling in a savanna landscape. A multivariate statistical method was first applied to identify land cover change intensity (LCCI), using four time-sequenced NDVI images derived from LANDSAT scenes. Three proxy land use change predictors: distance from roads, distance from surface water bodies, and a terrain characterisation index, were regressed against the LCCI using a multi-scale hierarchical adaptive model to identify scale dependency and spatial heterogeneity of LUCC processes. High spatial associations between the LCCI and land use change predictors were mostly limited to moving windows smaller than 10$\times$10km. With increasing window size, LUCC processes within the window tend to be too diverse to establish clear trends, because changes in one part of the window are compensated elsewhere. This results in a reduced correlation between LCCI and land use change predictors at a coarser spatial extent. The spatial coverage of 5-l0km is incidentally equivalent to a village or community area in the study region. In order to reduce spatial variability of land use change processes for regional or national level LUCC modelling, we suggest that the village level is the optimal spatial investigation unit in this savanna landscape.

A Study on the Policy of Reserved Forests in Korea - mainly, on the designation and cancellation of reserved forests - (보안림정책논고(保安林政策論考) - 보안림(保安林)의 지정(指定) 및 해제(解除)를 중심(中心)으로 -)

  • In this study, the present institution of reserved forests in Korea has been criticized through the analysis of the present situation of reserved forests in Korea, and mainly, on designation and cancellation of them because of this important institution thought as restriction of forest ownership. Reserved forest land in Korea as of the end of 1962 is 996,915 chungbo in area, or about 14.8% of the total forest land area, 6,750,324 chungbo in Korea, and we can find that the area of reserved forest land has increased remarkably since the Liberation in 1945, in comparison with about 180,000 chungbo-a little over 1% of the total forest land area, 16,000,000 chungbo, through Southern and Northern Korea till the Liberation in 1945. This fact clearly proves that Korean forests are extremely devastated since the Liberation in 1945, and in Korea we can find that reserved forest policy is very important in forest policy, consequently, reserved forest institution must be dealt with care. Moreover, the area of reserved forest land, 996,915 chungbo, which is divided into 43,820 chungbo of national forest land, 59,302 chungbo of public forest land, 893,793 chungbo of private forest land, and private forest land is excellently large, or about 89.7% of the total area of reserved forest land. In this number, we may understand the fact that reserved forests have the most influences on private forests, therefore, we may recognize that it is necessary for reserved forest constitution which is infringement of private right to be carried out carefully. From the first beginning, the institution of reserved forests is serious restriction to the forest ownership. Consequently, when the area of reserved forest land grows, it interferes seriously with the free forest management and the desire for forest own ership is decreased, at the same time, forest enterprise results in obstruction. Especially, Korean forests are destroyed extremely at present, so, intensification of reserved forest institution is unavoidable for completion of the national aim which forests have, but the author thinks that reserved forest institution must be as avoidable as possible, and we have to obtain good results by supervision of forest management which is regulated in the Forest Law. Consequently, designation of reserved forests must be minimized, and although forests were already designated as reserved forests they must be cancellated as fast as fast as possible and put them free in the owner's hands when they are in cancellation conditions. According to the provision of Article 18 of the Forest Law concrete cases designated as a reserved forest are enumerated for the purpose of maintaining the forest ownership and avoiding to give the forest authorities a free hand in order to protect forest owners from one-sided damage. Therefore, the forest authorities must not abuse the institution of reserved forests, and it is not good tendency to give only the authorities a free hand in eesignation and cancellation of reserved forests, and especially, when the forest owners object to that, establishing some legal organization like the reserved forest council in each province in order to hear about impartial opinions, and it is more suitable than administrative disposal by the same organization. The compensation of damages for reserved forests by the provision of Article 25 of the Forest Law is a different problem by forest policy, but the results of compensation of damages regulated in the Forest Law are wholly lacking up to now, the author thinks that this is caused to poor forest cover, the forest owner's unconcern and insincerity of administrative authorities. Therefore, the government must enlarge the range of compensation and minimize the forest owner's economic sacrifice, also, the government must mollify the conditions of the legal restrictions to reserved forests, and harmonize with functions of national conservation and economy. This means that it is necessary to modify the restrictive conditions for the effective utilization of forest resources within the range in which can be attained the purpose of designation, from permanent prohibition of cutting. Except the reserved forests of fish habitat, public sanitation, maintaining scenery and navigational mark ect., most of reserved forests are prohibited from cutting, and the present situation of forests in Korea are extremely devastated and those forests are not so expected in cancellation possibility in near future. Therefore, when the forest owners apply for national purchase of those reserved forests, the government had better nationalize them, protect and manage to reduce the forest owner's economic sacrifice.

Recent Spatial and Temporal Changes in Means and Extreme Events of Temperature and Precipitation across the Republic of Korea (최근 우리나라 기온 및 강수 평균과 극한 사상의 시.공간적 변화)

  • In this study, the spatial and temporal patterns of changes in means and extreme events of temperature and precipitation across the Republic of Korea over the last 35 years (1973-2007) are examined. Over the study period, meteorological winter (December-February) mean minimum (maximum) temperature has increased by $+0.54^{\circ}C$/decade ($+0.6^{\circ}C$/decade), while there have been no significant changes in meteorological summer (June-August) mean temperatures. According to analyses of upper or lower $10^{th}$ percentile-based extreme temperature indices, the annual frequency of cool nights (days) has decreased by -9.2 days/decade (-3.3 days/decade), while the annual frequency of warm nights (days) has increased by +4.9 days/decade (+6.8 days/decade). In contrast, the increase rates of summer warm nights (+8.0 days/$^{\circ}C$) and days (+6.6 days/$^{\circ}C$) relative to changes in summer means minimum and maximum temperatures means are greater than the decreasing rates of winter nights (-5.2 days/$^{\circ}C$) and days (-4.3 days/$^{\circ}C$) relative to changes in winter temperatures. These results demonstrate that seasonal and diurnal asymmetric changes in extreme temperature events have occurred. Moreover, annual total precipitation has increased by 85.5 mm/decade particularly in July and August, which led to the shift of a bimodal behavior of summer precipitation into a multi-modal structure. These changes have resulted from the intensification of heavy rainfall events above 40mm in recent decades, and spatially the statistically-significant increases in these heavy rainfall events are observed around the Taebaek mountain region.

A Characteristics and Improvement of Thermal Environment in Summer of Protected Horticulture Complex Using CFD Simulation (CFD 시뮬레이션을 이용한 시설원예단지 여름철 외부 열환경 특성 및 개선방안)

  • Agricultural or rural landscape provides various ecosystem services. However, the ecosystem services function is declining due to various environmental problems such as climate change, land use change, stream intensification, non-point pollution and garbage. The A1B scenario predicts that the mean air temperature of South Korea will rise $3.8^{\circ}C$ degrees celsius in 2100. Agricultural sector is very vulnerable to climate change, so it must be thoroughly predicted and managed. In Korea, the facility horticulture complex is 54,051ha in 2016 and is the 3rd largest in the world(MAFRA, 2014). Facilities of horticultural complexes are reported to cause problems such as groundwater decrease, vegetation and insects diversity reduction, landscapes damage and garbage increase, compared with the existing land use paddy fields. Heat island phenomenon associated with climate change is also accelerated by the high heat absorption of horticultural sites. Therefore, we analyzed the heat island phenomenon occurring in the facility of horticultural complex in Korea. As an improvement measurement, I examined how much air temperature is reduced by putting the channel and the open space. In the case of the Buyeo area, the Computational Fluid Dynamics (CFD) simulation was analyzed for the average summer temperature distribution in the current land use mode at $38.9^{\circ}C$. As an improvement measurement, CFD simulation after 10% of 6m water channel was found to have an effect of lowering the summer temperature of about $2.7^{\circ}C$ compared with the present average of $36.2^{\circ}C$. In addition, CFD simulations after analyzing 10% of the open space were analyzed at $34.7^{\circ}C$, which is $4.2^{\circ}C$ lower than the present. For the Jinju area, CFD simulations were analyzed for the average temperature of summer at $37.8^{\circ}C$ in the present land use pattern. As an improvement measure, CFD simulations after 10% of 6m water channel were found to have an effect of lowering the summer temperature of about $2.6^{\circ}C$ compared to the current average of $35.2^{\circ}C$. In addition, CFD simulations after analyzing 10% of the open space were analyzed at $33.9^{\circ}C$, which is $3.9^{\circ}C$ lower than the present. It can be said that the effect of summer temperature drop in open space and waterway has been proven. The results of this study are expected to be reflected in sustainable agriculture land use and used as basic data for government - level policy in land use planning for climate change.

Estimating Fine Particulate Matter Concentration using GLDAS Hydrometeorological Data (GLDAS 수문기상인자를 이용한 초미세먼지 농도 추정)

  • Fine particulate matter (PM2.5) is not only affected by anthropogenic emissions, but also intensifies, migrates, decreases by hydrometeorological factors. Therefore, it is essential to understand relationships between the hydrometeorological factors and PM2.5 concentration. In Korea, PM2.5 concentration is measured at the ground observatories and estimated data are given to locations where observatories are not present. In this way, the data is not suitable to represent an area, hence it is impossible to know accurate concentration at such locations. In addition, it is hard to trace migration, intensification, reduction of PM2.5. In this study, we analyzed the relationships between hydrometeorological factors, acquired from Global Land Data Assimilation System (GLDAS), and PM2.5 by means of Bayesian Model Averaging (BMA). By BMA, we also selected factors that have meaningful relationship with the variation of PM2.5 concentration. 4 PM2.5 concentration models for different seasons were developed using those selected factors, with Aerosol Optical Depth (AOD) from MODerate resolution Imaging Spectroradiometer (MODIS). Finally, we mapped the result of the model, to show spatial distribution of PM2.5. The model correlated well with the observed PM2.5 concentration (R ~0.7; IOA ~0.78; RMSE ~7.66 ㎍/㎥). When the models were compared with the observed PM2.5 concentrations at different locations, the correlation coefficients differed (R: 0.32-0.82), although there were similarities in data distribution. The developed concentration map using the models showed its capability in representing temporal, spatial variation of PM2.5 concentration. The result of this study is expected to be able to facilitate researches that aim to analyze sources and movements of PM2.5, if the study area is extended to East Asia.

A Pursuit of Innovation in the Korean Genetics-Genomics Research System through a Culturalist Strategy (문화적 전략을 통한 한국 유전학-유전체학 연구체계의 혁신 모색)

  • The Korean genetics and genomics reveal a firm willingness to participate in and contribute to the production of creative scientific knowledge at a world level at present, though they have short past histories of introduction from the Western counterparts and those of education for the next generations. But the Korean genetics and genomics have been developed in a fragmented and biased manner. By reconfiguring the various research projects of genomics into the Genome Project of Korea, which reflect a worldly trend in life science, but have been established in a scattered fashion in Korea, and incorporating some neglected areas of genetics, such as human genetics and theoretical and population genetics which can be reconstructed in a new way, a genetics-genomics research system can be formulated on the multi-tiered perspective of concept, knowledge, and institution, while the system being a subsystem of the national research system of life science in Korea. Innovation can be pursued in the systematic practice through a culturalist strategy. The culturalist strategy with the practice based on the research system consists of 1) intensification of fundamentalness of genetics and genomics, 2) advancement of communitarianism in geneticist-genomicist community, 3) research on the cultural bio-species along with the promotion of scientific arts and culture, and 4)formation of the Korean science studies of genetics-genomics and the diffusion of the knowledge produced. The first two strategy components are the ones that intends to bring out changes in the structural aspect of the scientist community in Korea. The third is the one that attempts to magnify the interface between the scientist community and the Korean society at large and increase its connectivity between both, while the fourth is the one that has an intentionality toward the Korean society outside of the scientist community. This culturalist strategy is intended to increase the cultural constructivity of the genetics-genomics research system in Korea.
