Twenty-three patients with chronic shoulder pain beyond 6 months after the fracture of the greater tuberosity underwent arthroscopic treatment and were retrospectively assessed after an average of 29 months(range, 22 to 40 months). There were 18 men and 5 women with the average age of 39 years(range, 24 to 61 years). Fourteen were isolated fractures and nine were related to acute anterior instability episode. The average displacement of the fracture was 2.3mm(range, 0 to 4mm) on the anteroposterior view of the plane radiographs. At the time of arthroscopy, all patients had partial thickness rotator cuff tears in the articular surface. The cuff tears were located on the tuberosity fracture area and were an Ellman's grade I to n in depth. With the arthroscopic debridement or repair of the tear depending on the condition of the tear itself, as well as the subacromial decompression, the UCLA score revealed good to excellent results in 20 and fair in 3 patients. Nineteen of the patients had returned to the previous level of activities. The patient with a higher activity demand revealed a lower level of activity return(p=0.034). The partial thickness rotator cuff tear should be considered in patients with chronic shoulder pain after the minimally displaced fracture of the greater tuberosity, and arthroscopic debridement or repair is an appropriate procedure.
Remicade is a therapeutic biosimilar natural antibody in which the mouse variable domain has been linked to the human constant domain. It is a chimeric monoclonal antibody specific to tumor necrosis factor-alpha (TNF-α) and has been developed for the treatment of rheumatoid arthritis. To investigate the biological activity of the Remicade antibody, we carried out a bioinformatics study using a protein data bank to characterize the TNF-α antigen binding mechanism of the Remicade natural antibody. Because the production of the Remicade antibody is often limited by genetic instability of the natural antibody-producing cell, we generated a Remicade single-chain variable domain fragment antibody (Remicade) in which a heavy chain variable domain (VH) is joined with a light chain variable domain (VL) by a polypeptide linker. Furthermore, Remicade was fused to a leucine zipper (RemicadeScZip) for higher production and higher antigen-binding activity than Remicade. The Remicade and Remicade ScZip were expressed in Escherichia coli and purified by a Ni+-NTA-agarose column. As expected, the purified proteins had migrated as 28.80 kDa and 33.96 kDa in sodium dodecyl sulfate-polyacrylamide electrophoresis. The TNF-α antigen binding activity of Remicade was not observed by ELISA and western blot. In contrast, RemicadeScZip showed antigen-binding activity. Additional bio-layer interferometry analysis confirmed the antigen-binding activity of RemicadeScZip, suggesting that the leucine zipper stabilized the folding of RemicadeScZip in a denatured condition and improved the TNF-α antigenbinding activity.
The host rocks of brick-shaped stone pagoda in the Bunhwangsa temple are lots of kinds andesitic rocks, which has gone through mechanical and chemical weathering. As the overall observation, the pagoda is serious damages by air pollutants, and the northeast parts show the much advanced state of turning white, while the southeast parts are heavily cracked in the materials. The rocks of brick-shaped pagoda body are in a relatively stable condition of weathering and damage except for the abrasion and cracks of the corners. The rocks of the pagoda roof suffer from more symptoms including multiple peel-offs, exfoliation, cracks forming round lines, and falling off stone pieces. The pagoda roof rocks are dominated by the thriving leafy lichens and mosses, especially, there are higher plants (selaginella involvens, dandelions) taking root actively between the brick stones and content mortar. There are even light gray precipitates like stalactites between the rocks of the body, In particular, the 1st and 2nd floor in the east side and the body parts in the north side are the most serious. Their major minerals are calcite, gypsum and clay minerals. The rocks of the stylobate and the tabernacle in all the four directions are composed mainly of granitic rocks. The materials consisting of the tabernacles show the severe splits and distortion, which causes the structural instability. The stylobate rocks are heavily contaminated by some weeds with the often marks of inorganic contamination by secondary hydroxides. The central part of the east stylobate has been sinking, while that of the 1st floor west stylobate is protruded nesting a line of cracks. Accordingly, the inside of the tabernacle is always humid with the constant introduction of rainwater. The stone lion standing in the southeast and northeast side are alkali granite, while that in the southwest and northwest lithic tuff. Each of the stone lion also coated with various colored lichens, mosses, algae, bacteria and bryophyte. The external materials of the pagoda have deteriorated the functions of the rocks and made the loss, falling off, and biological contamination even worse due to the surface weathering. Thus it's urgent to come up with scientific restoration and conservation measures through clinical tests.

Pheochromocytoma is derived from the chromaffin tissue. The typical finding of pheochromocytoma is paroxysmal hypertension accompanied with various signs and symptoms that are due to the excess of catecholamines or other bioactive substances. Yet the diagnosis is sometimes difficult to make because its clinical presentation is quite variable. Especially, hemoptysis is a very rare symptom, so the diagnosis is often missed or delayed. Without making the correct diagnosis and then subsequently administering treatment, the condition may be fatal. We herein report on a 68 year-old woman who was admitted because of abdominal pain and hemoptysis. The initial radiologic findings suggested pulmonary edema with alveolar hemorrhage. The urine catecholamine levels were elevated and she developed catecholamine-induced cardiomyopathy. We performed bronchial arterial embolization and we administered alpha blocker medication for controlling the hemoptysis and hypertension. After the temporary symptomatic improvement, her clinical course was aggravated by pneumonia and pulmonary edema. In spite of performing definitive surgery for pheochromocytoma, she died of postoperative hemodynamic instability.
This study presents the results of a series of laboratory scale slope failure experiments aimed at clarifying the process and the condition leading to the initiation of rainfall-induced slope failures. For the evaluation of hydrologic response of the model slopes in relation the process of failure initiation, measurements were focused on the changes in volumetric water content during the initiation process. The process leading to failure initiation commences by the development of a seepage face. It appears reasonable to conclude that slope failures are a consequence of the instability of seepage area formed at the slope surface during rainfall period. Therefore, this demonstrates the importance of monitoring the development seepage area for useful prediction about the timing of a particular failure event. The hydrologic response of soil slopes leading to failure initiation is characterized by three phases (phase I, II and III) of significant increase in volumetric water content in association with the ingress of wetting front and the rise of groundwater level within the slope. The period of phase III increase in volumetric water content can be used to initiate advance warning towards a failure initiation event. Therefore, for the concept outlined above, direct and continuous monitoring of the change in volumetric water content is likely to provide the possibility for the development of a reliable and effective means of predicting the occurrence of rainfall-induced slope failures.
This study was conducted to investigate the effects of some soil conditioners, such as polyacrylamide(PAM), polyvinylalcohol(PVA) and Bitumen emulsion, on aggregate formation and stability, wetting angle, sorptivity and penetrability of the soil with different textures : sand, sandy loam, loam and clay loam. A pot experiment was carried out to find out the effect of treatment on the germination and growth of lettuce with three textures : sand, sandy loam and silt loam. Soil aggregates larger than 2mm in untreated soils at dry condition were naught in sand, 45% in sandy loam, 80% in loam and 90% in clay loam. Treatments of soil conditioners tended to increase the occurrence of soil aggregate larger than 2mm, which were 20~25% in sand, 55~75% sandy loam, but not affected greatly aggragate occurence in loam and clay loam. The aggregate instability was decreased by the soil conditioner treatment. The wetting angles of the soils were greatly changed by hydrophobic of Bitumen, but those were changed slightly by PVA and PAM application. The sorptivity and penetrability data indicated that the effects of different materials on these parameters differed depending upon soil texture. Application of PVA and PAM were no effect exceptive in sand. Application of Bitumen revealed that water movement was not showed in all soils. The germination rate, root weight and top plant weight of lettuce were increased in all soils by PAM treatment as compared to untreated soils. Particularly the greater effect was occurred in sand soil than in sandy loam and silt loam. PAM increased greatly the moisture content and air phase of soils.
This study is to consider applicability of spiral bolt strain gauge as an instrument measuring behavior of soft ground foundation and rock slope. When the instrument was installed on the ground, it can be useful to identify the state of ground behavior because it has the characteristics of flexibility, as well as to apply the ground reinforcement because it has higher pull-out resistance to the ground. From the measurement of behavior to soft ground foundation, the strain shows a stable state in the beginning, then was observed significant change in the upper and the middle of spiral bolt strain gauge after 400 days. This is analyzed that ground loosening, which is due to occurred frequent earthquake of magnitude 1~2 with increased rainfall, lead to the instability of the ground. From the measurement of behavior to rock slope, the strain shows a stable state with very little change in a period of 0~50 days and the biggest strain at 4.2 m (P6) in a period of 50~100 days, then other places except P6 was maintained at a stable state in a period of 100~160 days. The reason is analyzed because that blasting for excavated limestone surrounding was affected to the largest at P6. However, based on the size of strain change by behavior of the soft ground foundation and rock slope, it is considered that the present condition are not effected on stability of retaining structure and rock slope. In conclusion, the proposed spiral bolt strain gauge can be useful to measure behavior of soft ground foundation and rock slope, and also to be measured behavior as well as reinforcement of the target ground.

In recent years, many construction projects become large and complicated, and construction accidents also steadily increase, which grows interest in the safety and maintenance during construction. Many of the construction accidents are related to temporary construction and structures, but the safety evaluation and management during construction are unclear and indefinite due to the short operating period and continuous change in the formation of the temporary structure. The system support, which is one of the temporary structures to support the pouring load of concrete, was proposed to easily install and dismantle members with connection parts pre-manufactured. The use of the system support is increasing to improve the safety of the temporary structure during construction. However, the system support, which consists of multiple members, still has uncertainties in connectivity between members and supports of vertical members. Therefore, this study analyzed the structure, load, and accident cases of the system support to define the damage scenarios for member connection, support condition, and lateral displacement. The decrease rate of the critical load was analyzed according to the damage scenarios based on the defined unit structure of the system support. In addition, this study provided vulnerable members for each damage scenario, which could induce instability of the temporary structures during design, construction, and operation of the structure.
The study measured a subway construction worker's Macpa stress by Heart Rate Variability measuring instrument and conducted a survey of Korean job stress from subway construction workers. Also, the study analyzed the relationship between Macpa stress index and Korean job stress result and suggested managing stress method for each item. According to National Statistical Office data, the first line subway in Seoul was started to open in 1974. The extended total length is 996 kilometers until 2019. Many aged workers are currently working at subway construction sites due to the avoidance of young workers since the past until now. It means that the elderly has a substantial portion among subway construction workers. The productivity has been adversely affected by health problems due to the aging of workers, job stress due to heavy work, and personal health problems. So, the regulation and policies on job stress health management are being strengthened. The data were measured Macpa stress by machine measuring heart rate variability and conducted Korean job stress survey(shortened) from Sa-sang to Ha-dan line Busan subway construction workers for analyzing the relationship. Independent variable were age, job duration, job position, employment type, working type in this study. Macpa's dependent variable was stress index and Korean job stress survey(shortened)'s dependent variables were job requirements, job autonomy, relationship conflict, job instability, organizational structure, inappropriate compensation, working place culture, and total score. SPSS 12.0 K Statistics Program was used for statistical analysis. Kruskal-wallis test, a nonparametric statistical analysis, was used because the data are difficult to be assumed as normal distribution. As a result, the paper indicated the significant correlation between Macpa stress index and Korean job stress(short version). The elderly workers presented higher Macpa index and higher job stress due to aging and heavy-duty work. The majority workers were daily workers who had unstable working condition and uncertainty about the future. The study suggested a manual that could reduce job stress for subway construction workers and future study deriving management tool through analyzing job stress factor is necessary.
At the mudstone slope located on the roadside of the Seokri area in Donghae-myeon, Pohang, Gyeongsangbuk-do, this study was performed to analyze the effects of rainfall on the stability of slope through seepage analysis according to the precipitation type of the mudstone slope, referring to the actual case of slope failure. For this, precise geological survey, geophysical exploration and drilling survey for the slope where the failure occurred were performed and followed by analysis of detailed soil layer. For the section where failure surface located, the durability reduction of rocks was measured through slaking/swelling tests and the permeability was measured through in-situ permeability tests for each soil layer. In addition, the change of strength parameter and process of instability were analyzed by back analysis, using Talren 97 and Slope/W programs, in the slope. By applying different precipitation conditions to the geographical conditions of the slope that had actual failure records, the slope stability was analyzed by seepage analysis according to duration of rainfall and rise of groundwater level resulting from the flow of rainfall caused by development of geological structures and the slope surface condition.
