A study on facility and program coditions of a welfare hall for the elderly (노인복지관의 시설 및 프로그램 실태)

  • Because of developing medical science, lengthening the average life span and so on, the population of the elderly goes on increasing. The population of the healthy and high-income old people are increasing, also. So this study aimed to grasp the facilities and program situation of the welfare hall for the elderly as a welfare-leisure institution. The elderly be going to a hall are anticipated too many, based on the number of the hall per local old people. The majority of the hall are organized similar spaces and progams in them. There are almost not different things among the welfare halls and the locations. So they should be set on the aged' higher requirements and the old man's need varying in different localities. Also, 'gyoungrodang' remodeled in facilities and programs near their houses is likely to be recommended as the nest best thing.

Study on Food Habits of the Elderly in Institution (시설노인의 식생활태도에 관한 연구)

  • The purpose of this study is to compare food habits of four institution(public boarding home, public mursing home, private boarding home, meal service in welfare center) in Pusan, Korea. Data were collected from 119 of were more than 65 years in the above institutions. Chi-square test was the main data analysis method. More than a quarter of the respondents(27%) showed fairly good level of activity, while a half of them(52.9%) answered the middle level. More than 80 percent of the respondents reported that their food attitude was "Good". There were more elderly women than elderly men among the four institutions. Although the four institutions have served snacks on the regular basis, they did not provide the residents with nutritious food. Most of the residents want to have snacks between meals, but they did not perfer candy. All the subjects didn't point out any particular problem of institution. They were accustomed with their old food served by their institution. As food perferences, almost all of the respondents preferred cooked rice and cooked rice with cereals. Only the residents of private boaring home liked cooked rice with red bean. In case of soup, all of residents liked any kind of soup. The most preferred most menu was beef. But only residents of private boarding home did not like pork. All residents like any all kind of kimchi except kimchi made by radish. In preference of cooking method of a side dish, seasoned food and vegetables were the most favorable menu by the respondents. Compared to the private home, the institutions give residents effective plan to prepare meals. Even though this study tried to find differences in attitudes of dietary life among the institutions, it could not find any meaningful difference among them. Korean elders in the institutions seem to have no proper that they do not have any proper sense of evaluating their dietary life.

A Study on Relations between Health Status and Physical Therapy in Rural Area Elderly Women (일부 농촌지역 여성노인들의 건강특성과 물리치료와의 상관관계)

  • Purpose: This study examined the health status and physical therapy in elderly women in rural areas to provide basic data on the promotion of health and physical therapy management of elderly women in rural areas. Methods: The subjects were 110 elderly women. The data was collected through individual interviews, and the tool developed for this study was a structured questionnaire based on the literature. The reliability coefficient was 0.57 ~ 0.79(Chronbach' $\alpha$). Results: Village 1 made the most of an oriental medicine clinic and a neighborhood clinic, whereas village 2 mainly used a public health center. Arthritis/ neuralgia and high blood pressure/low blood pressure were the most common complaints in the two villages; a medical institution was used once a month by more than 50% of subjects. Most rural elderly women used physical therapy, and the prevalence of arthritis/neuralgia was high. The most common problem in the two villages was poor health. The improvement in mobility was higher in the more healthy women, who also had less need for treatment at a medical institution. Conclusion: Most rural elderly women were concerned about poor health and used physical therapy. Therefore, rural medical institutions need to pay attention to the medical service and preventive activity to reduce the incidence and severity of neuromuscular syndrome in rural elderly women.

A Study on the Status of Utilization of Health Care Institution by Geriatric Patients -Focus on the Utilization of Physical Therapy- (노인환자 보건의료기관 이용실태 분석)

  • This study was conducted to provide basic materials required to enforce and develop welfare policies, as well as the health system, for the aged, by surveying the status of health care utilized by the daily increasing old population and the importance of receiving physical therapy. Data that need in this research was gathered from over ages of 65, during the period from Jan 4, 1996 to Jan 31, using the inquiries previously made by geriatric researchers and through literatures investigator by this writer. The data were analyzed by $X^2$, Z-test, Likert scale. The findings were as follows : 1) General characteristics of subjects. People in the age group between 65 and 69 were 55.6% and the highest number, while male were 37% and female 63%. Analysis of income group disclosed 60.6% whose monthly income, including the pocket money given by children, was less than 200,000 won. 91.1% of the elderly people surveyed owned houses; only 36.4% live with spouses; while 15.6% live alone. 2) Characteristics with respect to utilization of health care institution. 56% of the total medical institutions used by the elderly people were clinics and the rates of chronic disease and musculoskeletal disease were 73.2%. 3) Characteristics with respect to approach of health care institution. 45.1% of the respondent stated it took 20 minutes to arrive at hospital, and bus accounted for 48.6% of all transportation means used to go to hospital. 4) Degree of cognition with respect to the rights of geriatric patients. (1) There is no financial support from the government for geriatric patients(71.4%). (2) Government financial support is needed for geriatric patients(95.3%). (3) Have never been regionally surveyed or called upon for interviews with respect to treatment desire and problems relating to geriatric patients(87.2%). (4) Health and medical policies for geriatric patients must be established rapidly(98.4). (5) Expansion and construction of specialized medical facilities for geriatric patients such as elderly hospital and medical center are needed(90.2%). (6) Government's welfare policies for the elderly people is insufficient(82.0%) 5) Degree of cognition on importance of physical therapy with respect to geriatric patient. (1) Physical therapy is considered most effective in treating geriatric patients(82.9%). (2) Physical therapists specializing in only elderly people must be need of separately(76.2%). (3) It is desirable for medical specialists to visit geriatric patients at home to provide physical therapy(82.9%). (4) Hospitals specializing in physical therapy for geriatric patient are required(85.6%). Based on the result for this research, the following suggestions are presented to facilitate the utilization of health care institution for the welfare of geriatric patients. Medical facilities such as elderly hospital and geriatric patient's medical center specializing in elderly people must be constructed as early as possible; and home-visiting physical therapist system must be important to treat chronic geriatric patients; our government must establish policies to provide the old ages with means for the health care and curing chronic diseases, and carry out the plans of reasonable distribution and effective untilization of medical resources.

  • As the aging of this society, instead of growing numbers old protective functions of families decreased according to increasing tendency of nuclear families, increasing rates of females' participation of economic activities. Now, the problem of supporting the old is in need of social supporting system the cope with this are based on these factors are like this: 1) It is necessarily the instruction of medical welfare service. 2) The overlapped service is avoided and the service of specialized. 3) Most of the welfare service institution is Seoul and local city hall, so it needs the institution is transferred. 4) It needs the increase the number of home helper. 5) When the service is offered, the government must considers finance, elderly health condition and preference. 6) It needs the department for the welfare service for the old who stay at home.

  • Japan is one of the countries where the number of the elderly people is most rapidly increasing. This study aims to locate the suitable areas for the lodgings of elderly people, using Geographical Information System (GIS). We focused on the areas where the residents can live a usual and customary life, which includes walking, shopping, hospitalizing etc. Combining the geographical and municipal data along with some other information obtained from social factors, we performed selection of the optimal locations for welfare institutions. Out of 413 areas within Nagasaki City three different areas were found suitable for such elderly people lodging.

A Study of the Factors Influencing the Staff Members' Recognition of the Level of Ethical Management in the Elderly Welfare Institutions (노인복지기관 종사자의 윤리경영수준 인식에 대한 영향 요인 연구)

  • The purpose of this study is to investigate the effects of staffs' general characteristics and institutional characteristics on the staff members' recognition of the level of ethical management in the elderly welfare institutions and to provide basic materials for raising standard of ethical management in the elderly welfare institutions. For this purpose, we conducted a questionnaire survey using structured self-filling-type questionnaires for 245 staffs of elderly welfare institutions in Seoul and Gyeonggi-do. This study sets general characteristics of staffs and institutional characteristics as the independent variables and the staff members' recognition of the level of ethical management as the dependent variable. The variables are investigated through the descriptive statistics such as frequency, percentage and average analysis, and the difference of effect on the staff members' recognition of ethical management in the elderly welfare institutions by each independent variable was analysed by t-test, ANOVA and multiple regression analysis. The results of the study shows that in terms of general characteristics of staffs, the higher male to female ratio and the higher educational level; and in institutional characteristics, the less experience in basic ethics education and the better ethical working environment of the institution are the positive factors for that the staff members estimate the level of ethical management of their workplace high. Based on these results, for making staff members recognize the level of ethical management in the elderly welfare institutions higher, we should create actual atmosphere of the ethical working environment in the institution and implement not formal but step-by-step intensive ethics education that make employees can apply it in the real situations. In conclusion, this study gives the practical suggestions for improving the staff members' recognition of ethical management level in the practice field of elderly welfare on the fast-changing trends including downsizing and privatization.

The Relations among ADL, Self-efficacy, and Life Satisfaction of Elderly Institution Residents (시설노인의 일상생활능력, 자기효능감 및 삶의 만족도와의 관계연구)

  • Purpose: This is a descriptive research to examine the levels of ADL, self efficacy, and life satisfaction of elderly institution residents, and the relations among the factors. Method: Data were collected through a questionnaire survey of elders aged over 65 at nursing homes in 9 areas throughout the country. Measures were the physical function scale, the general self efficacy scale, and LSI Z. Collected data were analyzed using SAS/PC11.0. Result: First, the mean levels of ADL, self efficacy, and life satisfaction were 2.83 (0.85), 2.79 (0.63), and 1.18 (0.42). Second, the relations among three factors were showed positive correlations. Third, (1) The level of ADL was statistically significantly different according to health condition, religious activity, volunteer activity, and friend meeting. (2) The level of self efficacy was statistically significantly different according to health condition, source of help, religious activity, volunteer activity, friend meeting, and elderly school & senior center. (3) The level of life satisfaction was statistically significantly different according to health condition, source of help, religious activity, volunteer activity, friend meeting, and elderly school & senior center. Conclusion: Conclusively, it is required to enhance ADL and self-efficacy in order to improve elderly institution residents’ life satisfaction.

Cognitive and Physical Function by the Social Disengagement in Institution and Community Living Elderly (시설노인과 지역사회 거주노인에서 사회적 유리에 따른 인지기능과 신체기능)

  • Purpose: This study was conducted to compare the level of cognitive and physical function by the level of social disengagement in institutions and community dwelling elderly. Method: The study subjects were 209 people whose age ranged from 65 to 94. The data were surveyed with social disengagement index, MMSE-K, and ADL, and analysed by SPSS 11.0 using frequency, t-test, ANCOVA and ANOVA. Results: 1) The level of social disengagement was 1.67 in institution and 3.25 in community elderly, and showed statistically significant difference (F=41.07, p<.001). 2) There were statistically significant difference in MMSE-K (F=15.51, p<.000), and activity of daily living (F=9.65, p=.002) between two residence after control the age, gender, religion, allowance and number of children. 3) There were statistically significant difference in MMSE-K (F=6.55, p=.002) by social disengagement in community dwelling elderly. Conclusions: Programs for successful aging to reduce social disengagement should be developed that stimulate social activities and then improve cognitive function for the elderly. For these, establishing social policies in nation and local government is needed. Especially, these policies should be conducted among the elderly with low-income living in institutions.

Factors Affecting the Healthcare Utilization of Spinal and Joint Surgery in Elderly Patients (65세 이상 노인의 척추·관절 수술별 의료이용에 미치는 영향요인 분석)

  • Background: The purpose of this study is to analyze the current status and factors of elderly patients' hospitalization for hip replacement, knee replacement, and general spine surgery. Methods: National health insurance data in 2018 was provided by the National Health Insurance Service. We used multiple regression to analyze factors associated with the medical utilization of hip replacement, knee replacement, and general spine surgery in elderly patients over 65 years old. The dependent variables are the length of stay and total health expenditure. The independent variables are the demographic-social factors (sex, age, region, insurance type, income level) and surgery-related factors (institution type, location of the hospital, surgery classification). Results: The most common factor affecting surgery was the location of medical institutions. Compared with the medical institutions located in metropolitan, the length of stay in rural medical institutions was higher and total health expenditure was lower. The lower quartile of income, the higher the length of stay and total health expenditure. In addition, the variables of age, type of health insurance, and type of medical institution were statistically significant. Conclusion: In this study, we confirmed the effect of sociodemographic factors and medical institution factors on the Healthcare Utilization of spinal and joint surgery.
