Effect of Virtual Reality Rehabilitation Program with RAPAEL Smart Glove on Stroke Patient's Upper Extremity Functions and Activities of Daily Living (라파엘 스마트 글러브를 이용한 가상현실 재활프로그램이 뇌졸중환자의 상지 기능과 일상생활활동 수행에 미치는 영향)

  • Purpose : This study examined the effects of a virtual reality rehabilitation program on stroke patients' upper extremity functions and activities of daily living (ADL). Methods : The subjects were equally and randomly divided into an experimental group (n=16) to whom a virtual reality rehabilitation program was applied and a control group (n=16) who received traditional occupational therapy. The intervention was applied five times per week, 30 minutes per each time, for six weeks. Jebsen-Taylor hand function test was conducted and the subjects' Manual Function Test was measured to examine their upper extremity functions before and after the treatment intervention, and a Korean version of modified Barthel index was calculated to look at their activities of daily living. Results : After the intervention, the upper extremity functions and activities of daily living of the participants in both groups significantly improved (p<.05). However, the improvements in these parameters among the participants in the virtual reality rehabilitation program were significantly greater than those in the control group (p>.05). Conclusion : The virtual reality rehabilitation program is a stable and reliable intervention method for enhancing the upper limb functions and activities of daily living of stroke patients.

Simvastatin Induces Osteogenic Differentiation and Suppresses Adipogenic Differentiation in Primarily Cultured Human Adipose-Derived Stem Cells

  • Recent in vitro and in vivo animal studies have reported that statin, a cholesterol-lowering drug, stimulate osteogenic differentiation. In the present study, we investigated the effect of simvastatin on osteogenic and adipogenic differentiation in primarily cultured human adipose-derived stem cells (hADSCs). The simvastatin treatment significantly increased the positive cell numbers in alkaline phosphatase and von Kossa staining, and enhanced the expression levels of bone morphogenic protein (BMP)-2, core binding factor alpha 1 (cbfa1), collgen type I and osteonectin mRNAs. Lastly, hADSCs were cultured in the adipogenic media with or without simvastatin to examine the effect of simvastatin on adipogenic differentiation. In the RT-PCR analysis, there were notable decreases in mRNA expression of aP1, C/EBP-$\alpha$ and PPAR-$\gamma$ in hADSCs cultivated in simvastatin-added medium, compared to those in simvastatin-free medium. It suggests that the adipogenic differentiation was significantly inhibited by simvastatin treatment. These observations indicate that simvastatin induces osteogenic differentiation and suppresses adipogenic differentiation in hADSCs.

Preliminary Study for Development of Korean Medicine Clinical Practice Guideline for Pancreatic Cancer (췌장암 한의 임상진료지침 개발 예비 연구)

  • Objectives: The purpose of this study is to investigate preliminarily for development of the Korean medicine clinical practice guideline (CPG) for pancreatic cancer through the analysis of existing CPGs. Methods: Through searching the medical database such as Pubmed, SCOPUS, CNKI, Google Scholar, etc. The global CPGs within recent three years were collected and analyzed. In particular, recommendations related to the Korean medicine or Chinese medicine were made primarily in the Guidelines of Diagnosis and Therapy in Oncology with Traditional Chinese Medicine. Results: The six CPGs were mentioned in this study. The academic societies and organizations developing the CPGs were located in China, Japan, Europe and America. The contents of the CPGs were the clinical questions and statements, surgical therapy, adjuvant therapy, radiation therapy, chemotherapy, palliative medicine, risk assessment, palliation and supportive care, follow-up and recurrence, Tumor-Node-Metastasis (TNM) staging. In the Guidelines of Diagnosis and Therapy in Oncology with Traditional Chinese Medicine, the etiology, mechanisms, herbal drugs, Chinese medicine assessment, complication, syndrome differentiation (SD), Chinese medicine treatment were described. Conclusions: In order to develop the proper Korean medicine CPG for pancreatic cancer and to adapt the correct integrative treatment program on the pancreatic cancer, institutional arrangements for cooperation with Korean medical communities and standardization of SD should be performed.

  • Objectives: To grasp the traditional stream of cancer treatment inherited from the previous doctors. Methods: The author's research has been performed cancer treatment based on JIJU(積聚) taking the original text as a reference. Results & Conclusion: The general three outlines and five detailed rules of cancer treatment have been obtained as follows. The first outline of cancer treatment is that reinforced vital function makes cancer reduce naturally. the second is that Harmless cancer can coexist in human beings and aging with them. and the third is that Elimination and reinforcing therapies should be executed in appropriate era and those therapies should utilize appropriate methods. The first detailed rule of cancer treatment is when using reinforcing therapy, it must applicate mildly and when using elimination therapy, it must applicate calmly. The second detailed rule is that the methods of cancer treatment are different from each cancer stage. The concentration should be made on reinforcing therapy at early stage while reinforcing and elimination therapies must be conducted together at middle stage. At terminal stage reinforcing therapy is the sole method to be taken. The third detailed rule is that the basis property of cancer drug is warm nature and extremely biased property should be avoided and when complication arises (eg. inflamatory disease, cancer fever, etc), cold or cool nature can be applied. The fourth detailed rule is that Cancer drug must have the effect eliminating the blood stasis, phlegm and excessive fluid, all together. The fifth detailed rule is that Physicians have to control patient's stress or stress related symptom and teach patients about right way of taking care of themselves and patients should take hygienic rules with their free will by themselves (eg diet, exercise, stress, etc)

Prognosis Followoing the Arthrocentesis for the Painful TMJ (악기능 장애에 있어 악관절세척술의 효과)

  • Purpose : The aim of this study was to report a follow-up study on the prognosis following the arthrocentsis for the painful temporomandibular dysfunction not responsible to the conservative splint therapies. Arthrocentsis of TMJ is a simple precedure that can be performed in the out-patient clinic under the local anesthesia without any reported complications. Method : Seventy patients had been followed after the arthrocentsis for over 6 months. Maximum mouth opening, TMJ pain, TMJ noise, and their changes by time were examined and compared statistically. The effectiveness of the treatment was evaluated in terms of the postoperative range of maximal mouth opening (MMO) and the degree of postoperative pain score. Predictors which was analyzed were age, duration of painful locking, MMO, the degree of pain, preoperative clicking and the amounts of irrigation fluid. Result : The result of this study were as follow; 1) Mouth opening was improved from 32.6 mm to 42.4 mm in the maximum inter-incisal distance. 2) TMJ pain was decreased in 45.7%. 3) TMJ clicking and noise disappeared in 60.0%, but recurred in 40.0%. Conclusion : Amounts of irrigated solution recovered to normal MMO and the appeareance of perioperative clkicking may be predictors of the successful results of arthrocenetesis of ADD without reduction of TMJ.

Effect of Vagus Nerve Electrical Stimulation to Cortisol Level Control and Heart Rate Variability(HRV) - Pilot Study (미주신경 전기자극이 심박변이도와 스트레스호르몬 분비에 미치는 효과 - 파일럿 연구)

  • Purpose: Stress tends to cause sympathetic hyperactivity and increase blood cortisol levels. The vagus nerve is a parasympathetic nerve that is involved in relaxing the body. The purpose of this study was to investigate the effects of electrical stimulation of the vagus nerve on stress hormone (cortisol) levels and heart rate variability (HRV) in humans. Methods: A total of 10 healthy subjects participated in the pilot experiment. All subjects underwent electrical vagus nerve stimulation for 20 minutes in the cymba conchae of the left ear. An electro cardiogram meter was used to measure HRV. Blood samples were collected and analyzed for cortisol levels. The standard deviation of all normal N-N intervals (SDNN), root mean square of successive differences (RMSSD), low frequency, high frequency, and cortisol levels were compared in pre and post mean values after the intervention. Data were analyzed using the Wilcoxon's signed-rank test. Results: Coritsol levels were observed to decrease after vagus nerve electrical stimulation. The mean SDNN and RMSSD values were increased after the intervention, but not significantly. Conclusion: Vagus nerve electrical stimulation has been shown to increase the ability of the parasympathetic nerve to adapt to upward regulation and stress. Vagus nerve electrical stimulation could thus be an effective treatment for modern social psychological stress control.

The Effect of Gluteus Medius Taping on Balance and Walking in Chronic Stroke Patients (만성 뇌졸중 환자의 마비측 중간볼기근 테이핑이 균형과 보행에 미치는 영향)

  • Purpose : Kinesio taping is a therapeutic method used in the treatment of various musculoskeletal and neuromuscular deficits limited evidence the effects of gluteus medius kinesio taping in neurologic patients. Therefore, this study aimed to determine the effects of gluteus medius kinesio taping on balance ability and gait function in after a stroke. Methods : Twenty-four post-stroke patients were included in this study. Gluteus medius taping group and sham gluteus medius taping group were divided into intervention. Kinesio tape was applied the gluteus medius muscles. In all the subjects, the balance ability was measured using the force plate and timed up & go test (TUG) and gait function was assessed using the 10-meter walking test at time points of both before and after the taping. Result : There was a significant difference in balance ability and gait function between the two groups before and after gluteus medius taping group (p<.05). The gluteus medius taping group showed a significant difference between the groups (p<.05). Conclusion : The results suggest that taping may be a useful method during rehabilitation programs for stroke patients. Application of Kinesio taping to the gluteus medius muscles was found to be useful especially in improving balance ability.

  • Backround : The paradigm of recent education has been shifting from existing style of professor-oriented, passive and rote teaching to learner-centered education. Rather than mere delivery of knowledge, today's idea of education uses various audiovisual media to let learners gain more problem-solving skills, judgment, cognitive thinking ability, and creativity to apply to real practice. Also, while current trends and change in policy ask for related industry to require practice-centered teaching learning model, Problem-Based Learning (PBL) is quite effective that it activates problem-solving skills as well as application of National Competency Standards (NCS). Purpose : The purpose of this study was to suggest a teaching learning model article as an approach to apply web-based PBL for patient & medical charge management practices. Discussion & Conclusion : This paper the cases on PBL and presents the teaching learning model on web-based PBL as an approach to applying web-based PBL, which fits Medical Information System Department that combines health-medical treatment and computer applications, to practical health administrative affairs.

Effects of Early Muscle Strengthening Exercises on Pain, Function and Sleep Quality for Rotator cuff Partial Tear Patients (회전근개 부분 파열환자의 조기 근력강화운동이 통증, 견관절의 기능 및 수면의 질에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the effects of early muscle strengthening exercise on pain, shoulder function and sleep quality for rotator cuff tear patients. Method : The subjects of this study, partial tear of the rotator cuff diagnosed patients, four patients were picked up, who were agreed with this research. This experiment is comprised two experimental groups, with two control groups. The experimental groups recieved early muscle strengthening exercise for 6 weeks, which consisted of 4 times per week, 40 min of supervised exercise program. The control groups received a general movement treatment of 6 weeks which consisted of 4 times per week, 40 min, Pain, shoulder function and sleep quality measured by VAS, PPT and ROM, CMS, SST and PSQI. Result : In comparison of VAS, PPT and CMS, SST and PSQI was the experimental group showed a more improved. Conclusion : Therefore, early muscle strengthening exercise is useful to improve the pain, shoulder function and sleep quality for rotator cuff tear patients.

The Effects of Air-Pressure Cervical Traction Treatment on Forward Head Posture, ROM and Pain in the Tension Headache in the Adults (성인의 긴장성 두통에 에어견인요법이 앞쪽머리자세와 ROM 및 통증에 미치는 영향)

  • Purpose : The purpose of this study was to ascertain the effects of air-pressure cervical traction on the tension headache in the adults. Method : All subjects were treated with air-pressure cervical traction while 15 minutes a day, 3 times per week, for 4 weeks. The effects of air-pressure cervical traction were evaluated by HIT-6(headache impact test), VAS, and flexion, extension, left right flexion, left right rotation of cervical ROM. The measured data were analyzed by using repeated ANOVA. Especially in order to investigate the effect of air-pressure cervical traction. The comparison with among three measurement time. Results : The results of this study were as follows; 1) Prior and two weeks measurement did not show significant differences. The head forward posture than before the experiment, each experimental group was significantly increased(p<.05). 2) Prior and two weeks measurement did not show significant differences. The experimental group than ever cervical ROM of the movable range increased significantly(p<.05). 3) The experimental group was significantly lower than before the VAS(p<.05). 4) HIT score of the experimental group was significantly lower than before(p<.05). Conclusion : These findings suggest that air-pressure cervical traction had effect on the tension headache in the adults.
